1.A Case of Cutaneous Mucormycosis in a patient with Acute Lymphocytic Leukemia.

54-year-old male patient with acute lymphocytic leukemia was consulted to our department. He had had a few painful erythematous necrotic plaques on the chest, which suggested initslly vasculitis or pyoderma. On histological examination, characterisitic histological findings of non-septated large hyphae were observed in the dermis and subcutaneoustissue, Although organism was not isolated on Sabouraud agar media containing cycloheximide, the diagnosis of cutaneous mucormycosis was made by the characteristic histological nattlre. He died after 2 weeks with amphotericin B intravenous injections so that additional fungus culture was not performed.

2.Effects of Age and Left Ventricular Hypertrophy on Left Ventricular Diastolic Function in Normotensives and Hypertensives.

Transmitral left ventricular diastolic inflow velocities determined by pulsed Doppler in 47 normotensives, 80 hypertensives and 44 age-matched hypertensives were measured to evaluate the effects of age and left ventricular hypertrophy on left ventricular diastolic fuction in normotensives and hypertensives. The results were as follows: 1) The peak E velocity in normotensives(77.82+/-13.53cm/sec) was significantly different from those of hyeprtensives(58.13+/-11.80cm, p<0.01) and age-matched hypertensives(59.31+/-10.94cm/sec, p<0.01). The peak E/A in normotensives(1.31+/-0.45) was significantly different from those of hypertensives(0.83+/-0.21,p<0.01) and age-matched hypertensives(0.83+/-0.20, p<0.01). 2)In normotensives, simple linear regression analysis revealed strong inverse correlations of peak E with age(r=-0.758, p<0.01), peak E/A with age(r=-0.748, p<0.01), but no significant correlation of peak E and peak E/A with LVMI(p>0.05). 3.Clinical Observation of Legg-Calve-Perthes' Disease: Preliminary Report

The authors did clinical analysis for 37 cases, ranging from 5 to 13 year old of age and following result obtained. 1. In group I partial involved type had brightful prognosis even in the neglected the cases. 2. Ischeal brace did not give any help to group II & III but in group I, it gave some help to the prognosis. 3. In untreated cases of old age group developed early arthritic changes. 4. The inital radiological bony changes in early L-C-P. cases was the demineralization of the subchondral area and subsequently subohondral vaccum phenomena followed.

4.Microvascular Loop Graft in the Treatment of the Kienbock's Disease

Two patients with Kienbocks disease were treated with microvascular loop graft to obtain the neovascularization of the avascular lunate. After each follow-up of forty-six months and eleven months, their results were complete relief of pain, definite decrease of sclerotic change in the radiologic finding and restoration of normal range of motion. This procedure is considered as a good method to help for the treatment of the Kienbocks disease.

5.A Clinical Study of the Myelography with Metrizamide

The authors have studied 60 cases of back pain with respect to diagnostic accuracy of myelography with metrizamide from January, 1983 to Decembar, 1984 at the Department of Orthopaedic Surgery, College of Medicine, Kyung Hee University. The results were as followes; 1. Of the 60 cases, 57 cases (95%) showed excellent and good findings of myelography. 2. In case of the marginal clearance of the nerve-sleeve in myelography, the excellent finding was 93.3%.3. The side effect of myelography was slight and 44% of all patients complained of headache, which disappeared within 24 hours after onset in 92% of them. 4. The accuracy rate of myelography with metrizamide was 89. 2%.

6.The Effect of Somatosensory Evoked Potential by Stretching of Spinal Dura in the Dog

Spinal instrumentation is popular nowadays. After applying the Harrington distraction rod, there might be a severe, tragic complication, paraplegia. Recently SSEP is widely used to detect the early change of the spinal cord dysfunction. The force to correct the spinal deformity is usually distraction. The distraction force will stretch the spinal dura. But the relation of neural deficit between distraction force and duration is not clearly determined yet. Also the anatomical level of neural deficit is not known. The purpose of this study is to find that the distraction force of 200gm on the spinal dura will change the SSEP by the stimulation of the posterior tibial nerve and if there is change of SSEP, what portion of nerve stimulation pathway is responsible for. Results are as followings. 1) The distraction force of 200gm makes change of SSEP wave. 2) P2 wave is most susceptible, N1 wave is less susceptible and P1 is relatively contant for one hour dural traction. 3) After release of traction, the wave is partially reversible but not completely for next one hour recovery period. 4) While the wave of SSEP is changed by the dural distraction force, the nerve conduction between the nerve root just outside of the spinal dura and calf shows constant latency. That means the anatomical level responsible for the change of SSEP wave is proximal to the spinal dura, not the peripheral nerve distal to the spinal dura.

8.Vascularized Fibula Graft for Restoration of the Large Bone Defect

We analyzed 34 cases of large bone defect caused by trauma, infection and tumor resection from July, 1979 to June, 1985. The bony defect were restored by vascularized fibular graft or vascularized fibular transposition. 22 cases were the traumatic segmental loss and 7 cases were infected ones and 5 cases were the loss following tumor resection. Vascularized fibular graft was performed in 19 cases and vascularized fibular transposition was performed in 15 cases. The 33 cases(97%) of 34 cases had obtained sound bony union and average union time had been 18.6 weeks. Even local infection in the bone, vascularized fibular graft or transposition can be performed. Grafted or transposed fibula had been hypertrophied about 1.2 times in anteroposterior and lateral film which had been measured in the center of the fibula. The most common complication was the clawing of first or second toe. If the ipsilateral fibula is intact, the vascularized fibular transposition is better and if not, vascularized fibular graft is preferred. Operation using the vascularized fibula is a excellent method for the restoration of large bone defect in long bone.

9.Congenital Absence of the Flexor Digitorum Profundus: A Case Report

The absence of palmaris longus musle is common, about 10% in general population, but absence of another muscle(S) in forearm and/or hand is very rare. We experienced a case of conenital absence of flexor digitorum profundus in both hands. It was confirmed by surgical exploration. This anomaly seems to be extremely rare case in the English literature.
