1.The Correlation between the Change in the Degree of the Canal Paresis in Bithermal Caloric Test and the Degree of Hearing Loss after Chemical Labyrinthectomy in Patients with Meniere's Disease.

BACKGROUND AND OBJECTIVES:The chemical labyrinthectomy with aminoglycoside is a good method for vertigo control in Meniere's disease. It is relatively non-invasive and easy procedure, but hearing loss is a awful complication. We aimed to evaluate the relationship between the change in the degree of canal paresis in bithermal caloric test and the degree of hearing change. MATERIALS AND METHODS:Sixty-five patients of Meniere's disease who underwent chemical labyrinthectomy with streptomycin between October 1997 and December 2001 were included in this study. The minimum follow-up period for the patients was 24 months. Medical records were analyzed retrospectively. Criteria had been prescribed in 1995 by the American Academy of Otolaryngology-Head and Neck Surgery were used. Forty patients were tested a caloric response before and after the chemical labyrinthectomy. We analyzed the vertigo control rate, the change of the canal paresis and the change of hearing level. RESULTS:Vertigo was controlled in 90.8% of the 65 patients. Twelve patients of total 65 patients were decreased hearing level more than 10 dB after this procedure. Even though the severity of canal paresis increased, the hearing ability of most patients was unaffected. Some patients who had only slight canal paresis happened to suffer from hearing loss. CONCLUSION:There was no correlation between the change in the degree of canal paresis in the bithermal caloric test and the degree of the hearing change.

2.Conservative Treatment of Acute Isolated Injuries to the Posterior Cruciate Ligament: Prospective Study.

PURPOSE: We assessed prospectively the results of acute, isolated, posterior cruciate ligament injuries treated conservatively. MATERIALS AND METHODS: Between August 1999 and December 2001, the study population consited of 11 patients. The patients were treated conservatively and were prescribed full extension long leg splint with posterior tibial support for 6 weeks and then posterior tibial support brace for another 6 weeks. Patients completed a subjective and objective evaluation at 6 weeks, 3 month, 6 month and each year for an average of 17.5 months. RESULTS: Two patients underwent reconstructive surgery because of combined posterolateral rotatory instability. The nine patients were treated conservatively, the mean Orthopadishe Arbeitsguppe Knie score was 93.2 points, International Knee Documentation Committee score in 5 patients was A and 4 patients was B. CONCLUSION: Our study suggests that patients with acute, isolated posterior cruciate ligament injuries treated conservatively may get good clinical results, but we are planning a long term follow-up evaluation to reveal change in laxity, incidence of arthrosis and their correlation.

3.Dry eye in patients with fibromyalgia.

BACKGROUND: Dry eye is a disorder of the tear film caused by reduced tear production or excessive tear evaporation. A high incidence of sicca symptoms has been noted in fibromyalgia (FM) patients resulting in serious complications. This study was conducted to evaluate the incidence of dry eye by pathogenic mechanisms and to investigate the relationship between dry eye and symptom severity in FM patients. METHODS: Fifty patients that visited Maryknoll Hospital between March 2006 and December 2006 were included in the study. Enrolled patients underwent a Korean fibromyalgia impact questionnaire (KFIQ), ocular surface disease index (OSDI), Schirmer test and tear break up test (TBUT). The KFIQ and OSDI are used to assess the severity of fibromyalgia symptoms and dry eye symptoms, respectively. The Schirmer test and TBUT are used to assess reduced tear production and excessive tear evaporation, respectively. We assessed the OSDI, Schirmer test, TBUT, age and disease duration in terms of the KFIQ. RESULTS: The incidence of dry eye was high in FM patients (12.0% by the Schirmer test and 92.0% by the TBUT test). The OSDI positively correlated with the KFIQ (p=0.006). A positive correlation was observed between the Schirmer test and the KFIQ (p=0.024), but not between the TBUT and the KFIQ. CONCLUSIONS: In this study, dry eye was found to be common in FM patients. Excessive tear evaporation was a more common cause of dry eye than reduced tear production. The patients with more severe dry eye symptoms tend to have more severe FM symptoms.

Spindle cell lipoma is a relatively rare adipocytic neoplasm that is easily mistaken for a liposarcoma, and is histologically characterized by a mixture of uniform spindle cells and mature fat cells. It occurs predominantly in male patients aged 45~65 years, and in most cases it arises in the subcutaneous tissue of the neck or shoulder. We report a case of a 45-year-old woman with spindle cell lipoma arising from the left pleural cavity.

