1.Recurrent retinal detachment in silicone oil-filled eyes treated with two-port 27-gauge pars plana vitrectomy

AIM:To evaluate the feasibility and advantages of two-port 27-gauge pars plana vitrectomy for treatment of recurrent retinal detachment in silicone oil dwelling eyes.METHODS:A retrospective study.Two-port 27-gauge pars plana vitrectomy was performed in 7 eyes with recurrent retinal detachment without remove of silicone oil.Proliferative membrane peeling, endolaser treatment, and drainage of subretinal fluid were performed.Scleral bulking was performed if necessary.RESULTS:Sucessful retina re-attachment was accomplishied in all eyes.No visually significant intraoperative complication occurred.All eyes expericend quick recoveray of visual acuity and mild postoperative irritation.One eye developed a recurrent inferior retinal detchment in 20d after the surgery using non-remove technique, and recovered after conventional silicone oil removal and retinal reattachment surgery.CONCLUSION:Two-port 27-gauge pars plana vitrectomy is an efficacious (and potentially cost-efficient) method to treat early stage recurrent retinal detachment in silicone oil dwelling eyes.
2.Experimental study of liver fibrosis reversal effect of warming-yang compound formula ganzhifu.

OBJECTIVETo observe the effect of Ganzhifu (GZF), a Chinese compound formula for Warming-yang on experimental hepatic fibrosis, and to explore its mechanism.
METHODSFifty-nine Wistar rats were divided into the normal control group and the model group. After the hepatic fibrosis model was established in rats by subcutaneous injection of CCl4 and feeding 10% alcohol in the model group, the rats were randomly sub-divided into the Ganzhifu group, colchicine group and model group, they were treated with Ganzhifu, colchicine and normal saline of same volume once a day respectively for 30 days as one treatment course. The serum liver function and hepatic fibrosis indexes, liver tissue contents of hydroxyproline (Hyp), malondialdehyde (MDA) and superoxide dismutase (SOD) activity were measured at the end of the experiment. Histologic changes of liver tissue were observed and scored by the modified inflammatory activity and fibrosis semiquantative scoring system (SSS), and collagen type I , III and IV in liver tissue were stained and semiquantitive analysis conducted.
RESULTSIn the Ganzhifu group, liver function and inflammatory activity and fibrosis SSS scores were markedly improved, indexes of hepatic fibrosis, liver contents of Hyp, MDA and collagen type I, III and IV were significantly decreased, and the SOD activity was increased, the changes were more significant as compared with those in the model group. The protective effect of Ganzhifu against hepatic fibrosis was the same as that of colchicine.
CONCLUSIONGanzhifu can alleviate or reverse hepatic fibrosis in experimental hepatic fibrosis, and antioxidation may be its mechanism.

3.Influence of wen-yang herbs on hemodynamics in liver fibrotic rats.

OBJECTIVESTo observe the influence of wen-yang herbs on the hemodynamics in liver fibrotic rats.
METHODSWistar rats with liver fibrosis, induced by carbon tetrachloride and alcohol, were randomly divided into a treatment group and a control group. The treatment group was administered wen-yang herbs and the control group saline. At the end of the experiment, the hemodynamic markers of the liver and the mesentery, the liver function and hydroxyproline content of liver tissues between the two groups were compared. Blood volume of the livers and hydroxyproline content of liver tissues were also determined.
RESULTSBlood volume of the liver and mesentery (P < 0.01) and blood flow velocity of small vein of mesentery (P < 0.05) of the treatment group were distinctly higher than the control group. The hydroxyproline content (P < 0.01) of the treatment group was remarkably reduced and liver function was improved.
CONCLUSIONWen-yang herbs can activate microcirculation of the liver and mesentery, decrease the deposit of collagen in the liver and improve liver function.

4.Application of damage control orthopedics in 41 patients with severe multiple injuries.

