Pedunculated hepatocellular carcinoma was first described by Roux in 1987, and Goldberg and Wallerstein presented a case with pathological description in 1934. Since then 37 cases have been reported in the world literature. A pedunculated hepatocellular carcinoma, occurred in a 69-year-old woman, was encountered. It was attached to the Glission's capsule of the right lobe of liver inferiorly, and was connected to the porta hepatis bt a mesenteric pedicle containing arteries, veins, bile ducts and nerve bundles. The tumor mass was completely encapsulated, and measured 8.5x8x6 cm and 255 gm. It was composed of hepatocellular carcinoma of a well-differentiated, acinar type. Tumor cells were positive for HBsAg, but negative for alpha-fetoprotein on peroxidase-antiperoxidase staining. The liver exhibited macronodular cirrhosis on gross examination. This tumor is thought to have on in the supernumerary lobe of liver.

The elbow region is the most common site at which the ulnar nerve is prone to a localized neuropathy. Diagnosis of ulnar nerve entrapment at elbow is made on the basis of history, lacal neurologic finding, nerve conduction studies and electromyography. Electrophysiologic test is simple, relatively quick, and accurate method for the diagnosis of peripheral nerve lesion. Therefore, electrodisgnosis makes it possible to identify and localize the early stage of entrapment lesion. Definitive diagnosis of ulnar nerve entrapment at elbow requires electromyographic demonstration of decreased ulnar nerve conduction velocity across the elbow and depressed sensory nerve action potentials. The purpose of this study were to analyze the electrophysiologic findings of patients with an established ulnar nerve entrapment at elbow compared to normal eonduction velocity of our laboratory. And we evaluate the correlation of clinical symptom with the electromyographic findings. Twenty five subjects were studied, 22 men and 3 women, aged from 5 to 62 years. The results were as follows . 1. The mean motor conduction velocity of ulnar nerve across elbow were 32.85±10.21 m/sec. The longer duration of symptom were, the slower conduction veloctity across elbow were. 2. Among the 25 patients, motor conduction velocity of ulnar nerve at forearm segment decreased in 15 patients(60%). As the motor conduction velocity of the ulnar nerve across elbow became slower, thst of forearm segment were slower. 3. In 19 patients (76%) among the 25 patients, sensory evoked potentials revealed prolonged distal latency or were not evoked. 4. On needle electrodiagnostic study, positive sharp wave or fibrillation potentials could be found in abductor digiti minimi first dorsal interosseus and/or flexor carpi ulnaris muscles in 22 patients(88%).

The purpose of this study was to prevent the dilutional effect of excessive absorption of irrigating solution by using furosemide intraoperatively during transurethral resection of the prostate. 30 patients, who belonged to physical status II or III of ASA classification, were selected randomly and divided with two groups as follows: G1 (N=15): Not-administrated furosemide (control group) G2 (N=15): Administrated furosemide (Experimental group) All patients were premedicated with Hydroxyzine (1mg/kg, IM) and were performed continuous epidural anesthesia with 2% lidocaine (1-1.5mg/segment). For irrigating solution, 5% D-sorbitol was used and Hartman's solution were given for maintenance fluid and fixed the height of irrigating container to 60 cm from symphysis pubis. With the starting of operation, 20 mg furosemide was administrated to experimental group. The blood samples for the values of Na⁺, K⁺, Glucose and BUN were performed at the periods of preoperation, 10 min, 20 min, 30 min after the starting of operation and immediate postoperation. Based these date, serum osmolality and effective osmolality were calculated. The results were as follows: 1. The sodium concentration of control group was decreased statistically significantly at 10 min, 20 min, 30 min after the starting of operation and immediate postoperative period as comparing with the preoperation value (p<0.05). But that of experimental Group was not changed significantly. 2. The serum osmolality and effective osmolality were decreased statistically significantly at 30 min after the starting of operation and immediate postoperative period as comparing with the preoperation value (p<0.05). But those of experimental group were not changed significantly. These results show that the dilutional effect of excessive absorption of irrigating solution might be prevented by using furosemide intraoperatively. And so we recommend the use of furosemide during TURP, especially in patients with congestive heart failure or renal failure.

We investigated the effect of 2% dorzolamide and 0.12% UF-021 on intraocular pressure and peripapillary and optic nerve head microcirclation with Heidelberg Retina Flowmeter. The 40 eyes of 40 normal subjects were divided 2 groups. 2% Borzolamide and 0.12% UF-021 were instilled in each group. The IOP and microcirculation of peripillary retina and optic nerve head were measured before and 90min, 4, 8, 12, 24 hours after instillation. The IOP was decreased at 90 minutes, 4, 8 hours after instillation of 2% dorzolamide and maximal reduction of IOP was observed at 90 minutes (P<0.05). The IOP was decreased at 90min, 4, 8, 12 hours after instillation of 0.12% UF-021 and maximal reduction of IOP was observed at 4 hours(P<0.05). But, there was no statistically significant changes of microciculation in both groups(P>0.05). We suggest that 2% dorzolamide and 0.12% UF-021 reduce the IOP, but do not alter the mictocirculation of peripapillary retina and optic nerve head.
