1.Effects of shenmai injection on the clinical efficacy of noninvasive ventilation in patients with severe respiratory failure caused by chronic obstructive pulmonary disease.

OBJECTIVETo study the effect of Shenmai Injection (SMI) on the clinical efficacy of noninvasive positive-pressure ventilation (NPPV) in treating patients with severe respiratory failure (RF) caused by chronic obstructive pulmonary disease (COPD).
METHODSPatients in the control group (n=23) were treated with conventional therapy plus NPPV, and those in the treatment group (n=25) received the same treatment but with intravenous infusion of SMI 30 ml once a day additionally. The treatment lasted for 10 days. Arterial oxygen saturation (SaO2) was continuously monitored in all patients, arterial blood gas values was determined before treatment and at the 6th hour, the 24th hour, the 3rd day and the 7th day after treatment, and consciousness, breath, heart rate and severity of dyspnea were observed during the treating period.
RESULTSAll patients in the treatment group were markedly improved and discharged from hospital, while in the control group 20 were improved and discharged from hospital, but the other 3 failed to be remitted and received the conventional invasive mechanical ventilation. The duration of hospitalization and ventilation were 11.3 +/- 2.5 d and 5.8 +/- 2.8 d in the treatment group, and 13.2 +/- 2.7 d and 7.5 +/- 3.0 d in the control group, showing significant difference in the two indexes between the two groups (P < 0.05). The dyspnea score and the arterial blood gas values determined on the 3rd and 7th day were significantly improved in the treatment group than those in the control group (P < 0.05 or P < 0.01). The incidence of endotracheal intubation caused adverse reaction, such as facial-skin crush wound, abdominal distention and contaminated infection in the treatment group was obviously lessened as compared with the control group.
CONCLUSIONSMI could shorten the duration of hospitalization and mechanical ventilation, reduce trachea cannulation, and decrease the incidence of complications and adverse reactions in the patients with severe RF due to COPD treated with NPPV.

Objective:
To evaluate the predictive value of cardiac magnetic resonance (CMR)-derived parameters on the improvement of left ventricular function in patients with acute viral myocarditis.
Methods:
Forty patients, who referred for acute viral myocarditis in our hospital from September 2011 to September 2015, were prospectively enrolled in this study.All patients were examined by CMR during hospitalization for acute viral myocarditis (baseline) and after 12 months.The CMR sequences include: two dimension steady state free precession, 2D SSFP; triple inversion recovery, triple IR; early gadolinium enhancement; phase sensitive inversion recovery turbo field echo, PSIR TFE.
Results:
Thirty out of 40 patients with susceptive acute viral myocarditis met the CMR criteria of acute viral myocarditis (Lake Louise Criteria) (LL+ ) and the other 10 patients did not meet the diagnostic criteria (LL-). 4.Analysis of the application and effect of the PDCA cycle nursing management model in the treatment of peri-implant mucositis

