1.A Case of Sweet's Syndrome Associated with Gastric Adenocarcinoma.

Sweets syndrome has 4 cardinal features, that is, fever, neutrophil polymorphonuclear leukocytosis of the blood, raised painful plaques on the limbs, face and neck, histologically a dense dermal infiltration with mature neutrophil polymorphs. Hematologic malignancies are associated with 10-15% of cases, but gastric adenocarcinoma associated case is very rare. A 57-year-old male patient admitted to Korea University affiliated hospital because of fever and painful erythematous plaques of both limbs. No response to antibiotics and antihistamines for 1 week, so we referred to dermatologic department for skin biopsy. After then under the impression of Sweet's syndrome, oral prednisolone 60 mg/day prescribed and skin lesions were progressively regressed. For the purpose of screening of associated disease, we did abdominal CT scan and panendoscopy with biopsy which revealed adenocarcinoma of stomach. We couldn't find out any other etiologic agents.

3.Tolerance of Patients and postoperative Results: Strabismus Surgery under Topical Anesthesia.

The results of corrective surgery in strabismus patients are not always predictable. To lower the chance of reoperation, intraoperative or postoperative adjustment is performed. The authors performed intraoperative adjustment surgery in 24 patients with topical Anesthesia with 0.5% tetracaine. We evaluated the severity of pain and the strabismus angel postoperatively. Follow-up showed success rate of 95.8% immediately after surgery. 79.2% on 6 weeks after surgery and 70.8% after 1 year. Among 24 patients, 17 patients felt no pain and 7 patients had on slight pain on muscle traction. The above results suggest that we can not prevent long-term recurrence after adjustment operation using topical anesthetics, this method causes no intolerable pain and making this a useful treatment for strabismus angle.

4.A Case of Exophthalmos due to Traumatic Orbital Encephalocele.

Exophthalmos after trauma may develop because of retrobulbar hemorrhage, carotid cavernous fistula and orbital roof fracture. The orbital roof is composed of two layers of strong bone that makes it the strongest among the orbital walls and reports of its case are hardly found. However, rarely strong impact such as in automobile accidents causes these fractures and exophthalmos may develop due to herniation of brain tissue through a traumatic defect in the roof of the orbit. The authors experienced gradually increasing exophthalmos in a 33-year old man who had developed a subdural hematoma, orbital roof fracture and anterior temporal skull fracture due to an automobile accident. We report a case of orbital roof fracture in which herniation of brain was shown in orbital computed tomography and exophthalmos disappeared after dural suture.

5.Studies on the life cycle of Culex pipiens pallens in Korea.

The present studies were carried out, under natural room condition from June to July in 1969. The period required for growth of eggs and larva varied according to various temperatures and methods of rearing. The average number of eggs laid by the Culex pipiens pallens was 137 per egg raft. The period of gonotrophic cycle was from 3 to 6 days: incubation period of eggs was from 1.5 to 2 days: larva period was from 10 to 14 days: pupa period was from 2 to 3 days. Accordingly, the period from the beginning of gonotrophic cycle to the adult mosquitos in next generation required 25 days at the longest and 16.5 days at the shortest.

6.Cross-reactivity between sera from dogs experimentally infected with Dirofilaria immitis and crude extract of Toxocara canis.

This study was performed to investigate whethere there is cross-reactivity between Dirofilaria immitis and three intestinal nematodes of dogs. In ELISA, D. immitis-infected dog sera obtained at the 4th molting stage (9-11 weeks) and microfilaremic stage (25-30 weeks) were shown to be highly reactive with crude extract of T. canis. In immunoblotting, some antigenic fractions, 44, 57, 88, 100 kDa of crude extract of T. canis, were found to be positive reaction with sera of dogs infected with D. immitis. However, little or no cross-reaction were observed between sera of D. immitis-infected dogs and crude extract antigen of T. vulpis or A. caninum. These result suggest that there are partial cross reaction between sera of D. immitis-infected dogs and the antigen of T. canis.

7.The Relationship Between Nutritional Assessment and Obesity Index.

