1.A Clinical Study On The Occurrence Of Food Impaction.

The purpose of this study was to investigate the causes of food impaction and to explore solutions as well. For this study, 39 patients with food impaction wee selected. 77 contact areas in these patients were investigated mobility, tightness of contact area, gingival index, plaque index, attachment loss, alveolar bone loss, proximal caries, marginal ridge distance and occlusal relationships. The results were as follows ; 1. Teeth without distal support were found to be the most frequent site of food impaction (41.6%). Food impaction was found to be more frequent in the upper teeth (66.2%) than the lower teeth (33.8%). 2. Food impaction was found in tight contact cases (71.4%). Alveolar bone loss was not found in the early stage of food impaction (83.1%) 3. The distance between the marginal ridges of food impaction sites (mean=0.48mm) was shorter than that of the control group. (mean=0.77mm) (p<0.001) 4. In 18.2% of t he cases, proximal carries were found at the food impaction site. 5. Food impaction affected patient's occlusion with the following frequencies ; cusp to marginal ridge relationship (72.7%), cusp to fossa relationship (3.9%) and stepped relationship (23.4%).

2.Perception assessment of esthetics of upper anterior teeth.

STATEMENT OF PROBLEM: The beauty has a little different meaning according to a time, culture, and nation. PURPOSE: This study was undertaken to determine the Korean perception of the altered upper anterior dental esthetics including the lack of symmetry, the midline deviation, the gingival exposure, the inclination of incisal plane, the type of incisal plane, and the type of gingival line. MATERIAL AND METHOD: 670 subjects were participated in this survey. A questionnaire accompanied by 12 sets of computer-manipulated images using 3D MAX 4.2 software was used to record the ranking of the geometric preference related to the anterior esthetic discrepancies in three or four degrees of alteration. The statistical significance of the differences between the groups was determined by a one-way ANOVA and a t-test. RESULTS: The results obtained were as follows: 1) The Korean perception of the anterior dental esthetics according to the subjects' occupation, sex, and age was most affected by occupation. 2) The masked image emphasizing the dentition and lips appeared stranger than the non-masked image at the same alteration. 3) The lack of symmetry, which was expressed as a unilateral discoloration of the tooth, showed incongruity in any teeth of the anterior dentition. The incongruity was more severe as the degree occurred closer to the midline. 4) The deviation of midline was showed more severe strangeness as the degree of deviation increased. However, more than half of the subjects did not perceive a deviation of 5mm. 5) During smiling, the exposure of the upper gingiva showed more severe incongruity as the degree of gingival exposure increased. 77% of the subjects perceived strangeness at the gingival exposure of 4.5mm. 6) The inclination of the incisal plane appeared stranger as the degree of inclination increased. 62% of subjects perceived strangeness at the 7.5.inclination of the incisal plane. 7) The type of incisal plane showed increasing trangeness in the order of convex/downward, straight/horizontal, and concave/upward. 80% of subjects perceived strangeness at concave/upward. 8) The type of gingival line was showed increasing incongruity in the order of the same, a little above, and a little under the zenith of the lateral incisor to the line joining the zenith of the central incisor and the canine. However, less than half the subjects did not perceive strangeness at any alteration of the gingival line. CONCLUSION: The Korean perception of the upper anterior dental esthetics was different to the westerner's perception in the some respects.

3.Assessment of Left Ventricular Diastolic Function by Doppler Echocardiogram in Hypertensives with Disproportionate Septal Thickening.

Hypertensive left ventricular hypertrophy(LVH) was considered to be a physiologic adaptation to the increased afterload of left ventricle, but recent studies revealed that LVH was one of the most important target organ damage in essential hypertensive patients & cardiovascular morbidity was increased in patients with hypertensive LVH. Hypertensive LVH could be classified into three types : concentric LVH, disproportionate septal thickening(DST), and left ventricular dilatation. Relatively high incidence of DST in hypertension has bee reported after clinical introduction of echocardiogram. But, the mechanisms for the development of DST and its clinical significance have not been elucidated exactly. In order to assess left ventricular diastolic function in hypertensives with DST, the authors performed phonocardiogram, M-mode, and pulsed Doppler echocardiogram in 15 normotensive control(group A : 5 male, 10 female, 44.4+/-7.7 years), 15 hypertensives without LVH(group B : 5 male, 10 female, 45.5+/-8.6 years), 85 hypertensives with DST(group C : 9 male, 16 female, 47.5+/-8.6 years) and 15 hypertensives with concentric LVH(group D : 8 male, 7 female, 47.7+/-6.1 years). The obtained results were as follows : 1) Left ventricular ejection fraction was 71.6+/-6.3% in group A, 71.9+/-7.5% in group B, 731+/-7.0% in group C, and 70.3+/-10.3% in group D. 4.A STUDY ON THE MICROLEAKAGE OF THE IPS EMPRESS CERAMIC CROWN ACCORDING TO MARGIN TYPES AND RESIN CEMENT.

