1.Supracondylar Fractures of the Femur Treated by Interlocking Nailing

Between Jan. 1976 and May 1987, Sixteen supracondylar fractures of the femur were treated by interlocking nailing. Of the 16 cases, 7 cases were intercondylar, 12 cases were segmental fractures of the femur, 4 cases were open fractures in the type of wound. 1. The simple supracondylar fracture involving the distal 9cm of the femur can be treated by interlocking nailing which makes firm fixation and allows early ambulation. 2. If the supracondylar fracture is accompanied with intercondylar fracture, careful reduction and transcondylar fixation by means of tibial bolt or cancellous screws are essential before the insertion of the nail. 3. The shape of the bent nail must be depended on the type of the fracture. If the nail is to be inserted into the anterolateral aspect of the distal fragment, the degree of bending of the nail is 400cm, and posteromedial is 110cm in radius. 4. The authors made several sagittal holes the at dorsum of the nail. So the screw can be easily inserted to the nail perpendicularly or obliquely, when the insertion of the screw through the transverse holes is difficult. 5. Of the 15 cases treated by interlocking nailing, 5 cases were excellent, 4 cases were good, 2 cases were fair, 4 cases were failure by schatzker assessment.

2.A Study of Congenital Dislocation of the Hip

The author reviewed eighteen congenitally dislocated hips in Kyung Hee University Hospital during the three and a half year period (rom September, 1973 to March, 1977. The following observations were made. 1. The average age of the patients at the first visit was 17.9 months. 2. All 16 patients were female. 3. There were 2 bilateral dislocations and 14 unilateral dislocations. 4. Left side was dislocated in 12 hips snd right side in 6 hips. 5. Limping was the chief complaint in 12(75%) of the patients. 6. Arthrogram gave valuable information as to make correct diagnosis and treatment. 7. Twelve hips were successfully treated by closed methods and 6 hips required operative treatment. 8. Following reduction, cast immobilization was continued for an average of 5 months in the conservative series. 9. In the conservative series, the acetabular index decreased from 33.4 degree before treatment to 28.1 degree immediately after removal of cast. 10. The results were good or excellent in all except for 2 cases of avascular necrosis and 1 case of redislocation.

3.Giant Chondroblastoma: A case report

Chondroblastoma is a rare primary bone tumor, The radiologic findings are so similar as giant cell tumor, osteolytic osteogenic sarcoma, enchondroma, bone tuberculosis, etc, but the treatment and prognosis are quite different, the differential diagnosis is mandatory. We experienced unusual huge giant chondroblastoma (10X7X5cm in size) occured in distal femur.

4.Experimental Study for Peripheral Nerve Reconstruction using Skeletal Muscle and Omentum

The experimental study for peripheral nerve reconstruction was performed using degenerated skeletal muscle and omentum. At the sciatic nerves of 26 rabbits, about 10mm gaps were made under ketamine anesthesia. Degenerated skeletal muscles were made through the procedure of freezing with liquid nitrogen and thawing in distilled water. And omenta were taken through midline abdominal incisions. On 14 rabbits of experimental group, the nerves were connected with the degenerated skeletal muscles and surrounded with the omenta. On 6 rabbits of control group, the nerve gaps were connected with the degenerated skeletal muscles without surrounding with the omenta. And the other 6 rabbits of control group, the gaps were lets without further treatment. After 3, 6, 9 and 12 weeks, the results were compared using light microscope and transmission electron microscope. There were contractions and fragmentations at the skeletal muscles during the freezing and thawing. There were nerve regenerations through the degenerated skeletal muscles. The nerve tissues of the experimental group were more abundant than the control group using the degenerated skeletal muscles only.

5.Noninvasive serum biomarkers for liver steatosis in nonalcoholic fatty liver disease: Current and future developments

Nonalcoholic fatty liver disease (NAFLD) affects approximately 30% of the population worldwide and includes nonalcoholic fatty liver, nonalcoholic steatohepatitis (NASH), and cirrhosis. Since NAFLD-associated diseases begin with steatosis, the early diagnosis of steatosis helps to prevent the progression of NASH and fibrosis. In addition, more convenient and easily diagnosable serum biomarkers are becoming crucial in disease diagnosis. In this report, we summarize the known serum biomarkers for liver steatosis and provide guidance for their application in clinical practice.
6.Mutagenic Assessment of Olmesartan Cilexetil by Bacterial Mutation Assay.