BACKGROUND: Obesity has been accepted as the rusk factor of major chronic diseases in our society. And nutritional assessment is the important issue in respect to obesity evaluation. A self-administrated semiquantitative food frequency questionnaire of which validity is recognized is used in nutritional assessment. But the relationship between nutritional assessment resulted from the questionnaire and the obesity index has not been built until now, so we studied about this relationship. METHODS: Among 324 adults who had visited health screening center from June'95 to August '96, 270 adults were evaluated except who had any chronic disease such as diabetes mellitus, pulmonary tuberculosis, thyroid disease by screening data or low calorie diet for aimed weight loss or inadequate Questionnaire. Nutritional intake was assessed by the self-administrated semiquantitative food frequency questionnaire. We evaluated the activity grade, obesity indices such as body mass index(BMI), waist to hip ratio(WHR), percent body fat using bioelectrical impedance fatness analyzer and serum total cholesterol, HDL-cholesterol, triglyceride. Then we examined the correlation between obesity Indices and nutritional variables each other. We analyzed these results using Pearson's correlation coefficient. RESULTS: We evaluated for a group of 164 men and 106 women whose mean age was 43.9 years, mean BMI was 23.2kg/m2, mean WHR was 0.85 and mean percent body fat was 21.2%. Correlation coefficient between calorie intake to recommended calorie ratio(ICRCR) and BMI was 0.13(P<0.05), WHR was 0.11(P> OR =0.05), and percent body fat was 0.07(P> OR =0.05). Fats intake to recommended fats ratio(IFRFR) and obesity indices didn't show the significant correlation between them. We could not find the significant correlation between cholesterol intake and serum total cholesterol, triglyceride but the significant negative correlation between cholesterol intake and serum HDL-cholesterol(r=-0.14) was found. CONCLUSIONS: Only BMI among obesity indices and ICRCR showed low(r=0.13) correlation coefficient although it is significant. Our results shows that obesity is the complex syndrome influenced by genetic, social, psychological, medical variables besides nutritional impacts.

9.Three Cases of Membranous Reticular Inflammatory Polyp of the Colon.

Inflammatory polyposis of the colon is rare portion of the colon polyp entity. We experienced three patieats who suffered from colitis of unknown'etiology and discovered membranous retkular with/ without filiform polyp in the distal sigmoid colon of the them. Case 1, 56-year-old female, combined with basaloid cercinoma of the rectal canal that confirmed by pathology after abdominoperineal resection. So we obtained specimen of inflammtory polyp end tried to evaluate the mechanism of it. We found healed ulcer under the mucosal bridge. Case 2. 55-year-old male, had been suffered from irritable bowel syndrome. Prior to visit outpatient clinic, he complained tenesmus with diarrhea for a week. Inflammatory polyp was discovered by colonoacopy but his symptoms relieved by tranqulizers only. Case 3. 57-year-old male, has been a good health. He undertook colonoscopy for routine check thst revealed membranous reticular with filiform inflammatory polyp from distal sigmoid colon to hepatic flexure. Such shape of inflammatory polyp was not documented in Korea, so we reported three cases of it with review of the literature.

10.Femur Shaft Fracture in Children Treated by Charnley's Traction Unit

The treatment and results of 6 cases of the femoral shaft fracture in children are prensented and the method of Charnleys traction unit used most frequently at Korea General Hospital is described. Many Kinds of traction for the femoral shaft fracture in children was introduced. But nearly all methods which depend on heavy traction are open to the following criticism; 1) It is impossible to diminish the traction force alone without jeopardising the stablity of the reduction. 2) Gravity is not used to help in correcting the deformity of backward angulation. 3) Backward angulation of the distal fragment can never be corrected by traction in the axis of the femur. To do this the direction of the traction must be delfected away from the axis of the femur. Charnleys traction offers the following features: 1 The foot is supported at right angles to the tibia. 2. The external popliteal nerve and calf muscles are protected from pressure against the slings of the splint. The tibia is suspendcd from the Steinmann pin inside the plaster so that an air space develops under the tibia as the calf muscles lose their bulk. 3. External rotation of the foot and distal fragment of the femur is controlled. 4. The tendo Achillis is protected from pressure sores. 5. Comfort; the patient is unaware of the traction when applied through the medium of the nail. Age distribution of the patients was from 2 years to 13 years. Initial angulation after the manual reduction was from 0 to 25 degree posteriorly and from 5 to 9 degree laterally. Angulation after radiological union was from 0 to 15 degree posteriorly and from 0 to 8 degree laterally. Overriding before reduction was from 0.9 to 3.2cm and after reduction from 0.3 to 1.2cm. The duration of the inmobilization in Charnleys traction unit was from 15 days to 57 days. The duration of radiological union was from 51 days to 94 days.
