2.Association of Hemoglobin A1c with Visceral Fat Measured by Computed Tomography in Nondiabetic Adults.

OBJECTIVE: A prediabetes hemoglobin A1c (HbA1c) level of 5.7%-6.4% is considered a risk factor for diabetes mellitus and cardiovascular disease (CVD) in the USA. In this study, we assessed the HbA1c and visceral fat levels as CVD risk factors in health check-up examinees who were not yet diagnosed with diabetes. METHODS: Totally, 507 study subject were categorized as per criteria of the American Diabetes Association, depending on whether the HbA1c level was > or =5.7% or <5.7%. Lipid levels, blood pressure, BMI (kg/m2), total abdominal, and visceral fat levels were measured by computed tomography. RESULTS: The mean of HbA1c in the male group was larger than the mean in the female group and their values were, respectively, 6.03+/-0.8% and 5.88+/-0.72%(p<0.05). Only the mean values of age and visceral fat area were different between HbA1c> or =5.7% and <5.7% in both male and female group (p<0.05). Visceral fat levels were significantly associated with HbA1c in the group of HbA1c > or =5.7% (odds ratio=1.005, 95% CI 1.002~1.008). CONCLUSIONS: Visceral fat levels were significantly higher and correlated with the group which HbA1c level is > or =5.7%. This finding suggests that subjects who have high levels of HbA1c should be carefully monitored during prediabetes and should have chance to have health education programs.

3.Pharmacologic Effects of Atropine Penalization in Amblyopic Eyes.

PURPOSE: To analyze the pharmacologic effect of atropine penalization in amblyopic eyes. METHODS: Distant and near visual acuity, near point of accommodation (NPA), and pupil size were measured in the sound eye of 40 children with amblyopia before and after receiving atropine sulfate 1% solution. Amblyopic children were divided into 2 groups according to the time of follow-up visit: 4-days and 7-days after injection; the same tests were performed on all the scheduled follow-up visits. This study included a control group of 20 normal children who received the same test on the initial visit and the scheduled follow-up visits. RESULTS: Visual acuity of the sound eye at distance showed greater decreases in children with 4-day follow-up visits compared with 7-day visits in the amblyopic group (p = 0.24). There was no significant difference in near visual acuity between the amblyopic group and the normal control group by follow-up interval (p = 0.98, p = 0.75). Near point of accommodation significantly increased in the 4-day visit group in both the amblyopic and the normal control groups (p = 0.04 and p = 0.03, respectively), Pupil size was significantly maintained in the 4-day visit, amblyopic group (p = 0.01). CONCLUSIONS: Atropine effectively decreased the vision of the sound eye in amblyopic children. However, to maintain the pharmacologic effect, two separate atropine instillations per week (on a weekday and the weekend) should be considered as a treatment for amblyopia.

4.Polypoid Amelanotic Melanoma on the Thigh.

Amelanotic maliganat melanoma is a comparatively rare disease. It accounts for 1.8~8.1% of all malignant melanoma. It is sometime difficult to diagnose amelanotic malignant melanoma because there is no pigmentation, clinically. Polypoid melanoma is a variant of nodular melanoma, which in depth seldom reaches the reticular dermis. The main part of the tumor is located above the nearby epidermis, raised in the form resembling cauliflower. We report a rare case of amelanotic malignant melanoma with polypoid feature in a 78-year-old woman who presented a single bright red nodule on the left thigh.

5.Characteristics of Attention in Children with Post-Injury Attention-Deficit Disorder and Developmental Attention-Deficit Hyperactivity Disorder.

OBJECTIVES: Children with post-injury attention deficit disorder (ADD) exhibit deficiencies in academic functioning, social skills, and self-control, that are comparable to developmental attention-deficit hyperactivity disorder (ADHD). The similarities and differences of the two conditions have to be studied because some children with traumatic brain injury have shown premorbid ADD and seem to be less responsive to psychostimulants. METHODS: Characteristics of cognition and attention of post-injury ADD (n=12) and developmental ADHD (n=12) boys were investigated by administering KEDI-WISC and Visual and Auditory TOVA. RESULTS: No differences in KEDI-WISC subtests were found between two diagnostic groups. Post-injury ADD boys show lower commission error (lower impulsivity) in visual and auditory TOVA, and lower variability of reaction time (higher information-processing consistency) in auditory TOVA than developmental ADHD boys (p<0.05, p<0.01). CONCLUSION: Post-injury ADD and developmental ADHD are different in their attention components suggestively, which warrants further study for differential diagnosis and proper treatment.

