2.Clinical comparison of tacrolimus and CsA in end stage renal disease of diabetic renal allograft recipients

One year after the operation, 21 patients ( 63.63 %) were subjected to anti-hypertensiom, 5 patients ( 15.15 %) to anti-hyperlipidemia treatment and 3 patients ( 9.09 %) to liver protection treatment due to abnormal liver function in FK506 group, while in the CsA group 28 patients ( 90.32 %) were subjected to anti-hypertension, 13 patients ( 41.94 %) to anti-hyperlipidemia treatment and 11 patients ( 35.48 %) to liver protection treatment due to abnormal liver function. There were significant differences between the two groups.Conclusion The small dose low concentration FK506 is effective in the end stage renal disease of diabetic renal allograft recipients and side effect incidence of it was low, and it has almost the same influence on the glucose metabolism as the CsA.

4.Early removal of the urethral catheter after transurethral plasma kinetic resection of the prostate in the treatment of BPH.

OBJECTIVETo investigate the feasibility and effect of early removal of the urethral catheter after transurethral plasma kinetic resection of the prostate (PKRP) in the treatment of benign prostatic hyperplasia (BPH).
METHODSWe equally randomized 128 BPH patients treated by PKRP to an experimental group and a control group, urethral catheters removed at 1 -2 days for the former and at 5 -7 days for the latter. We compared the relevant indexes and clinical effects between the two groups.
RESULTSThe baseline data were not significantly different between the two groups. Compared with the controls, the experimental group showed a significantly shorter postoperative hospital stay ([6.8 +/- 1.9] d vs [3.7 +/- 1.5] d, P < 0.05) and lower infection rate (25.0% vs 10.9%, P < 0.05). All the patients were followed up for 3 -6 months postoperatively. At 3 months after surgery, both the experimental and the control groups showed remarkable improvement in the International Prostatic Symptoms Scores (4. 9 +/- 2. 2 vs 5. 3 +/- 2. 3), maximum urine flow rate ([21.5+/- 5.6 ] ml/s vs [19.1 +/-4.9 ] ml/s) , and residual urine ( [ 16.8+/- 10.3 ] ml vs [18.9 +/- 12.3 ] ml), but with no significant differences between the two (P > 0.05) , and no significant differences were observed in postoperative complications (P >0.05).
CONCLUSIONEarly removal of the urethral catheter after PKRP, with its advantages of shorter postoperative hospital stay, lower infection rate, and no influence on the long-term effect, deserves to be recommended as a routine method in the treatment of BPH.

5.Effect of Yangxue Qingnao Granule on Ultrastructures and Expression of p38 Mitogen Activated Protein Kinases in CA1 Area of Hippocampus of Vascular Dementia Rats

Objective To observe the effect of Yangxue Qingnao Granule on the ultrastructures and expression of p38 mitogen activated protein kinases (p38MAPK) in CA1 area of hippocampus in vascular dementia rats. Methods 180 Sprague-Dawley rats were randomly di-vided into sham group, vascular dementia model group (model group) and Yangxue Qingnao Granule treatment group (treatment group). The vascular dementia was modeled with modified Pulsineli's four-vessel occlusion. The ultrastructure of CA1 area was observed with trans-mission electron microscope, while the expression of p38MAPK in CA1 area was detected with immunohistochemstry and Western blotting 1, 2, 4 and 8 weeks after modeling. Results In the model group, pyknosis, nuclear dissolution, heterochromatin margination and mitochon-dria swelling were found in most of the neurons in CA1. In the treatment group, the distribution of chromatin was well-proportioned, and mi-tochondrion and other organelle were normal. In the model group, the expression of p38MAPK increased at each time point compared with the sham group (P<0.01), and peaked 4 weeks after modeling, and decreased in the treatment group compared with the model group (P<0.01). Conclusion Yangxue Qingnao Granule can improve the ultrastructure of neuronal in CA1 area of hippocampus of vascular dementia rats, which may relate with the inhibition of the expression of p38MAPK.
6.Effect of Yang-Xue Qing-Nao Granules on Protein Expression of BDNF and bFGF in CA1 Area of Hippocampus among Vascular Dementia Rats

