2.The Comparison of Therapeutic Effect between Imipramine and Desmopressin on Enuretic Patients.

PURPOSE: Nocturnal enuresis is one of the most common disorders of childhood, occurring in 15% of 5 year-old children. Although usually self-limiting, justification for early treatment has been founded in psyc hological impact on the child. Many investigators have reported upon the effectiveness of prospectively studied to compare the therapeutic effect between imipramine (Tofranil) and desmopressin (Minirin/1-Desamino-8-D-Arginine Vasopressin: DDAVP) on the monosymptomatic enuretic patients. MATERIALS AND METHODS: 83 enuretic patients (primary enuresis 64 cases, secondary enuresis 19 cases) were randomized th one of two groups: imipramine group(44 cases) or desmopressin group (39 cases). They were free of other abnormalities in the screening tests. In addition to drug therapy, all of cases were performed motivational counselling, reduction of fluid in take prior to bedtime and voiding diary. The efficacy of drug was measured in reduction of the number of wet nights per week. During the treatment period, 83 cases were classified as excellent (0 to 1 wet night per week), good (over 59% reduction of wet night)and failed responder(less than 50% reduction of wet night). RESULTS: Average age of imipramine group and desmopressin group was 9.3 years (range 5-17) and 9.6 years (range 5-17), respectively. The number of wet nights per week decreased respectively from a mean of 6.1 to 3.4 in imipramine group are from a mean of 6.4 to 2.3 in desmopressin group. Average therapeutic duration and overall response rate in the imipeamine group and desmopressin group was 9.1 weeks, 6.7 weeks (p<0.05) and 90.9%, 95.9%, respectively (p<0.05). The overall relapse rate of imipramine group and desmopressin group was 40.0% and 40.8% during the follow-up period of 3 months after cessation of medication in excellent responders. There was no difference in the therapeutic effects between primary and secondary enuresis on each drug therapy. There was also on no difference in the therapeutic response according to constipation. No serious side effects were observed in both groups. CONCLUSIONS: These data suggest that the overall effects of imipramine and desmopressin are excellent. But desmopressin has more effective therapeutic response and more shorter therapeutic duration as compared with imipramine.

4.Clinical Comparison of Vesicoureteral Reflux in Infants and Children more than 1 Year Old.

PURPOSE: We performed a retrospective study to determine the comparative characteristics of vesicoureteral reflux (VUR) between the infants less than 1 year and children more than 1 year old and whether early treatment in infantile VUR may be of benefit to preserve the renal function and to prevent the new renal scar. MATERIALS AND METHODS: All of 62 cases (infant group 19 and children group 43) with primary VUR were managed as the ureteral reimplantation from January 1991 to December 1996. RESULTS: The average age of patients was 7.1 months in infant group and 6.0 years in children group. The sex ratio of male to female showed no difference between infant and children group (1.7:1 Vs 1.5:1). The average follow up periods were 20.5 and 23.7 months in infant and children groups, respectively. The high grade reflux and renal scarring in infant group were significantly more frequent than those in children group (P<0.05), but the prevalence of urinary tract infection in children group was significantly higher than that in infant group (P<0.05). There was no significant difference in the rates of bilaterality and in the sex ratio between infant and children group. The success rate of operation was 100 percent in both groups. CONCLUSION: We suggest that the early surgical treatment in infants with bilateral and higher grade VUR may be of benefit to the preservation of renal function and the decrease of renal parenchymal change.

5.A Case of Pseudomonas Gangrene.

Forytfour-year-old female inpatient, who was gravely ill, developed vesicobullae on the greenish patches surrounded by narrow red to brown halo in the left lower extremity. Two years ago, she had treated by surgical operation and radiation therapy for cervix cancer. Blood and bullous fluid cultures showed Pseudomonas aeruginosa. She developed gangrenous and mummificated skin lesions after septic shock therapy but died due to disseminated intravascular coagulation.

