1.The Present Status of Venereal Disease Morbidity for prostitutes of Foreign Soldiers.

Two hundreds and ninty-four prostitutes of foreign soldiers of K2 area in Taegu were investigated for their general circumtances, clinical and bacteriological studys from Mar. 1978 to May 1979. The results were as foIIows: The age group of 20-24 year-old constituted highest proportion at the level of about 53.4% Averge age of total casea was about 26.2. The propartion of those who were more than 35 occupied about 7.2%. The positive rate of Cram negative displococci (GND) on dirert smers showed highest in December and, January and lowest in-April. Wcekly distribution shwed a higher in second and fourth week. Nongonococcal infect.ions occupied about 45.4%. and total raes. The causative oanism. were niostly Gram negative and Gram positive bacilli. The mothly spells and perons with gonorrhea priod prevaIence rate showed a higher rate in Becember, January and July. The prevalence ratio of gonorrhea and syphilis was 13: 1. The examined cases occupied about 34.3, during first 2 month is after registration. The seasoal vari tion of registrators and cancellators were both most in winter. The positive rate of GND was ahout 53.1% in new comers less than two month-examinations after regiatration. Cervical erosion or chronic cervicitis .;howed about 44.8% among the examined 96% cases. Gonococci were isoIated about 25.6% among the ones with cervical lesions on Thayer-Martin media. On the other hand, the ones without lesions were about 11.3% comparatively. Syphilis period prevalence rate was about 8. 5 A. The titers of VDRL slide test were low (1; 8 or less). Two-thirds of positive cases of GND were treated by penicillin, although tetracycline and spectinomycin were also used in a few cases. The penicillin hypersensitivity sho- wed positive results in 9.7% of all treated cases.

2.A Case of Granuloma Faciale.

A case of 38 year old woman had noted 3 lesions with asymptomatic, reddish brown colored, coin sized, slightly elevated, confluent patches on the right postauri-cular for about 20 years. The infiltrate in the patches felt moderately firm to the touch. the lesions were suspected to be chronic dermatitis group, erythema multiforme or granuloma annulare clinically, but the histopathologic features showed the typical findings of granuloma faciale. Literature was briefly reviewed for the discussion and comparision of clinical features and histopathological findings of author's case with the 2 cases previously reported in Korea.

3.High Veloctiy Missile Wounds In Extremities

It is socially fortunate that there is little chance in the civil hospital to experience the victims of high velocity missiles. However it is reasonable thought to educate doctors about the gunshot and explosive injuries who might be mobilized in emergency condition. Authors have experiences to treat the high velocity missile wounds. In order to provide valid data to be an educational material, we clinically analyzed 41cases of those injuries who were treated at the Capital Armed Forces General Hospital from 19xx to 19xx. The results obtained were as follows: 1. In 15 cases there were only soft tissue injuries, The remaining 26 cases had the bone injuries and six of them had two injury sites. 2. In 28 cases the lower extremities were injuried, and they out-numbered the injury of upper extremities. The most frequent site of injuries was the thigh (31.7%). 3. Most common associated injuries were the periphenal nerve injuries, which numbered 10 cases. 4. The early operative treatments were given in 5 out of 32 cases having bone injuries. And the secondary operations, including bone graft and intemal fixation, had to be done in 10 out of the remaining 27 cases due to delayed union or nonunion. 5. There was no infection in cases having only the soft tissue injuries. But the localized osteomyelitis occurred in 4 cases among the cases having bony injuries. 6. Factors affecting the result of high velocity missile wounds were presence of bony involvement, site and extent of injuries, associated thoracoabdominal injuries and presence of peripheral nerve injuries and infection. 7. The evacuation time, chance of early adequate wound management, site of injury and extent of injury were the important factors in deciding the method of treatment. We suggest that the more selective and aggressive measures should be taken in the management of bony injuries.

4.A Study on the Pre-hospital Emergency Care in workplace through the Analysis of Fatal Work-place Injuries.

