1.Clinical Observation of Prucalopride in the Treatment of Functional Constipation

OBJECTIVE:To observe the clinical efficacy and safety of prucalopride in the treatment of functional constipation (FC). METHODS:In prospective self-controlled study,87 female patients with FC were selected and given prucalopride 2 mg,1 time per day. The total course was four weeks. The defecate frequency,stool property and constipation symptom grade were ob-served before and after treatment. Related adverse drug reaction were observed during treatment. RESULTS:During the period of treatment,the defecate frequency was gradually increased,the stool property was gradually improved,and the constipation symp-tom grade of the patients was also improved. The difference had statistical significance before and after treatment(P<0.01). No sig-nificant adverse drug reaction was found during the study. CONCLUSIONS:Prucalopride is a safe and effective treatment for pa-tients with FC.
3.The diagnostic value of Epworth sleepiness scale combined with portable sleep monitor in patients with obstructive sleep apnea syndrome

Objective To evaluate the diagnostic value of Epworth sleepiness scale(ESS) combined with portable sleep monitor (PM) in patients with obstructive sleep apnea syndrome (OSAHS).Methods The questionnaire of ESS was used to evaluate 156 patients with snoring,and then line PM check,monitoring apnea hypoventilation index (AHI),oxygen reduction index (ODI),the lowest oxygen saturation (LSpO2),average blood oxygen saturation (MSpO2).Results One hundred and twenty-four patients were diagnosed to be OSAHS and 32 patients be simple snore by PM in 156 patients.A total of 124 cases,including mild 42 cases,moderate 47 cases,severe 35 cases.There were significant difference in ESS score,AHI,ODI,LSpO2 and MSpO2 between OSAHS in various degrees and patients with simple snore [simple snore:(5.8 ± 2.9) scores,(3.2 ± 1.3) times/h,(3.3 ± 2.6) times,0.911 ± 0.121,0.973 ± 0.132;mild OSAHS:(9.7 ± 3.5) scores,(12.8 ± 23.3) times/h,(14.2 ± 5.7) times,0.875 ± 0.083,0.902 ± 0.095 ; moderate OSAHS:(12.9 ±4.8) scores,(27.6 ± 4.5) times/h,(25.6 ± 6.1) times,0.816 ± 0.069,0.851 ± 0.071 ;severe:(16.1 ± 5.2)scores,(43.6 ± 7.6) times/h,(38.6 ± 7.8) times,0.701 ± 0.061,0.795 ± 0.063] (P < 0.05).With the severity of OSAHS increased,ESS score,AHI,ODI increased gradually,LSpO2,MSpO2 gradually declined,there were statistically significant differences (P < 0.05).ESS score was positively correlated with AHI and ODI (r =0.42,0.51,P < 0.05),and negatively correlated with LSpO2 and MSpO2 (r =-3.70,-4.50,P < 0.05).Conclusions There is a good relativity between ESS score and the monitoring index of PM.Combination of ESS with PM could be used as a primary diagnostic method in patients with OSAHS,especially in primary-care hospitals.
4.Efficacy and safety of Jianwei xiaoshi oral liquid combined with mosapride on treating postprandial distress syndrome

Objective To investigate the clinical efficacy and safety of Jianwei xiaoshi oral liquid combined with mosapride on treating postprandial distress syndrome( PDS ). Methods Eighty cases patients with PDS diagnosed according to Rome-Ⅲ PDS diagnostic criteria were randomly assigned into treatment group and control group. Patients in treatment group were treated with jianwei xiaoshi oral liquid( 1 bottle/times,3 tiems/d,after meals)and mosapride,and in the control group were taken mosapride(5 mg/times,3 times/d, before meals)only for 4 weeks. After the end of treatment,the improvement of the symptoms,the total symptoms score,the total effective rate and adverse drug reaction were recorded. Results After the treatment,symptoms were relieved in both groups. The total symptoms score of the treatment group was(2. 90 ± 1. 39)scores,lower than that in before((11. 14 ± 1. 86)scores),and the difference was significant(t=22. 328,P﹤0. 01). The total symptoms score in treatment group was(2. 90 ± 1. 39)scores,significantly lower than the control group((4. 94 ±2. 03)scores;t =5. 217,P ﹤0. 01). The total effective rate was 87. 80%(36/41),66. 67%(26/39)in treatment and control group,and the difference was significant( t=5. 122,P﹤0. 05). There was no significant in terms of adverse drug reaction in two groups during the treatment. Conclusion Jianwei xiaoshi oral liquid combined with mosapride is a safe and effective treatment of PDS patients,which is better than using mosapride alone.

