1.Two Cases of Bullous Dermatitis Herpetiformis.

Dermatitis herpetiformis occurs rarely in childhood and certain features of the disease in children differ from its manifestation in adults. The eruptions are commonly bullous, show little predilection for the typical regional pattern of the adults, may be nonpruritic, have a shorter course, and often are resistant to the usual therapy. A review of literature shows that in patients over 50 years of age or older the tendency is to manifecst predominantly, although not as strikingly for children, a vesicobullous type of dermatitis herpetiformis with large bullae, varying 1 to 5cm or more in diameter. The authors present two cases of bullous dermatitis herpetiformis confirmed by histology; the first case is a 5 year-old boy who has tense bullae, pustules, and pigmentations on the inguinal region, lower abdomen, buttocks, face, and extensor surfaces of the extremities of 1 yars duration; the second case is a 44 year old housewife who has eruptions of vesicles, bullae, and erosion on the buttccks, back, abdomen, ext;remities, oral cavity, and vulva of 15 days' duration. The authors reviewed the relevant literature and discussed the clinicaI feat ures of both cases thoroughly.

3.Intrathecal Endothelin-1 Reduced the MAC of Isoflurane in the Rat.

BACKGROUND: Recent evidences suggest that anesthetic action within the spinal cord is important in suppressing somatic responses to painful stimuli. Intrathecal endothelin-1 (ET-1) is known to have antinociceptive effect. The purpose of this experiment was to determine whether intrathecal ET-1 may influence the minimum alveolar concentration (MAC) of isoflurane in rats and access the role of the spinal cord as the sites of anesthetic action in blocking somatic responsiveness. METHODS: In Sprague-Dawley rats fitted with an indwelling intrathecal catheter, we determined the MAC of isoflurane using a tail-clamp technique as a painful stimulus, combined with end-tidal anesthetic sampling. In experiment 1, the control MAC was determined and changes of control MAC were observed after intrathecal ET-1 (4x10-2 nmol, 4x10-3 nmol) administration. In experiment 2, we observed the effects of L or N type Ca++ channel blocker such as verapamil (50 g) or W-conotoxin (0.5 g) on the MAC after measurement of the control MAC. In experiment 3, after measurement of the control MAC, ET-1 (10-2 nmol) was administered intrathecally and the MAC was determined again. Next, intrathecal verapamil (50 g) or W-conotoxin (0.5 g) was injected. After that, the MAC was determined again. RESULTS: In experiment 1, ET-1 decreased the MAC of isoflurane and its effect was sustained over 2 hours. In experiment 2, the MAC, determined following administration of verapamil or W-conotoxin, was not different from that of the control. In experiment 3, the MAC was decreased after ET-1 administration and then increased following injection of verapamil or W-conotoxin. CONCLUSIONS: These results suggested that ET-1, in relation to calcium, might play an important role in determining the MAC of isoflurane in the spinal cord.

4.3-Dimensional Gait Analysis of Korean Adults.

OBJECTIVE: The aim of this study is to present the basic reference data of age and sex specific gait parameters for Korean adults. METHOD: The basic gait parameters were extracted from 70 healthy adults, 38 men and 32women, 21 to 69 years of age using VICON 340 Motion Analyzer. RESULTS: The temporal gait parameters and kinematic parameters of Korean adults were similar to other western reference data. CONCLUSION: Significant sex differences exist in the maximal pelvic tilting, pelvic internal rotation and foot internal rotation and plantar flexion. There is a statistically significant age variability for the hip and knee flexion at stance phase.

5.Change of Blood Pressure after Aortic Declamping during Open Heart Surgery.

BACKGROUND: A decrease in mean arterial pressure (MAP) often accompanies aortic declamping during open heart surgery (OHS). But, there are no many reports to evaluate the extent of the decrease in MAP following the aortic declamping during OHS. METHOD: Twenty nine patients undergoing OHS, 17 patients for repair of cushion defect and 12 patients for valve replacement were evaluated. Anesthesia was provided by fentanyl (30 g/kg bolus followd by 0.3 ug/kg/min infusion) and intermittent inhalation of N2O-Isoflurane. An indwelling radial artery catheter was used to measure MAP. Systemic vascular resistance (SVR) was calculated using the formula. Measurements were made just before aortic declamping (control), and 1,3,5,7 and 10 minutes after declamping. RESULTS: MAP and SVR were decreased significantly until 7 minutes after aortic declamping compared to those of control value, and have a significant difference in decrease pattern of MAP following time interval after declamping between the patients for repair of cushion defect and the patient for valve replacement. The extent of decrease in MAP and/or SVR showed no significant relationship with difference the duration of bypass, patients body temperature and age. CONCLUSION: The results of this study indicated that there is a significant decrease in MAP and SVR after aortic declamping and it persists for about 7 minutes, without significant relation with bypass time, body temperaure and age.