5.24-hour Blood Pressure Monitoring in Chronic Cerebrovascular Disease: Decreased Nocturnal Blood Pressure Dip.

BACKGROUND: Periventricular white matter changes(PVWC) in chronic cerebrovascular disease are regarded to be caused by chronic low perfusion and as forms of small arterial disease of cerebral circulation. In some reports, these are suggested to be concerned with the fluctuation of blood pressure. The purpose of this study was to elucidate the diurnal pattern of BP in chronic cerebrovascular disease and the relationship between severity of PVWC and decrement of nocturnal dip through the ambulatory 24 hour blood pressure monitoring (ABPM). Subjects and METHODS: ABPM were carried out every 30 minutes in 36 patients with chronic cerebrovascular disease, prospectively. Twenty-five had PVWCS and/or multiple lacunar infarction (MLI) with various grades, 11 had large artery territorial infarction (LATI) without PVWC. The patterns of ABPM in each group were analyzed by disease entity and severity of PVWC. RESULTS: There was no differences in the two groups; PVWC/MLl and LATI, in properties(age, sex, etc.) and possible stroke risk factors (hypertension, diabetes mellitus, hypercholesterolemia, smoking, etc.) (P<.05). The pattern of ABPM of PVWC/MLl group showed the decrement of nocturnal dip of BP unlike LATI group (P<.05). The decrement of nocturnal dip of BP correlated with severity of PVWC (P<.05). CONCLUSION: We conclude that the diurnal pattern of BP in small artery disease is different from that of large arterial disease group and related to the severity of PVWC.

6.Coronal microleakage of four temporary restorative materials in Class II-type endodontic access preparations.

OBJECTIVES: The purpose of this study was to evaluate the microleakage of 4 temporary materials in teeth with Class II-type endodontic access preparations by using a glucose penetration model. MATERIALS AND METHODS: Glucose reaction test was performed to rule out the presence of any reaction between glucose and temporary material. Class II-type endodontic access preparations were made in extracted human premolars with a single root (n = 10). Each experimental group was restored with Caviton (GC), Spacer (Vericom), IRM (Dentsply-Caulk), or Fuji II(GC). Microleakage of four materials used as temporary restorative materials was evaluated by using a glucose penetration model. Data were analyzed by the one-way analysis of variance followed by a multiple-comparison Tukey test. The interface between materials and tooth were examined under a scanning electron microscope (SEM). RESULTS: There was no significant reaction between glucose and temporary materials used in this study. Microleakage was significantly lower for Caviton and Spacer than for Fuji II and IRM. SEM observation showed more intimate adaptation of tooth-restoration interfaces in Caviton and Spacer than in IRM and Fuji II. CONCLUSIONS: Compared to IRM and Fuji II, Caviton and Spacer can be considered better temporary sealing materials in Class II-type endodontic access cavities.

7.Pelizaeus-Merzbacher Disease: Two Cases in a Family Diagnosed by the Clinical Features and Magnetic Resonance Imaging.

Pelizaeus-Merzbacher disease(PMD) is very rare x-liked recessive leukodystrophy, rather dysmyelinating d due to defect of biosynthesis of proteolipid protein, a component of myelin. Its usual clinical features are stridor, pendular nystagmus, extrapyramidal sign, pyramydal sign, developmental delay and cognition deterioration. Until now PMD has not been published in our country. A 4-year 3 month-old boy developed abnormal eye movement at 3 months of age. His psychomotor development was delayed. On our examination, definite horizontal pendular nystagmus was noted. He showed severe dysarthria, quadriparesis with generalized hyperreflexia, bilateral extensor plantar signs. All laboratory findings including cerebrospinal fluid study were normal. Brain magnetic resonance image (MRI) -revealed diffuse and symmetrical white matter changes in cerebrum and cerebellum. His 23-year-old younger maternal uncle also showed similar clinical history and brain MRI findings. Although the diagnosis of PMD depended on histopathologic investigation, recently MRI can make an important contribution to the diagnosis of PMD with characteristic clinical features. So we could diagnose him and his uncle as PMD.

8.Medium Term Results of a Mobile Bearing Total Knee Replacement.