7.Therapeutic effect of nasogastric tube and nasointestinal tube enteral nutrition for severe traumatic brain injury

Objective To investigate the therapeutic effect of enteral nutrition in patients with severe traumatic brain injury (sTBI) by nasogastric tube and nasointestinal tube.Methods A retrospective case control study was made on 64 patients with sTBI hospitalized from October 2013 to December 2015.The patients were assigned to nasogastric tube group (n=32) and nasointestinal tube group (n=32) according to the random number table, and the same nutrient solution was given to all patients.Eighteen male and 14 female patients aged (37.8±8.7)years were identified in nasogastric tube group, and the Glasgow coma score (GCS) was (4.7±1.3)points.Twenty males and 14 females aged (39.7±6.5)years were identified in nasointestinal tube group, and the GCS was (4.1±1.2)points.Triceps skinfold thickness, body mass index, hemoglobin, level of serum albumin and level of prealbumin were measured and compared between groups before and after operation.Incidence of complications was recorded as well.Results All the above-mentioned indexes of the two groups did not differ significantly before the therapy (P>0.05), but showed significant differences 7 and 15 days after therapy (P<0.05).Incidence of complications in nasointestinal tube group (17 lung infection, nine gastrointestinal bleeding, 11 electrolyte imbalance) was lower than that in nasogastric tube group (seven lung infection, two gastrointestinal bleeding, one reflux esophagitis, four electrolyte imbalance) (P<0.05).Conclusion Nasointestinal tube enteral nutrition is better than nasogastric tube enteral nutrition in improving nutritional status and reducing complications in patients with sTBI.
8.Design of test phantom for MRI clinical quality control

Objective To design a test phantom for clinical routine quality control of magnetic resonance imaging (MRI).Methods The commonly used phantom such as Magphan SMR170 and Victoreen 76-903 were used for experiments with the same parameters,and the test data of all the modules were compared.The advantages and disadvantages of all the modules were analyzed,and an optimization scheme was proposed accordingly.Results The phantom could be applied to the detection of high-image-quality MRI machine,and solved the problems in positioning,size matching and etc.Conclusion The new phantom meets the demand of magnetic resonance imaging clinical routine testing,and provides a certain reference for phantom optimization design.
9.Qianlongtong capsule elevates the Smad4 gene expression in prostate stromal cells.

OBJECTIVETo investigate the effects of the plasma containing Qianlongtong Capsule (QLT)-containing plasma on the expression of the Smad4 gene in prostate stromal cells in vitro and provide some experimental evidence for the treatment of benign prostatic hyperplasia (BPH) with Chinese medicinal compound.
METHODSFifteen cases of BPH were equally randomized to three groups to be treated with QLT at a high dose (6 capsules once), a medium dose (3 capsules once), and a low dose (1.5 capsules once), tid, for 7 days consecutively. QLT-containing plasma was collected from the patients. Prostate stromal cells were identified by immunofluorescence when they became monolayered and cultured in the QLT-containing plasma for 24 hours, followed by detection of the expression of the Smad4 gene by real-time quantitative PCR and that of the Smad4 protein by Western blot.
RESULTSAfter treatment with the QLT-containing plasma, the expression of the Smad4 gene in the stromal cells was significantly increased in a dose-dependent manner as compared with the blank control and no-QLT groups (P < 0.01). The expression of the Smad4 protein was also markedly elevated after treatment. The differences were statistically significant between the blank control and medium-dose groups (P < 0.01), low-dose and medium-dose groups (P < 0.05), and high-dose and the other groups (P < 0.01), but not between the blank control and low-dose groups (P > 0.05).
CONCLUSIONQLT-containing plasma could inhibit the proliferation and improve the apoptotic index of prostate stromal cells in vitro, which was related to the elevation of the mRNA and protein expressions of Smad4.
