1.Oxygen saturation level: An indicator for amputation.

Twenty two patients presenting with chronic non-healing wound of the foot were evaluated. The ages range from 49 to 72 years old. There were 15 males and 7 females. Sixteen patients had NIDDM. Two were diagnosed to have Buergers disease with four others had both NIDDM and Buergers disease. Using a pulse oximeter with a sensor attached to the toes, the oxygen saturation level of the distal limbs was determined. Twelve patients were amputated. The mean oxygen saturation level of the non-amputated group was 91.2%, while that of the amputated group was 32.8%. In the non-amputated group, all the wounds improved. It is therefore suggested that determination of oxygen saturation level using pulse oximeter is a safe and convenient way of predicting wound healing. A level below 80% is correlated with poor healing which may assist the surgeon in his decision to amputate. (Author)

2.Early results of supracondylar femoral osteotomy in severe knee flexion contractures in polio patients.

Twenty one polio patients with severe knee flexion contractures admitted at the Santo Tomas University Hospital, Clinical Division from 1984 to 1990 were studied. There were 11 males and 10 females with age ranging from 12 to 18 years with a mean of 14.3 years. The degree of knee flexion ranged from 45 to 120 degrees with an average of 62.5 degrees. Clinical data included the ranges of motion, the stability of the hip, knee and ankle. Manual muscle testing as well as limb length and girth were recorded. the technique as described by Huckstep was conducted either as a one or two staged procedure with or without preliminary skeletal tractions. One patient required an internal fixation on the osteotomized fragments for stability. Two patients had residual flexion contracture of 5 ad 10 degrees after removal of cast. Only one patient abandoned her braces and opted to use her wheel chair despite healed osteotomy. (Author)

4.Knowledge, Attitude And Practices Of Adults In Relation To Sun Exposure And Photodamage

Background: Protection from sun exposure is key in the prevention of photodamage and skin cancer, and is particularly important in countries that experience high ultraviolet exposure. We compare the knowledge, attitude and behaviour towards sun exposure in Singapore between adults with and without photodamage. We also describe the clinical features of patients with photodamage in Singapore.
Methods: 532 subjects were recruited from the dermatology specialist outpatient of a tertiary hospital in Singapore. Each subject was assessed clinically by a dermatologist for evidence of photodamage, and answered a questionnaire assessing his knowledge, attitude and behaviour towards sun exposure and protection.
Results: Subjects with photodamage were older, and had lower education and employment rates compared to subjects without photodamage. There was no significant difference in knowledge on the harmful effects of sun exposure and on sun protection or in sun avoidance behaviour (other than use of protective sunglasses) between the two groups, though more patients with photodamage felt that they take adequate sun protection measures. Of note, only a low percentage of subjects in both groups (24.5% of subjects with photodamage and 23.1% of subjects without photodamage) practise regular use of sunscreen.
Conclusion: There was no significant difference between the knowledge, attitudes and behaviours of subjects with and without photodamage, though demographic differences between the two groups exist. Regular sunscreen usage is low in Singapore, a country with high exposure to ultraviolet light, and measures to educate and modify the behaviour of the public need to be developed.
5.Radiological Assessment of Porous-coated Acetabular Component with Screw Fixation in Primary Total Hip Arthroplasty

This is a follow up study on 124 cases of primary total hip arthroplasty with Harris-Galante porous-coated acetabular component with the line-to-line technique, performed at the Department of Orthopaedic Surgery, Pusan National University Hospital from January 1986 to February 1992. The follow-up ranged from 24 months to 92 months, the average being 53 months. The results were as follows; 1. At the last follow-up, a radiolucent line was found in 20 cases(16.1%), but the width was less than one millimeter. Among the 20 cases, the continuous radiolucent line was found in two cases(1.6%), the progressive radiolucent line in five cases(4.0%). 2. Areas of non-contact(initial gap) between the porous mesh of the acetabular component and the bone were seen on the immediate postoperative radiographs of 35 cases(28.2%) among 124 cases. A radiolucent line developed around 13 cases(34.2%) of the 35 cases that were associated with an initial gap, but a radiolucent line developed around only seven cases(7.8%) of the 89 cases(71.8%) not associated with such a gap. 3. At the last follow-up, a vertical migration was found in five cases(4.0%), an angulation change of the acetabular cup in two cases(1.6%), but a horizontal migration of the acetabular cup was not found. 4. There was no evidence of the disruption of the titanium porous mesh of the acetabular component, and loosening, bending or breakage of the screw for fixation of the acetabular cup. And there was no revision for acetabular loosening. In conclusion, because overall loosening rate was less than 5%, the results were very satisfactory. Considering an initial gap is a important factor of acetabular loosening, we think that it is important to make less initial gap during operation.

