1.Reactive oxygen species-dependent apoptosis induction by water extract of Citrus unshiu peel in MDA-MB-231 human breast carcinoma cells.

BACKGROUND/OBJECTIVES: Although several recent studies have reported the anti-cancer effects of extracts or components of Citrus unshiu peel, which has been used for various purposes in traditional medicine, the molecular mechanisms for their effects remain unclear. In the present study, the anti-cancer activity of a water-soluble extract of C. unshiu peel (WECU) in MDA-MB-231 human breast carcinoma cells at the level of apoptosis induction was investigated. MATERIALS/METHODS: Cytotoxicity was evaluated using the MTT assay. Apoptosis was detected using DAPI staining and flow cytometry analyses. Mitochondrial membrane potential, reactive oxygen species (ROS) assay, caspase activity and Western blotting were used to confirm the basis of apoptosis. RESULTS: The results indicated that WECU-induced apoptosis was related to the activation of caspase-8, and -9, representative initiator caspases of extrinsic and intrinsic apoptosis pathways, respectively, and caspase-3 accompanied by proteolytic degradation of poly(ADP-ribose) polymerase and down-regulation of the inhibitors of apoptosis protein family members. WECU also increased the pro-apoptotic BAX to anti-apoptotic BCL-2 ratio, loss of mitochondrial membrane potential and cytochrome c release from mitochondria to cytoplasm. Furthermore, WECU provoked the generation of ROS, but the reduction of cell viability and induction of apoptosis by WECU were prevented when ROS production was blocked by antioxidant N-acetyl cysteine. CONCLUSIONS: These results suggest that WECU suppressed proliferation of MDA-MB-231 cells by activating extrinsic and intrinsic apoptosis pathways in a ROS-dependent manner.

2.Bilateral Obstructive Uropathy Caused by Congenital Bladder Diverticulum Presenting as Hypertensive Retinopathy

A congenital bladder diverticulum (CBD) is caused by inherent muscular weakness instead of obstruction of the bladder outlet. The major clinical conditions are recurrent urinary tract infection (UTI) and voiding dysfunction. This report describes a 15-year-old male adolescent who developed sudden visual disturbance resulting from hypertensive retinopathy. The cause of hypertension was bilateral obstructive uropathy caused by enlarged paraureteral bladder diverticula. After the non-functioning right kidney and ureter and the bilateral diverticula were removed, the left ureter was reimplanted in the bladder. Pathologic findings showed chronic pyelonephritis and partial loss of the bladder musculature in the diverticular wall. This observation indicates that dilated CBD can cause latent UTI, ureteral obstruction, hydronephrosis, and secondary hypertension.

3.Endodontic management of a C-shaped maxillary first molar with three independent buccal root canals by using cone-beam computed tomography.

The aim of this study was to present a method for endodontic management of a maxillary first molar with unusual C-shaped morphology of the buccal root verified by cone-beam computed tomography (CBCT) images. This rare anatomical variation was confirmed using CBCT, and nonsurgical endodontic treatment was performed by meticulous evaluation of the pulpal floor. Posttreatment image revealed 3 independent canals in the buccal root obturated efficiently to the accepted lengths in all 3 canals. Our study describes a unique C-shaped variation of the root canal system in a maxillary first molar, involving the 3 buccal canals. In addition, our study highlights the usefulness of CBCT imaging for accurate diagnosis and management of this unusual canal morphology.

4.Workers intake too much salt from dishes of eating out and food service cafeterias; direct chemical analysis of sodium content.

The average sodium intake of Koreans was reported to be 5,279.9 mg/day, which is one of the highest intake levels worldwide. The average Koreans intake 19.6% of sodium from kimchi, showing kimchi as the main contributor of sodium in this country (Ministry of Health and Welfare, 2005). The sodium content of dishes that are frequently chosen by workers, and which were served by foodservice cafeterias were chemically analyzed. The average sodium content of one meal provided by 10 foodservice cafeterias was 2,777.7 mg. Twenty-one, one-dish-meals, frequently chosen by workers for a lunch menu, were collected at 4 different restaurants for each menu by one male, aged in the twenties and analyzed chemically also. Workers who eat lunch at a workplace cafeteria everyday could intake about 8 g of salt at a one-time meal and those who eat out for a one-dish-meal would intake 3-8 g of salt without counting sodium content from the side dishes. From these study results, one could estimate that over 10 g of salt could be possible for a single meal for workers who eat out everyday. A nationwide nutrition campaign and education for low salt diets for restaurant owners and foodservice providers should be seriously considered.

5.A Study on Perception and Performance of Health Assessment among Clinical Nurses.

PURPOSE: The purpose of this study was to describe perception and performance of health assessment among clinical nurses. METHODS: The sample of this study consisted of 177 staff nurses working at university affiliated hospitals and a self-report questionnaire was used to collect data. Data were analyzed with SPSS WIN 12.0 program. RESULTS: The overall mean scores of perception and performance of health assessment were 3.67 and 1.71 respectively. There were no significant differences according to educational level, working department, and type of health assessment courses taken for perception of health assessment. The variable significantly affecting health assessment performance was the working department. There was a statistically significant correlation between perception and performance of health assessment. CONCLUSION: These findings suggest the need to develop a nursing education strategy to increase the performance of health assessment.

6.The Relationship between the Histological Findings and beta ig-h3 in IgA Nephropathy.