Objective:
To explore the application and effect of the PDCA cycle nursing management model in the treatment of peri-implant mucositis.
Methods :
Thirty patients with peri-implant mucositis were treated nonsurgically. Before treatment, the 30 patients had no history of systemic diseases, drug allergies, or bad habits. According to the principle of single-blind randomized control, the patients were divided into two groups: 15 patients were assigned to the control group and received routine clinical nursing and oral hygiene education according to the doctor′s prescription; and 15 patients were assigned to the intervention group, in which the PDCA cycle nursing management model was adopted. The four steps of “plan, do, check and act” were carried out. The plaque index (PL), gingival index (GI) and probe depth (PD) in the two groups were recorded before treatment and 3 and 6 months after treatment.
Results:
There was no significant difference in the PL, GI or PD between the intervention group and the control group before treatment (P > 0.05). Three months after treatment, the PL in the intervention group was 1.25 ± 0.44, while the PL in the control group was 1.49 ± 0.39, with a significant difference (t=2.56, P=0.008); the GI in the intervention group was 1.21 ± 0.43, while the GI in the control group was 1.56 ± 0.37, with significant difference (t=2.94, P=0.006); and the PD in the intervention group was 4.39 ± 0.41 while the PD in the control group was 4.47 ± 0.52 mm, with no significant difference (t=2.24, P=0.062). Six months after treatment, the PL in the intervention group was 1.26 ± 0.48, while the PL in the control group was 1.51 ± 0.42, with a significant difference (t=2.66, P=0.007); the GI in the intervention group was 1.34 ± 0.28, while the GI in the control group was 1.74 ± 0.48 (t=2.98, P=0.008); and the PD in the intervention group was 4.46 ± 0.52 mm, while the PD in the control group was 4.54 ± 0.66, with no significant difference (t=2.28, P=0.077).
Conclusion
The PDCA cycle nursing management model can enhance patients′ awareness of oral health maintenance, reduce gingival plaque accumulation, and effectively improve the health status of peri-implant tissues.
5.Cystathionine β-synthase affects the proliferation of lung adenocarcinoma A549 cells through the AKT/cyclin D1 pathway

[摘 要] 目的:探究胱硫醚β合成酶(CBS)通过AKT/cyclin D1通路影响肺腺癌A549细胞的增殖及其分子机制。方法: 通过cBioPortal数据库分析肺腺癌组织中CBS和AKT/cyclinD1相关蛋白的表达,收集的国人肺腺癌组织通过免疫荧光法验证CBS的表达。WB法检测A549细胞及其移植瘤中半胱氨酸代谢和AKT/cyclin D1通路相关蛋白的表达。将pCW57.1-myrAKT质粒分别与RNA干扰对照质粒pGIPZ-CBS-nc-shRNA、干扰质粒pGIPZ-CBS-shNRA1和pGIPZ-CBS-shNRA2共转染A549细胞,共转染组又分多西环素(DOX)诱导组(DOX+)和不诱导组(DOX-),通过细胞集落形成实验检测转染后各组A549细胞的集落形成能力。通过CBSwt A549细胞、CBSI278TA549细胞移植瘤实验检测过表达CBS对移植瘤生长的影响,AzMC荧光染色法和免疫组织化学法分别检测移植瘤组织中H2S水平和Ki-67阳性率。结果:数据库分析和国人组织标本检测均显示,与癌旁组织比较,肺腺癌中CBS呈低表达(均P<0.01);与SV-40细胞比较,A549、SKMES1和NCIH460细胞中CBS蛋白呈低表达(均P<0.01),而p-AKT和cyclin D1蛋白均呈高表达(均P<0.01)。A549细胞中过表达AKT显著促进细胞集落的形成(P<0.01),在此基础上敲减CBS可进一步促进细胞集落的形成(均P<0.01)。移植瘤实验显示,与DOX- CBSWT组比较,DOX+ CBSWT组中CBS、RB和P21蛋白表达升高、H2S含量增加、肿瘤体积变小、Ki-67阳性率降低(均P<0.01);与DOX- CBS I278T组比较,DOX+ CBSI278T组中CBS表达升高(P<0.01),但H2S水平和移植瘤体积变化均不明显。结论: 肺腺癌组织和A549细胞中CBS呈低表达,而AKT/cyclin D1通路呈激活状态,过表达或敲减CBS可抑制或促进A549细胞及其移植瘤的生长,CBS和AKT/cyclin D1通路相关蛋白可能是肺腺癌的潜在靶点和标志物。
7.Mutation characteristics of angioimmunoblastic T-cell lymphoma: an analysis of 75 cases.

