1.Combination Therapy with Griseofulvin and Immunotherap (DNCB, DPCP)on Plane Warts.

BACKGROUND: Although several kinds of treatment methods(destructive therapy, immunotherapy, etc) for plane warts have been attempted but there have been no entirely satisfactory treatments, because the plane wants are seen to recur frequently. OBJECTIVE: This study was aimed to evaluate the combination therapy of griseofulvin and immunotherapy(dinitrochlorobenzene=DNCB, diphenylcycloprope none=DPCP) on plane warts. METHOD: Sixteen patients(age range, 9 to 41; mean age, 21.8 years) with VPJ were treated with the combination therapy with griseofulvin(500mg/day) and single contact immunotherapy(DNCB or DPCP).

2.A Case of Linear IgA Bullous Dermatosis.

We describe an 18-year-old male with generalized pruritic, tense annular or polycyalic blistera on normal-appearing skin or erythematous bases. Histopathological fmdings of an iatact early blistering showed subepidermal blistering and papillary neutrophilic gnicroabscesses. A direct immunofluctescent study of a section from the skin adjacent to a lesioti revealed linear deposits of IgA at the dermoepidermal junction. There were no deposits of IgG, lgM, or C3. In 1.0M NaC1 eplit skin, IgA was found to be bound linearly on the epidermal roof. The patient respmded weIl to eteroid and dapsone therapy.

3.The Surgical Treatment of Acute Rupture of the Lateral Ligaments of the Ankle

Inappropriate treatment of the injury of the lateral collateral ligaments of the ankle produce residual instability, which causes serious disability. There is considerable divergence of opinion as to whether fresh rupture of the lateral ligaments of the ankle are best treated by conservative or opreative method. We have conducted a trial to analyze thirty patients, who were treated by immediate open surgical rupair of rupture of the lateral ligaments of the ankle between April 1991 and February 1992. The results were as follows: 1. All patients were active, young men, Injuries to left ankle were three times more than right side. The most common cause of injury is occurred during sports, especially association football. 2. The decision to operate was based on 1)Talar tilt angle of the injured ankle measures 8-10 degrees more than that of the stress uninjured ankle. 2)A positive arthrogram showing contrast leakage into peroneal tendon she-aths & anterolateral aspect of the lateral malleolus. 3. Diagnostic accuracy rate of arthrography is 94.5%, stress radiography 61.1 %. Arthrography is more reliable than stress radiography in the diagnosis of acute injuries to the lateral ligament of the ankle. 4. At operation, isolated rupture of anterior talofibular ligament was in 5 cases, isolated rupture of middle calcaneofibular ligament in 3 cases, concomittant ruptures of anterior talofibular and middle calcaneofibular ligements in 22 cases. 5. Location of tears of anterior talofibular ligament was mostly mid-portion, middle calcaneofibular ligament in mid-portion or distal portion. 6. On radiologic results, average angle of talar tilt was 14.4° preoperatively, 5.2 preoperatively. There was correction of talar tilt angle of average 9.2°. 7. Clinical results of treatment were excellent in 9 patients, good in 6 patients, fair in 4 patients, poor in 2 patients by authors rating system. The satisfactory result was achieved in 15 patients(71.4%). 8. Postoperative complication was reduced mobility in 4 cases, functional instability 1 case, neuroma in operation scar in 2 cases, minor sensory loss in 4 cases. 9. Primary surgical repair gave good results in the aspect of radiologic & fun ctional instability, but clinical results were not satisfactory, due to high incidence of postoperative complication.

5.Chronic Recurrent Folliculitis with Atypical Hyperpigmented Scarring in an AIDS Patient.

Bacterial, fungal, and viral infections of the skin with extended skin involvement can occur during the early phase of human immunodeficiency virus infection. A significant reduction in circulating CD4+ lymphocytes in the late stage of the disease may cause tumors of the skin such as Kaposi's sarcoma. A 40-year male patient, a former sailor who had multiple sexual contact with native African women, presented with multiple tender follicular pustules and fibrotic brown patches on both his legs. these had been present for 6 months. The skin lesions were healed leaving brown pigmentation. Laboratory examinations revealed the presence of leukopenia, thromocytopenia and a reversed T4/T8 ratio. The ELISA and Western blot analysis to human immunodeficiency virus were positive. A skin biopsy from a brown patch showed early stages of scar tissue and perivascular hemosiderin deposition. We herein report a case of acquired immunodeficiency syndrome patient with atypical dark brown scarring atrophic patches on the lower legs following purulent bacterial folliculitis. This may have been an early manifestation of Kaposi's sarcoma from a preceding skin lesion.