6.The Tibial Condylar Fractures with Dissociation of Metaphysis and Diaphysis Treated by Surgical Methods

The tibial plateau fracture involving the articular surface clinically is a very important injury which often can cause disabilities of the knee joint. Open reduction and internal fixation have been proposed as the optimal treatment of patient with Schatzker's type VI tibial plateau fracture because of the decrease in the functional impairment of the knee joint by allowing early motion. But the injury to the soft tissue is usually severe, which may lead to wound complications after internal fixation. Also delayed union, nonunion, and infection after such operation is not a few. Recently some authors have studied about the external fixation or minimal internal fixation as the treatment for tibial plateau fracture. The purpose of the current study was to compare the results of two different operative methods. The authors analyzed 9 cases of Schatzker's type VI tibial plateau fracture surgically treated at the Pusan National University Hospital from March 1991 to August 1994. The follow-up period ranged from 1 year to 4 year 7 months. The results were obtained as follows. 1. Of the 9 cases, 7 were male and 2 were female, the major population were the 6th and 7th decades. 2. The most common cause of this injury was traffic accident in 7 cases(78%). 3. About the fracture pattern, the most commom type was intercondylar fracture type in 5 cases(56%), the next one lateral condyle fracture type in 4 cases (44%). 4. Common associated injuries were ipsilateral fibula fracture in 8 cases(89%) and ipsilateral femur fracture in 4 cases(44%). 5. Six cases were treated by open reduction and internal fixation, and 3 cases were treated by external fixator. 6. According to Poster's criteria, the results were “Acceptable” in 7 cases(89%), “Unacceptable” in 2 cases(11%).

7.VEGF Expression and Microvessel Density in Oral Squamous Cell Carcinomas.

Angiogenesis is an essential process in tumor growth and metastasis. VEGF has been considered a leading candidate inducing tumor angiogenesis. VEGF expression was significantly correlated with clinical stage, lymph node matastasis, and prognosis of cancers of various parts of body. However, little has been known about the correlation between VEGF expression and clinicopathologic parameters in oral squamous cell carcinoma. The aim of this study was to correlate VEGF expression with the clinicopathological parameters and microvessel density. Forty six oral squamous cell carcinomas were analyzed using immunohistochemical method with primary antibodies to VEGF and CD31. VEGF expression was detected in 33 (71.7%) of the 46 cases. The microvessel density was significantly correlated with VEGF expression (P=0.002). There was no correlation between microvessel density and tumour size, clinical stage, and lymph node metastasis, respectively. VEGF expression did not correlate with the histological grade of tumour differentiation, tumour size, and clinical stages. The VEGF-positive rate seemed to be higher in patients with cervical lymph nodal metastasis than in those without it, but it was not statistically significant. In conclusion, the overexpression of VEGF in the oral squamous cell carcinoma seemed to be associated with a more aggressive course of the disease. Further study is necessary to define the role of VEGF in oral squamous cell carcinoma.

10.Evaluation of Prognostic Significance of AgNORs and PCNA during 9,10-dimethyl-1,2-benzantracene(DMBA)-induced Hamster Buccal Pouch Carcinogenesis.

The purpose of this study is to evaluate the prognostic significance of argyrophilic nucleoalr organizer regions (AgNORs) and proliferating cell nuclear antigen (PCNA) by using DMBA hamster buccal pouch carcinogenesis which provides a good experimental model in reproducing steps from precancerous lesions to invasive squamous cell carcinomas. The buccal pouches of 50 Syrian hamsters were applied with 0.5% DMBA in mineral oil three times a week to reproduce various lesions from precancerous ones such as hyperkeratosis or epithelial dysplasia to invasive squamous cell carcinomas. Their sections were stained with H & E, and silver colloid, and processed immunohistochemically by being applied with monoclonal antibody to PCNA. The histopathologic examainations were done and the counts of AgNORs were evaluated. The PCNA labelling indices on each lesions were evaluated. The correlation between histopathological grades and counts of AgNORs or PCNA labelling indices were evaluated. The number of AgNORs was 2.22+/-0.22 in control group, 3.46+/-0.72 in carcinoma in situ (CIS), 3.78+/-0.63 in squamous cell carcinoma (SCC), respectively. AgNORs significantly increased in severe epithelial dysplasia, CIS, and SCC compared with normal tissue (P<0.05). The PCNA Labeling Index (LI) was 39.47+/-6.68% in control group, 79.61+/-4.14% in CIS, and 85.43+/-6.25% in SCC, respectively. PCNA LI also significantly increased in epithelial dysplasia, CIS, and SCC compared with normal tissue (P<0.05). The number of AgNORs, AgNOR area, and PCNA LI slightly increased in the advancing front than in the center of SCC, but, it was not statistically significant. It appeared that there were a good correlation between the number of AgNORs and PCNA LI (Pierson correlation coefficient : 0.649, P<0.001). These results suggested that the number of AgNORs and the PCNA LI could be useful markers for evaluating the risk of malignant transformation and prognosis of SCC. It was thought that the clinical usefulness of these markers should be verified by using human tissue specimens.
