1.Effects of neonatal capsaicin treatment on stress induced analgesia and hyperalgesia in Tail-Flick test in male rats

Background & Objective: It is reported that acute forced swimming stress induces analgesia immediately,
and chronic stress induces hyperalgesia. Whereas in response to nociceptive stimulation, small-diameter
C-fibers of the excitatory system in the dorsal horn of the spinal cord are activated, therefore, in
the present study, the effects of C-fiber lesion in stress and dexamethasone-induced analgesia and
hyperalgesia in acute and chronic forms were investigated using Tail-Flick test. Methods: Adults Wistar
male rats (180-200 g) were assigned into three groups (n=7): C-normal (intact C-fibers), sham (received
capsaicin vehicle at neonate stage) and C-lesion (received capsaicin at neonate stage). Forced swim
stress (10 min/day) in water (18±1 ºC) was considered as acute stress and repeated daily forced swim
stress as chronic stress, also single-dose of dexamethasone (2 mg/kg, i.p.) was considered as acute
dexamethasone and repeated for three days as chronic dexamethasone. Neonatal capsaicin treatment
was used for C-fibers depletion. The nociceptive thermal threshold was assessed using Tail-Flick test.
Results: In C-lesion group, thermal pain sensitivity was reduced (P<0.001). Acute stress in C-normal
group, reduced pain (P<0.001) and in C-lesion group, it caused deeper antinociception in Tail-Flick
(P<0.001). Chronic stress and acute-chronic dexamethasone in C-normal group, created hyperalgesia
(P<0.001) and induced analgesia in C-lesion groups (P<0.01).
Conclusion: It seems that presence of C-fiber is so important in thermal pain transmission in Tail-Flick
test; therefore, C-fiber lesion, reduces pain sensitivity (analgesia), increases antinociception effects of
acute stress, decreases hyperalgesia of chronic-stress and acute-chronic dexamethasone

2.Sensitization to common allergens among patients with allergies in major Iranian cities: a systematic review and meta-analysis

Various allergens are implicated in the pathogenesis of allergic diseases in different regions. This study attempted to identify the most common allergens among patients with allergies based on the results of skin prick tests in different parts of Iran. Relevant studies conducted from 2000 to 2016 were identified from the MEDLINE database. Six common groups of allergen types, including animal, cockroach, food, fungus, house dust mite, and pollen were considered. Subgroup analysis was performed to determine the prevalence of each type of allergen. The Egger test was used to assess publication bias. We included 44 studies in this meta-analysis. The overall prevalence of positive skin test results for at least one allergen was estimated to be 59% in patients with allergies in various parts of Iran. The number of patients was 11,646 (56% male and 44% female), with a mean age of 17.46±11.12 years. The most common allergen sources were pollen (47.0%), mites (35.2%), and food (15.3%). The prevalence of sensitization to food and cockroach allergens among children was greater than among adults. Pollen is the most common allergen sensitization in cities of Iran with a warm and dry climate; however, sensitization to house dust mites is predominant in northern and southern coastal areas of Iran.

4.Prevalence of Strongyloides stercoralis and other intestinal parasitic infections among mentally retarded residents in central institution of southern Iran.

OBJECTIVETo determine the prevalence of intestinal parasitic infections among mentally retarded residents of rehabilitation center of Bandar Abbas, Hormozgan province, southern Iran.
METHODSA cross-sectional study was carried out in central rehabilitation institute of Hormozgan province in summer 2010. Fecal samples of all 133 residents (72 males, 61 females) aged 3-52, were collected in triplicate. Specimens were examined by direct smear, formalin-ether concentration techniques and stained by permanent Trichrome, Ziehl-Neelsen stains. Statistical analysis was conducted by SPSS 13.5.
RESULTSIntestinal parasitic infections were seen in 48.5% (64 out of 133 subjects: 53.4% in males and 46.6% in females). Strongyloides stercoralis with 17.3% showed the highest incidence followed by Entamoeba coli (9.8%), Blastocystis hominis (7.5%), Giardia lamblia (2.3%), Endolimax nana (2.3%), Hymenolepis nana (0.8%), Oxyuris vermicularis (0.8%), and Chilomasix mesnili (0.8%). Double infections were found to be as: Strongyloides stercoralis + Giardia lamblia (2.3%), Entamoeba coli + Giardia lamblia (1.5%), Entamoeba coli + Blastocystis hominis (1.5%), Oxyuris vermicularis + Entamoeba coli (0.8%), Strongyloides stercoralis + Entamoeba coli (0.8%), respectively.
CONCLUSIONSOur findings reveal that strongyloidiasis is a common disease among mentally retarded population in southern Iran.