6.Mechanism of Amelioration of Cisplatin Nephrotoxicity by Procaine Treatment in Mice.

BACKGROUND: Procaine binds to DNA and reduces cisplatin nephrotoxicity, but the mechanism is poorly understood. We explored whether procaine amelioration of cisplatin nephrotoxicity was related to down-and/or up-regulation of inflammatory response gene tumor necrosis factor-alpha (TNF-alpha), oxidative stress indicator gene heme oxygenase-1 (HO-1) or cell cycle inhibitor gene p21. METHODS: Cisplatin and procaine were intraperitoneally injected to mice at a single dosage of 16 and 80 mg/kg, respectively. Renal evaluation was performed 72 hours after cisplatin administration. The expression of transcripts and proteins was analyzed using real time RT-PCR and Western blot, respectively. RESULTS: Procaine treatment moderately attenuated necrotic changes of renal proximal tubules and increases in BUN and creatinine concentration by cisplatin administration. Kidney platinum level between the cisplatin (cis) group and the cisplatin + procaine (CisPro) group was not different. Although the level of TNF-alpha mRNA increased 4-fold higher in the Cis group than in the control, this increase was not attenuated by procaine treatment. Gene expression of p21 and HO-1 was elevated 175 and 4-times higher in the Cis group than in the control, respectively. But their expression was no further elevated, rather significantly reduced in the CisPro group compared to the Cis group. Protein abundance of p21 and HO-1 was paralleled by their respective mRNA expression. CONCLUSIONS: Procaine amelioration of cisplatin nephrotoxicity is likely to be achieved through processes other than the regulation of TNF-alpha, HO-1 or p21 gene expression.

7.Prediction of Prognosis by Acetazolamide Brain Perfusion SPECT in Patients with Arteriovenous Malformation.

After surgical operation in patients with arteriovenous malformation (AVM), normal pressure perfusion breakthrough (NPPB) is one of the major complications. Brain perfusion SPECT with acetazolamide stress was known to be useful to evaluate the vascular reserve in several neurological and neurosurgical conditions. The authors performed acetazolamide brain perfusion SPECT in patients with AVM and compared the brain perfusion in the post-operative clinical courses. The acetazolamide brain perfusion SPECT was helpful in defining the prognosis of the patients with AVM. We describe 4 patients with AVM who had acetazolamide brain perfusion SPECT to examine the prognosis.

8.In-vitro investigation of the mechanical friction properties of a computer-aided design and computer-aided manufacturing lingual bracket system under diverse tooth displacement condition

OBJECTIVE:
The purpose of this study was to compare the static (SFF) and kinetic frictional forces (KFF) of a computer-aided design and computer-aided manufacturing lingual bracket (CAD/CAM-LB) with those of conventional LB (Con-LB) and Con-LB with narrow bracket width (Con-LB-NBW) under 3 tooth displacement conditions.
METHODS:
The samples were divided into 9 groups according to combinations of 3 LB types (CAD/CAM-LB [Incognito], Con-LB [7th Generation, 7G], and Con-LB-NBW [STb]) with 3 displacement conditions (no displacement [control], maxillary right lateral incisor with 1-mm palatal displacement [MXLI-PD], and maxillary right canine with 1-mm gingival displacement [MXC-GD]; n = 6/group). While drawing a 0.016-inch copper or super-elastic nickel-titanium archwire with 0.5 mm/min for 5 minutes in a chamber maintained at 36.5℃, SFF and KFF were measured. The Kruskal-Wallis method with Bonferroni correction was performed.
RESULTS:
The Incognito group demonstrated the highest SFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD, Stb-MXC-GD] < [7G-MXC-GD, 7G-MXLI-PD, 7G-control] < [Incognito-MXLI-PD, Incognito-control, Incognito-MXC-GD]; p < 0.001). However, there were no significant differences in SFF among the 3 displacement conditions within each bracket group. Within each displacement condition, the Incognito group demonstrated the highest KFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD] < Stb-MXC-GD < 7G-MXLI-PD < [7G-control, 7G-MXC-GD] < [7G-MXC-GD, Incognito-MXLI-PD, Incognito-control] < [Incognito-control, Incognito-MXC-GD]; p < 0.001). MXC-GD exhibited higher KFFs than MXLI-PD in the same bracket group.
CONCLUSIONS
The slot design and ligation method of the CAD/CAM-LB system should be modified to reduce SFF and KFF during the leveling/alignment stage.
