1.Application effects of Blatchford risk assessment system combined with grading nursing care in patients with upper gastrointestinal hemorrhage

Objective To explore the application effects of Blatchford risk assessment system combined with grading nursing care in patients with upper gastrointestinal hemorrhage (UGIH). Methods A total of 73 UGIH patients who were hospitalized in Wenzhou People's Hospital from June 2016 to January 2017 were collected in this study using convenience sampling. The participants were divided into two groups. The control group (n=34) received routine nursing care while the observation group (n=39) received grading care according to Blatchford risk assessment. The Blatchford risk assessment score was evaluated for the observation group before and after intervention. The re-bleeding rate was compared between the two groups. Results The Blatchford score was significantly lower after 1 week of treatment than the day of admission,and the Blatchford score on the day of discharge was lower than 1 week of treatment and the day of admission (P< 0.05). The re-bleeding rate (5.13%) of the observation group was significantly lower than that (35.29%) of the control group (P<0.05). Conclusions The Blatchford risk score system combined with grading nursing care can effectively reduce the re-bleeding rate in UGIH patients and may reduce the mortality rate and improve the clinical nursing quality.
2.Impacts of workplace violence and professional identity on nurse work engagement in Class Ⅲ Grade A hospitals

Objective? To understand the influence of workplace violence and professional identity on nurses' work engagement, so as to provide guidance for clinical nursing management. Methods? From July to September in 2018, nurses from 4 ClassⅢ Grade A hospitals in Wenzhou city were selected as subjects randomly. The General Data Questionnaire, the Workplace Violence Scale, Professional Identity Scale and the Work Engagement Scale were used in the study. A total of 880 questionnaires were distributed and 862 valid ones were retrieved with the effective recovery rate of 97.95%. Results? In the 862 participant nurses, 247 (28.65%) nurses suffered from zero frequency workplace violence, 354 (41.07%) nurses suffered from low frequency workplace violence, 202(23.43%) nurses suffered from intermediate frequency workplace violence, 59(6.84%) nurses suffered from high frequency workplace violence. The total score of professional identity was (110.89±19.26) points;the total score of work engagement was (59.72±8.95) points;workplace violence scores were negatively correlated with the nurses' work engagement (P<0.05);the total score of nurses' professional identity and the scores of all dimensions were positively correlated with the scores of work engagement (P<0.05);Multiple linear regression analysis showed, nurse professional identity, workplace violence, professional title, working duration and education are the main influencing factors of their work engagement (P< 0.01). Conclusions? The nurses' work engagement needs to be improved, and professional identity, workplace violence, professional title, length of work and education are the main influencing factors. It is suggested that clinical nursing managers should take measures to reduce the frequency of workplace violence, improve the working environment for the nurses so as to improve their working engagement level and ensure the quality of clinical nursing service.
3.Study on effects of diversified management model for increasing quality of bowel preparation in patients with colonoscopic examination

Objective To investigate the application effect of diversified management models for bowel preparation in colonoscopic examination.Methods A total of 100 inpatients with colonoscopic examination in the gastroenterology department of this hospital from October 2021 to March 2022 were selected and divided into the control group and observation group by adopting the random number table method,50 cases in each group.The control group was given the routine nursing and the observation group implemented the diversified management model nursing.The Boston bowel preparation scale(BBPS)score,intestinal preparation qualifi-cation rate,awareness of intestinal tract preparation knowledge and patients satisfaction on the nursing service were compared between the two groups.Results The BBPS score in the observation group was(7.10±0.99)points,which was significantly higher than(6.08±1.00)points in the control group,and the difference was statistically significant(t=5.095,P<0.001).The intestinal preparation qualification rate in the observation group was also significantly higher than that in the control group[92%(46/50)vs.76%(38/50)]and the difference was statistically significant(x2=5.556,P=0.018).The awareness rate of intestinal tract prepara-tion knowledge and nursing service satisfaction score in the observation group were higher than those in the control group(P<0.05).Conclusion Implementing the diversified management model can effectively in-crease the cleanliness of intestinal tract preparation,improve the quality of intestinal tract preparation and pa-tients satisfaction on the nursing service and is conducive to improve the diagnosis and treatment effects of the patients.
