In the chronic osteomyelitis of the ilium the infection is often so diffuse that removing all sequestra and small cavities are practically impossible. Either the partial or total resection of the ilium has been thought as a standard method of treatment in these situations. According to authors experience, the partial resection is not sufficient for removing such diffuse lesions, and all the patients that underwent the total resection of the ilium have been suffering from the instability with severe Trendelenburg gait because of the loss of the abductor origin. These results are far from a satisfactory one in modern orthopedic field in which many reconstructive surgeries have been done for the disabled patients. Removal of only one cortex followed with the closed suction irrigation technique are applied in three cases. The result of this operation is as good as that removing the total ilium in controlling the infection, and there are no patients who are suffering from the gait disturbance. The inner or outer cortex could be safely and widely removed without affecting the hip joint of the affected side.

80 cases of Cysticercosis were studied and analysed, which were diagnoses by biopsy at Seoul National University in the past three years from January-1978 to March-1980. The results were summerized as follows. 1. The relative incidence ratio of cysticercosis among 18,941 surgical specimens was 0.42%. 2. The age group between 11 and 50 years old occupied 87.5%. 3. Of the 80 cases, there were 38 males and 42 female patients, without significant sex predominence. 4. The common sites of cysticercosis were upper extremities (35.0%) and anterior surface of the trunk (37.5%) 5. Epidemiologically, there was marked predominence of rural area over urban area. 6. There were 8 cases (10%) of definite cerebral cysticercosis and 4 cases (5%) of suspicious cerebral cysticercosis. 7. In the cases of nodular masses of extremities, the parasitic infection, especiaily cysticercosis celluiosae, must be included in differential diagnosis distinguished from other soft tissue tumors.

CT and myelography, separately or combined together, have been used for the study of spine lesion. Metrizamide enhanced CT is a new diagnostic modality for the study of spine lesion. The purpose of this study was to compare the efficacy of metrizamide enhanced CT with those of CT and metrizamide myelography. Among 163 surgically proven cases, metrizamide CT was carried out on 49 cases, CT on 50 cases and myelography on 64 cases, at Department of Orthopedic Surgery, Seoul University Hospital for 3 years period from August 1981 to July 1984. 1. Overall diagnostic accuracy of CT was 90%, that of metrizamide myelography was 82.8%and that of metrizamide CT was 93. 9%. 2. Diagnostic accuracy of metrizamide CT in HIVD was 95%, in spinal stenosis 94.4% and in spondylolisthesis 80%. 3. Most of complications were observed in myelography, even they were minor. 4. It was concluded that plain CT was advisable on a clinically typical HIVD, considering complications and expenses of myelography and metrizamide CT. 5. Metrizamide CT was indicated on previously operated cases or atypical HIVD, spinal stenosis and spondylolisthesis.

Double-orifice mitral valve is a relatively rare congenital abnormality, usually discovered at autopsy or surgery. In most cases, the double-orifice mitral valve causes no hemodynamic effects, sometimes it is regurgitant, and rarely is stenotic. Appreciation of this echocardiographic abnormality is important because double orifice mitral valve is often associated with other congenital anomalies and this echocardiographic findings may be confused with other cardiac abnormalities. The authors report a case of isolated congenital double-orifice mitral valve in a 42-year-old woman. Data from the literature are reviewed and the echocardiographic images of the malformation are described.

Recent development of dental implantology has taken an interest in the bone graft procedure. This is a review of literature, published from 1994 to November 1999. This study is provided by MEDLINE search. In this study, 718 patients received 829 graft with placing 2,677 endosseous implants. In mandible, nonvascularized or vascularized block bone grafts provided better results (success rate 95.2%) than particulate grafts(83.6%). But in maxilla, particulate grafts provided better results(93.7%, 86.2%) and more cases especially in sinus elevation. There were many cases using autogenous bone graft and revealed good results, but allogenic or alloplastic bone graft materials also were used by many surgeons.

BACKGROUND AND OBJECTIVES: Perfluorocarbon exposed sonicated dextrose albumin (PESDA) microbubbles has been suggested to facilitate thrombus disruption under the transcutaneous ultrasound (US). Thus, we investigated whether such a noninvasive approach could augment thrombolytic effect of fibrinolytic agent in an experimental thrombotic model. MATERIALS AND METHODS: Thrombus formation was induced with electrical injury in the rabbit iliofemoral arteries (n=20): Thrombus occlusion was documented by angiography in all arteries. In the control group, only tissue plasminogen activator (t-PA, 3 mg/kg) was administered intrav-enously in five rabbits. In the Group 1 (n=9), injured arteries were exposed to transcutaneous US (20 kHz, 30 W/cm2, continuous mode) with t-PA (3 mg/kg). In the Group 2 (n=6), the same treatment was given while administering PESDA continuously (10 ml/min, intravenous). Angiographic results were evaluated at 10 minute interval for 1 hour respectively. RESULTS: In the control group, two of five iliofemoral arteries (40.0%) were recanalized and one of nine iliofemoral arteries (11.1%) was recanalized in Group 1. In contrast, four of six iliofemoral arteries (66.7%) were recanalized angiographically in Group 2 (p=0.392 vs. control group: p=0.047 vs. Group 1). However, late reocclusion occurred in all iliofemoral arteries of Group 2. CONCLUSION: Although PESDA with transcutaneous US significantly enhanced initial angiographic patency rate of t-PA, it was associated with high rate of reocclusion. Further studies will be necessary for clinical application of this noninvasive method in acute arterial occlusion.

It is rare to meet an uncontrolled hyperthyroid patient in elective surgery. But, in emergency surgery, recognition of hyperthyroidism may be difficult, as trauma or pain may mask hyperthyroidism. The patient may be unaware of its existence or incapable of transmitting the information. During or after surgery, untreated hyperthyroidism can cause thyrotoxic crisis (thyroid storm), which may be a highly fatal complication. Thyrotoxic crisis is an extreme accentuation of signs and symptoms of throtoxicosis. Clinical manifestations are sinus tachycardia or atrial fibrillation, marked increase in systolic pressure and pulse pressure, high temperature, profuse sweating, tremor, dehydration, tachypnea, extreme restlessness and agitation, delirium, and frank psychosis. In this case, atrial fibrillation and hypertension developed after induction, and severe thyrotoxic symptoms appeared after emergence, which were caused by undetected hyperthyroidism.