OBJECTIVETo probe the feasibility and efficacy of damage control orthopedics (DCO) in treating severe multiple injuries.
METHODSA retrospective analysis was made on the clinical data of 41 patients (31 males and 10 females, aged 18-71 years, mean: 36.4) with multiple injuries admitted to our department and treated by DCO from January 1995 to December 2005.
RESULTSAs a first-stage therapy, devascularization of internal iliac arteries was performed in 29 patients with pelvic fractures combined with massive bleeding, including ligation of bilateral internal iliac arteries in 21 patients and embolization of bilateral internal iliac arteries in 8. And early external fixation of pelvis was performed in 10 patients. Ten patients with severe multiple injuries combined with femoral fractures were managed with primary debridement and temporal external fixation and 2 patients with spinal fractures combined with spinal cord compression received simple laminectomy. Thirty-one patients received definite internal fixation after resuscitation in intensive care unit. The overall mortality rate was 12.1% (5/41) with an average injury severity score of 41.4. The main causes of death were hemorrhagic shock and associated injuries. Complications occurred in 7 patients including acute respiratory distress syndrome in 3 cases, thrombosis of right common iliac artery in 1, subphernic abscess in 2 and infection of deep wound in lower extremity in 1. After treatment, all the patients got cured.
CONCLUSIONSPrompt diagnosis and integrated treatment are keys to higher survival rate in patients with severe multiple injuries. In this condition, DCO is an effective and safe option.

5.Expression analysis of a disintegrin and metalloprotease 8 during hepatocellular carcinoma induced by diethylnitrosamine in mice

Objective To study the expressed dynamic change of a dis-integrin and metalloprotease 8 ( ADAM8 ) during hepatocellular carci-noma ( HCC ) induced by diethylnitrosamine ( DEN ) in mice.Methods Seventy mice were randomly divided into two groups:nor-mal group ( n=10 ) and experimental group ( n=60 ).The mice in experimental group were fed daily with 30 μg · mL -1 DEN in drin-king water for 24 weeks.Ten mice were respectively sacrificed at 4 , 8 , 12 , 16 , 20 and 24 weeks, then the livers were taken from the mice and divided into two sections.One was used to detect the occurrence of liver cancer by HE staining, the other was used to detect the ADAM8 expression by Western blot during HCC induced by DEN in mice.The normal control mice were also detected as experimental mice.Results HCC model was successfully induced at 24 weeks after DEN induction.The expression of ADAM8 was gradually increased and reached the peak level at 24 weeks after DEN induc-tion.Conclusion ADAM8 were remarkably differently expressed during HCC induced by DEN in mice.ADAM8 could play important roles in promoting the occurrence and development of HCC.
6.Chimeric Ad5F35 adenoviral vector-mediated expression of mutant IκBα induces apoptosis of leukemia cells.

Constitutive activation of nuclear transcription factor-κB (NF-κB) exists in a variety of leukemia, and induction of apoptosis through blocking NF-κB activation may be an alternative strategy for leukemia treatment. The aim of this study was to investigate the inducing effect of modified adenovirus 5-based adenovirus vector (i.e. chimeric Ad5F35 Vec)-mediated expression of mutant IκBα (IκBαDN) on apoptosis of HL-60 cells. The recombinant Ad5F35-IκBαDN Vec carrying IκBαDN cDNA which deleted the first 1-70 amino acids coding sequences at 5' terminal of human IκBα was transfected into HL-60 cells. The apoptosis, NF-κB DNA binding activity, the expressions of IκBα, cIAP-2 and xIAP in HL-60 cells were detected by DNA binding assay, flow cytometry, real-time quantitative polymerase chain reaction and Western blot respectively. The results showed that apoptosis rates were 22.53 ± 2.999%, 6.08 ± 2.464% and 4.86 ± 1.366% for Ad5F35-IκBαDN Vec-infected or blank vector of Ad5F35-EGFP Vec-transfected and untransfected HL-60 cells respectively, which showed a significant difference between Ad5F35-IκBαDN Vec-transfected and untransfected cells (p < 0.001) and between Ad5F35-IκBαDN Vec-transfected and Ad5F35-EGFP Vec-transfected cells (p < 0.001, p < 0.002), while NF-κB DNA binding activity was decreased, the truncated IκBα was expressed, and IκBα mRNA expression was up-regulated, but the expression of cIAP-2 and xIAP mRNA was down-regulated after transduction for 48 hours. It is concluded that the chimeric Ad5F35 Vec can effectively mediate the expression of IκBαDN cDNA in HL-60 cells, leading to the inhibition of NF-κB DNA binding activity and inducing apoptosis of HL-60 cells.

7.Study on the interface of human hepatocyte L-02 polypropylene:simple culture method of human hepatocyte with spheroidal aggregate culture.

