1.Association of peripheral blood estradiol, progesterone and testosterone levels with irritable bowel syndrome.

OBJECTIVETo investigate the relation of peripheral blood estradiol, progesterone and testosterone levels with irritable bowel syndrome (IBS).
METHODSForty-eight patients with IBS identified according to Rome II diagnostic criteria and 30 healthy subjects as controls were analyzed for peripheral blood sex hormone levels by radioimmunoassay and corresponding software.
RESULTSIn male patients with IBS, blood testosterone level was significantly lower than that of the control group (P<0.05), but blood estradiol and progesterone showed no significant differences between the two groups (P>0.05). In the female patients, blood estradiol level was significantly lower than that of the control group (P<0.05), whereas blood progesterone and testosterone levels had no significant differences between the two groups (P>0.05).
CONCLUSIONPeripheral blood testosterone level in male IBS patients and estradiol level in female patients are lower than those of healthy subjects, suggesting that IBS might be associated with blood sex hormone disorder.

2.Characterization of a novel impurity in bulk drug of lisinopril by multidimensional NMR technique.

During the routine impurity profile of lisinopril bulk drug by HPLC (high-performance liquid chromatography), a potential impurity was detected. Using multidimensional NMR (nuclear magnetic resonance) technique, the trace-level impurity was unambiguously identified to be 2-(-2-oxo-azocan-3-ylamino)-4-phenyl-butyric acid after isolation from lisinopril bulk drug by semi-preparative HPLC. Formation of the impurity was also discussed. To our knowledge, this is a novel impurity and not reported elsewhere.

3.Research progress on the association between environmental pollutants and incidence of diabetes

The research focus of diabetes, a common chronic metabolic disease, has shifted from individual factors to environmental factors at the population level. Epidemiological studies suggest an association between exposure to environmental pollutants and the risk of diabetes. Major environmental pollutants include organochlorine pesticides, polychlorinated biphenyls, phthalates and their metabolites, and arsenics, which primarily enter the human body through the skin, respiratory tract, and digestive system. Long-term exposure to these pollutants can affect the pathology of diabetes through various mechanisms, such as promoting insulin resistance, causing insulin secretion deficiencies, inducing oxidative stress-induced glucose metabolism disorders, and affecting DNA methylation. However, the potential damaging mechanism of the impact of environmental pollutants on diabetes remain unclear. Limitations such as insufficient sample sizes, uncertainties regarding exposure time and dosage, and differences between single and co-exposures. In the future, it is necessary to focus on exploring and analyzing the mechanisms of environmental pollutant exposure on diabetes to develop effective prevention strategies, control and reduce the incidence and development of diabetes, and provide new insights into its diagnosis and treatment.
4.Mutation analysis of SLC12A1 gene in nine Chinese patients with Bartter syndrome type Ⅰ

Objective To analyze the mutations of SLC12A1 gene in nine Chinese families with Bartter syndrome type I (BS1),and analyze the relationship between genotype and phenotype.Methods The next generation sequencing was used to detect mutations in nine BS1 patients including eight with antenatal BS (aBS) and one with classical BS (cBS).Clinical characteristics and biochemical findings at the first admission as well as follow-up were reviewed.Results 15 different mutations of SLC12A1 gene were identified,including 11 novel ones.Among nine probands,seven were compound heterozygotes,two were homozygotes.All patients presented with polydipsia and polyuria,and eight with growth retardation.All patients had lower than-normal serum chloride concentration,metabolic alkalosis,and elevated basal renin activity and aldosterone,and seven had hypokalemia.Through treatment of indomethacin and/or potassium chloride,biochemical indicators could roughly restored normal.Conclusion These findings will enrich the human gene mutation database (HGMD) and provide valuable references to the genetic counseling and diagnosis for Chinese population.
5.On-site Determination of Element Concentrations in Marine Sediments and Comparative Study of Analytical Methods

