1.A Case of Recurred Malignant Proliferating Trichilemmal Tumor of the Scalp.

PURPOSE: To report the case of recurred malignant proliferating trichilemmal tumor on scalp METHODS: Case report and literature review RESULTS: A 40-year-old man presented with a recurred malignant proliferating trichilemmal tumor, which developed on the occipital area. It was 8 x 9 x 4.5 cm in size, protruded, firm and non-tender mass. This tumor was widely excised including normal skin margin and pericranium. Immediate reconstruction using free latissimus dorsi muscle flap and skin graft were done. Histopathologic examination revealed abrubt, compact trichilemmal keratinization in the central area of lobular epithelial proliferation. CONCLUSION: There has been no recurrence of tumor during follow-up period of nine months without adjuvant chemotherapy or radiotherapy.

2.A Case of Kimura Disease on Parotid Gland.

Kimura disease is a relatively uncommon chronic inflammatory disorder of unknown etiology. Kimura disease occurs mainly in oriental young males. The majority of lesions occur in the head and neck and most patients have involvement of regional lymph nodes and major salivary glands. Although Kimura disease is similar to malignancy clinically and radiologically, it is benign disease showing good prognosis in only conservative management. So it is important to familiarize clinical, histological and radiological findings. We experienced a case of mass which developed on parotid gland of 40-year-old male. On impression of malignancy, total parotidectomy was performed. The Histological report was Kimura disease. There has been no recurrence during follow-up period of two years.

3.Clinical importance of F-waves as a prognostic factor in Guillain-Barré syndrome in children.

PURPOSE: A limited number of studies have examined the link between F-wave abnormalities and clinical presentation in pediatric Guillain-Barré syndrome (GBS). Therefore, this study examined the importance of F-wave abnormalities as a prognostic factor in pediatric GBS patients. METHODS: The records and electrodiagnostic studies (EDS) of 70 GBS patients were retrospectively evaluated, and divided into 2 groups according to the results of EDS. Group A (n=33) presented with F-wave abnormalities, and group B (n=26) exhibited normal findings. We compared laboratory reports, clinical features, response to treatment, and prognosis between the 2 groups. RESULTS: Motor weakness was the most frequently observed symptom for either group. Clinically, the incidence of fever and upper respiratory symptoms differed between the 2 groups, while the prevalence of abnormal deep tendon reflex (DTR) was significantly higher in group A than B (P<0.05). Patients diagnosed with GBS had received intravenous immunoglobulin treatment: 94% in group A and 58% in group B. Furthermore, significantly greater numbers of patients in group A showed H-reflex abnormalities and poor prognosis compared with group B (P<0.05). CONCLUSION: This study demonstrated that F-waves are a clinically important prognostic factor in GBS. F-wave abnormalities were associated with abnormal DTR and poor prognosis in patients. Limited studies have examined the link between F-wave abnormalities and clinical results; therefore, further randomized controlled studies are needed to confirm the clinical characteristics and efficacy of treatments.

4.Gene Therapy Using Hepatocyte Growth Factor Expressing Adenovirus Improves Skin Flap Survival in a Rat Model.

Hepatocyte growth factor (HGF) is a potent angiogenic factor that can stimulate the production of blood vessels in ischemic tissue. We investigated whether gene therapy using HGF-expressing adenovirus could enhance skin flap survival. Sprague-Dawley rats were randomly divided into three groups. Rats were subdermally injected with HGF-expressing adenovirus (HGF virus group), recombinant HGF (rhHGF group), or phosphate buffered saline (PBS group) 2 days before and immediately after 3 x 9 cm caudal flap elevation. The survival area of the skin flap, the ratio of blood flow, CD31-positive vessels and, VEGF expression were examined. Skin flap viability was significantly increased in the HGF virus group compared to the rhHGF and PBS groups (71.4% +/- 5.9%, 63.8%+/- 6.4%, and 39.2% +/- 13.0%, respectively) (P = 0.025). Furthermore, the blood flow ratio was significantly increased in the HGF virus group. In the HGF virus group, the number of CD31-positive vessels and vascular endothelial growth factor (VEGF) expression were significantly increased. Gene therapy using HGF-expressing adenovirus increase VEGF expression, the number of viable capillaries, and blood flow to the flap, thereby improving skin flap survival.

5.Ileal W-Neobladder in Women with Bladder Cancer: Early Experience.

PURPOSE: The anatomical and voiding functional outcomes of orthotopic neobladders in women with bladder cancer were investigated. MATERIALS AND METHODS: Since July, 1998, 5 women (median age 64.8) underwent an orthotopic bladder substitution following a radical cystectomy for bladder cancer. The median follow-up was 19.6 months (range 12 to 35). After urethral sparing cystectomy, an ileal W-neobladder was constructed and supported by an omental flap. Both the early and late complications were analyzed. A pressure flow study was performed to evaluate the voiding function, and a radiologic study including a MRI was performed to observe the anatomy. RESULTS: There were no perioperative deaths and no early and late complications related to the surgery except for acute pyelonephritis, which occurred in one patient 22 months after surgery. Four patients were continent day and night and one patient had nighttime incontinence. MRI showed that the urethra was connected to the dependent portion of the neobladder, which was supported by an omental flap in all patients. No recurrence of cancer in the native urethra was noted. All patients were satisfied postoperatively. CONCLUSIONS: Orthotopic bladder substitution after a standard radical cystectomy in women provides satisfactory anatomical and functional outcomes. An omental flap could improve the voiding function by supporting the neobladder and making a urethroneobladder junction located in the dependent portion of the neobladder.