Hypertension is a serious health problem due to high frequency and concomitant other diseases including cardiovascular and renal dysfunction. Olmesartan cilexetil is a new antihypertensive drug associated with angiotensin II receptor antagonist. This study was conducted to evaluate the mutagenicity of olmesartan cilexetil by bacterial reverse mutation test using Salmonella typhimurium (TA100, TA1535, TA98, and TA1537) and Escherichia coli (WP2 uvrA). At the concentrations of 0, 62, 185, 556, 1667, and 5000 microg/plate, olmesartan cilexetil was negative in both Salmonella typhimurium and Escherichia coli regardless of presence or absence of metabolic activation system (S9 mix). These results demonstrate that olmesartan cilexetil does not induce bacterial reverse mutation.

7.Effect of Lovastatin(Mevacor(R)) on Serum Lipids of Patients with Primary Hyperlipidemia.

A new hypolipidemic agent, lovastatin, hydroxy-methyl-gultaryl coenzyme A reductase inhibitor was administred to 25 patients with primary hyperlipidemia 20 to 40 mg daily for 12 weeks and sequential changes of serum lipid profile were analysed as follow. 1) Mean average at baseline period of serum total cholesterol, triglyceride, HDL and low desity lipoprotein cholesterol were 271, 179, 51 and 185 mg/dl respectively. 2) Total cholesterol showed 20% decrease at 4th week and 23% decrease at the end of 12th week while low density lipoprotein cholesterol decreased 31% and 33% respectively. 3) Triglyceride dropped 7% at 8th week and 3% at 12th week. High density lipoprotein cholesterol increased 4% at 4th week and showed 2% decrease at the end of study. 4) Only one patient complained of moderate abdominal pain, which subsided after 2 weeks drug withdrawal. In conclusion, lovastatin was well tolerated and effective, in the treatment of primary hyperlipidemia.

8.A Case of Intradermal Nevus Combined with Epidermal Cyst.

The combination of an intradermal nevus with epidermal cyst or other adnexal tumors has often been described since 1952. Most of these reports have involved the coexistence of common epidermal cysts and melanocytic nevi. Herein, we report a 21-year-old Korean male, who initially presented to our department with a rapidly enlarging preexisting nevus on his back. The mass was surgically excised and histopathologically showed intradermal nevus combined with epidermal cyst.

9.One Case of Insulinoma.

Insulinoma is a rare tumor, occurring more often in women and in the older age range. Eighty percent of patients have a single benign tumor, usually 2 cm in diameter, located with about equal frequency in body, head or tail of pancreas and amenable to surgical cure. About 10% have multiple tumors. The remaining 10% of patients have metastatic malignant insulinoma. The symptom of insulinoma is characterized by the periodic attack of hypoglycemia of blood sugar level below 50mg%, by fasting or exertion, and rapid relief of symptom by oral or intravenous administration of glucose. Symptom often lead to misdiagnosis as a neurologic or psychiatric disorder. A case described by authors was 44-year old female with the chief complaints of the loss of consciousness, epileptic seizure although she has been treated by anticonvulsants. Serum blood sugar and insulin level during fasting suggested insulinoma but abdominal computed tomography shows no definitive mass in pancreas. Celiac angiography revealed insulinoma. She transferred to the department of General Surgery and was performed enucleation. Microscopic findings shows the islet cell tumor of pancreas. A brief review of the literature was made.

10.Effects of Dehydration on Vasopressin and Oxytocin Immunoreactive Neurons of the Hypothalamus in the Mongolian Gerbil.

The effects of dehydration on vasopressin and oxytocin immunoreactive neurons in the hypothalamus was investigated by using a immunohistochemistry. Adult Mongolian gerbil[Meriones unguiculates] were deprived of drinking water. Dehydrated animals were sacrificed on the 7th, 14th and 21st day of water retriction. The results are as follows : 1. The body weights were decreased about 1.8% daily. On the 21st day of dehydration, they were shown up to 45% compare to the control. 2. In the hypothalamus of the control group, majority of vasopressin and oxytocin immunoreactive neurons were located in the supraoptic and paraventricular nuclei. 3. Changes due to dehydrated stimulation were mainly observed in vasopressin immunoreactive neurons. And these changes in supraoptic nuclei were more severe than those in paraventricular nucleus. Size of vasopressin immunoreactived cells and of areas were increased as to proceed the dehydration. The numbers of those were increased on the 7th day of dehydration, and then they were continously decreased. 4. Although oxytocin immunoreactive neurons were slightly changed in numbers during dehydration, they were not shown conspicuous changes compare to vasopressin immunoreactive neurons. Thus it is appeared that vasopressin secretory neurons in the hypothalamus of Mongolian gerbil are affected by osmotic stress induced dehydration while oxytocin neurons may be affected by other factors.
