3.Postoperative analgesic effect of intraarticular bupivacaine or morphine after arthroscopic knee surgery.

Recent studies have shown that opiods can produce potent antinociceptive effects by interacting with opioid receptors in peripheral tissues. This study sougt to compare the effects of morphine with those of-bupivacaine administered intraarticularly upon pain after arthroscopic knee surgery. In a ramdomized manner, 60 healthy patients received either morphine(3 mg in 20 ml NaCl : n=20), bupivacaine(20 ml, 0.25% : n=20) intraarticularly at the completion of surgery, and others were not administered (n = 20) under general anesthesia after 1, 2, 4, 6, 12 and 24h of postoperative day, pain was assessed by a visual analogue pain scalps, time to first analgesic use were recorded. Pain scores were signicantly greater in the morphine group than two groups at 11. From 4th until the end of the study period, pain scores were significantly greater in the bupivaca.ne group than in the other two group. Anagesic requirements were significantly greater in the morphine group than two groups at 1h but were significantly greater in the bupivacaine group than in the other groups throughout the remainder of the study period. The results suggest that intraarticular morphine produces an analgesic effect of delayed onset but of remarkably long duration.

4.Peripheral Lymphocyte Counts and Abnormalities of Quantitative D. N. C. B. Sensitization in Urogenital Malignant Tumors..

Seventy five cases of urogenital malignant tumors were reviewed in regard to the survival, therapy and peripheral lymphocyte counts. Cellular immunity was also investigated in 15 cases of them by assessing their cutaneous delayed hypersensitivity responses to dinitrochlorobenzene (DNCB), compared with controls. I. The incidence of lymphopenia in the urogenital malignant tumors (20. 6%) was higher than that in the control group(10.0%). Most cases with lymphopenia showed poor survival, and data suggested that peripheral lymphocyte count was a useful parameter to estimate the prognosis of the urogenital malignant tumors. 2. Surgical treatments caused a temporal decrease of circulating lymphocyte counts by 27.6% which recovered to normal value within 4 weeks, and high dosage radiation therapy brought about persistent and higher degree of decrease of peripheral lymphocyte by 47.9% which persisted for up 1-2 years. 3. Cancer patients had a lower incidence of positive reaction (73.3% vs. 93.4% in controls) in investigation of cellular immune responses using quantitative DNCB contact sensitization. There may be a significant relationship between tumor stage and reactivity to DNCB in bladder cancers.

8.Platelet-rich Plasma in Arthroscopic Rotator Cuff Repair.

Rotator cuff tear is a common reason for shoulder pain. Although the surgical technique of rotator cuff repair is developing, high retear rate requires additional supplementary methods. Among these supplementary methods, as a kind of biologic augmentation, platelet-rich plasma (PRP) has been spotlighted and has recently been studied by many researchers. PRP, a concentrate of platelet extract obtained from whole blood, contains numerous growth factors. As this is known to play an important role in the tissue recovery process, it had been used for research in a variety of fields including orthopedics. Use of PRP has been attempted in surgical treatments of rotator cuff tear for better results; however, only a few large-scale research studies on the effect of PRP have been reported. Clinical results of each study are also variable. Therefore research using large-scale randomized, double-blind trials should be conducted in order to prove the application range, safety, and clinical effects of PRP.