8.Failures of the Nail in the Treatment of the Comminted Femoral Shaft Fractures with Interlocking Nail: Analysis of 6 cases

The intramedullary nailing is one of the most available method in the treatment of femoral shaft fracture. Since the development of Küntscher nail in 1940, new designs such as the Grosse-Kempf interlocking femoral nail, Ressell-Taylor Femoral nial, AO/ASIF universal nail, ACE titanium nail became widely used by more applicable level of femoral shaft fracture. But complication of intramedullary nailing was rarely seen. The authors analyzed the clinical and mechanical factors predisposing to failures of intramedullary interlocking nails in 6 cases whom treated the comminuted fracture of the femoral shaft, at the department of orthopaedic surgery, Sun General Hospital from 1991 to 1995 and the following results are obtained. 1. Initial fracture patterns were comminuted in all 6 cases. 2. The implant nails were AO nail in 4 cases, Grosse-Kempf nail in 1 case, Russell-Taylor nail in 1 case(nail diameter was 10mm to 12mm). 3. The failure sites of the interlocking nail were the initial bone fracture site in 2 cases, distal to the proximal screw hole in 1 case, proximal to the distal screw hole in 2 cases, fracture site and distal to the proximal screw hole in 1 case. 4. The average time to failure of implants were 11.5 months. 5. The average times to partical weight bearing were 6.3 weeks. In the analy of above results, by using diameter of nail more than 13mm and delaying weight bearing on the injured limb until there is clear radiographic evidence of early union of the femur fracture, we may reduce the metal failures of intramedullary interlocking nail in comminuted femoral shaft fractures.

9.Histomorphologic Changes of the Contralateral Testis in Unilateral Inguinal Cryptorchidism.

PURPOSE: The pathogenesis of infertility in unilateral cryptorchidism remains unclear. We studied prospectively to evaluate the cause concerning potential infertility in unilateral inguinal cryptorchidism.Materials and Methods: Between Feb 1998 and July 2000, 30 specimens were taken by ipsilateral undescended and contralateral descened testicular biopsies in 15 unilateral inguinal cryptorchid boys (age range: 1-11 years, mean: 4.7 years). Control testicular biopsies were performed in 5 hydrocele boys (age range: 1-9 years, mean: 5.1 years). We performed histomorphologic analysis including spermatogonia per tubule (S/T) value, Sertoli cell index (SCI), tubular degeneration phase V-VII (TDP V-VII), mean tubular diameter (MTD), and changes of peritubular interstitial tissue (thickened tubular basement membrane and peritubular fibrosis). RESULTS: Testis volume, S/T value, and MTD were significantly different between ipsilateral cryptorchid and contralateral testes. However, there was no significant difference between ipsilateral cryptorchid and contralateral testis in SCI, TDP V-VII, and changes of peritubular interstitial tissue. We found significant difference between contralateral and control testis in testis volume, S/T value, MTD, TDP V-VII, and changes of peritubular interstitial tissue except SCI.Conclusions: Decreased testis volume, S/T value, MTD and increased TDP V-VII of contralateral testis are associated with germinal hypoplasia. These findings may explain the pathogenesis of infertility in unilateral inguinal cryptorchidism.

10.A Case of Contact Dermatitis due to Garlic.

Garlic is a well known edible plant and has been used as a herbal medicine. It is a member of Liliaceae which contains allylpropyldisulfide, diallyldisulfide and allicin which can induce allergic contact dermatitis or primary irritant dermatitis applied topically. A 53-year-old female patient was presented with vesicles, crust and erythema on her left buttock. She applied crushed raw garlic on the lesion site for the treatment of residual skin lesion following herpes zoster.
