2.Disabling Pansclerotic Morphea.

Disibling pansclerotic morphea, a severe form of generalized morphea, has a relentless clisabling course, producing marked contract,ion and joint deformity due to sclerosis of the skin and the deeper structures. We report herein an unusual case of morphea in a 25 year old male who had suffered from chronic recurrent ulceration with scar like sclerotic changes overlying limb folds, ankles, nape and abdomen since 4 years of age. The indurated lesions on the chest and abdomen were ivory colored, sclerotic guttae and plaques having a, violac ous border. At l4 years of age, the skin lesions were diagnostically ambiguous, wherea.s the immunologic results revealed a selective IgA deficiency with partial T lymphacyte inactivation. About 10 years later, the follow up visit showed this severe form of morphea over generalized area and disabling contractures of knees, elbows and alkles, but IgA value returned nearly to the nomal level.

4.A Case of Elephantiasis Neuromatosa.

We report herein a case of elephantiasis neuromatosa seen in neurofibromatosis patient, who was a 33-year-old male and had been diagnosed at the age of 13. Since that time on, small nodule present on the mid-back area led to develop grotesquely large, dark-brown colored, flabby mass with thick verucoid skin change, measuring about 30 * 20cm in size. Histopathologic examination of excised mass demonstrated moderate hyperplasia of epidermis and typical neurofibroma findings with soft tissue and fibrous tissue overgrowth, but no atypical cells. The electron microscopic study showed many perineurial cells surrounded by the continuous bssal lamina of fibroblsst in the dermal lesion and numerous giant pigmented granules in melanocyte and basal keratinocyte in the cafe-au-lait spot.

6.Epidemiologic Survey of the Serlogic Markers of Hepatitis B Virus and Serologic Tests for Syphilis in Institutionalized Persons.

To survey the seroprevalence of hepatit!s B virus(HBV) in relation to serologic tests for syphilis(STS), using VDRL and TPHA test, the tests were done in the seri of 1252 institutionalized persons, who lived in Taegu Hope Village for the poor or crippled, and 1831 controls, who had the regular routine physical examination from March to May, 1987. The positive rates nf HBsAg and anti-HBs were 11.7%(146) and 27.0%(338'). compared with 6.25%(ll) and 18.4%(337) in controls, respectively. The prevalence of HBV including HBsAg and anti-HBs was higher in institutionalized persons (38,7+i) than in controls(24.6%)(p<0.0001). The positive rate of STS was higher in institutionalized persons(7.5%) than in controls(2.9%)(p<0.0001). There was a tvend for a higher rate of seropositivity in women than in men of institution- alized persons in the both diseases. The prevalence of HBV among STS-positive cases wa.s 42.6%(40) of instituti- onalized persons and 26.4%(14) of controls, but not, statistically significant, the gre,ater part of them distributing in 4th and 5th decades. The positive rates of I-tBi.Ag in institutionalized persons was 45.5%(5) of STS-positive cases and 25. 9%,'35) of STS-negative cases. No correlation was found between the serologic ma kers of HBV and VDRL titration.

8.Anticancer Activity of Intravesical Glyceryl Monooleate (GMO)-Paclitaxel Therapy in Murine Superficial Transitional Cell Carcinoma Model Induced by BBN.

Purpose: Paclitaxel, on systemic administration, is widely known to be effective in the treatment of bladder cancer. However, the intravesical use of paclitaxel has not been attempted because of its liposolubility and direct toxicity to the bladder mucosa. The purpose of this study was to evaluate the efficacy and toxicity of paclitaxel-loaded glyceryl monooleate (GMO) in the intravesical treatment of superficial bladder cancer, by enhancing its bioadhesiveness and bioavailability. Materials and Methods: 12 mice were divided into two groups, and bladder carcinomas induced by the addition of 0.05% BBN to their drinking water for 12 weeks. Group 1 received an intravesical instillation of 0.1ml GMO-paclitaxel-free buffer and Group 2 an intravesical instillation of 0.1ml GMO-paclitaxel. On day 21, the tumor incidence, bladder weight and toxicity were evaluated. Results: The incidence of cancer in groups 1 and 2 were 83 and 17%, respectively. The incidence of cancer was significantly reduced in group 2 compared to group 1 (p<0.05). There was a tendency for the average bladder weight in group 1 to be heavier than that in group 2, but there was no significant difference (p=0.375). There were no liver, kidney or bone marrow toxicities in either group. Conclusions: Intravesical GMO-paclitaxel therapy may have an inhibitory effect on the growth of superficial bladder cancer in a BBN-induced bladder cancer model; therefore, it could potentially be used in those patients showing little to no response to intravesical Bacillus Calmette- Guerin (BCG) or other anticancer drug therapies.

9.Epidemiologic Study of Paederus Dermatitis Prevailing in the Midwest Area of Kyungpuk Province.

BACKGROUND: Paederus derriattis is a peculiar linear lesion caused by an insect belonging to genus Paederus, family Staptylinidae, order Coleoptera. The deliberate or accidental contact of crushed coelomic fluid of th insect, on the skin causes acute lesions due to a t.oxin(pederin) being released. OBJECTIVE: The study was nade in order to investigate an epidemic status of the peculiar der matitis and its causative agent of factor prevailing in the midwest area of Kyungpuk province. METHODS: We performed field approaches on 633 cases(M: 303, F: 330) including cases enrolled in a local clinic, teamiring up with a local skin clinician over the 3-month period of August November in 1994. RESULTS: 1. The captive insect causing the present dermatits, was identified as Paederus fuscipes(PF). The epidemic might be attributeid to the climatic conditions with higher temperature than usual during the period of July-eary September in the past year by 3-4degrees C as well as a lower relative humidity. The active temperature zone for PF was presumed to be about 15.5+1.2degrees C to 28.9+1.0degrees C 2. There was the highest p;evalence of 412 cases(65.1%) in early September, followed by mid September, late August, early October and mid August in a small number of cases in decreasing order. The cases were not found after mid October. 3. All age groups were evealy represented. The sex ratio was 1: 1.1 with little difference. The majority of cases lived in Kiriich on city(80.3%), with a lesser outbreak in the neighboring coun ties and Kumi city. 4. The preferred site was the mainly exposed area such as face, neck and extremities. The number of lesion was single(60%) and multiple(40%). The lesion showed a common linear con figuration and acute irritant contact dermatitis consistent with the histologic finding. CONCLUSION: The unusual wiather conditions and poor environmental hygiene help to develop an ecosystem breakdown, which, in turn, sometimes influences the innocuous insects life to be harm ful being on humans, as like outbreak of PF dermatitis.

10.Renal Candidiasis in Premature Infants.

Candida is a saprophytic fungus in the human host. However, in an immune- suppressed host, the organism can become pathogenic. The most common site of end organ involvement in premature infants with candidemia is the kidneys. A conglomeration of fibrillar hyphae results in fungus balls, which may cause a urinary tract obstruction and acute renal failure in premature infants. We recently experienced two cases of premature infants who had fungus balls in their urinary tracts. They were treated successfully with percutaneous nephrostomy, in combination with antifungal agents.