5.The Effects of Air Pollution on Ischemic Stroke Admission Rate

The present study aimed to determine the relationship between the level of air pollutants and the rate of ischemic stroke (IS) admissions to hospitals. In this retrospective cross-sectional study, stroke admissions (January-March 2012 and 2013) to an emergency department and air pollution and meteorological data were gathered. The relationship between air pollutant levels and hospital admission rates were evaluated using the generalize additive model. In all 379 patients with IS were referred to the hospital (52.5% male; mean age 68.2+/-13.3 years). Both transient (p<0.001) and long-term (p<0.001) rises in CO level increases the risk of IS. Increased weekly (p<0.001) and monthly (p<0.001) average O3 levels amplifies this risk, while a transient increase in NO2 (p<0.001) and SO2 (p<0.001) levels has the same effect. Long-term changes in PM(10) (p<0.001) and PM(2.5) (p<0.001) also increase the risk of IS. The findings showed that the level of air pollutants directly correlates with the number of stroke admissions to the emergency department.

6.Correlation between cystatin-C, acute phase reactants, and retinopathy severity in diabetic patients

·AIM: To evaluate correlation of cystatin-C (Cys-C) with severity of diabetic retinopathy (DR) and acute phase reactants, including erythrocyte sedimentation rate (ESR) and high-sensitivity C-reactive protein(hs-CRP). ·METHODS: All diabetic patients who were referred for diabetic retinopathy (DR) screening during 1mo were enrolled. Demographic data were recorded. All patients have undergone full ophthalmic exam. At the same day, all patients were tested for hemoglobin A1c (HbA1c), ESR,hs-CRP,and Cys-C serum levels. · RESULTS: Sixty seven diabetics were enrolled, including 19 (28.3%) without retinopathy, 22 (32.8%) non - proliferative retinopathy, and 26 ( 38. 8%) proliferative retinopathy patients. The mean age, sex distribution, mean duration of diabetes, prevalence of hypertension and dyslipidemia, smoking status and HbA1c levels were not significantly different among the three groups. The mean levels of Cys - C increase significantly as retinopathy progress [1.1 ± 0. 48; 1. 22 ± 0.38;1.71 ± 0.92 (P=0.007), respectively]. In multiple regression analysis, just Cys - C was significantly associated with severity of DR (P=0.025). ·CONCLUSIONS: This study revealed that serum levels of Cys-C increase while DR progress independently of acute phase reactants. Therefore, it could be used as an associated marker by primary care physicians to distinguish patients at higher risk of severe DR. Larger randomized studies are warranted to confirm findings. Reviewing physiological role of the Cys-C, we proposed that the Cys-C may be a protective response to catalytic stress rather than being a pathogenic factor in microangiopathies.
7. Analysis of temporal trends of human brucellosis between 2013 and 2018 in Yazd Province, Iran to predict future trends in incidence: A time-series study using ARIMA model

Objective: To determine the temporal patterns of cumulative incidence of brucellosis using autoregressive integrated moving average models. Methods: This cross-sectional study employed yearly and monthly data of 1 117 laboratory-confirmed human brucellosis cases from January 2013 to December 2018 using the Yazd brucellosis national surveillance system. The monthly incidences constructed a timeseries model. The trend of cumulative incidence was perceived by tracing a line plot, which displayed a seasonal trend with periodicity. Thus, the ARIMA models were selected. Thereafter, Akaike information criteria (AIC) and Bayesian information criterion (BIC) values among different models indicated a preferable model from models which were expanded by diverse lags [(3, 0, 3), (2, 0, 3), (3, 0, 2), (4, 0, 3) and (3, 0, 4)]. Then, the achieved ARIMA model was applied to the forecasting cumulative incidence of monthly brucellosis incidences. All analyses were performed using Stata, version 11.2. Results: For the ARIMA (3, 0, 4) model, MAPE value was 56.20% with standard error 0.009-0.016, and white noise diagnostic check (Q=19.79, P=0.975) for the residuals of the selected model showed that the data were completely modelled. The monthly incidences that were fitted by the ARIMA (3, 0, 4) model, with AIC (25.7) and BIC (43.35) with a similar pattern of actual cases from 2013 to 2018 and forecasting incidences from January 2019 to December 2019 were, respectively, 0.50, 0.44, 0.45, 0.49, 0.55, 0.58, 0.56, 0.51, 0.46, 0.44, 0.45 and 0.49 per 100 000 people. Conclusions: In summary, the study showed that the ARIMA (3, 0, 4) model can be applied to forecast human brucellosis patterns in Yazd province, supplementing present surveillance systems, and may be better for health policy-makers and planners.

Objective To evaluate the overall prevalence of Toxoplasma gondii infection among birds and poultries in Iran. Methods Data were systematically collected from 1983 to 2016 in Iran on the following electronic databases: PubMed, Google Scholar, Science Direct, Scopus, Web of Science, Magiran, Irandoc, IranMedex, and Scientific Information Database. Additionally, the abstracts of national scientific congresses and dissertations were included. Results A total of 20 articles in the field of birds and poultries toxoplasmosis, totally examining 4563 cases with 754 positive results reporting the overall prevalence of infection from all parts of Iran could fulfill our eligibility criteria. The overall estimated prevalence included in chicken 20% (95% CI: 3%–38%) in chicken, pigeons 8% (95% CI: −17%–33%) and in sparrows 15% (95% CI: −25%–54%). Conclusion Although there is a lack in data about poultries and birds toxoplasmosis in Iran, our meta-analysis revealed that infection rate is high among birds and poultries in Iran. More studies are needed to manage controlling programs and prevention strategies among poultries in Iran.
