1.Prognostic Indicators in Tibial Shaft Fractures ; Serum Creatine Kinase Acticity

Serum creatine phosphokinase(CPK) levels were measured serially in 20 adults with closed tibial shaft fractures, CPK activity increased significantly after fracture(p < 0.01). High energy and delayed healing fractures had significantly higher levels of CPK than low energy and normally healing fractures, respectively. Thus, CPK determinations could be used to assess the severity of trauma and possible prognosis of the healing in tibial shaft fractures.

3.Clinical Study of 459 Cases Cardiovascular Surgery.

Between April, 1984 and September 1988, 459 patients underwent cardiovascular surgery at the Yeungnam University Hospital. Of these, 355 cases were open heart surgeries and 104 cases were non-open heart surgeries. There were 237 patients of acyanotic congenital cardiac anomalies, 40 patients of cyanotic congenital cardiac anomalies, and 85 patients of acquired heart diseases. The sex ratio of cardiovascular diseases was represented as 1:1.3 in male and female. The age distribution was ranged from 1 day to 65 years old. The common congenital cardiovascular anomalies were ventricular septal defect (38.7%), patent ductus arteriosus (25.5%), atrial septal defect (20.7%), Tetralogy of Fallot (8.3%), and pulmonary stenosis (2.4%) in order of frequency. Among 87 acquired cardiovascular diseases, 81 patients underwent operation for cardiac valvular lesions, 51 patients had mitral valve replacement and 13 patients had aortic valve replacement and 17 patients had double valve replacement. The overall mortality of cardiovascular surgery was 3.3% and mortality of open heart surgery was 3.9%.

4.A Clinical Study of the Tibial Condylar Fracture

The tibial condylar fracture is a fracture of the proximal end of the tibia, involving the srticular surface. This fracture frequently accompanied by soft tissue injuries such as collateral ligsment, cruciate ligament and menisci. Thirty cases of the tibial condylar fracture which treated Inchon Choong Ang Gill Hospital during the period from March, 1983 to February, 1986, the thirty cases could be followed for one year to four years. They have been analyzed according to its cause, classification, treatment and result. Among these 12 cases(40.0%) were treated by cast immobilization, 1 case(3.3%) by skeletal traction, 17 cases(56.7%) by open reduction and internal fixation. Twenty-three cases(76.7%) out of thirty revealed the rating of ACCEPTABLE according to Hohl and Lucks criteria.

5.Essential Osteolysis of Carpal and Tarsal Bones: A Case Report

Essential osteolysis is a rare, progressive, slow and bone-resorptive disease that mainly involves carpal bone, metacarpal bone, tarsal bone, metatarsal bone and elbow joint. Authors had recently experienced one case of “essential osteolysis” that had involved the bilateral carpal, metacarpal, bilateral tarsal, metatarsal bone, elbow and ankle joint without associated renal insufficiency, hypertension and significant familial history.

7.Role of Nitric Oxide in Leukocyte-Endothelial Interaction in Cerebral Venules during Reperfusion after Global Ischemia.

OBJECTIVE: Reactive oxygen metabolites and polymorphonuclear leukocytes have been implicated in the pathophysiology of reperfusion injury. The mechanisms involved in superoxide-mediated leukocyte adherence remain unclear, however, nitric oxide(NO) may contribute to this response. The present study is undertaken to elucidate mechamisms controlling NO based mechanisms that regulated leukocyte-endothelial interactions in the cerebral vasculature after global cerebral ischemia and reperfusion. METHODS: Pial venular leukocyte adherence of anesthetized newborn piglets was quantified by in situ fluorescence videomicroscopy through closed cranial windows during basal conditions and during 2hours of reperfusion after global ischemia induced by 9minutes of asphyxia. Nitric oxide synthase(NOS) was inhibited by local window superfusion of L-nitroarginine(NA); superfusion of sodium nitroprusside(SNP) was used to donate NO. RESULTS: The mean number of adherent leukocytes to cerebral venules in the 9minutes asphyxia and 2hours reperfusion group were 161+/-19 compared with 13+/-4 in the nonasphyxial group. Superfusion of L-NA through the cranial window for 2hours resulted in leukocyte adherence similar to that observed during the initial 2hours of reperfusion after asphyxia. Leukocyte adherence was not additionally increased in asphyxic animal treated with L-NA. SNP inhibited asphyxia induced leukocyte adherence back to control levels. CONCLUSIONS: Nitric oxide inhibits leukocyte adherence to cerebral venules during the initial hours of reperfusion after asphyxia, and that NO supplementation inhibit asphyxia induced leukocyte adherence back to control levels. These results indicate that NO is an important factor in ischemia-reperfusion induced leukocyte adherence.

9.Chiari Type I Malformation in a Child with Syringomyelia and Scoliosis: A case report.

The association between pediatric Chiari malformation and the development of syringomyelia has been well documented. Scoliosis in the patient with syringomyelia is thought to be secondary to anterior horn cell damage, which innervate the muscles of trunk, by an asymmetrically expanded syrinx. In pediatric patients, the neurologic signs and symptoms due to Chiari malformation and syringomyelia show much lower frequency but the incidence of scoliosis is very high. Thus, the MRI study for the diagnosis of the underlying syringomyelia and Chiari malfornation is essential in pediatric scoliosis patients, which may otherwise be misdiagnosed for idiopathic scoliosis. We present a case of Chiari type I malformation associated with syringomyelia and scoliosis.

10.A 3 year prospective study of survival rate of narrow diameter implants

PURPOSE: The purpose of present study was to retrospectively analyze the survival rate of narrow diameter implant less than 3.6 mm by initial stability and radiographic measurements. MATERIALS AND METHODS: In total, 24 patients who received 38 narrow diameter implants (≤ 3.6 mm in diameter, ≥ 7 mm in length) were enrolled in this retrospective study. The cumulative survival rate was calculated and various factors were investigated according to the implant platform diameter, body diameter, length, position, concomitant use of guided bone regeneration in implant placement and final prosthesis type. Initial stability was investigated with implant stability quotient (ISQ) value. The mesial and distal marginal bone level (MBL) change was calculated with radiography. RESULTS: The overall survival rate was 92.11%. Mean ISQ value and MBL change of survival implants was 66.26 and 0.14 ± 0.31 mm, respectively. None of the implants with platform diameters larger than the body diameter failed. CONCLUSION: In conclusion, the findings of present study suggest that narrow diameter implant could be predictable treatment in narrow alveolar ridge.
