5.The Usefulness of Fetal Fibronectin in Pregnant Women.

BACKGROUND: The purpose of our study is to evaluate the usefulness of cervicovaginal fetal fibronectin assay for the prediction of rupture of membrane and preterm labor. METHODS: A group of 39 pregnant women was involved in this prospective study. Out of 139 pregnant women, 96 were clinically diagnosed as ruptured membranes (group A). The remaining 43 of 139 pregnant women were clinically diagnosed as preterm labor(group B). The assay was performed by using the ROMCheckTM kit (Adeza Biomedical Corp., Sunnyvale, CA). RESULTS: In group 4, fetal fibronectin (fFN) positive rate is 55% (53 patients) and negative rate is 45% (43 patients). In group B, fFN positive rate is 56% (24 patients) and negative rate is 44% (19 patients). Both group of fFN positive patients show a significantly shorter interval from sampling to delivery than fFN negative patients. Also in group A, the percentage of fFN positive patients who delivered at less than 48 hours after sampling is greater than those with fFN negative patients and in group B, the preterm delivery rate is 79% with positive fFN and 37% with negative fFN. As a predictor for preterm delivery, the presence of fFN has the sensitivity 79%, the specificity 84%, the positive Predictive value 76% and the negative predictive value 86%. CONCLUSIONS: The result suggests that a positive fFN in pregnant women who have uterine contractions and ruptured membrane indicates a significant risk for preterm delivery and labor onset, and a negative fFN is a reassuring sign.

8.Vibrio Fetus Human Infection: Isolation from a Subacute Bacterial Endocarditis Case.

Vibrio fetus was isolated from blood specimens of a subacute bacterial endocarditis patient. The 38 year old male patient was admitted to Severance Hospital in January 1970 for 11 days and again in July 1970 for 13 days. Subacute bacterial endocarditis was the major condtion. Aortic insufficiency and cholestatic hepatitis were the accessory diagnosis. The organism was isolated during the second admission. V. fetus human infection is known to be very rare, and the present case appears to be the first case in Korea. V. fetus grows very slowly with increased carbon dioxide tension which favours the growth. It is a slightly curved, S-shaped and spiral gram-negative organism. Many antibiotics, effective to gram negative organisms, inhibit the growth of the organism. V. fetus is an animal pathogen causing disease in ruminants. The patient enjoyed raw beef dishes. He could be infected with the organism by eating raw beef.

9.Pulmonary Nocardiosis in a Renal Transplant Patient.

Nocardia is a significant opportunistic pathogen in patients with compromised immunity. The authors isolated N. asteroides from an abscess of the axilla and from respiratory specimens of a renal allograft patient with pneumonia. Direct smear of the sputum and bronchial washing specimens showed many branching, filamentous forms which were gram-positive and acid fast. Culture yielded slow growing small white colonies which became orange on further incubation. Aerial hyphae were produced. Identification of the species was based on typical cultural and biochemical tests. The isolate was susceptible to amikacin, minocycline and rifamipicin. The patient became afebrile after 8 weeks of treatment with cotrimoxazole and brief treatment with other antimicrobial agents.