OBJECTIVETo found new interface of human hepatocyte/poly propylene with good cytocompatibility for made polypropylene hollow fibers bioreactor of bioartificial liver in future.
METHODSUsing the macromolecular hydroperoxide groups on the polypropylene membrane surface as initiators, acrylamides were polymerized on the polypropylene membranes, under induction by both UV irradiation and Fe2+ reduction. Growth characteristics of human hepatocyte L-02 were detected when it was cultured on polystyrene, polypropylene and modified polypropylene membrane surface.
RESULTSWater contact angle measurement of the polypropylene and the modified polypropylene membranes decreased from (72 +/- 5) degrees to (30 +/- 4) degrees , which indicated that the hydrophilicity of the membrane was improved obviously after the grafting modification. Human hepatocyte L-02 could not adhere and spread on modified polypropylene membrane surface, and grown in spheroidal aggregate with higher density and higher proliferation ratio measured by MTT method.
CONCLUSIONSAcrylamide polymerized on the polypropylene membranes is a good method which not only improved human hepatocytes cytocompatibility but also found a new simple culture method with spheroidal aggregate culture of human hepatocyte.

8.Expression of a disintegrin and metalloprotease 8 during the drug -induced liver in mice

Objective To study the expressed dynamic change and roles of a disintegrin and metalloprotease 8 (ADAM8) during drug -induced liver injury induced by acetaminophen (AAP).Methods Mice were randomly divided into two groups:normal group and experimental group. The mice in experimental group were respectively drawn blood by remo -ving the eyeballs and serum was separated to detect the activity of aspar -tate aminotransferase (AST) and alanine aminotranferease (ALT) at 6, 24, 42 and 72 h after intraperitoneal injection with 550 mg? kg-1 AAP. The activity of serum AST and ALT in the mice of normal group was also detected.The expression of hepatic ADAM8 at protein and mRNA levels was detected by Western blot and RT -PCR in the mice of normal group and experimental group at different time points after AAP injection . Results Serum AST and ALT activity was first increased and then decreased after AAP injection in the mice.The expression of ADAM8 were significantly up -regulated (P <0.05) at 6 h, reached peak level at 24 h (P <0.05).Then gradually down -regulated and recovered near the normal level at 72 h after AAP injection.Conclusion ADAM8 were remarkably differently expressed during drug -induced liver injury induced by AAP, which may play an important role in promoting liver damage during drug -induced liver injury induced by AAP.

10.The role of nitric oxide in lung injury associated with acute necrotizing pancreatitis.

OBJECTIVETo discuss the role of nitric oxide (NO) in lung injury associated with acute necrotizing pancreatitis (ANP).
METHODSOne hundred and twenty SD rats were randomized into five groups: control group, ANP group, L-arginine (L-arg) pretreatment group, L-NAME pretreatment group, and mixed pretreatment group (n = 24 for each group). Rat ANP model was induced by intraductal administration of 3% sodium taurocholate. Alveolar macrophages (AMs) were obtained by bronchoalveolar lavage. The protein content of bronchoalveolar lavage fluids (BALF), the myeloperoxidase (MPO) of lung tissue and generation of tumor necrosis factor alpha (TNFalpha)and NO by alveolar macrophages were evaluated. The expression of TNFalpha mRNA and iNOS mRNA was also measured.
RESULTSLung injury was aggravated gradually with progression of the disease. The level of MPO of lung tissue and the protein content of BALF showed a steady increase with time and peaked at the 12(th) hour (10.8 +/- 0.6 U/g for MPO and 2,011.0 +/- 105.5 micro g/ml for protein, respectively). TNFalpha and NO secreted by AMs were elevated gradually and peaked at the 6(th) hour (1,624.2 +/- 149.2 pg/ml and 88.8 +/- 6.5 micro mol/L respectively) but decreased at the 12(th) hour. The expression of TNFalpha mRNA and iNOS mRNA was similar with the change of TNFalpha and NO. The parameters of the groups of L-arg, L-NAME and the mixed pretreatment were similar to those of ANP group. The parameters compared with those of the control group showed a significant difference (P < 0.05). The parameters of groups of L-Arg and L-NAME pretreatment in comparison with those of the ANP group showed significant difference (P < 0.05).
CONCLUSIONSOver production of NO mediated by iNOS aggravates lung injury caused by acute necrotizing pancreatitis. Administration of exogenous NOS substrate would worsen lung injury, whereas administration NOS inhibitor would alleviate lung injury.
