1.Efficacy and Safety of Amlodipine in the Treatment of Essential Hypertension.

The antihypertensive efficacy and safety for once daily dosing of amlodipine, a new calcium channel blocker, was studied in a series of 36 patients with essential hypertension. The starting dose of amlodipine were 5mg/day, which were doubled after 4 weeks, if normalization of diastolic blood pressure was not reached. Amlodipine induced a clinically significant reduction in blood pressure(17.7mmHg in systolic blood pressure and 11.9mmHg in diastolic blood pressure) with similar heart rates after 12 weeks therapy. The efficacy of amlodipine therapy was noted an excellent in 18 patients(51.4%), good in 8 patients(22.9%), fair in 5 patients(14.3%), and failed in 4 patients(11.4%). There were few side reactions such as fatigue, headache, constipation and indigestion, but all studied patients completed for 12 weeks therapy except one patients who did not return. And also there was no abnormal results of laboratory tests before and after the amlodipine therapy. This results indicated that amlodipine is an effective and safe new long-acting calcium channel blocker in the treatment of hypertension.

3.CAT Guided Needle Biopsy of Spinal Lesions

The utility of computed axial tomography(CAT) in the diagnosis of spinal disorders has been the subject of considerable interest since the introduction of CAT scanning. Computed tomographic guided needle biopsy of 12 consecutive patients with a variety of undermined spinal disorders resulted in a 100% yield of positive tissue diagnoses, without signlficant complications. The principal advantage of cat guidance is the continuous direct observation of the needle tip position in relation to the target volume. We think that the application of computed tomographic guided needle biopsy in the lesions of the spine is of great value, because it is the single most accurate method of obtaining diagnosis other than by open surgical biopsy.

4.Vascularized Fibula Graft for Restoration of the Large Bone Defect

We analyzed 34 cases of large bone defect caused by trauma, infection and tumor resection from July, 1979 to June, 1985. The bony defect were restored by vascularized fibular graft or vascularized fibular transposition. 22 cases were the traumatic segmental loss and 7 cases were infected ones and 5 cases were the loss following tumor resection. Vascularized fibular graft was performed in 19 cases and vascularized fibular transposition was performed in 15 cases. The 33 cases(97%) of 34 cases had obtained sound bony union and average union time had been 18.6 weeks. Even local infection in the bone, vascularized fibular graft or transposition can be performed. Grafted or transposed fibula had been hypertrophied about 1.2 times in anteroposterior and lateral film which had been measured in the center of the fibula. The most common complication was the clawing of first or second toe. If the ipsilateral fibula is intact, the vascularized fibular transposition is better and if not, vascularized fibular graft is preferred. Operation using the vascularized fibula is a excellent method for the restoration of large bone defect in long bone.

9.Detection of Human Papillomavirus DNA in Condylomata Acuminata Patients using Molecular Hybridization.

Condylomata acuminata are benign tumors which are mostly venereally transmitted. Common sites were coronal sulcus, perisnal area and prepuce. Among 28 patients, 21 acuminate lesions and 10 papular lesions were found. Twenty eight human genital warts in Korean were analysed by Southern blot hybridization. Sequences related to HPV6/11 are found in 89.3%(25/28) of the condylomata. HPV16 DNA was not found at sll. Subtype of HPV was determined by the restriction pattern of DNA cleaved with PstI restriction enzyme in 7 cases. Six cases of HPV6a and one case of HPV6c are found. The above results suggest that most of condylomata acuminata are caused by HPV6 and HPV11 in Korea.