PURPOSE: The LCS (low contact stress) mobile bearing knee replacement has been used extensively. The purpose of this study was to report at least five-year clinical results of a primary total knee replacement performed with LCS mobile bearing design. MATERIALS AND METHODS: From January 1992 to October 1997, author performed 80 total knee arthroplasties in 59 patients with LCS mobile bearing knee replacement. At the final follow-up, one patient had died, and ten patients had been lost to follow-up. The patients were evaluated with HSS (Hospital for Special Surgery) knee rating system and radiographic analysis. RESULTS: The fifty-six knees in the forty-two patients were examined for final clinical and radiographic analysis at five to ten years. The average active range of knee flexion was from 5 degrees to 112 degrees preoperatively and 0 degrees to 123 degrees at the final follow-up evaluation. The average HSS score was 60.7 points preoperatively and 88.9 points at the final follow-up evaluation. Of the 56 knees, none had a dislocation of the mobile bearing prosthesis, one case had a traumatic periprosthetic fracture and four cases required reoperation, one for infection, one patellar revision for patellar polyethylene breakage, one rotating platform change to thicker one for posterior instability during flexion and one meniscal bearing change for medial meniscal bearing wear. There was no significant progressive periprosthetic osteolysis on follow-up radiographs. CONCLUSION: After five to ten years of follow-up, the LCS knee replacement was found to be performing well, with durable clinical and radiographic results.

9.Results of a Mobile Bearing Total Knee Replacement: Comparison of Retention and Sacrifice of the Posterior Cruciate Ligament.

PURPOSE: The purpose of this study was to compare the results of posterior cruciate ligament retention and sacrificed total knee arthroplasties with mobile bearing design. MATERIALS AND METHODS: Between January 1992 and October 1997, the study consisted of two types of low contact stress(LCS) implants: a meniscal bearing implant(Group I) that retained the posterior crucaiate ligament(n=31), and a rotating platform implant(Group II) requiring sacrifice of this ligament(n=25). All patients were evaluated with pre-and postoperative range of motion and HSS(Hospital for Special Surgery)knee rating system and radiographic analysis at least five years following the knee replacement. RESULTS: The average active range of knee motion was 4 degrees-113 degrees (Group I), 5 degrees-109 degrees (Group II) preoperatively and 0 degrees-127 degrees (Group I), 0 degrees-118 degrees (Group II) at the final follow-up evaluation. The average HSS score was 61.7(Group I), 56.0(Group II) preoperatively and 90.1(Group I), 88.2(Group II) at the final follow-up evaluation. In group I, one patellar revision for patellar polyethylene breakage and one meniscal bearing change for medial meniscal bearing wear had been performed. In group II, one revision for infection and one rotating platform change had been performed for posterior instability during flexion. we had no dislocation of mobile bearing prosthesis, but 2 cases was required reoperation because of one traumatic periprosthetic fracture(Group 1) and one post. instability(Group 2). There was no significant progressive periprosthetic osteolysis on last follow-up radiographs in both groups. CONCLUSION: After five to ten years of follow-up, we found no signigicant difference between group I and II. However, it might be that subjective symptom is excellent in group I and surgical technique remains an important element of success with mobile bearing implants regardless of posterior cruciate ligament.
10.Evaluation of human embryo development in in vitro fertilization- and intracytoplasmic sperm injection-fertilized oocytes: A time-lapse study.

OBJECTIVE: We investigated whether the insemination method (in vitro fertilization [IVF] or intracytoplasmic sperm injection [ICSI]) affected morphokinetic events and abnormal cleavage events in embryonic development. METHODS: A total of 1,830 normal fertilized embryos were obtained from 272 IVF and ICSI cycles that underwent ovum retrieval culture using a time-lapse system (Embryoscope) from June 2013 to March 2015. All embryos were investigated by a detailed time-lapse analysis that measured the developmental events in the hours after IVF or ICSI insemination. RESULTS: No significant differences were observed between the two groups regarding clinical outcomes (p>0.05). ICSI-derived embryos showed significantly faster morphokinetics than those derived from conventional IVF, from the time to pronuclear fading to the time to 6 cells (p<0.05). However, no significant differences were found from the time to 7 cells to the time to expanded blastocyst (p>0.05). There were no differences in abnormal cleavage events between the two groups (p>0.05); they showed the same rates of direct cleavage from 1 to 3 cells, 2 multinucleated cells, 2 uneven cells, and reverse cleavage. CONCLUSION: The morphokinetics of embryo development was found to vary between IVF- and ICSI-fertilized oocytes, at least until the 6-cell stage. However, these differences did not affect the clinical outcomes of the embryo. Additionally, no significant differences in abnormal cleavage events were found according to the fertilization method.
