1.Computed tomography of osteitis condensans ilii

The CT is a more accurate technique for detecting sclerotic bony change of osteritis condensans ilii thanplain radiograph. We analysed a comparison between CT and plain radiography of osteitis condensans ilii, acorrelation between osteitis condensans ilii and women of childbearing age. The result were as follow: 1. Theincidence of osteitis condensans ilii is 5.3% on KUB, 11.7% on CT when the width of iliac sclerosis is more than7.5mm as diagnsotic criteria. 2. We observed a osteitis condensans ilii between 19 years and 51 years of age, mostfrequently in fourth decade. 3. The width of iliac sclerosis is 10-13 mm in 3 cases of osteitis condensans ilii onboth CT &KUB, 7.5-9mm in 4 cases of osteitis condensans ilii on CT only. 4. The incidence of osteitis condensansilii is increased significantly when the width of iliac sclerosis is less than 7.5mm as diagnostic criteria. 5.Relatively high correlation between osteitis condensans ilii and delivary in our study (66.6%)

2.Value of the lumbar lordotic angle taken angle from CT scanogram as an index of back pain

Normal" spinal posture is generally accepted as moderate lordosis of the cervical and the lumbar sections ofspine and moderate kyphosis of the thoracic and sacrococcygeal sections. Among these normal range of the lumbarlordosis is less well defined. CT is very useful method to identified the bony structure and adjacent soft tissueof the spine, so it is very available method to detect accrate lumbar lordotic angle by using lateral Scanogram.Analysis and comparison of lumbar lordotic angles, crossing angle are drawn each parallel line to the superior endplate of body of 1st. lumbar vertebra and to the inferior end plate of the 5th. Lumbar vertebra, in 174 cases withbackache and 50 cases normal groups at Kangnam General Hospital Public Corporation, from Aug. 1985 to Jul 1986.Male and female were almostly same affected in backache group (1.1:1). And most lumbar lordotic angles were 10 degrees to 40degrees between all backache age group. On backache group, over all mean lumbar lordotic angleswere 24.7±8.9degrees but no significant difference at mean value of the each disease, such as HIVD, DegenerationSpondylosis or No Remarkable Finding group. On control group, over all mean ones were 29.2±8.0degrees. So,significant difference of lumbar lordotic angle between backache and control group, and most difference is at 5th.decade group(p<0.01).

5.The Use of Flexible Intramedullary Nails in Fracture of the Tibial Shaft

Forty five cases of adult tibial shaft fractures were treated with flexible intramedullary nails under image intensifier at the department of orthopaedic surgery, Capital Armed Forces General Hospital during between April 1986 to July 1988. The results were summarized as follows: 1. 45 cases were all male, the average age was 24.3 years and the most common cause of fractures was sports injury(20 cases) and the second most common cause was traffic accident(18 cases). 2. 11 cases out of 45 cases were open fracture and 34 cases were closed fracture. Eight cases of open fraeture were grade I and three cases were grade II. Twenty eight cases were comminuted fracture. The most common level of the fracture was middle third of the tibial shaft. 3. Average interval from injury to Ender nailing was 13.5 days and the mean duration of permission to full weight bearing was 9.2 weeks. 4. Out of 2 cases of delayed union, the average time of clinical bone union was 9.7 weeks and that of roentgenological bone union was 14 weeks. 5. 11 cases of complication were noted among the 45 cases of tibial shaft fracture which were treated by PGP nailing such as angulation, rotation, ankle joint stiffness, delayed union, shortening, and soft tissue infection. 6. PGP nailing is considered to be a valuable method in treating tibial shaft fracture because of their relative ease of nailing, allowing early weight bearing and decreased complication.

7.Factors Affecting Measurement Error of Vertebral Axial Rotation and Reduction of Measurement Error through Training.

PURPOSE: To identify the changes of bony landmarks with rotation and to compare the reliability and accuracy of Perdriolle's method before and after training on the exact bony landmarks. MATERIALS AND METHODS: Simple A-P radiographs were taken with rotation from 5 degree to 50 degree at 5 degree interval using 10 dry human vertebrae (2 sets of T3, T6, T9 8 L3) with no bony abnormality. Axial rotation was measured before and after training on the exact bony landmarks by three observers, twice for each radiographs. Repeated measures of ANOVA were used for inter- < intraobserver variance and regression analysis for root mean square error and deviation of differences. RESULTS: Mamillary process, inner margin of lamina, proximal and distal costovertebral joints, upper margin of concave pedicle and tip of the transverse process on the convex side confused the exact outline of the pedicle, while neural foraminal margin was helpful in identifying the location of the indistinct convex pedicle and concave vertebral border. Of the 660 measurements before and after training 372 and 397 errors were made, respectively, of which 88.9% and 88.5% were within +/-5 degree(P > 0.05). Incidence of error greater than 5 degree was high at upper thoracic vertebra and 20 degree-40 degree of rotation regardless of training. The effects of training, however, was greatest at T3 and 20 degree-40 degree of rotation. Inter- < intraobserver variance was diminished at T3 and at the rotation between 20 degree-40 degree after training. CONCLUSIONS: We could conclude that training on the exact bony landmarks did not reduce the overall error incidence. Accuracy and reliability, however, was increased at T3 after training and the efficacy of training was more evident in moderate vertebral rotation than in small or large vertebral rotation.

9.Effects of Intra-Articular Injection and Subscapularis Motor Point Block on Painful Hemiplegic Shoulder.

OBJECTIVE: To evaluate the effects of intra-articular injection of shoulder and subscapularis motor point block on painful hemiplegic shoulder. METHOD: Thirty painful hemiplegic shoulder of recent onset stroke were divided randomly into three groups, i.e. group I: range of motion (ROM) exercise only, group II: intra-articular injection with ROM exercise, group III: subscapularis motor point block with ROM exercise. The intra-articular injection of shoulder was done with 20 ml of normal saline, 5 ml of 1% lidocaine, and 40 mg of triamcinolone. The subscapularis motor point block was done with 5 ml of 5% phenol under electromyographic guide. The ROM in external rotation and simple X-ray of shoulder in full abduction were checked in three groups at pre-treatment and post-treatment 3 week, and the glenohumeral abduction and scapulohumeral rhythm were obtained from the shoulder X-ray. RESULTS: There were significant (p<0.05) improvements of glenohumeral abduction, external rotation, and scapulohumeral rhythm of shoulder in group II and III, but improvement of only glenohumeral abduction was significant in group I. Angular increment of glenohumeral abduction and external rotation was most significant in group III compared with other two groups. CONCLUSION: These results suggest that the intra-articular injection of shoulder and subscapularis motor point block are potentially useful techniques in the prevention and management of the painful hemiplegic shoulder.
