We reviewed the clinico-histopathologic aspects. of 121 patients diagnosed as subcutaneons lipamas in 1981~1986, Additionally, the size of lipoma cells was statistica,lly compared with normal controls. The results were obtained as follows: The majority of patients visited the surgical department, specially general surgcry, whereas only 7 visited dermatology. The ratio of solitary to multiple type was 10:1. Females predominated in the patients with solitary type, outnumbering males in multiple type. While solitary type consisted of 98 patients (89.1%) with SL, 10 with FL and 3 with AL, multiple type had 7 with SL and 4 with AL. There were prominent incidences in the 5th and 6th decades followed by a slow decline. Faur of all 6 AL patients were multiple type. Males were affected in all but one. All AL patients showed non-infiltrating. The torso was mostly involved in 60.4% In 7 of all 17 AL lesions, the upper extremity was predorninantly involved. Sixty sevenr of the 136 masses were less than,3cm in diameter. In AL lesions, 76.5% was less than 3cm and likely to be smaller than other 2 types. The presumed time of onset was most common in 1~3 years The size of liporna cells was found to be slightly larger in only SL specimens than in controls(63.65+432pm and 56.32+6.50um respectively; p<0.0l).

Intussusception*

We reviewed 28 patients admitted due to urologic complications after obstetric & gynecologic operation and management since May 1972. The age distribution, developed urologic complications and it`s treatment are discussed. The prevalent age groups of urologic complications were 30`s and 40`s. The most common urologic complication was urinary fistula : 8 ureterovaginal, 8 vesicovaginal and 4 other fistulas. The results after 1st operation of urinary fistula showed an over-all success rate of 72.6%.

Granuloma gluteale infantum is a granulomatous lesion in the diaper area of infants. Athough usually seen in infants, it has been described in incontinent elderly patients. The causes of granuloma gluteale infantum are not clear, although child care equipment(ditergents1, diapers2, and starch3), corticosteroids2,4,5,6, candidial infection3 urine and feces7 have been postulated for its etiology. We report a case of granuloma gluteale infantum in a 5-month-old boy. He was healthy except for an asymtomatic 1.5×1.5cm sized, round, Rat-topped brownish and firm nodule on the perianal area, and two, 1×3cm and 1×6cm sized brownish plaques on the intertriginous area of the neck. The lesions were resistant to the treatment of systemic antibiotics. These nodules completely resolved in the three months after we tried to keep the diaper area clean without using any corticosteroids or other ointments and after changing from plastic diapers to cotton o A histological examination of the nodule on the perianal area showed mild hyperkeratosis, spongiosis, and focal infiltration of neutrophils in the epidermis. Among the many causes producing lower back pain herniated nucleus pulposus and bulging disk have occupiedlarge percentaages and uniformly showed defect on lateral aspect of contrast filled thecal sac. But it isessential to differentiate each conditions from the other because of their different treatment methods.differentiation at metrizamide myelography between a diffusely bullging disk(unlikely to cause nerve rootcompression) and a herniated disk is based on the curature, extent, and multipllicity of the extradural deformityof the anterolateral margin of the contrast filled sac and o hte presence of fusiform widening of the most distalpart of the affected nerve root. The deformities caused by a bullging disk are round, usually symmetrical(aothoughoccasionally more prominent on one side), do not extend above or below the disk space, and can show multiple levelinvolvement; the nerve root is uniform in caliber and normal in size(although some of severe bulging show fusiformwidening of the most distal part of the affected nerve root). The deformities caused by a herniated disk isangular and extends cephalad and/or caudal to the level of the disk space; the affected nerve root is usuallywidened in its most distal visible part. A consecutive series of 50 patients with low back pain and no pasthistory of back surgery who did metrizamide myelography underwent spine CT and /or laminectomy. Using the criterialisted above for differentiation of bulging from herniated disk on metrizamide myelography, the myelographicdiagnosis was correct in 32(22 patients) of 34(24 patients) (95%) surgically and/or computed tomographicallyconfirmed bulging disks and in all 26 (100%) surgically and/or computed tomographically confirmed herniated disks.

We report a case of mycosis fungoides in a 30 year-old man who had been treated for multiple open fractures after a traffic accident. Histopathologic findings showed epidermotrophism wiih hyperchromatic haloed lymphocytes and atypical lymphocytic infiltration in the upper dermis. The infiltrated cells were positively stained with CD45Ro, but not with CD20. He had no evidence of extracutaneous involvement by laboratory studies, Skin lesions improved markedly after treatment with systemic PUVA therapy combined with topical corticosteroids for 3 months.

We report herein s case of subcutaneous aspergillosis in a 41-year-old woman with lumbar spondylitis(L4~L5). The patient had received high doses of corticosteroids and broad-spectrum antibiotics and two times of operation due to lumbar problems. The diagnosis of Aspergillus fumigatus infection was established by the mycologic study of cultures from wound exudate and histopathologic examination of the second surgical specimens of subcutaneous soft tissue, which was assumed to be iatrogenically susceptible infection. Intravenous amphotericin B therapy was discontinued because of the immediste adverse effects. After twelve-week therapy of oral ketoconazole(400mg/day), open wound healed and spondylitis with neurologic signs eventually resolved. No evidence of recurrent infection was noted during six months of follow-up.

BACKGROUND: Alopecia areata is replete with data from studies using variuos therapeutic approaches, none of which is clearly superior to another. OBJECTIVE: This study was designed to evaluate the therapeutic effect of cryotherapy in alopecia areata. METHODS: Thirty three patents with AA(M:13, F:20) well conduted in this study, precluding 3 patients with the moderate and extensive lesions. Cryotherapy using a cotton swab dipped with liquid nitrogen was applied to the involved areas, repeated with 2 times of freeze thaw cycle of two seconds. In 13 cases of AA multiplex, the lesions were divided into two groups, each of which was undertaken cryotherapy and intralesional injection of triamcinolone(3mg/ml) for evaluation of their respeetive effects. The treatment was done weekly method of 1 to 11 times, Th therapeutic response was determined as the regrowth of terminal hairs on the treated area(s) at 12 Weeks. RESULTS: Of the 33 patients with AA, the overall responders were 22(66.7%). There seemed good response rates of 70.0 % in females, 71.4 % in AA multiplex and 72.2 % in third decade as well as 72.4 % in those with lower than 1 year of duration. The remarkable resionse had 3-4 times in treatment numbers. There were no untoward side effects in patients except mild erythema. In the 13 patients treated with both modalities, cryotherapy had a slightly higher esaonse than triamcinolone therapy (69.2% vs 61.5%, p>0.05 by x-test). CONCLUSION: The effect of cryotherapy therapy in AA patients is not less than the intralesional therapy of triamcinolone. In addition, cryotherapy may be recommendec as the new, safe therapeutic modality.