6.Prevalence and risk factors of peri-implantitis: A retrospective study.

PURPOSE: The study analyzed the prevalence of peri-implantitis and factors which may have affected the disease. MATERIALS AND METHODS: This study based on medical records and radiographs of 422 patients (853 implant cases) who visited Ewha Womans University Mokdong Hospital Dental Center from January 1, 2012 to December 31, 2016. Generalized estimation equations (GEE) was utilized to determine the statistical relationship between peri-implantitis and each element, and the cumulative prevalence of peri-implantitis during the observation period was obtained by using the Kaplan Meier Method. RESULTS: The prevalence rate of peri-implantitis at the patient level resulted in 7.3% (31 patients out of a total of 422 patients), and at the implant level 5.5% (47 implants out of a total of 853 implants). Sex, GBR, guided bone regeneration (GBR) and functional loading periods had statistical significance with the occurrence of peri-implantitis. Upon analysis of the cumulative prevalence of peri-implantitis in terms of implant follow-up period, the first case of peri-implantitis occurred at 9 months after the placement of an implant, and the prevalence of peri-implantitis showed a non-linear rise over time without a hint of a critical point. CONCLUSION: The prevalence of peri-implantitis at the patient level and the implant were 7.3% and 5.5%, respectively. Male, implant installed with GBR and longer Functional Loading Periods were related with the risk of peri-implantitis.

7.Efficacy of 3D-printed simulation models of unruptured intracranial aneurysms in patient education and surgical simulation

Objective:
The purpose of this study was to determine the efficacy of a 3D-printed aneurysm simulation model (3DPM) in educating patients and improving physicians’ comprehension and performance.
Methods:
This prospective study involved 40 patients who were diagnosed with unruptured intracranial aneurysms (UIAs) and scheduled for surgical clipping or endovascular coiling and randomly divided into two groups (the 3DPM group and the non-3DPM group). The 3DPM was used in preoperative consultation with patients and intraoperatively referenced by surgeons. The patients, 7 neurosurgical residents, and 10 surgeons completed questionnaires (5-point Likert scale) to determine the usefulness of the 3DPM.
Results:
Patients in the 3DPM group had significantly higher scores in terms of their understanding of the disease (mean 4.85 vs. 3.95, p<0.001) and the treatment plan (mean 4.85 vs. 4.20, p=0.005) and reported higher satisfaction during consultation (5.0 vs. 4.60, p=0.036) than patients in the non-3DPM group. During patient consultation, 3DPMs were most useful in improving doctor–patient communication (mean 4.57, range 4-5). During clipping surgery, the models were most useful in assessing adjacent arteries (mean 4.9, range 4-5); during endovascular coiling, they were especially helpful in microcatheter shaping (mean 4.7, range 4-5).
Conclusions
In general, 3DPMs are beneficial in educating patients and improving the physician’s performance in terms of surgical clipping and endovascular coiling of UIAs.
8.Administration of Goji (Lycium chinense Mill.) Extracts Improves Erectile Function in Old Aged Rat Model.

PURPOSE: This study investigated the effect of goji (Lycium chinense Mill.) on erectile dysfunction in old-aged rats. MATERIALS AND METHODS: Twenty-four 18-month-old male Sprague-Dawley rats (defined as old-aged rats) were used. Treatment groups contained eight rats each: a control group, goji extract of 150 mg/kg/day group, and goji extract of 300 mg/kg/day group. Treatment was by orogastric tube once daily for 6 weeks. After 6 weeks of treatment, testes weight, serum testosterone, superoxide dismutase, nitric oxide (NO)-cyclic guanosine monophosphate (cGMP)-related parameters, intracavernous pressure/mean arterial pressure, and histological changes were examined. RESULTS: Treatments with goji extracts increased serum testosterone level, increased the expression of endothelial NO synthase, neuronal NO synthase, and cGMP, improved the oxidative stress marker, and decreased corporal fibrosis. CONCLUSIONS: Our results indicate that goji extract may have a positive effect on erectile dysfunction via its antioxidant effects.

9.l-myc Polymorphism in Gastric Cancer, Lung Cancer, and Hepatocellular Carcinoma.

PURPOSE: This study was performed to see if a particular polymorphism in the l-myc, a nuclear oncogene at the 1p32 locus, might be associated with greater risk of gastric cancer, lung cancer and hepatocellular carcinomas (HCC) in Korean patients. MATERIALS AND METHODS: Genomic DNA, derived from patients diagnosed with gastric cancer (n=57), lung cancer (n=39), HCC (n=35) and healthy individuals (n= 176), was examined. The l-myc polymorphism under study was visualized by PCR followed by EcoRI digestion. RESULTS: There was no significant difference in the distribution of the l-myc polymorphism genotypes and allele frequencies between the cancer patients and the controls. CONCLUSION: The l-myc polymorphism does not appear to be indicative of elevated risk of cancers of the stomach, lung and HCC.

10.Rapid-Growing Intracranial Immature Teratoma Presenting Obstructive Hydrocephalus and Abducens Nerve Palsy: A Case Report and Literature Review

Intracranial immature teratoma is an extremely rare disease with poor prognosis and requires complicated treatment. Owing to the deep midline location of the tumor, total surgical resection of the tumor is challenging. We present our experience with a fast-growing pineal gland immature teratoma in a 4-year-old boy, who presented with obstructive hydrocephalus and abducens nerve palsy, which was treated with total surgical resection of the tumor. In addition, we aimed to determine the appropriate treatment modality for intracranial immature teratomas by reviewing the literature and investigating the prognosis.
