1.A Clinical Observation on Tumors of the Renal Pelvis and Ureter.

Primary tumor of the renal pelvis and ureter is relatively rare disease, but usually malignant. It's prognosis is dependent upon the early detection, accurate diagnosis, procedure for treatment, and tumor grade and stage. Therefore, urologist should endeavor after accurate diagnosis and definitive treatment. A clinical observation was done on the 11 patients with renal pelvic tumor and the 3 patients with ureteral tumor at the Department of Urology, Kyungpook National University Hospital during the period of 4 years from January, 1978 to December, 1981. The results were as follows: 1. There were 9 men and 5 women, giving a ratio of 1.8 to 1. The average age at presentation of the 14 patients was 51.4 years. 2. On excretory urogram, nonvisualization of the kidney was the most prevalent sign in 10 patients. Retrograde pyelogram was done in 11 patients in whom findings of IVP were not sufficient for diagnosis. Renal angi0graphy was done in 9 patients. 3. Urine cytology was positive in 2 of 14 patients. 4. Of the 13 patients treated, 12 were proved to have transitional cell carcinoma, and 1 was leiomyoma. In 3 of the 11 patients with renal pelvic tumor, Concomitant ureteral tumor was present at diagnosis. And in 2 of the 11 patients with renal pelvic tumor developed subsequent urothelial tumors in the ureter and/or bladder after simple nephrectomy. 5. Nephroureterectomy including resection of a bladder cuff was done in 11 patients, and in 2 of these was also performed lymphadenectomy. Simple nephrectomy was done in 2 patients with renal pelvic tumor under the misdiagnosis of renaltuberculosis. 6. Postoperative complications developed in 3 cases, wound infection, pneumothorax, and death, in each case.

3.Topographic Measurement of the Optic Nerve Head with Confocal Scanning Laser Tomography in Normal Third Decade of Korean.

In glaucoma patients, optic nerve head analysis is very useful in early detection and progression during follow-up. Among optic nerve head analysis methods, the confocal laser tomographic scanner(Heidelberg Retina Tomograph: HRT) is actively studied for accurate and highly reproducible analysis.We analyzed the optic nerve head of 62 normal eyes(male 40 eyes, female 22 eyes) in third decade of Korean with HRT, and calculated the mean value of each parameters in HRT and evaluated the difference between men and women. We also studied the distribution fo the value of each parameter in normal third decade of Korean. In addition, for the parameters of Rim/Disc area ratio, we calculated the mean value in 6 divisions of optic nerve head.The mean value of Disc area was 2.039mm3, Cup area 0.593mm3, rim area 1.716mm3 and Cup/Disc area ratio 0.237. Rim volume, Maximal cup depth, Height variation contour, Mean retinal nerve fiber layer thickness, and Retinal nerve fiber layer cross sectional area were statistically different between men and women. For Rim/Disc area ratio, the maximal value was noted in nasal inferior(0.873) region, and the minimal value was in temporal(0.579) region. There was no statistically difference between men and women in Rim/Disc area ratio.

4.The Effect of Succinylcholine on the Intracranial Pressure in Cats.

Succinylcholine (1.5mg/kg) without pretreatment, or succinylcholine (1.5mg/kg) with pretreatment by vecuronium (0.015 mg/kg, 0.045 mg/kg, 0.15 mg/kg) or d-tubocurarine (0.1 mg/kg, 0. 3 mg/ kg, 1mg/kg) was given to cats of the same genus (body weight 2.5-3.5kg) under subcutaneous urethane anesthesia to determine the effect of the intracrainal pressure increase in each method. The results are summarized as follows: 1) The intracranial pressure was significantly increased (0.005< p< 0.01)with the administration of succinylcholine(1.5mg/kg). 2)The intracranial pressure was not increased with the administration of vecuronium. 3)As the dosage of d-tubocuraine administration increased, the intracranial pressure was increased. 4)The increase of the intracranial pressure by succinylcholine was depressed dose-dependently by vecuronium pretreament. 5)The increase of the intracranial pressure by succinylcholine was depressed by d-tubocurarine(0.3mg/kg)pretreatment.

5.A Clinical Observation on 171 Nephrectomized Patients.

In an effort to study the underlying diseases, management and postoperative complications of nephrectomized patients, author analyzed the records of 171 nephrectomized patients who were treated at the Department of Urology, Kyungpook National University Hospital during the past 10 years. The results were as followers: 1. These 171 nephrectomized patients revealed giving rate of 14.9% to the total urologic operations during above periods. 2. The most frequent year distribution was 1981, being 31 patients, followed in frequency by 1980 in 29 and 1979 in 25. Over half of nephrectomized patients were seen since 1979. 3. The most commonly involved age group was the 3rd decade (24.6%), followed in decreasing order of frequency by 6th decade (18.1%) and 4th decade (17.5%). Sex ratio. male to female, was 1.9:1. 4. The most common causative disease was the tumor comprising 43 patients (25.1%) out of 171 nephrectomized patients, followed in order of frequency by renal injury in 40(23.4%), renal tuberculosis in 33(19.3%), renal infection in 21(12.3%) and hydronephrosis in 16(9.4%). 5. The most frequently employed surgical approach of nephrectomy was through translumbar (64.3%), followed by transperitoneal (30.4%), thoracoabdominal (4.7%) and posterior lumbar (0.6%). The translumbar approach was undertaken nearly in all nephrectomized patients except transperitoneal approach in 18 tumor patients, thoracoabdominal in tumor patients and transperitoneal approach in 26 renal injury patients. 6. Postoperative complications were developed in 21 patients and 2 of these were died. 7. One hundred and forty one patients (82.5%) were hospitalized between 8 and 21 days, 24 over 22 days and 6 below 7 days.