OBJECTIVES: This study was designed to evaluate the problems of pre-hospital Emergency medical care system (EMS) in workplace. We analysed 25 fatal work-place injuries during the recent 4 years and the work-place EMS of 8 enterprises located in Masan-city, Changwon-city and Kuje-island. METHODS: The safety managers and the members of Dept. of safety in the labor unions were interviewed about the work-place EMS. And we investigated on the injury reports, the work-place medical-room records and the medical records of emergency center for fatal 25 cases. RESULTS: The enterprises had the at-risk machines and processes, volatile materials and high-altitude working processes. There were duty doctors in only 3 enterprises but a few duty nurses or health-care providers in the others. The time spent for the education to the workers on safety was 24hrs/yr in 3 enterprises in 1998 but less than Bhrs in the others. There were medical service center in all enterprises but the ambulances in three. The time for activation of the ambulance was ranging from 5 minutes to 10 minutes in 6 enterprises, and from 10 minutes to 30 minutes in two. The patient transportation to the emergency center was possible within 30 minutes in all enterprises but there were no equipments for airway maintenance and shock management in all enterprises. The 15 (60%) fatal injuries were occurred at one enterprise. The 64% of casualties had the duration of job-employment more than 10yrs and the 68% were suffered the typical type of work-place injury as descending injuries, collisions and falls. Most of all primary calls for rescue were concentrated on the fire-service agencies. But in 85% of fatal injtories, the tome for the activation of ambulance was more than 10 minutes and no emergency care was taken in the field in 48 percent of casualtles. The transportation time to the emergency center was more than 30 minutes in 50 percent. It toolk from injury to death was less than one hour in the 40 percent of all cases, and from one hour to four in the 50 percent. The causes of death in the 68 percent were the head-and-neck injuries or thoracic injuries. CONCLUSIONS: We found that there was Insufficiency of the education associated with work-place injury for workers, manpower and facilities, equipments related to the work-place EMS, the problems of the delay in transportation system.

5.A Case of Idiopathic Calcinosis of the Scrotum.

Idiopathic calcinosis of the scrotum consists of multiple asymptomatic nodules of the scrotum without any underlying disease. We presented a case of idiopathic calcinosis of the scrotum and compared it with other reported cases in Korea, A 47-year-old farmer had slowly growing, asymptomatic, hard scrotal nodules which were 1cm in diameter and 6 in number for about twenty years. KUB X-ray film showed two radiopaque densities on the scrotal area. Histopathological finding revealed amorphous, basaphilic masses with acicular clefts which were confirmed as calcification in Von Kossa stain and surrounding foreign body granulomatous inflammatory reaction.

7.A Case of Sea Urchin Granuloma.

Sea urchin granuloma is a rare skin disorder, considered to be allergic or foreign body reaction caused by stick of spines of sea urchin. However, detection of the spine in the granuloma is quite difficult. A 27 year-old marine was seen with several asymptomatic hard nodules on the dorsum of hands, fingers, forearrns and knees at the injured site of the spines after a latent period of 6 rnonths. Histolagical examination revealed dermal granulomatous inflamation with hyperkeratotic and acanthotic epidermis. On examination with polarized microscopy, sea urchin spines in the granuloma were seen as bright white fragments in the dark field. Those nodules were regressed with intralesional injection of triamcinoIone.

8.A Case of Dermatomyositis Associated with Acute Lymphocytic Leukemia.

Dermstomyositis is a disease mainly of the skin, muscle and blood vessels in which erythematous and edematous changes in the skin are associated with muscle weakneas and inflammation. In adult the disease is commonly associated with underlying malignancy or reticulosis. A 23 year-old male had generalized weakness, intermittent fever, myalgia and purplish red patches on the face. Diagnosis of dermatomyositis was established by clinical manifestations, electromyogram, histologic findings of skin and muscle. He was treated with systemic steroid, with some improvement. Seven months after the onset of dermatomyositis, ecchymoses was noted on the eyelids and forearms. Bone marrow aspiration showed many immature lymphoblasts, characteristic of acute lymphocytic leukemia, He was treated with systemic steroid and vincristine, but no favorable effect was noted. He died of sepsis due to retal abscess on the 34th day in the hospital

10.Electrophysiological Properties of Reinnervated Motor Units by Transplant of Embryonic Spinal Cord in Rat

OBJECTIVE: Recently, we have demonstrated that embryonic ventral spinal cord motor neurons(ESMNs), transplanted into the distal stump of the axotomized tibial nerve, can grow into the denervated gastrocnemius muscle and form neuromuscular junctions. Our interest in study was to see whether these newly formed neuronal connections are physiologically active, and to electrophysiologically characterize the reinnervated motor units. MATERIAL AND METHODS: Three to eight weeks after transplantation rats were prepared for electrophysiological recording. Motor unit (MU) action potential and gross EMG were monitored. In eleven out of sixteen transplanted animals single unit and gross EMG were recorded after electrical stimulation of the transplant site. Total of 63 motor units were analyzed for their stimulus threshold (ST), latency and stimulus intensity, which is required to produce maximum firing. RESULTS: ST intensities activating MUs were significantly higher in transplanted animals than in controls. Maximum firing of MUs occurred at less than 1.2×T in the control but greater variation was observed in the transplanted animals ranging from 1.1×T to 1.6×T. No significant differences were found in the latencies of MU's firing following stimulation. MU firing was reversibly blocked by infusion of succinylcholine. CONCLUSION: we characterized the temporal, morphological and electrophysiological parameters of denervated skeletal muscle reinnervated by dissociated grafts of embryonic ventral spinal cord cells. MUs reinnervated by the embryonic motoneuronal transplant were physiologically active, with some properties similar to the MUs of the normal.
