1.Effect of Timing of Do-Not-Resuscitate Orders on the Clinical Outcome of Critically Ill Patients.

BACKGROUND: Many physicians hesitate to discuss do-not-resuscitate (DNR) orders with patients or family members in critical situations. In the intensive care unit (ICU), delayed DNR decisions could cause unintentional cardiopulmonary resuscitation, patient distress, and substantial cost. We investigated whether the timing of DNR designation affects patient outcome in the medical ICU. METHODS: We enrolled retrospective patients with written DNR orders in a medical ICU (13 bed) from June 1, 2014 to May 31, 2015. The patients were divided into two groups: early DNR patients for whom DNR orders were implemented within 48 h of ICU admission, and late DNR patients for whom DNR orders were implemented more than 48 h after ICU admission. RESULTS: Herein, 354 patients were admitted to the medical ICU and among them, 80 (22.6%) patients had requested DNR orders. Of these patients, 37 (46.3%) had designated DNR orders within 48 hours of ICU admission and 43 (53.7%) patients had designated DNR orders more than 48 hours after ICU admission. Compared with early DNR patients, late DNR patients tended to withhold or withdraw life-sustaining management (18.9% vs. 37.2%, p = 0.072). DNR consent forms were signed by family members instead of the patients. Septic shock was the most common cause of medical ICU admission in both the early and late DNR patients (54.1% vs. 37.2%, p = 0.131). There was no difference in in-hospital mortality (83.8% vs. 81.4%, p = 0.779). Late DNR patients had longer ICU stays than early DNR patients (7.4 ± 8.1 vs. 19.7 ± 19.2, p < 0.001). CONCLUSIONS: Clinical outcomes are not influenced by the time of DNR designation in the medical ICU. The late DNR group is associated with a longer length of ICU stay and a tendency of withholding or withdrawing life-sustaining treatment. However, further studies are needed to clarify the guideline for end-of-life care in critically ill patients.

2.The Work Related Psychosocial Factor and Disease among Health Professional.

The work-related psychosocial factor of health professional includes workload, decision latitude, interpersonal conflict, patient factor, job stress, etc. A total of 12,631 employees nationwide, the number of sample being proportional to the Korean Standard Industrial Classification, were diagnosed. Job demand, organization system, lack of reward, and occupational climate were more significant factors for health professionals than other occupations. The psychosocial factor increased the risk of chronic disease (hypertension, coronary heart disease, and musculoskeletal disease) and psychological diseases (alcoholism, depression, anxiety), shift work, specifically, was associated with an increased risk of cancer. It is required that worksite prevention programs be established in occupational as well as individual levels.

3.Effects of Reactivation of Latent Epstein-Barr Virus Using Polymerase Chain Reaction on Acute Hepatitis A in Children.

PURPOSE: We previously reported that concurrent reactivation of latent Epstein-Barr virus (EBV) in children with hepatitis A virus (HAV) infection is common and EBV reactivation with HAV infection adversely affects the clinical features of hepatitis. However, the incidence of concurrent reactivation was not accurate because the detection of EBV reactivation was based on serologic methods. Therefore, we studied the effects of polymerase chain reaction (PCR)-proven EBV reactivation, thus a more precise concurrence, on acute HAV infection in children. METHODS: PCR were conducted in 34 patients, who had enrolled previous study and diagnosed with acute HAV infection between January 2008 and June 2010. Their medical records were reviewed. RESULTS: Among 34 patients with acute HAV infection, 12 patients (35.3%) had EBV reactivation which was proven using serologic and molecular biologic techniques. There were significant differences in the peak levels of AST and ALT between the reactivated and non-reactivated groups (p=0.001 and p<0.001, respectively). The duration of full recovery from hepatitis was more prolonged in the reactivated group (p<0.001). Clinical parameters, such as serum protein (p<0.001) and albumin concentrations (p<0.001), atypical lymphocyte count (p=0.001), prothrombin time-international normalized ratio (PT-INR, p<0.001), and splenomegaly (p<0.001), showed significant differences. The clinical features in the reactivated sub-group >10 years of age revealed more liver dysfunction compared to the non-reactivated sub-group. A comparison with a previous study was performed. CONCLUSION: PCR-proven reactivation of latent EBV in children with HAV infection is common and EBV reactivation with HAV infection adversely affects the clinical features of hepatitis, especially in older children.

4.The Assessment of Worker's Health Status by SF-36.

