1.Incidental gallbladder opacification after intravascular contrast infusion.

Opacification of the gallbladder after parenteral administration of contrast material usually indicates diminished renal function but can occur with normal renal function. The authors recently experienced 13 cases of such vicarious excretion of contrast media. Among 13 patients. 9 patients had renal disease unilateral ureteral stone(n=3), staghorn calculi(n=2), acute renal failure from sepsis(n=1), renal contusion(n=2) and unilateral renal artery involvement by dissecting aortic aneurysm(n=1). Of these 9 patients, 3 patients showed abnormal serum creatinine levels at the time of presentation or shortly thereafter. In 6 patients, injection of a large amount of contrast media was possibly the additional cause of vicarius excretion. There was no detectable cause in the remaining 4 patients. Heterotopic excretion of contrast media is clearly a complex phenomenon, the reason for which it is difficult to establish with certainty in each individual case. Anyhow, vicarius excretion of contrast media occurs more frequently than previously thought, and it can occur not only with abnormal renal function but with normal function also.

2.Visual outcomes and operative complications following cataract surgery in Sorokdo Hansen patients.

PURPOSE: To evaluate the visual outcomes and operative complications following cataract surgery in Sorokdo leprosy patients. METHODS: Forty three eyes of 35 leprosy patients with cataract underwent phacoemulsification and posterior chamber intraocular lens implantation surgeries. Eyes with small pupil and insufficient pupildilatation were treated with iris retractor and iris sphincterotmy. RESULTS: The mean age of patients was 71.0 +/- 8.2 years and the mean follow up was 4.5 +/- 2.0 months. The mean visual acuity significantly increased from 0.34 +/- 0.23 to 0.73 +/- 0.21 (p<0.05). Sixteen eyes (38%) gain 5 or more lines and 35 eyes (83%) gain 2 or more lines in corrected visual acuity. Common intraoperative events were 8 (19%) iris retractor use and 6 (14%) posterior capsular tear. CONCLUSIONS: Phacoemulsification and posterior chamber intraocular lens implantation surgeries in aged Sorokdo Hansen patients were safe and efficient to improve visual quality and life expectations.

3.p53 mutation in patients with ulcerative colitis in rectal biopsy.

OBJECTIVES: Long standing ulcerative colitis (UC) has been known to be one of the precancerous diseases of colorectal cancer. Although the frequent loss of p53 allele (LOH) and aneuploidy were reported as the molecular events in carcinoma and dysplasia known as the precursor of UC, p53 genetic alteration was not reported in indefinite dysplasia and UC involved mucosa in long standing UC. Therefore, we investigated the mutational inactivation of the p53 gene in UC patients who showed dysplastic mucosa, as well as non-dysplastic mucosa on H & E stain and, secondly, if there is p53 mutation, we examined the relationship between p53 alteration and clinical data. METHOD: Sixteen patients with UC who had different duration of colitis were studied by endoscopic examination with rectal mucosal biopsies, p53 gene alterations were detected by PCR-SSCP for exon 4-8 and immunohistochemical staining with p53 monoclonal antibody. RESULTS: Among 16 patients, 2 patients (12%) showed dysplasia on H-E stain. The p53 point mutations were detected in 4 (two dysplasia and 2 normal looking mucosa) on PCR-SSCP. 4 patients who had p53 gene mutation were positive in immunohistochemical staining. With regard to clinical characteristics, these patients with p53 point mutation showed poor response to medical treatment. CONCLUSION: These results suggest that the p53 mutation may be an early molecular event of cancerous change in UC.

4.Regrowth of Grafted Lower Half of the Hair Follicle.

We have examined the regenerative capabilities of the human scalp hair follicle after grafting the lower half of the follicle. Twenty-eight of 32 intact whole-hair follicles isolated from the human scalp regenerated hairs when grafted onto the forehead of the same person. Seven of the 15 lower-half follicles regenerated complete hair follicles 8 months after grafting showed that the lower-half follicle implant reconstituted the complete hair follicle. The sebaceous gland was not regenerated, but there was an outgrowth in the sebaceous gland regPark ion. Some grafts formed epithelial cysts. Two years after grafting, the histological examination of the regenerated follicle from the lower-half implant showed that the sebaceous gland was completely regenerated. While an intact follicle shows prominent naked shaft outgPark Park Parkrowth, the sheath grows concomitantly with the shaft in lower-half follicles in culture. If grafted lower-half follicles were located too deep, the regrown sheath could not reach the epidermal layer. In this situation, the formation of an epidermal cyst was likely.

5.Expression of Protein Kinase C Isoform mRNAs in the Developing Rat Heart.