In recent years, marked increase in incidence of congenital syphilis has occured throughout the world due tochanges in social norms and development of penicillin-resistant strains. Early diagnosis plays an important rolein congenital syphilis as the clinical manifestations may simulalte many other conditions in the paediatric agegroup. The authors analized 52 cases of congenital syphilis admitted tot he department of paediatrics, ChosumUniversity Hospital, clinically and radiologically. Among them, 18 cases were born in this hospital and 34 caseswere admitted from OPD, during the period of 8 years from Jan. 1975 to Dec. 1982. The reults obtained were asfollows; 1. In 28 of 34 cases (82%), the first clinical manifestations were below the age of 3 months. 2. Amongthe 52 cases, a male predominance was observed with a male to female ratio of 2:1. 3. The serologic test (VDRL) ofthe 52 studied cases showed reactive in 49 cases (94%), and that of syphilitic mothers except 6 cases, rective inall studied cases. 4. The major manifstations of the 52 cases were bone tenderness (12%) and swelling of thejoints(7%) inskeletal system, hepatosplenomegaly (79%) and skin lesions (73%) in extraskeletal one. 5. Theradiological skeletal changes were detected in 45 of 52 cases(87%), and the commonest findings were detected in 45of 52 cases (87%), and the commonest findings were metaphsitis(83%) and periostitis(81%). The most characteristictype of metaphysitis were transverse trophic line(74%) and zone of rarefaction (65%). 6. The comonest bones to beaffected were growing metaphyses of the long bones, particulary about the wrist and the knee. The order offreqeuncy were radius (80%), ulna(80%), tibia(77%), femur(69%) and hymerus(40%).

8.Metabolism of C(14)-glucose by Moniezia expansa and Diphyllobothrium mansoni.

The adult worms of cestodes, Moniezia expansa and Diphyllobothrium mansoni employed in this experiment. The worms were divided into three portions, i.e. immature , mature and gravid proglottids, and each proglottids were incubated in a certain incubation period, and the glucose uptake rate, total CO2 production rate, tissue concentration and their radioactivities were employed as previous reports(Rim et al., 1965). The glucose uptake rate by M. expansa was a mean value of 6.46+/-1.23 micromole per hour per gram of wet wt. and the rate by D. mansoni was a mean value of 18.8+/-0.8 micro-mole per hour per gram of wet wt. The higher rates were observed in the mature proglottid of M. expansa and in the immature proglottid of D. mansoni . The total CO(2) production rates by the worms averaged 14.0+/-2.37 micro-mole per hour per gram in M. expansa and 17.51+/-1.54 micro-mole per hour per gram of wet wt. The relative specific activities of respiratory CO(2)(R.S.A CO(2)) averaged 22.2+/-5.15 percent in M. expansa and 54.2+/-2.2 per cent in D. mansoni. In the both worms, the higher values were obtained in the mature proglottids. Therefore, the average value of 8.84+/-2.66 per cent of glucose utilized by M. expansa and 8.23+/-0.50 percent of glucose utilized by D. mansoni from the medium glucose was oxidized into respiratory CO(2). The tissue concentrations of glycogen were a mean of 2.21+/-0.46 percent per gram of wet wt. in M. expansa and 7.56+/-1.24 percent per gram of wet wt. in D. mansoni. The higher concentration of glycogen was observed in the gravid proglottids of M. expansa, however the gravid proglottids of D. mansoni showed lower concentration of glycogen than the other proglottids. The turnover rate of glycogen pool yielded with a mean of 0.04+/-0.01 miligram per hour per gram of wet wt. of M. expansa, whereas a mean of 1.66+/- 0.46 miligram per hour per gram wet wt. of D. mansoni. Therefore, a mean value of 2.58+/-0.93 per cent(R.G.D gly) of glucose utilized by M. expansa and 53.6+/-1.4 percent by D. mansoni was incorproated into the glycogen . These data account for that at least 11.42 per cent of the utilized glucose by M. expansa and 61.83 per cent of the utilized glucose by D. mansoni participated in furnishing the oxidation into respiratory CO2 and the synthetic process into glycogen.

9.A Case of Linear IgA Bullous Dermatosis.

We describe an 18-year-old male with generalized pruritic, tense annular or polycyalic blistera on normal-appearing skin or erythematous bases. Histopathological fmdings of an iatact early blistering showed subepidermal blistering and papillary neutrophilic gnicroabscesses. A direct immunofluctescent study of a section from the skin adjacent to a lesioti revealed linear deposits of IgA at the dermoepidermal junction. There were no deposits of IgG, lgM, or C3. In 1.0M NaC1 eplit skin, IgA was found to be bound linearly on the epidermal roof. The patient respmded weIl to eteroid and dapsone therapy.