This study was conducted to understand health status by general characteristic, and to find out relationship between social support and worker's health status. Health status was measured using SF-36(Medical Outcome Study Short Form 36), a 36 item self administered Instrument. The finding of this study were as follow; Mean scores of health status by sex were higher in male. The younger worker reported good health on physical functioning and role limitation-physical than did the older worker, but the older worker reported good health on social functioning and mental health. Mean scores of health status were higher in high income and white worker. When the relationship between social support and health status, social functioning, role limitation-emotion, mental health, vitality, general health were significantly related. Therefore we reviewed the evidence that socio-economic condition are associated with health status in this study, and that the strength of the social support was a important to maintain health.

6.Relationship between Stress and Biomarkers.

Stress can induce modifications in the central nervous(CNS), autonomic nervous and neuroendocrine system. Thus, the stress response has long been measured in laboratory experiments by biochemical changes in the hormone systems that are referred to as the sympathetic nervous system(SNS) and pituitary-adrenocortical axes(HPA). These axes react to acute stress or chronic stress. The activation of these two particular pathways result in elevated serum levels of catecholamines, cortisol, ACTH, dopamine, and others hormones. But there is considerable debate about the relevance of traditional laboratory stress findings to real-life situation. The neurobiology of stress is a key step to the understanding of stress-induced changes of immune functions. The immune system operates in communication with brain and endocrine system. Because of this extensive communication, the immune system can influence how we feel and behave. The stress are associated with endocrine and autonomic changes that can inhibit immune system function. The concept of neurocardiology renders plausible the various theoretical constructs of stress as they relate to circulatory vascular disease. Detailed reviews of the anatomic connections between the brain and the heart and of experimental and clinical data on the role of the CNS in cardiac dysfunction can be found elsewhere. In this study, we reviewed that stress was associated with cardiovascular disease mortality through the known cardiovascular risk factors(hypertension, heart rate variability, homocycteine, and clotting system).

7.Risk factors for hemorrhagic stroke in Wonju, Korea.

Although stroke is a great public health challenge in Korea, there have have been few epidemiologic studies of the risk factors stroke. A case-control study was performed to evaluate the risk factors for hemorrhagic stroke in Wonju, Korea. Ninety-five subarachnoid hemorrhage (SAH) and 102 intracerebral hemorrhage (ICH) patients aged 21-86 years, and 267 controls were recruited among the inpatients of Wonju Christian Hospital during 1994-1995. Information was gathered through interview and examinations. After adjustment for age and sex, family and past history of hypertension, drinking habits, age of menarche, height, weight, body mass index, waist and hip circumference, earlobe crease, prothrombin time, white blood cell count, hemoglobin, and total cholesterol were all found to be significantly associated with both SAH and ICH. The risk factor significantly associated only with ICH was smoking habits. In multiple logistics analyses, the independent risk factors for SAH and ICH were the same. Those included family and past history of hypertension, age of menarche, earlobe crease, prothrombin time, white blood cell count, hemoglobin and total cholesterol. In general, the risk factors for SAH and ICH were similar with each other, except smoking habits. Risk factors found in this study congruent with previous studies were family and past history of hypertension, drinking habits, body mass index, prothrombin time, white blood cell count, and hemoglobin. Those incongruent or rather newly found were age of menarche, a big physique, earlobe crease, and total cholesterol.

8.Effect of Noise Exposure and Psychosocial Factors on Blood Pressure in Manufacturing Workers.

This study was conducted to assess the effects of noise exposure and psychosocial factor on blood pressure in manufacturing workers. The study subjects are 414 workers (243 males, 171 females) employed at the noisy department. The mean age of low exposed group (<85dB) was 34.65+/-9.53 years, and that of high exposed group (> or =85dB) was 36.37+/-11.15 years. The difference in mean age wart not significant. The mean and distribution of working duration, smoking status, drinking status were not significantly different between two groups. The mean systolic blood pressure of the low exposed groups was 120.01+/-12.06 mmHg, and that of high exposed group wart 126.27+/-13.84 mmHg. The mean diastolic blood pressure of the low exposed and the high exposed group were 77.18+/-10.83 mmHg, and 83.46+/-11.22 mmHg respectively. These differences of blood pressure were statistically significant(p<0.05). The workers in noisy department have significantly less work environmental satisfaction, higher job demand, and higher social support. The mean values of psychosocial distress were higher in the workers of the noisy department, but the difference was not statistically significant. This study was to speculate whether the work environmental satisfaction and social supports modify the association between the noise exposure level and the blood pressure. The results showed that work environmental satisfaction could not modify the association between the blood pressure and the noise exposure. Social support at work did not modify the association. Furthermore, we evaluated the high job strain from a combination of high job demand and low job control at work. Compared to the low strain group, the olds ratio of the high job strain group for hypertension in diastolic blood pressure were statistically significant, but not in systolic blood pressure. Hierarchical multiple regression analysis was used to determine whether the independent variables contributed to explaining the blood pressure. After controlling for possible confounders, we found that the noise exposure level was a correlate of the diastolic blood pressure. But no association between the noise exposure level and the systolic blood pressure. No significant result was found for psychosocial factor.

