1.Echocardiographic Diastolic Left Atrial Emptying Index in Hypertensive Patients.
Korean Circulation Journal 1984;14(1):95-102
M-mode echocardiography has been demonstrated to be a sensitive indicator of the sequential changes occurring in the heart in systemic arterial hypertension. The changes in function as well as anatomic features have been recorded. Previous analysis has focused on the anatomy and systolic function of the left ventricle, analysed in hypertensive heart disease. 13 normal subjects(Group I), 14 hypertensive patients without evidence of cardiac involvement(Group II) and 13 hypertensive patients with echocardiographic evidence of left ventricular hypertrophy(Group III) were compared using M-mode echocardiographic data and clinical. findings. The results are as follows : 1) Mean age are; Group I : 41 years(19-62), Group II : 49 years(25-67) and Group III : 53 years(30-83). There are no significant age difference between Group I and II. Sex ratio are 4:9, 8:6 and 7:6 respectively. 2) Mean arterial pressure are; Group I : 98+/-5mmHg, Group II : 126+/-7 mmHg and Group III : 131+/-101mmHg. 3) Left ventricular posterior wall thickness are; Group I : 0.87+/-0.12cm, Group II : 0.89+/-0.12cm and Group III : 1.32+/-0.18cm(p>0.05 in I vs II). 4) Left atrial dimension are; Group I : 2.77+/-0.68cm, Group II : 2.96+/-0.44cm and Group III: 3.12+/-0.60cm. 5) Left ventricular mass arel Group I : 194+/-51gm, Group II : 192+/-51gm and Group III : 318+/-77gm(p>0.05 in I vs II, p<0.001 in I vs II and II vs III). 6) Ejection fraction are; Group I : 0.57+/-0.09, Group II : 0.60+/-0.01 and Group III : 0.52+/-0.01)p>0.05). 7) Mitral valve EF solpe are; Group I : 113.70+/-23.26mm/sec, Group II : 81.14+/-17.22mm/sec and Group III : 69.92+/-22.70mm/sec(p<0.05 in I vs II, p<0.001 in I vs III and II vs III). 8) AEI are; Group I : 0.70+/-0.04, Group II : 0.56+/-0.07 and Group III: 0.48+/-0.07)p<0.001 in I vs II, I vs III and II vs III). From the above results, the atrial emptying index seems to be an early indicator of abnormalities of left ventricular diastolic compliance in hypertensive patients.

Acute osteomyelitis is usually shown as a localized area of increased activity (hot uptake) in the infectious focus on bone scintigraphy. In our patient, absence of radioactivity (cold lesion) was noted in the distal metaphyseal and diaphyseal regions of his right femur. Initial x-ray was interpreted as normal except for mild soft tissue swelling in the right thigh. The lesion was confirmed as an acute osteomyelitis with subperiosteal abscess on surgery. Staphylococcus aureus was the etiologic organism. We describe a case of acute osteomyelitis in a one-year-old boy shown as a cold lesion on bone scan.

PURPOSE: We compared the first postoperative diagnostic and post-therapy scans of patients who received therapeutic doses of I-131, to investigate the difference in clinical outcomes between patients with concordant findings of diagnostic and post-therapy scans and patients with discrepant (more lesions in post-therapy scan) findings. MATERIALS AND METHODS: The first postoperative diagnostic and post-therapy radioiodine scans of one hundred forty three patients with well differentiated thyroid carcinoma were reviewed. Diagnostic scans were obtained following ingestion of 185 MBq of I-131 and post-therapy scans were obtained after therapeutic dose of 3.7~9.3 GBq of I-131. Successful ablation was defined as no radioiodine uptake on diagnostic radioiodine scan and normal range of serum thyroglobulin level (<10 ng/ml) during serum TSH elevation. RESULTS: Discrepant scan findings were noted in 25 (17.5%) patients. Twenty-two patients (15.4%) showed more lesions in post-therapy scan and 3 patients (2.1%) showed stunning effect. Nine (64.3%) of 14 patients with distant metastasis revealed metastatic lesion(s) only on post-therapy scan. Stunning effect was considered as sublethal damage in 1 patient and treatment by a diagnostic dose in 2 patients. Ablation was achieved in 52.4% (75/143) of all patients. Ablation rate and mean cumulative radioiodine dose were not different statistically between concordant and discrepant groups. CONCLUSION: There were 17.5% difference between diagnostic and post-therapy scan findings when using 185 MBq of I-131 as a diagnostic dose. However, 64.3% of distant metastases were revealed only on post-therapy scan. Ablation rate and mean cumulative radioiodine dose were not different statistically between concordant and discrepant groups. The stunning effect was considered as not only sublethal damage but also treatment by a small diagnostic dose of radioiodine.

10.p53-mediated Inhibitory Mechanism on HIV-1 Tat is Likely to be Associated with Tat-Phosphorylation.

Journal of the Korean Society of Virology 1998;28(1):39-52
HIV-1 tat, a strong transactivator, is essential for the HIV-1 replication and AIDS progression. The Tat function is markedly inhibited by human anti-oncogene p53. This work was initiated to identify the p53-associated inhibitory mechanism on tat-mediated transactivation. Inhibitory function of P53 was confirmed by co-transfection of tat-expressing Jurkat cells with LTR-CAT plasmid, or H3Tl cells (LTR-CAT integrated HeLa cells) with different ratio of pSV-tat/pCDNA-p53 plasmids. Results from the direct protein-protein .interaction between soluble p53 and tat, and yeast two-hybrid experiments showed that the co-suppression mechanism is unlikely to be due to the direct interaction. CAT activity was not affected by tat in Jurkat cells which were transfected with p53-promoter-CAT or p53-enhancer-CAT, suggesting that the tat-mediated p53 suppression is not directly associated with p53-promoter. Finally, we have tested protein kinase activity in p53-tranfected Jurkat cells, which might phosphorylate HIV-1 tat, resulting in inhibition of tat function. Some of our data lead us to assume that the p53-mediated tat inhibition is likely to be associated with p53-associated, signaling-mediated phosphorylation of tat, resulting in the dysfunction of tat. This study is now under investigation.
