Objective The purpose of our study was to report the results of using arthroscopic Remplissage and Bankart repair in patients who had an engaging Hill-Sachs lesion with significant glenoid bone loss. Methods We retrospectively reviewed 49 consecutive patients who underwent arthroscopic Remplissage and Bankart repair for anterior shoulder instability with a mean duration of follow-up of 29.0 months (24-35 months). At the time of surgery the mean age of 42 men and 7 women was 28.4 years. All patients were diagnosed as recurrent anterior shoulder dislocation with a bony lesion of glenoid and an engaging HillSachs lesion. An arthroscopic Remplissage and Bankart repair using metal anchor was performed in all cases.ASES score, Constant-Murley score and Rowe score were used to evaluate the stability and the function of the shoulder. Results Patients' active forward elevation significantly(P=0.007) improved from 162.9°±17.1°preoperatively to 170.9°±7.4° at final follow-up. The external rotation was 56.0°±17.6° before the surgery compared with the 54.1°±17.1° postoperatively(P=0.511 ). The ASES score, Constant-Murley score and Rowe score was 84.7±11.3, 93.3±8.7 and 36.8±8.5 preoperatively compared with 96.0±3.4, 97.8±3.6 and 89.8±12.5 postoperatively. Significant difference could be found with regard to ASES score (P=0.000), ConstantMurley score (P=0.005) and Rowe score (P=0.000). One redislocation happened and a subluxation was noticed in three patients (8.3%). Conclusion Arthroscopic Remplissage combined with Bankart repair can achieve satisfactory for recurrent anterior shoulder dislocation accompany with engaging Hill-Sachs lesion.
3.Analysis of 138 cases of drug eruption

Objective To update the knowledge on the sensitizing drugs and clinical features of drug eruption. Methods The clinical data on 138 patients hospitalized for drug eruption in the Department of Dermatology, Institute of Dermatology, Chinese Academy of Medical Sciences, from January 2005 to June 2007, were collected and retrospectively analyzed. Results Totally, 178 episodes of drug eruption were observed in these patients during the tested period. The major sensitizing drugs included antibacterial agents (31.46%), non-steroidal anti-inflammatory drugs (28.09%), traditional Chinese medicines (15.73%). Amoxicillin triggered 20 episodes of drug eruption and was the most common causative drug. Oral administration was the predominant sensitizing route of administration (54.17%). Of all the drug eruptions, 33.71% manifested by erythema multiforme, 28.09% by fixed drug eruption, 22.47% by exanthematous drug eruption. Severe types of drug eruption were mainly caused by traditional Chinese medicines and anti-gout drugs. Conclusions Antibacterial agents and non-steroidal anti-inflammatory drugs have become the major sensitizing drugs of drug eruption, especially amoxicillin. The frequency of traditional Chinese medicine-induced eruptions are increasing. Furthermore, caution is warranted for the drug eruption caused by oral administration.
4.Clinical characteristics of acute non-ST segment elevation myocardial infarction

Objective To study the clinical characteristics of acute non-ST segment elevation myocardial infarction(NSTEMI).Methods 211 patients of NSTEMI and STEMI underwent coronary artery angiography and echocardiogram.Patients'history and symptome were collected and the data of coronary artery angiography and echocardiogram were analyzed.Results Compared with STEMI,NSTEMI patients had more risk factors and postinfarction angina pectoris;severe coronary artery disease and three coronary vessel disease.But NSTEMI had relatively little effects on cardiac function.Conclusion NSTEMI always has more severe coronary artery disease and postinfarction ischemic effects.So more attention should be paid to its standard therapy.
5.Synthesis and antifungal evaluation of chalcone derivatives combined with fluconazole against drug-resistant Candida albicans

Chalcone is a common scaffold in natural products with optimal properties and biological activities.In this study, we designed and prepared eight new coumarin-chalcone derivatives (5a-5h), and confirmed their structures by 1H NMR and 13C NMR. Their in vitro antifungal activity combined with fluconazole (FLC) against drug-resistant Candida albicans was tested by microdilution method.The results indicated that most chalcone derivatives showed good antifungal activity against drug resistant Candida albicans with FLC, particularly with compound 5g displaying better antifungal activity (MIC50 = 5.60 μg/mL) than FLC (MIC50 = 200 μg/mL) when combined with FLC, so, these derivatives could be used as synergists of antifungal drugs.
6.Effects of group psychotherapy on damage of theory of mind in patients with early-onset schizophrenia

Objective:To explore the effect of group psychotherapy on damage of theory of mind (TOM) in patients with early-onset schizophrenia.Methods:Totally 60 patients with early-onset schizophrenia were included and divided into the intervention group (n = 30) and the health education group (n = 30).The patients in the intervention group were offered 10 twice a week 2-hour sessions of group psychotherapy,and those in the control were offered group health education twice a week.All participants completed baseline measures and post-intervention measures with the Eye Emotion Recognition and Theory of mind Picture-sequencing Task (TOM-PST).Results:Totally 23 patients in the intervention group and 29 patients in the health education group finished the post-intervention measures.After 5-week intervention,subscores of sad and fear and total score of emotion recognition,total score of TOM-PST,understanding first order error belief,second order error belief,third order error belief,sense of reality,detecting fraud compared to baseline had statistically significant difference in intervention group.In health education group,subscores of joy emotion recognition,understanding first order error belief,third order error belief,total score of TOM-PST compared to baseline had statstically significant difference.The patients with group psychotherapy got higher scores of total score of TOM-PST and understanding second order error belief and third order error belief than the health education group after 5-week intervention (P < 0.05).Conclusion:This study suggests the group psychotherapy could partially improve theory of mind with early-onset schizophrenia,and promote the recovery of social cognition.
7.Establishing reference intervals of the routine laboratory tests for hepatorenal function in Shanghai by biochemistry