6.A Single Isolated Cotton-Wool Spot not Associated with Systemic Diseases.

PURPOSE: To present a case of a single isolated cotton-wool spot in a healthy patient. CASE SUMMARY: A 43-year-old woman with no systemic disease complained of sudden inferior field defect of the right eye. On fundoscopic examination of the right eye, a single soft white spot was observed in the superotemporal vascular arcade. An elevated hyper-reflective area in the inner retina corresponding to a white spot was observed on optical coherence tomography. Automated perimetry revealed an arcuate field defect which was wider than the cotton-wool spot. A complete workup for systemic diseases was within normal range except a positive rheumatoid factor. At the 2-week follow-up, the patient's subjective symptom improved and the white spot decreased. After 6 months, the cotton-wool spot disappeared and slit-like retinal nerve fiber layer (RNFL) defects corresponding to the location of the cotton-wool spot was noticed. One year later, disc and RNFL defect were unchanged but the previous field defect disappeared. CONCLUSIONS: Isolated cotton-wool spots can occur without serious systemic diseases in healthy patients. A single isolated cotton-wool spot in the present case disappeared spontaneously leaving permanent structural damage on the retina over time.

8.Swept Source Optical Coherence Tomography Findings in Gas-Filled Eyes after Macular Hole Surgery.

PURPOSE: To investigate the ability to determine the postoperative status of macular hole (MH) in gas-filled eyes using swept source optical coherence tomography (SS-OCT). METHODS: Ten eyes of 10 patients who underwent vitrectomy, internal limiting membrane peeling and gas tamponade for idiopathic MH were included in this study. The macular area was examined using SS-OCT on postoperative days 1, 2 and 30. RESULTS: MH status was assessed in 6 eyes (60%) on postoperative day 1 and MH closure was confirmed in 5 of the 6 eyes (83%). MH remained closed in all 5 eyes at 1 month after surgery. Only gas-retinal interface was obtained in 4 eyes (40%) and unclosed MH was found in 1 of the 4 eyes (25%) at 1 month after surgery. CONCLUSIONS: The MH status can be determined using SS-OCT in the early postoperative period even in gas-filled eyes. Confirming early MH closure with SS-OCT can serve as an important guide to significantly shorten the duration of prone positioning while maintaining high closure rates.

10.Terminally Differentiating Eosinophils Express Neutrophil Primary Granule Proteins as well as Eosinophil-specific Granule Proteins in a Temporal Manner.

Neutrophils and eosinophils, 2 prominent granulocytes, are commonly derived from myelocytic progenitors through successive stages in the bone marrow. Our previous genome-wide transcriptomic data unexpectedly showed that genes encoding a multitude of neutrophil primary granule proteins (NPGPs) were markedly downregulated during the end period of eosinophilic terminal differentiation when cord blood (CB) cluster of differentiation (CD) 34+ cells were induced to differentiate toward the eosinophil lineage during a 24-day culture period. Accordingly, this study aimed to examine whether NPGP genes were expressed on the way to eosinophil terminal differentiation stage and to compare their expression kinetics with that of genes encoding eosinophil-specific granule proteins (ESGPs). Transcripts of all NPGP genes examined, including proteinase 3, myeloperoxidase, cathepsin G (CTSG), and neutrophil elastase, reached a peak at day 12 and sharply declined thereafter, while transcript of ESGP genes including major basic protein 1 (MBP1) attained maximum expression at days 18 or 24. Growth factor independent 1 (GFI1) and CCAAT/enhancer-binding protein α (C/EBPA), transactivators for the NPGP genes, were expressed immediately before the NPGP genes, whereas expression of C/EBPA, GATA1, and GATA2 kinetically paralleled that of eosinophil granule protein genes. The expression kinetics of NPGPs and ESGPs were duplicated upon differentiation of the eosinophilic leukemia cell line (EoL-1) immature eosinophilic cells. Importantly, confocal image analysis showed that CTSG was strongly coexpressed with MBP1 in differentiating CB eosinophils at days 12 and 18 and became barely detectable at day 24 and beyond. Our results suggest for the first time the presence of an immature stage where eosinophils coexpress NPGPs and ESGPs before final maturation.