Epsilon 3xenergy dispersive X-ray fluorescence spectrometer (EDXRF) was used to analyze the major and minor elements in marine sediments collected from the Indian Ocean on-site. Results obtained by EDXRF were compared with those by inductively coupled plasma-mass spectrometry (ICP-MS) and inductively coupled plasma-optical emission spectrometry (ICP-OES) in the laboratory A total of 24 elements in marine sediment samples,namely Al2O3,MgO,Fe2O3,Na2O,K2O,CaO,Mn,P,Ti,Ba,Sr,V,Zn, Zr, Co,Ni,Cu,Ga,Rb,Y,Nb,Mo,Nd,Pb and Th were measured,and a good agreement was found for these elements(R2>0.9),except for Al2O3,and Ga,Nb and Th with content lower than 30 μg/g. The mean relative deviations of EDXRF method were less than 10% for all of the above mentioned major elements, except for MgO in low concentration and P in high concentration. The mean relative deviations were less than 25% for all of the above mentioned trace elements,except for Sr(>700 μg/g),Mo(<20 μg/g) and Nb. In general, precision and accuracy of EDXRF method were good enough for detection of marine sediments. Taking into account all these facts, the EDXRF method proposed here was proved to be an effective tool for element determination in marine sediment on-site,and it would provide a new technical support to investigate the spatial distribution of mineral resources in marine geological survey.
6.Predictive value of neutrophil/lymphocyte ratio for stroke-associated pneumonia in patients with acute ischemic stroke

Objective:To investigate the predictive value of neutrophil/lymphocyte ratio (NLR) at admission for stroke associated pneumonia (SAP) in patients with acute ischemic stroke (AIS).Methods:Patients with AIS admitted to the First Affiliated Hospital of Kunming Medical University from January 2019 to June 2020 were retrospectively included. The demographic information, vascular risk factors, severity of stroke at admission, and NLR data of the patients were collected. Multivariate logistic regression was used to analyze the independent correlation between NLR and SAP. The NLR was divided into quartile groups to further analyze the trend relationship between NLR and SAP. Receiver operating characteristic (ROC) curve was used to evaluate the diagnostic value of NLR for SAP. Results:A total of 316 patients with AIS were enrolled, including 200 males (63.29%) with an age of 63.86±13.78 years. The median baseline Nationanl Institutes of Health Stroke Scale score was 4 (interquartile range, 2-9), and the median NLR was 4.42 (interquartile range, 3.17-6.70). Ninety-three patients (29.43%) experienced SAP. Multivariate logistic regression analysis showed that NLR was an independent risk factor for SAP in patients with AIS (odds ratio 1.189, 95% confidence interval 1.077-1.313; P<0.001). Moreover, SAP risk increases with the increase of NLR ( Ptrend<0.001). Compared to the first quartile, the risk of SAP increased 9.991 times in the fourth quartile (95% confidence interval 2.912-34.279; P<0.001). ROC curve analysis showed that the area under the curve of NLR for SAP prediction was 0.793 (95% confidence interval 0.737-0.850), with an optimal cutoff value of 5.475. The sensitivity and specificity for predicting SAP were 66.67% and 79.82%, respectively. Conclusion:NLR at admission is an independent risk factor for SAP in patients with AIS and has certain predictive value for SAP.
7.Qualitative research on oncology nurses' working experience and occupational stability

Objective To discuss oncology nurses' true feelings and working experience,so as to investigate the influencing factors of their occupational stability and clinical nursing quality.Methods Qualitative research methods were adopted and semi-structured open-ended questionnaire was used to conduct individual in-depth interviews with 16 oncology nurses from Level 3 of 1st class hospital,and analysis methods of qualitative contents were used for data analysis.Results Reasons for not wanting leave Oncology Department included:working environment,familiarity with the working process,good protective measures,harmonic colleague relationship,not wanting children for now and worries about not adapting to the new department.The influencing factors of oncology nurses' occupational stability included:drug damage,working environment,hard work,bad colleague relationship,no special treatment and so on.Conclusions Corresponding measures should be taken according to the influencing factors of nurses' instability,so that nurses can work for the patients without concerns.
8.Clinical Study of Jinmaitong Composita(复方筋脉通) on Diabetic Peripheral Neuropathy

Objective: To verify the effect of Jinmaitong composita (JMTC) on red blood cell aldose reductase activity (RBC-AR), red blood cell sorbitol (RBC-S) and nerve conductive velocity in diabetic peripheral neuropathy (DN) patients. Methods: Sixty-six patients with DN were randomly divided into two groups, 33 patients in the treated group treated with JMTC and 33 patients in the control group treated with Jingui Shenqi capsule (JGSQ). RBC-AR, RBC-S and nerve transmission speed were observed before and after three months treatment.Results: Level of RBC-AR, RBC-S apparently decreased and nerve conductive velocity increased (P<0.05, P<0.01) after JMTC treatment.Conclusion: JMTC can improve the nerve conductive velocity significantly with a lowering of RBC-AR and RBC-S and has a good result in treating diabetic peripheral neuropathy.
9.Application of the best evidence for screening and evaluating cancer-related fatigue in patients with lung cancer undergoing chemotherapy

