1.The effect of double silicone tube intubation on surgical outcome of endonasal dacryocystorhinostomy.

PURPOSE: This study is to evaluate the effectiveness on patency rates of two silicone tube intubation comparing with one silicone tube intubation in endonasal dacryorhinostomy. METHODS: The 99 patients' charts were reviewed on patency rate and the cause of failure in patients with nasolacrimal duct obstruction, who had undergone endonasal DCR with two sets of silicone tube intubation. Another 100 patients who underwent endoscopic DCR with single silicone tube intubation, were enrolled in this study as the control group to compare the patency rates and the causes of failure between the two groups. RESULTS: Double silicone tube intubation group showed much higher lacrimal patency rates (89.9%) than that (84.0%) of single silicone intubation group. It seems to be due to less membrane formation (4%) at the site of internal ostium by sufficient bougination in two silicone tube intubation group compared with one silicone tube intubation group (16%). Two silicone tube intubation in endonasal DCR was more effective in preserving the lacrimal patency and avoidance of membranous obstruction at newly created nasal ostium (P=0.03). There were no significant differences in the size of nasal cavity, sex and age distributions, and postoperative cares between the two groups. CONCLUSIONS: Two silicone tube intubation can be considered to be a method of improving success rate in endonasal DCR.

2.A Case of Neonatal Death due to Group B beta-Hemolytic Streptococcal Sepsis.

Since 1970, the group B streptococcus(GBS) has been a significant cause of neonatal sepsis in the West. Two distinct forms of disease occur in neonates. Early-onset disease which occurs within 7 days after birth is characterized by respiratory distress, apnea, shock, pneumonia, and occasionally meningitis. Late onset disease usually occurs at 3-4 weeks of age(ranging from 7 days to 3 months) and presents occult bacteremia or meningitis. The GBS is thought to be a rare causative agent for neonatal sepsis in Korea. Lately, we experience a case of early-onset GBS sepsis who died at 3rd day of life. We present this case with brief review of literatures.

3.A Case of Idiopathic Kaposi's Sarcoma of the Skin and Oropharynx.

Kaposi's sarcoma is one of the important features of acquired immunodeficiency syndrome (AIDS) patients, renal transplanted patients, young African men, and elderly Ashkenazi Jews. It is well recognized as the most common malignancy asssociated with AIDS and there is also an increased incidence among patients with iatrogenic immunosuppression secondary to renal transplantation. However, a few cases of Kaposi's sarcoma in immunosuppressed patients not related to the renal trans-plantation had been reported. In this study, we present a case of Kaposi's sarcoma involving the oropharynx and the skin of a patient who was not associated with AIDS nor treated with special immunosuppressive agents for renal transplantation.

5.Ginsan Enhances Humoral Antibody Response to Orally Delivered Antigen.

BACKGROUND: There have been several reports describing the capability of ginseng extracts as an adjuvant. In this study, we tested if ginsan, a polysaccharide extracted from Panax ginseng, was effective in enhancing antibody response to orally delivered Salmonella antigen. METHODS: Ginsan was treated before oral salmonella antigen administration. Salmonella specific antibody was determined by ELISA. mRNA expression was determined by RT-PCR. Cell migration was determined by confocal microscopy and flow cytometry. COX expression was detected by western blot. RESULTS: Ginsan treatment before oral Salmonella antigen delivery significantly increased both secretory and serum antibody production. Ginsan increased the expression of COX in the Peyer's patches. Various genes were screened and we found that CCL3 mRNA expression was increased in the Peyer's patch. Ginsan increased dendritic cells in the Peyer's patch and newly migrated dendritic cells were mostly found in the subepithelial dome region. When COX inhibitors were treated, the expression of CCL3 was reduced. COX inhibitor also antagonized both the migration of dendritic cells and the humoral immune response against oral Salmonella antigen. CONCLUSION: Ginsan effectively enhances the humoral immune response to orally delivered antigen, mediated by CCL3 via COX. Ginsan may serve as a potent vaccine suppliment for oral immunization.

6.A Case of Idiopathic Hypereosinophihc Syndrome Assoclated with Pulmonary Infiltration.

The idiopathic hypereosinophilic syndrome consists of peripheral blood eosinophilia of 1500/mm3 or more without a known cause, plus signs and symptoms of organ eosinophilia. The prognosis of HES without treatment is poor. However, about one third of the patients with this syndrome may respond to corticosteroid thrapy. Morever, the majority of the remainder may have a favorable response to hydroxyurea. We present here a case of hypereosinophilic syndrome without any identifiable causes, involving bone marrow, liver, lungs and cervical lymph node. We tried corticosteroid as a treatment but it showed no response. However the hydroxyurea showed good response.

7.Determinants of Successful Percutaneous Transluminal Coronary Angioplasty.