BACKGROUND: TGF-beta is involved in the pathogenesis of various kidney diseases characterized by glomerulosclerosis and tubulointerstitial fibrosis. It is reported that urinary TGF-beta reflects the grade of interstitial fibrosis in glomerular disease. Here, we evaluated the relationship between the histological findings and beta ig-h3 in IgA nephropathy. METHODS: In patients with IgA nephropathy, we measured blood pressure (BP), serum creatinine, 24-hour urinary protein excretion (UTp), creatinine clearance (Ccr), serum and urine beta ig-h3 levels, and urine TGF-beta levels at the time of renal biopsy. Histologic findings were semiquantitively scored according to the extent of glomerulosclerosis (GG), tubulointerstitial fibrosis (TIG) and hyaline arteriolosclerosis (HA) by the criteria suggested by To. Semiquantitive scoring of immunohistochemistry for beta ig-h3 was done. RESULTS: Mean BP 95.4+/-14.5 mmHg, serum creatinine 1.06+/-0.35 mg/dL, 24-hour UTp 1, 423+/-1, 439 mg/day, and Ccr was 97.84+/-59.73 mL/min. The number of patients that showed GG 3 were 5, GG 2 was 1, GG 1 were 12. And, the number of patients that showed TIG 3 were 2, TIG 2 were 5, TIG 1 were 11. HA was shown in 4 patients. beta ig-h3 immunostaining was observed in glomerular Bowman's capsules and basement membrane of proximal tubules. The degree of beta ig-h3 immunostaining was positively correlated with the degree of glomerulosclerosis (r=0.72, p<0.001), interstitial fibrosis (r=0.91, p<0.001), serum creatinine (r=0.592, p<0.05) and Ccr (r=-0.626, p<0.05), but not with 24-hour UTp. Serum and urine beta ig-h3 levels did not correlate with any of these parameters. CONCLUSION: Renal beta ig-h3 expression in patients with IgA nephropathy may be related to glomerulosclerosis and interstitial fibrosis. However, urinary beta ig-h3 levels did not represent the pathologic changes of IgA nephropathy. Long-term study to measure renal beta ig-h3 expression and urinary beta ig-h3 is required to elucidate the roles of beta ig-h3 in IgA nephropathy.

San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, from Jan. 1, 2000, to Dec. 31, 2002. The malignant tumor of lung and bronchus was the most frequent incidence and the tumor of stomach, liver and biliary tract, rectum and colon were next in order of frequency. But, there was the most frequent dental consultation in the malignant tumor of head and neck, owing to the many oral diseases. Male prediction (69.7%) was existed in the admission patients with major malignant tumors and oral diseases. The most common age group of the admission patients with the malignant tumors and oral diseases was the sixty decade(29.8%), followed by the fifty, forty & seventy decade in order. In the content of chief complaints on the admission patients with major malignant tumors and oral complications, peak incidence was occurred as toothache (33.2%), followed by mucosal pain, mastication difficulty, dental extraction, oral bleeding in order. In the diagnosis group of oral complications in the patients with the malignant tumors, periodontitis, pulpitis, mucositis and xerostomia were more common. In the treatment group of oral complications in the patients, the most frequent incidence(32.6%) was showed in primary endodontic drainage(pulp extirpation, occlusal reduction and canal opening drainage) and followed by incision and drainage, scaling, medications & oral hygiene instruction, continuous oral dressing in order.]]>

The primary glomerular disease was the most frequent cause of renal disease, and renal tubulo-interstitial diseases, chronic renal failure and acute renal failure were next in order of frequency. But, there was the most frequent dental consultation in the chronic renal failure, owing to the many oral complications. Male prediction(55.2%) was almost existed in the dental consultation patients with renal diseases. But, there was the female prediction(61.9%) in the primary glomerular diseases. The most common age group of the dental consultation patients with renal diseases was the fifty decade(26.5%), followed by the thirty, sixty and fourty decade in order. In the contents of chief complaints on the dental consultation patients with renal diseases, peak incidence was occurred as toothache(40.3%), followed by oral ulcer, mucositis and intraoral bleeding in order. In the pathologic diagnosis of oral complications in the dental consultation patients with renal diseases, pulpitis(16.1%) was the frequent disease and periodontitis, advanced caries, oral ulcer, gingivitis, mucositis, periapical abscess and periodontal abscess were next in order of freguency. In the care group of oral complications, the most freguent incidence(28.9%) was showed in the endodontic treatment(primary endodontic drainage), and followed by scaling, incision and drainage, only medications, restoration, ointment application onto the ulcer and daily dressing in order.]]>

This is a retrospective study on the care of odontogenic infections in admission patients with major bleeding disorders. The study was based on a series of 514 patients treated at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, from Jan.1, 2000, to Dec.31,2002. The obtained results were as follows : 1. The cardiovascular disease was the most frequent cause of the systemic diseases with major bleeding disorders, and liver disease, cerebrovascular disease and renal failure were next in order of frequency. But, there was the most frequent dental consultation in the liver disease, owing to the many odontogenic infectious diseases. 2. Male prediction (66.3%) was almost existed in the odontogenic infectious patients with major bleeding disorders. But, there was slight female prediction (53.4%) in the cardiovascular disease. 3. The most common age group of the odontogenic infectious patients with major bleeding disorders was the fifty decade(27.2%), followed by the forty, sixtyand thirty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with major bleeding disorder, peak incidence was occurred as toothache (42.2%), followed by intraoral bleeding, ulcer pain, dental extraction in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis and periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(44.2%) was showed in primary endodontic drainage(pulp extirpation, occlusal reduction and canal opening drainage) and followed by the incision and drainage, the medications and oral hygiene instruction, scaling, indirect pulp capping in order.