Objective To establish reference intervals of nine laboratory projects for hepatorenal function and verify its reliability by indirect method . Methods ALL test results were extracted from physical examinations that were stored in the laboratory information system of Zhongshan Hospital during 2012 to 2014.Using Skewness-Kurtosis test to detect the normality of data , if not the original data were transformed through BOX-COX transformation to obtain an approximatenormal distribution .Outliers were identified and omitted by Turkey method .The indirect reference intervals were taken by applying Hoffmann method.The reference change value ( RCV) was selected to inspect the statistical significance between the calculated and published reference intervals .Results Among those nine laboratory projects ,thedifferences betweenthereference intervalsfor ALT , AST, AKP, GGT, TP, BUNwerelessthan their RCV ( <60.12%, 38.12%, 19.9%, 41.53%, 8.53%, 37.5%) , there were no significant differences between the direct method.There were slight differences between the calculated reference intervals and published reference intervals for the lower limit of the LDH and the upper limit of the ALB (>26.65%,9.92%) ,and there was a significant difference between the calculated reference intervals and the current reference interval in the laboratory for the lower limit of the UA of male (>26.65%).Conclusion This research further proved the reliability of indirect reference intervals and this technique deserved to be promoted and applied by other clinical laboratories.
8.Clinical results on repair of massive rotator cuff tears

Objective To evaluate the clinical results of arthroscopic repair of massive rotator cuff tear. Methods The study involved 16 patients with massive rotator cuff tears treated arthroscopically from September 2007 to June 2009. There were 6 males and 11 females at average age 61.5 years (45-75 years). The rotator cuff tears was repaired with arthroscopic double-row reconstruction. The range of motion, pain, strength of flexed elevation and function evaluation score were all recorded before operation and at final follow-up. The results were evaluated by t test and compared according to age and course of disease. Results All patients were healed without complications and the outcome was improved significantly ( P < 0.01 ). The mean VAS score was improved from preoperative 5.6 to postoperative 1.7,the average forward flexion from 69. 1°to 151.2°, the average external rotation from 14.7° to 32.2°, and internal rotation from L1 level to T10, the mean Constant-Murle from 39 to 85, the mean UCLA from 10.4 to 28, the mean SST from 2.8 to 8.8 and the strength of flexed elevation from 10.7% of normal side to 65.0%. Compared with preoperation, there was statistical difference in aspects of pain, range of motion, muscle strength and function in postoperation (P < 0.01 ). Conclusion Arthroscopic doublerow fixation can attain satisfactory results in repair of massive rotator cuff tear.
9.Pevalence rate of healthcare-associated infection in a maternal and child health care hospital

Objective To understand healthcare-associated infection(HAI)in a maternal and child health care hos-pital,so as to provide scientific evidences for further targeted surveillance.Methods A cross-sectional survey was performed by bedside visiting and medical record reviewing.Results Of 768 hospitalized patients,9(1 .18%)had HAI,the top 3 highest prevalence rates were found in obstetrical intensive care unit (9.09%),neonatal intensive care unit (5.80%)and gynecological department II(2.22%).Antimicrobial usage rate was 30.34%(n=233),134 of which (57.51 %)were prophylactic use,165 were mono-therapy(70.82%).A total of 5 pathogenic bacteria were isolated,the number of Streptococcus agalactiae ,Klebsiella pneumonia ,Enterococcus faecalis ,and Staphylococcus saprophyticus was 2,1 ,1 ,and 1 respectively,except Streptococcus agalactiae ,the other 3 strains were multidrug-resistant organisms(MDROs).Conclusion Surveillance on MDRO infection should be paid much attention,the oc-currence of MDRO infection should be reduced through targeted and bundle intervention.
10.Studies on cognitive function in patients of first-episode schizophrenia with and without metabolic syndrome

Objective To discuss the difference of cognitive function in patients of first-episode schizophrenia with and without metabolic syndrome.Methods Event related potential detection and Wechsler Memory Scale (WMS) was determined in 57 patients of first-episode schizophrenia with metabolic syndrome (research group) and 57 patients of first-episode schizophrenia without metabolic syndrome (control group),and the psychiatric symptom in two groups was assessed by the application of the positive and negative symptoms scale (PANSS).Results The memory quotient score was (78.51 ± 12.54) scores in research group,and (92.01 ± 17.49) scores in control group,and there was significant difference between two groups (t =-2.130,P=0.047).The short-term memory,instantaneous memory impairment in research group were lower than those in control group(t =2.151-3.054,P=0.007-0.046).The latency of P300 was (325.10 ±22.46) ms in research group and (284.60 ±22.46) ms in control group,and there was significant difference (t =3.454,P =0.003).The amplitude was (6.56 ± 1.61) μ V in research group and (9.78 ±1.73) μV in control group,and there was significant difference (t =-4.307,P =0.000).Conclusion The patients of first-episode schizophrenia with metabolism syndrome has more severe cognitive impairment than those without metabolism syndrome.
