1.A Study of HBsAg in Semen and Saliva (Preliminary Report).

A group of 68 men who visited Urology Department of Severance hospital, including 11 patients with prostatitis and 57 patients with infertility were investigated for HBsAg from the specimen of serum, saliva and semen. HBsAg was detected by using a highly sensitive radioimmunoassay technique. The following results were obtained. 1. A case of 11 patients with prostatitis (9%) and 6 cases of 57 patients with infertility (10.5%) were HBsAg-positive in serum. 2. Of the 7 cases being HBsAg-positive in serum, 5 cases were HBsAg-positive in saliva (71%) and 3 cases in semen (43%) . 3. Type B hepatitis may be transmitted by parental, oral and probably venereal routes.

2.Normal PSA Values according to Age in Healthy Men without Prostatic Disease.

From January 1991 to June 1993, serum PSA according to age were tested by monoclonal radioimmunometric assay using ELSA-PSA 2 kit of CIS industry in 380 healthy men between 30 and 79 years of age who had normal urinalysis, no history of prostate disease and normal prostates on digital rectal examination and transrectal ultrasonography. The mean PSA values according to age were 0.775+/-0.462ng/ml in the 30 to 39-year group, 0.908+/-0.704ng/ml in the 40 to 49-year group, 0.842+/-0.468ng/ml in the 50 to 59-year group, l.393+/-0.942ng/ml in the 60 to 69-year group and 1.737+/-0.937ng/ml in the 70 to 79-year group. The upper limits of PSA (mean+2S.D.) according to age were 1.7ng/ml in the 30 to 39-year group, 2.3ng/ml in the 40 to 49-year group, 1.8ng/ml in the 50 to 59-year group, 3.3ng/ml in the 60 to 69-year group and 3.6ng/ml in the 70 to 79-year group. These results showed that mean PSA and upper limits of PSA increased significantly after 60 years of age(P values<0.01).

3.Isolated avuision fracture of the tibial attachment of the posterior cruciate ligament

Five patients with isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament were treated in our hospital from September, 1975 to May, 1978. Among the five patients. one was treated conservatively and the others by surgical repair using catgut suture, cancellous bone screw fixation and staple fixation. Four patients were followed from 4 to 10 months poatoperatively. Satisfactory results were obtained in four patients (one was loat in follow up).

4.Changes in Vertebral Rotation Following Segmental Pedicle Screw Instrumentation and Rod Derotation in Idiopathic Thoracic Scoliosis : Part I - CT Evaluation.

The employment of segmental pedicle screw instrumentation has greatly enhanced the correction of scoliotic deformity. However there is still much controversy on the ability of the system to correct the rotational deformity. This prospective study with 12 idiopathic throacic scoliosis patients subjected to segmental pedicle screw instrumentation was performed to determine the effect of the system on the rotational deformity of idiopathic throacic scoliosis. In addition to the standing films to measure the magnitude of scoliotic curvatures, preand postoperative CT were utilized for the measurement and comparison of angle of rotation relative to the sagittal plane(RAsag.), relative rotation with reference to the upper and lower end vertebrae(RAend), relative rotation with reference to the sacrum(RAsac.), frontal translation(TF) and sagittal translation(TS) for the apical vertebrae. Despite the high correction rate obtained in the frontal plane(average 71.1%) and normalization of sagittal contour, the rotational corrections were statistically insignificant in all parameters and showed no significant correlation to the correction of the curvatures nor horizontal plane displacement as represented by the frontal and sagittal displacement. In conclusions, the segmental pedicle screw instrumentation with rod derotation does not produce significant derotation by itself.

5.A Clinical Study on 212 Case of Pelviscopic Surgery.

Two-hundred and twelve cases of pelviscopic operation were performed at Pusan St. Benedict Hospital from November, 1992 to December, 1995. The results were as follows; The common indications were ovarian cyst(32.2%), ectopic pregnancy(16.9%), and endometriosis(14.7%). Frequent types of surgery were salpingo-oophorectomy(22.6%), salpingectomy(10.8%), and oophorectomy(10.0%). The mean age of the patients was 33.4 years. The mean parity of the patients was 1.0. The mean duration of hospitalization was 4.8 days. The most common complication was hemorrhage at the trocar sites. In our conclusions, pelviscopic surgery made minimal postoperative hemorrhage with postoperative discomfort, low incidence of complications, and short duration of hospitalization. Pelviscopic surgery was useful and recommended for the treatment of gynecologic diseases.

6.Impact of Treatment With Statins on Prostate-Specific Antigen and Prostate Volume in Patients With Benign Prostatic Hyperplasia.

