2.Early removal of the urethral catheter after transurethral plasma kinetic resection of the prostate in the treatment of BPH.

OBJECTIVETo investigate the feasibility and effect of early removal of the urethral catheter after transurethral plasma kinetic resection of the prostate (PKRP) in the treatment of benign prostatic hyperplasia (BPH).
METHODSWe equally randomized 128 BPH patients treated by PKRP to an experimental group and a control group, urethral catheters removed at 1 -2 days for the former and at 5 -7 days for the latter. We compared the relevant indexes and clinical effects between the two groups.
RESULTSThe baseline data were not significantly different between the two groups. Compared with the controls, the experimental group showed a significantly shorter postoperative hospital stay ([6.8 +/- 1.9] d vs [3.7 +/- 1.5] d, P < 0.05) and lower infection rate (25.0% vs 10.9%, P < 0.05). All the patients were followed up for 3 -6 months postoperatively. At 3 months after surgery, both the experimental and the control groups showed remarkable improvement in the International Prostatic Symptoms Scores (4. 9 +/- 2. 2 vs 5. 3 +/- 2. 3), maximum urine flow rate ([21.5+/- 5.6 ] ml/s vs [19.1 +/-4.9 ] ml/s) , and residual urine ( [ 16.8+/- 10.3 ] ml vs [18.9 +/- 12.3 ] ml), but with no significant differences between the two (P > 0.05) , and no significant differences were observed in postoperative complications (P >0.05).
CONCLUSIONEarly removal of the urethral catheter after PKRP, with its advantages of shorter postoperative hospital stay, lower infection rate, and no influence on the long-term effect, deserves to be recommended as a routine method in the treatment of BPH.

3.Clinical comparison of tacrolimus and CsA in end stage renal disease of diabetic renal allograft recipients

One year after the operation, 21 patients ( 63.63 %) were subjected to anti-hypertensiom, 5 patients ( 15.15 %) to anti-hyperlipidemia treatment and 3 patients ( 9.09 %) to liver protection treatment due to abnormal liver function in FK506 group, while in the CsA group 28 patients ( 90.32 %) were subjected to anti-hypertension, 13 patients ( 41.94 %) to anti-hyperlipidemia treatment and 11 patients ( 35.48 %) to liver protection treatment due to abnormal liver function. There were significant differences between the two groups.Conclusion The small dose low concentration FK506 is effective in the end stage renal disease of diabetic renal allograft recipients and side effect incidence of it was low, and it has almost the same influence on the glucose metabolism as the CsA.

Objective To observe the effect of Yangxue Qingnao Granule on the ultrastructures and expression of p38 mitogen activated protein kinases (p38MAPK) in CA1 area of hippocampus in vascular dementia rats. Methods 180 Sprague-Dawley rats were randomly di-vided into sham group, vascular dementia model group (model group) and Yangxue Qingnao Granule treatment group (treatment group). The vascular dementia was modeled with modified Pulsineli's four-vessel occlusion. The ultrastructure of CA1 area was observed with trans-mission electron microscope, while the expression of p38MAPK in CA1 area was detected with immunohistochemstry and Western blotting 1, 2, 4 and 8 weeks after modeling. Results In the model group, pyknosis, nuclear dissolution, heterochromatin margination and mitochon-dria swelling were found in most of the neurons in CA1. In the treatment group, the distribution of chromatin was well-proportioned, and mi-tochondrion and other organelle were normal. In the model group, the expression of p38MAPK increased at each time point compared with the sham group (P<0.01), and peaked 4 weeks after modeling, and decreased in the treatment group compared with the model group (P<0.01). Conclusion Yangxue Qingnao Granule can improve the ultrastructure of neuronal in CA1 area of hippocampus of vascular dementia rats, which may relate with the inhibition of the expression of p38MAPK.
6.Investigation of efficiency of immunotherapy on allergic rhinitis and analysis of its correlative factors

Objective To observe the efficiency of immunotherapy on allergic rhinitis and explore irs correla- tive factors.Methods 185 patients with AR were treated with immunotherapy in this department between 1993 and 2003.Specific allergens were selected through skin test.The patients were given subcutaneous injections with al- lergen extract produced by Xiehe Hospital.The doses were gradually increased to the maintenance dose.The clinical effectiveness evaluation based on the symptoms and signs before and after immunotherapy was judged as marked, moderated and poor response.The possihie factors which affected the efficacy of immunotherapy were selected.These factors included sex,age,illness duration,disease condition,chronic sinusitis,asthma,duration of immunotherapy and reaction condition of the skin test.The correlative factors with efficacy were sifted with multifactorial stepwise re- gression analysis.Results The marked,moderated and poor responses were 22.9 %,51.7 %,and 25.4 % respective- ly.The efficacy of immunotherapy correlated well with the duration of immunotherapy(P

9.Micro -incision phacoemulsification with silicone oil removal for eyes with silicone oil tamponade combined cataract after vitrectomy

AlM: To observe the clinical effect of micro-incision phacoemulsification combined with silicone oil removal from pars plana in eyes with silicone oil tamponade after vitrectomy.
METHODS:Forty-three patients (43 eyes) with cataract and silicone oiltamponade after vitrectomy underwent micro-coaxial phacoemulsification combined with silicone oil removal from pars plana between February 2013 and November 2013. lntraocular lens was implanted at stage l in all cases. The best corrected visual acuity was observed at 3mo after operation, and the complications were recorded.
RESULTS: The best corrected visual acuity in 39 cases (91%) was improved at 3mo after operation, and the best outcome was 0. 6. There were still 4 cases (9%) without improvement of visual acuity. There was no incidence of complications such as posterior capsular rupture, residual silicone oil, corneal endothelial decompensation, hyphema, vitreous hemorrhage, intraocular lens dislocation, secondary glaucoma and ora serrata mutilation after operation in all patients.
CONCLUSlON: Micro - invision phacoemulsification combined with silicone oil removal from pars plana is an effective and safe method for eyes with cataract and silicone oil tamponade after vitrectomy.