9.Risk factors for tooth loss over 3 years according to the number of pre-existing teeth: data from a rural cohort.

OBJECTIVES: The aim of this study was to identify the risk factors for tooth loss over three years, according to the number of pre-existing teeth, in middle and old aged people in a rural area. METHODS: This was a 3-year follow-up study including participants from KoGES-ARIRANG. Eight-hundred and fifty-five participants took part in both initial and follow-up oral examinations and surveys, conducted between 2010 and 2016. The number of existing teeth was determined through oral examination, and the subjects' sociodemographic characteristics, subjective periodontal statuses, and oral health behaviors were identified through oral questionnaires via 1:1 interviews. Chi-square analysis was used to compare tooth loss in participants with less than 20 pre-existing teeth to tooth loss in those with 20 or more pre-existing teeth. Multivariate logistic regression analyses were used to identify risk factors for tooth loss. RESULTS: Four-hundred and thirty-nine (51.4%) subjects lost more than one tooth over three years. After adjusting for demographic characteristics, subjective periodontal statuses, and oral health behaviors in subjects with 20 or more pre-existing teeth, the odds for tooth loss were as follows: 2.11 (1.42-3.14) times higher in people aged over 65 years compared to less than 65 years, 1.59 (1.06-2.38) times higher in those with a low (below elementary school) compared to high educational level, and 2.01 (1.33-3.02) times higher in those with a severe compared to normal subjective periodontal status. CONCLUSIONS: The risk of tooth loss was different in subjects with less than 20 pre-existing teeth compared to those with 20 or more pre-existing teeth. Therefore, the approach to prevent tooth loss should be different depending on the number of teeth present.

10.Epidemiological studies of Clonorchis sinensis in Mangyeong riverside areas in Korea.

In an attempt to clarify the epidemiological feature of C. sinensis in Mangyeong riverside area, the prevalence of clonorchiasis, infestation rate of the cercariae in Parafossarulus manchouricus, and detection rate of the metacercariae in fresh-water fishes were investigated from March 1 to September 30, 1983 at the upper, middle and lower reaches of the river. The results obtained were summarized as follows:Detection rate of C. sinensis egg among inhabitants was 8.2 percent out of a total of 1,266 persons. but the differences in detection rates were not statistically signifcant among upper, middle and 1ower reaches. According to sex, the detection rates were 10.3 percent in male and 6.1 percent in female (p<0.05), but by age groups, increases of the rates were observed as increase in age (p<0.05). Out of a total of 380 fresh-water fishes of 32 different species, 93 fishes (25 percent) of 12 species were found positive with Clonorchis metacercariae, and there were differences in infection rates of the metacercariae among the fishes in 3 parts of the river; 11 percent in upper, 35 percent in middle, and 34 percent in lower reaches respectively. The metacercarial detection rates from various fishes were 97 percent in Pseudorasbora parva, Cultriculus eigenmanni (85 percent), Gnathopogon strigatus (67 percent), Microphysogobio yaluensis (50 percent), Gnathopogon coreanus (47 percent), Pungtungia herzi(44 percent), Abbottina rivularis (40 percent), Moroco oxycephalus (33 percent), Coreoleuciscus splendidus (32 percent), Gnathopogon majimae (26 percent), Rhodeus ocellatus (7 percent), and Aphyocypris chinensis (3 percent) respectively. Although very few P. manchouricus were collected at upper reach, 12 snails (0.7 percent) among a total of 1,713 were found infected with Clonorchis cercariae. Also the cercariae of Echinochasmus japonicus (7.99 percent), Lexogenes liberum (0.99 percent), Cyathocotyle orientalis (0.75 percent), Exorchis oviformis (0.23 percent) and Asymphylodora japonica (0.05 percent) were detected from the snails.