PURPOSE: We investigated the impact on prostate-specific antigen (PSA) and prostate volume (PV) of statin medication for 1 year in patients with benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: We retrospectively investigated 791 patients in whom BPH was diagnosed. For analysis, the patients were divided into four groups according to their medications: group A, alpha-blocker; group B, alpha-blocker+statin; group C, alpha-blocker+dutasteride; group D, alpha-blockers+statin+dutasteride. To investigate changes in serum PSA, PV, and total cholesterol, we analyzed the data at the time of initial treatment and after 1 year of medication. RESULTS: After 1 year, group A showed a 1.3% increase in PSA and a 1.0% increase in PV. Group B showed a 4.3% decrease in PSA and a 1.8% decrease in PV. The difference in PV reduction between groups A and B was statistically significant (p<0.001). Group C showed a 49.1% reduction in PSA and a 22.9% reduction in PV. Group D showed a 51.6% reduction in PSA and a 24.5% reduction in PV. The difference in PV reduction between groups C and D was not statistically significant (p=0.762). By use of a multivariate logistic regression model, we found that the probability of PV reduction after 1 year was more than 14.8 times in statin users than in statin nonusers (95% confidence interval, 5.8% to 37.6%; p<0.001). CONCLUSIONS: Statin administration reduced PSA and PV in BPH patients. This finding may imply the improvement of lower urinary tract symptoms and prevention of cardiovascular disease and chemoprevention of prostate cancer with statin treatment.

8.Wilms' Tumor in a Horseshoe Kidney.

The incidence of horseshoe kidney is about 1 in 400 cases. The presence of Wilms' tumor with a horseshoe kidney is unusual, and the occurrence of Wilms' tumor in a horseshoe kidney is estimated at 0.4 to 0.9% of all Wilms' tumors. We report the case of a 5-year-old boy who presented with a stage IV Wilms' tumor in a horseshoe kidney. The patient was treated with preoperative chemotherapy followed by surgical resection and adjuvant chemotherapy. This case illustrates the role of preoperative chemotherapy for preserving renal function and aims to highlight the multimodality treatment of Wilms' tumor.

9.Tumor Establishment Features of Orthotopic Murine Bladder Cancer Models.

PURPOSE: Animal tumor models are important for the evaluation of novel therapeutic modalities. Since the initial report of an orthotopic bladder tumor model, several modifications have been proposed to improve the tumor take rate. Here we compared the HCl-pretreated and electrocauterization-pretreated orthotopic murine bladder tumor models. MATERIALS AND METHODS: MBT-2 murine bladder cancer cells were transurethrally implanted in the bladder of syngeneic C3H/He mice. The mice were divided into three groups according to pretreatment methods (electrocautery, HCl, and control group) and were subjected to pretreatment before instillation of MBT-2 tumor cells into the bladder. Mice were sacrificed on day 21, and bladders were harvested, weighed, and examined histopathologically. RESULTS: The tumor take rate of the control, electrocautery, and HCl groups was 0%, 54%, and 100%, respectively. The tumor take rate of the HCl group was significantly higher than that of the control group (p<0.01) and the electrocautery group (p=0.01). Pathologic reports revealed that all established bladder tumors were high-grade papillary urothelial carcinomas. CONCLUSIONS: The HCl pretreatment model was a preferable murine bladder tumor model for evaluating further therapeutic interventions.

10.Association of Arteriovenous Fistula Thrombosis with Anticardiolipin Antibody and Lipoprotein(a) Value in Patients with Maintenance Hemodialysis.

OBJECTIVES: Arteriovenous fistula (AVF) is Achilles hill of patients receiving maintenance hemodialysis, but thrombosis of AVF is a frequently encountered problem in maintenance hemodialysis patients. AVF thrombosis or occlusion is known to be associated with old age, underlying diabetes mellitus, increased fibrinogen and factor VIII, short AVF maturation time, low dialyzer blood flow, etc. Recently, several reports suggested that high titer of IgG anticardiolipin antibody (IgG-ACA) is associated with single or repeated clotting of AVF and elevated Lp(a) level is associated with vascular access occlusion in patients under maintenance hemodialysis. This study is to investigate the relationship between AVF thrombosis and the presence of elevated titiers of LP(a) and IgG-ACA. METHODS: This study included 20 patients with end stage renal disease under hemodialysis via AVF. Ten subjects have had one or more episodes of AVF obstruction (group A). Another 10 subjects without episodes of AVF obstruction (group B) were selected matching with age, sex, underlying disease, duration of hemodialysis, blood glucose level and lipid profile of subjects in group A. The IgG, IgM anticardiolipin antibody titers with indirect ELISA method and LP(a) level with turbidimetric assay were measured and analysed. RESULTS: Four subjects in group A showed positive IgG-ACA titer what of all subjects in group B were negative titer (p=0.03). Only one subject in group A and two subjects in group B showed positive IgM-ACA titer (p>0.05). The median value of Lp(a) was 32.75 (mg/dl), 43.1 (mg/dl) in group A and group B respectively and was not significantly different each other (p>0.05). CONCLUSIONS: In end stage renal patients receiving hemodialysis, positive IgG-ACA titier seems to be an independent risk factor of AVF thrombosis.