The purpose of this study was to compare the microleakage of the IPS Empress ceramic crown according to 1) margin types such as rounded shoulder and bevel, 2) margin locations such as enamel and dentine, 3) resin cement types such as Variolink and Bistite. For this study, 80 extracted premolar teeth were used. After 10 experimental dies were prepared for each group, the IPS Empress ceramic crowns were fabricated on the dies and cemented with one of two types of resin cement. After 1,000 cycles of thermal changes were given with the thermocycling unit, each specimen was sectioned with a low speed diamond saw in both the buccolingual and the mesiodistal direction. The microleakage was then measured with a stereo microscope. The obtained results were as follows : 1. Every experimental group showed microleakage. The microleakage of the crowns which had a rounded shoulder on enamel cemented with Variolink was the lowest of all. The resulting rank of the mean microleakage was rounded shoulder on enamel using Variolink(0.601mm), rounded shoulder on enamel using Bistite (0.794mm), bevel on enamel using Variolink (0.826mm), rounded shoulder on dentine using Variolink(1.054mm), bevel on enamel using Bistite(1.087mm), rounded shoulder on dentine using Bistite(1.176mm), bevel on dentine using Variolink(1.258mm), and bevel on dentine using Bistite(1.467mm). 2. The statistically significant differences in the microleakage were found in 4 cases: rounded shoulder on enamel using Variolink and bevel on dentine using Variolink, rounded shoulder on enamel using Variolink and bevel on dentine using Bistite, rounded shoulder on enamel using Bistite and bevel on dentine using Bistite, bevel on enamel using Variolink and bevel on dentine using Bistite. 3. The microleakage of rounded shoulder finish line was lower than that of bevel finish line. The statistically significant differences were found with respect to the type of finish lines. 4. The microleakage of the finish line on enamel was lower than that of finish line on dentine. The statistically significant differences were found with respect to the location of the finish lines. 5. The microleakage of the Variolink cement was lower than that of the Bistite cement. The statistically significant differences were found with respect to the resin cements.

6.Evaluation of hepatic masses by angio-CT.

Authors performed angio-CT(portal &/or arterial CT) in 35 patients with various hepatic masses. We obtained portal CT in 32 and arterial CT in 13 patients. The contrast material was injected into superior mesenteric artery for portal CT and into hepatic artery for arterial CT after transfemoral cathetrization of these arteries. We concluded that portal CT was superior to other imaging modalities highly sensitive in detecting hepatic masses, especially, early hepatocellular carcinomas, daughter nodules, and subclinical metastatic hepatic malignancy, With this method, detection rate of early hepatocelluar carcinomas improved 62% in number of patients and 90% in number of masses. Detection rate of subclinical hepatic metastases improve 60% each in number of patients and in number of masses. It was also useful for the detection of portal vein invasion. Arterial CT was useful in differential of masses and in defining the characteristics of entire or part of the masses because this method revealed hemodynamic patterns more definitely.

With advancement of endoscopical instruments and technique, gallstone diseases can be managed with laparoscopic cholecystectomy which has rapidly and radically changed the surgical treatment of gallstone diseases. The ideas of laparoscopic surgery was introduced by Germany gynecologist Semm, in 1967 and was first performed by French surgeon Mouret, Dubois in 1987. Although many reports of early laparoscopic cholecystectomy were excellent, many surgeons want to know the surgical results of laparoscopic cholecystectomy are enough good to perform comparing with the conventional cholecystectomy, especially in early complications and late complications. 9.A Case of Diffuse Leukoencephalopathy Caused by 5-Fluorouracil Derivatives.

A stomach cancer patient is reported, who presented with asterixis and dysarthria, after the administration of 5-fluorouracil derivatives, and progressed to a state of arnnesia and disorientation. At first impression was metastatic brain tumor however7 the result of a cerebrospinal fluid cytology for malignancy was negative, and magnetic resonance imaging study(MRI) revealed diffuse hyperintense lesion of the white matter, indicative of leukoencephalopathy. Upon discontinuing the drugs, patient recovered significantly.lly predominant muscle weakness from distal myopathy with rimmed vacuole formation While we present a young female.