This study was aimed to observe the effect of Yang-Xue Qing-Nao(YXQN) granules on the expression of brain-derived neurotrophic factor (BDNF) and basic fibroblast growth factor (bFGF) in CA1area of hippocampus among vascular dementia (VD) rats.A total of 72 SD rats were randomly divided into the sham operation group,VD model group (model group) and YXQN treatment group (treatment group).The VD rat model was prepared by modified Pulsineli's four-vessel occlusion.The learning and memory abilities of rats were detected by the Morris water maze.The protein expressions of BDNF and bFGF were detected by the immunohistochemical analysis.The results showed that compared with the sham operation group,there was obvious learning and memory disorders in the model group with increased protein expressions of BDNF and bFGF in CA1 area of hippocampus at difference time points (P<0.01).Compared with the model group,the learning and memory abilities of rats in the treatment group were significantly improved; and the protein expressions of BDNF and bFGF in CA1 area of hippocampus were significantly increased at difference time points (P<0.01).The expression was the highest on the 4th week.It was concluded that YXQN granules can improve the learning and memory abilities of VD rats.Its mechanism may be related to the upregulation of protein expression of BDNF and bFGF.

9.Micro -incision phacoemulsification with silicone oil removal for eyes with silicone oil tamponade combined cataract after vitrectomy

AlM: To observe the clinical effect of micro-incision phacoemulsification combined with silicone oil removal from pars plana in eyes with silicone oil tamponade after vitrectomy.
METHODS:Forty-three patients (43 eyes) with cataract and silicone oiltamponade after vitrectomy underwent micro-coaxial phacoemulsification combined with silicone oil removal from pars plana between February 2013 and November 2013. lntraocular lens was implanted at stage l in all cases. The best corrected visual acuity was observed at 3mo after operation, and the complications were recorded.
RESULTS: The best corrected visual acuity in 39 cases (91%) was improved at 3mo after operation, and the best outcome was 0. 6. There were still 4 cases (9%) without improvement of visual acuity. There was no incidence of complications such as posterior capsular rupture, residual silicone oil, corneal endothelial decompensation, hyphema, vitreous hemorrhage, intraocular lens dislocation, secondary glaucoma and ora serrata mutilation after operation in all patients.
CONCLUSlON: Micro - invision phacoemulsification combined with silicone oil removal from pars plana is an effective and safe method for eyes with cataract and silicone oil tamponade after vitrectomy.
10.Progress and application of 27 -gauge micro-incision vitrectomy

Abstract?Since the introduction of 25-gauge/23-gauge ( 25G/23G ) sutureless micro -incision vitrectomy surgery ( MIVS ) at the beginning of the 21 century, we have stepped into an era of micro-incision vitreoretinal surgery more than10a. The current 25/23G MIVS provide numerous advantages over the conventional 20G vitrectomy surgery including simplified surgical procedure, shortened operating time, decreased complications as well as smaller sclerotomy wound.As a result, vitreoretinal surgeons have been shifting gradually from 20-gauge to 25/23-gauge vitrectomy in the past decade. As the 25/23G MIVS adapted worldwide, however, its drawbacks were also increasingly reported. Most criticism regarding to current 25/23G MIVS are focusing on would sealing related complications.Based on stablished notion regarding vitrectomy -“the smaller the better”, researchers and doctors were keeping on exploring the next generation of vitrectomy system. Thanks to the innovation and development in new generation vitrectomy machines, high lumen output light source, more delicate manufacturing technology and clear wild angle fundus view system, Dr.Oshima from Japan launched the first 27-gauge vitrectomy surgery system.It provided us novel surgical experience with smaller sclerotomy wound and faster cut rate. Further development and refinement of vitrectomy with 27-gauge or more are still on its way and will continue in the future. Undoubtedly, MIVS would be heated debated regarding its pro/con, complications, indications and future development.Combined with our own experiences, here we briefly reviewed the 27-gauge vitrectomy surgery.