In Order to evaluate determinants of successful percutaneous transluminal coronary angioplasty (PTCA), PTCA was performed for 172 coronary arterial lesions in 120 patients(89 male, 31 female) at Yeungnam university hospital from Sep. 1992 to Aug 1993. The corinary artery luminal diameter at the site of the original stenosis was eveluated from end-diastolic frames of identical projections of the preangioplasty and immediate post angioplasty. The coronary luminal and balloon diameters were measured with using of computer measuring system. Overall success rate of 172 attempted lesions was 87.2%. Success rate of female patients was 93.5% and higher than those of male patients. According to the clinical diagnosis, success rate in stable angina was 93.7% and higher than those of post myocardial infarction angina, unstable angina and acute myocardial infarcrion. Success rate of American Heart Association type C lesion was 65.5% and lower those of type A(95.7%), type B (89.%). There was significantly difference in preangioplasty luminal stenosis, elastic recoil and length of lesion between successful PTCA group and failed PTCA group. Success rate of lesion location at a bed >45° and presence of intracoronary thrombus were lower than than those of other angiographic findings. In coclusion, primary angioplasty success was affected by specific angiographic factors, Stenosis severity, thrombus, lesion location at a bend >45°, elastic recoil, and length of lesion were the principle of determinants of coronary angioplasty success rate.

8.The effects of acupuncture treatment for smoking cessation: Preliminary study for high school student.

BACKGROUND: Smoking is the single most considerable factor, which may likely affect one's health most adversely. Therefore, an effective control upon smoking has been the most important issue for all of the practitioners. Furthermore, the latters concern an the alternative medicine has been steadily mounting these days. Accordingly, a series of clinical group study has been mapped aut in order to clarify the effect and the outcome of acupuncture treatment and its rate of success. METHODS: The survey has been conducted fram April 7th, 1998 for six months with the voluntary help of 130 students as our study objects, who were the 1st, 2nd and 3rd graders at two metropolitan high-schools in Seoul A basic questionnaire has been prepared and collected at the first interview. The acupuncture treatment has been alternately administered at each side of ears once a week for the respective object-students and the information had been questioned in the survey. When 5 months lapsed with no treatment after the initial four weeks of intensive application, a further follow-up survey was repeated all over again(once every week for 4weeks and another after 6 months). For the analysis of our finding, the SAS V6.12 has been used together with the one way ANOVA test, t-test, and Cochran-Armitage trend test. RESULTS: Those who managed to stop smoking after the acupuncture treatment of one week, four weeks and six months respectively were found as follows; 2 students(61%), 12 students(37.5%), and 8 students(25%), proving that the success rate for smoking cessation after six months was 25%. Those who decreased their number of smoking more than half were; 13 students(39.4%), 15 students(46.9%), 25 students(75%). The number of withdrawal symptom and the smoking cessation as well as the smoking reduction rate were not found co-related each other. The frequency of their smoking, the frequency of the smoking attempt, the period of their abstinence from smoking, the facts whether their family-members of relatives smoked or not, the FTQ(Fagerstrom Tolerance Questionnaire), the frequency of their drinking and the motivation types of smoking(Why test) were found not influential upon the change of smoking tastes and their intensity of smoking wish by the treatment of the auricular acupuncture. CONCLUSIONS: The change of smoking taste and the change on the intensity of smoking wish as the outcome of acupuncture treatment for the stop-smoking has been statistically siganificant.

Hypertrophic osteoarthropathy(HOA) is a systemic disorder characterized by proliferative periostitis of long bone and polysynovitis. Clubbing of the fingers and toes usually accompanies HOA. This syndrome is most frequently with lung cancer. Although disappearance of HOA with removal of the tumor is well known, very few typical cases are reported in Korea. We report case of lung cancer with HOA, which was dramatically relieved after the surgical resection of the tumor.

10.A Clinical Review for Resected Intrahepatic Cholangiocarcinoma.

BACKGROUND/AIMS: An intrahepatic cholangiocarcinoma, which has been defined as a primary adenocarcinoma from the intrahepatic bile duct to the second-order branch of the main hepatic duct, has a poor prognosis due to late diagnosis and low resectability. The purpose of this study is to define the prognostic factors that affected the survival of patients. METHODS: To evaluate the clinical manifestation and pathologic characteristics and the value of surgical management in relation to survival, we retrospectively reviewed the clinical records of 37 patients with an intrahepatic cholangiocarcinoma who had been underwent hepatic resection at the department of surgery, Hanyang University Hospital from April 1986 to April 2001. Cumulative survival rate and clinicopa-thologic factors that may influence the prognosis were analyzed statistically. Statistical significance was calculated by the Kaplan-Meier and compared by log rank test with statistical significance defined as p<0.05. RESULTS: The median survival time of the patients was 24.6 months (mean, 23.97 months), with 2 and 3-year survival rates of 33.3% and 27.3%, respectively. Univariate analysis showed that TMN stage and gross type correlated significantly with prognosis. Age, sex, tumor marker, Clonorchis Sinensis, intrahepatic duct stone and cell differentiation were not significantly correlated with prognosis. CONCLUSION: In case of the intrahepatic cholangiocarcinoma, without surgical treatment, survival time is not longer than mean 6 months from the first diagnosis. According to this study, we strongly recommend to early diagnosis and proper hepatic resection with lymph node dissection, be recommended to raise the long-term survival rate and improve the quality of life.