Objective:To summarize the best evidences for screening and evaluation of cancer-related fatigue in patients with lung cancer undergoing chemotherapy and apply them to clinical nursing practice, so as to improve clinical nursing service, reduce the level of cancer-related fatigue and improve the quality of life of patients.Methods:Using the convenient sampling method, a total of 48 patients with lung cancer undergoing chemotherapy who were admitted to Department of Respiratory and Critical Care Medicine in Shandong Provincial Hospital Affiliated to Shandong First Medical University from May 6 to June 28, 2019 were selected as the control group, while 50 patients who were admitted from September 2 to October 30, 2019 were selected as the observation group. Joanna Briggs Institute (JBI) Practical Application of Clinical Evidence System (PACES) was used as a review tool to screen the detection rate of cancer-related fatigue of patients after the application of evidence and investigate the implementation rate of nurses' review standards. The scores of Nurses' Knowledge, Attitude and Practice Questionnaire on Cancer-related Fatigue Management and the Chinese version of European Organization for Research and Treatment Quality of Life Questionnaire-Core 30 V3.0, EORTC-C30 V3.0 (EORTC-C30 V3.0) were compared before and after the use of evidence. The t test was used to compare the measurement data between the two groups, and the χ 2 test or Fisher's exact test was used to compare the enumeration data. Results:The implementation rates of review indicators after the application of evidence reached 86% (43/50) to 100% (50/50) . After application of the evidence, scores of knowledge, attitude and practice dimensions of Nurses' Knowledge, Attitude and Practice Questionnaire on Cancer-related Fatigue Management were respectively (99.50±1.37) , (39.06±1.00) and (30.69±1.08) , which were all higher than those before the evidence was applied, and the differences were statistically significant ( t=-16.023, -7.258, -19.769; P<0.05) . The detection rates of mild, moderate and severe fatigue in the observation group were 52% (26/50) , 26% (13/50) and 6% (3/50) , respectively. The scores of body, role, cognitive, emotional function and general health status in the Chinese version of EORTC-C30 V3.0 in the observation group were higher than those in the control group, while the score of fatigue dimension was lower than that in the control group, and the differences were statistically significant ( t=-3.193, -2.487, -3.743, -2.202, -1.067, -3.100, 3.273; P<0.05) . Conclusions:The best evidence for screening and evaluation of cancer-related fatigue in patients undergoing chemotherapy for lung cancer can provide evidence-based evidence for clinical fatigue management, and its clinical application can improve the identification and management of cancer-related fatigue, standardize nurse behavior, and improve the quality of life of patients.
10.Risk factors of slow blood flow or no reflow in STEMI patients after emergency PCI and their prognosis

Objective To analyze the clinical characteristics and prognosis of patients with coronary slow flow(CSF)or no reflow(CNR)after emergency PCI.Methods A total of 443 elderly STEMI patients who underwent emergency PCI in Department of Cardiology of the First Affilia-ted Hospital of Tsinghua University from January 2017 to August 2023 were recruited,and ac-cording to thrombolysis in myocardial infarction(TIMI)flow grading after operation,they were divided into CSF/CNR group(TIMI grade≤2 flow,n=40)and normal flow(NCF)group(TIMI grade 3 flow,n=403).Logistic regression analysis was used to analyze the risk factors for CSF/CNR in STEMI patients after PCI.The prognosis was analyzed in the two groups.Results The CSF/CNR group had advanced age,longer durations from onset to admission and from onset to balloon opening,higher mortality rate,and larger proportions of chronic heart failure(CHF),type 2 diabetes mellitus(T2DM),pulse rate>100 times/min,SBP<100 mm Hg(1 mm Hg=0.133 kPa),Killip grade>1,left anterior descending artery disease,LVEF<50%,and using glycopro-tein Ⅱb/Ⅲa inhibitors and intraaortic balloon counterpulsation,but lower male ratio and DBP when compared with the control group(P<0.05,P<0.01).Multiple logistic regression analysis revealed that T2DM(OR=1.320,95%CI:0.618-2.123,P=0.046),mean SBP(OR=1.239,95%CI:0.752-1.525,P=0.04)and SBP<100 mm Hg(OR=1.330,95%CI:0.836-1.675,P=0.013)were independent risk factors,while male was a protective factor(OR=-1.755,95%CI:-2.338--1.171,P=0.002)for CSF/CNR in STEMI patients after PCI.Conclusion T2DM and lower SBP are independent risk factors,while male is a protective factor for CSF/CNR in STEMI patients after emergency PCI.