BACKGROUND AND OBJECTIVES: Protein kinase C (PKC) is known to be related with development of various cells. In the heart, each isoform reacts differentially against agonists and the reaction changes during development. In this study, the roles of PKC isoforms (alpha, beta, gamma, delta, epsilon, zeta) were investigated through the localization of mRNA expression in the developing rat heart with in situ hybridization histochemistry. MATERIALS AND METHOD: The mRNA expression pattern of PKC isoforms (alpha, beta, gamma, delta, epsilon, zeta) was investigated with in situ hybridization histochemistry in developing and adult rat hearts. Whole body parasagittal sections were used for embryonal day 14 (E14), E16, E18 and heart sections were used for just born (P0), postnatal day 7 (P7), P14, P21 and adult rat. RESULTS: The expression of PKC alpha was found from E14, peaked at P7, and gradually decreased to adult level. The expression of PKC beta was observed from P14, peaked at P21, and decreased to adult level. The expression of PKC delta in the heart was observed from E14, peaked at P0, and abruptly disappeared at P14. The expression of PKC epsilon was observed from E14, peaked at P0, after that gradually decreased and disappeared at adult rat heart. The expression of PKC gamma and zeta was not found from any stage of developing rat heart. CONCLUSION: From these results, it is suspected that each PKC isoform may be differentially related with development of heart. The strong expression of PKC alpha, delta, epsilon around perinatal period, rapidly developing stage, suggests that PKC alpha, delta, epsilon may be related with rapid development of rat heart. And the late postnatal expression of PKC beta suggests that PKC beta may be related with maturation of rat heart.

6.A Cause of Erectile Impotence: Subclinical Penile Cavernosal Fibrosis.

Histological findings in corpus cavernosum were retrospectively evaluated in 36 patients with erectile impotence. Biopsies were obtained during insertion of penile prosthesis from both corpora. The most prominent histological findings were fibrosis(63.9%) and hyalinization(50%). Peyronie`s disease was not clinically diagnosed in any patient. Our findings suggest that clinical penile cavernosal fibrosis is common and may be a significant etiologital factor in patients with erectile impotence.

8.Effects of Orally Administered Baclofen in the Animal Model for Neuropathic Pain.

The aim of this study was to compare with the effects of baclofen using an animal model of neuropathic pain. The sciatic nerve of rats was ligated unilaterally about dorsal half-portion in the tight according to the method of Seltzer and his colleague. After surgical operation, the rats showed painful symptoms of the ipsilateral hind paw, suggesting the possibility of spontaneous pain. And then, the paw withdrawal latency to the local heating on the paw through the glass plate and the frequency of paw withdrawal response to innocuous mechanical stimulation with modified von Frey filaments were determined to compare with the effects of pre-and post-medication of baclofen, respectively, at postoperative 3, 7, and 10 days. The results obtained were as follows: 1) The thermal hyperalgesia and mechanical allodynia produced by partially tight ligation of sciatic nerve appeared continously postoperative 3 days later. 2) In the hyperesthetic rats, the thermal hyperalgesia was inhibited from the 3rd posroperative day with orally administered baclofen 0.2mg and 1.0mg. 3) In the hyperesthetic rats, the mechanical allodynia was inhibited with baclofen 0.2mg, but not with baclofen 1.0mg, These results suggest that baclofen have more specific effects on thermal hyperalgesia than mechanical allodynia.

9.An experimental study on the effect of acidity of papaverine HCl on fibrosis of the corpus cavernosum of rabbit.

This study was designed to investigate whether fibrosis of the corpus cavernoum(CC) induced by repeated intracavernous injection of papaverine HCl was related to pH of the drug. Forty-eight New Zealand White rabbits weighing 2.5kg-3.0kg were divided into 4 groups of 12 animals each. Group 1 received isotonic saline. Group 2; pH 2.0 papaverine HCl. Group 3; pH 3.0 papaverine HCl and Group 4; pH 4.0 papaverine HCl. The solution (0.1ml isotonic saline or 2mg. papaverine HCl of seeh pH) was injected intracorporeally twice every week for 6 months. Every one month since beginning of the injection, 2 animals of each group were sacrificed. the penis was observed with H and E and Masson`s lrichomme stain microscopically. No sign of Fibrosis was found in the CC of the group l even after 6months of injection. However, Group 2. 3 and 4 began to show focal of diffuse thin fibrotic changes after 2 months of injection. The fibrotic changes progressed more and more in accordance with the Frequency of papaverine HCl injection. After 4 months of injection. Group 2 showed diffuse thick fibrosis whereas Group 3 and 4 showed diffuse thin or focal thick fibrosis. In conclusion, intracavernous papaverine itself seems to play a main role to induce fibrosis of CC. However, the acidity of the papaverine HCl also might be responsible to development of the fibrosis when injected repeatedly for a long period.

10.Vestibular Neurectomy in the Treatment of Intractable Peripheral Vertigo: Case Report.

Vestibular neurectomy is known as an effective procedure in the management of intractable peripheral vertigo from Meniere's disease and other episodic peripheral vertigo disorders. Various approaches have been developed for selectively sectioning the vestibular nerve, in order to preserve hearing and avoid facial nerve injury. Vestibular neurectomy is performed in two patients with Meniere's disease to control intractable episodic vertigo through retrolabyrinthine approach. Vertigo was improved with preserving their hearing. We report the surgical technique and advantages of retrolabyrinthine vestibular neurectomy in the treatment of vertigo.