6.Correlation of Peripapillary Atrophy with Optic Disc Cupping and Disc Hemorrhage in Primary Open Angle Glaucoma.

To investigate the correlation of peripapillary atrophy with optic disc dupping and disc hemorrhage in primary open angle glaucoma(POAG) patients, tomographic measurements were performed with Heidelberg Retina Tomograph(HRT, Heidelberg Engineering, Germany). We analyzed cup to disc area ratio(C/D ratio) and peripapillary atrophy to disc area ratio(P/D ratio) from 28 eyes of 14 POAG patients with peripapillary atrophy(Group A), only C/D ratio from 28 eyes of 14 POAG patients without peripapillary atrophy(Group B), and only P/D ratio from 28 eyes of non-glaucomatous subjects with peripapillary atrophy(Group C). Group A was subclassified into thos with disc hemorrhage (Group A-1) and those without dixd hemorrhage(Group A-2), and changes of visual field defects were also analyzed from each subgroup witn Humphrey automated perimetry (Allergan Humphrey, USA, C24-2) at the six-month follow-up. Cup to disc area ratio(0.54+/-0.160)in the Group A was signifcantly higher than that(0.406+/-0.130) in the Group B, and P/D ratio was higher in Group A(0.619+/-0.203) than in the Group C(0.484+/-0.130). P/D ratio was also higher in Group A-1(0.725+/-0.34) than in the Group A-2(0.605+/-0.273). There was no significant difference in mean deviation(M/D) and corrected pattern standard deviation(CPSD) between Group A-1 and Group A-2. The results led us to the conclusion that peripapillary atrophy had significant correlation with glaucomatous damage on optic disc and disc hemorrhage in glaucoma patients. Therefore more attention should be paid tod a glaucoma patient especially when he or she presents with peripapillary atrophy.

7.A Study on the Sourcees of Postoperative Wound Infection in Patients with Open Fracture

The purpose of this paper is to define the bacterial sources of wound infection after surgery including debridement for open fractures. Total number of patients were 123 during surgery July 1975 to Oct. 1976 at Hangang Sacred Heart Hospital. Out of 123 cases 17 were infected postoperatively. Serial bacterial cultures were performed for specimens taken from the contaminated wound in emergency room and by exposing the Petri-dishes in the operating room. Additional cultures were carried out for 17 specimens taken from the infected wounds postoperatively. Positive bacterial cultures from the initial wounds were obtained in 71 cases (57.7%) out of 123 contaminated wounds in emergency room. Setting monitoring of air contamination in conventional operating room, 51 colonies of bacterial growth per 1 hour exposure were noted and common organisms were colliform bacilli, streptococci and staphylococci in order. Postoperative wound infection rate was 13. 8%. Common pathogens were staphylococcus aureus, coliform bacilli and pseudomonas and kanamycin was most effective antibiotics. The retrospective bacteriological studies for 17 infected cases elucidated following results: 1) Organisms of same species; from initial wound and infected wound postoperatively 7 cases 2) Organisms of same species; from air contamination in the operating room and infectected wound postoperatiyely 5 cases 3) Organism of same species; from air contamination in the operating room, initial wound and infected wound postoperatively 1 case 4) Others (contagious or endogenous origin) 4 cases The results reveal that the initial contaminated open fracture and air contamination during surgery are the main sources of postoperative infection. Four cases which the infectious sources were not determined included in contagious origin dogmaticaIly.

8.Effects of Orally Administered Baclofen in the Animal Model for Neuropathic Pain.

The aim of this study was to compare with the effects of baclofen using an animal model of neuropathic pain. The sciatic nerve of rats was ligated unilaterally about dorsal half-portion in the tight according to the method of Seltzer and his colleague. After surgical operation, the rats showed painful symptoms of the ipsilateral hind paw, suggesting the possibility of spontaneous pain. And then, the paw withdrawal latency to the local heating on the paw through the glass plate and the frequency of paw withdrawal response to innocuous mechanical stimulation with modified von Frey filaments were determined to compare with the effects of pre-and post-medication of baclofen, respectively, at postoperative 3, 7, and 10 days. The results obtained were as follows: 1) The thermal hyperalgesia and mechanical allodynia produced by partially tight ligation of sciatic nerve appeared continously postoperative 3 days later. 2) In the hyperesthetic rats, the thermal hyperalgesia was inhibited from the 3rd posroperative day with orally administered baclofen 0.2mg and 1.0mg. 3) In the hyperesthetic rats, the mechanical allodynia was inhibited with baclofen 0.2mg, but not with baclofen 1.0mg, These results suggest that baclofen have more specific effects on thermal hyperalgesia than mechanical allodynia.

9.Two Cases of Ureteral Strictures Associated with Contralateral Renal Agenesis.

10.Nocturnal Penile Tumescences in Normal Korean Adult Males with Relation of Age.

Nocturnal penile tumescence (NPT) evaluation is the single most important step and widely used technique for the differential diagnosis between psychogenic and organic impotence. It is based on findings that male subjects have erection in association with rapid eye movement sleep. A normal pattern of NPT indicates that neural supply, vascular supply and penile structures are intact. Nocturnal penile tumescence minitoring (NPTM) in the abscence of rigidity evaluation in the assessment of the impotent male is of questionable value, but we conducted a study of NPT in 41 healthy male subjects aged 20 to 69 by means of portable tumistore NPT monitor to provide normative data for clinical utility in diagnosis and prognosis of impotence. The results indicate that NPT occurs consistently in a healthy male population and its expression is significantly affected by age.