Objective: To investigate the characteristics of gene mutations in angioimmunoblastic T-cell lymphoma (AITL). Methods: Seventy-five AITL cases diagnosed at the Department of Pathology, Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine, Shanghai, China from June 2021 to June 2023 were included. Their formalin-fixed and paraffin-embedded or fresh tissues were subject to targeted next generation sequencing (NGS). The sequencing data was collected, and the distribution and type of gene mutations were analyzed. Results: 492 potential driver mutations were identified in 74 out of the 84 genes. Targeted sequencing data for the 75 AITL patients showed that the genes with mutation frequencies of ≥10% were TET2 (89.3%), RHOA (57.3%), IDH2 (37.3%), DNMT3A (36.0%), KMT2C (21.3%), PLCG1 (12.0%), and KDM6B (10.7%). There were significant co-occurrence relationships between TET2 and RHOA, TET2 and IDH2, and RHOA and IDH2 gene mutations (P<0.05), respectively, while TET2 and KDM6B gene mutations were mutually exclusive (P<0.05). Conclusions: The study reveals the mutational characteristics of AITL patients using NGS technology, which would provide insights for molecular diagnosis and targeted therapy of AITL.

8. Clinical analysis of 382 immediately breast reconstruction after mastectomy in Beijing City, Tianjin City and Hebei Province from 2012 to 2016

Objective:
To summarize the status of immediate breast reconstruction (IBR) after mastectomy in Beijing City, Tianjin City and Hebei Province.
Methods:
A retrospective analysis was made on the data of 382 cases with breast cancer who were treated and followed up successfully with immediate breast reconstruction after mastectomy from January 2012 to December 2016 in Beijing City, Tianjin City and Hebei Province. Clinic data of the followed-up 382 cases (all female, age (38.5±4.2) years (range: 24 to 70 years)), including general information, tumor information, sugery methods, and treatments after surgery were collected. The survival status, metastasis,complications and prognosis were followed up. Cosmetic effcet was evalated by Harris method, and life quality by Functional Assessment of Cancer Therapy-Breast scale (FACT-B). χ2 test was used to compare the difference between year 2012 and year 2013 to 2016. Bonferroni method was used to correct the inspection level, which was 0.05/10=0.005. The trend of IBR rate (ratio of IBR to modified radical mastectomy) from 2013 to 2016 was analyzed by trend χ2 test.
Results:
There was 46 cases in stage 0, 152 cases in stage Ⅰ, 165 cases in stage Ⅱ, 19 cases in stage Ⅲ. Twenty-five cases was treated by neoadjuvant chemotherapy, 231 by chemotherapy and 35 by radiotherapy. The proportion of implant reconstruction was 48.7% (186/382), more than expanded of 21.5% (82/382), with latissimus dorsi of 12.0% (46/382), TRAM of 8.9% (34/382), DIEP of 2.1% (8/382), and latissimus plus implant of 6.8% (26/382). According to the Harris standard, the excellent and good rate of the cosmetic effect of the reconstructed breast was 93.7%. From January 2019 to December 2021, overweight and obese children who visited in health outpatient Center of Hunan Children's Hospital were studied to explore and analyze the rate, related factors and patterns of multimorbidity of overweight and obesity-related diseases in children in Hunan Province. Univariate and multivariate logistic regression models were used to analyze the multimorbidity-related factors of overweight and obesity-related diseases in children. Association rules (apriori algorithm) were used to explore the multimorbidity patterns of overweight and obesity-related diseases in children. A total of 725 overweight and obese children were included in this study. The multimorbidity rate of overweight and obesity-related diseases in children was 46.07% (334/725). Age, waist circumference, the frequency of food consumption such as hamburgers and fries and adding meals before bedtime were multimorbidity-related factors of overweight and obesity-related diseases in children. The multimorbidity associated with nonalcoholic fatty liver disease (NAFLD) was relatively common. The patterns with the top three support degrees were "NAFLD+dyslipidemia","NAFLD+hypertension" and "NAFLD+hyperuricemia". The patterns with the top three confidence and elevation degrees were "Hypertension+dyslipidemia => NAFLD","Hyperuricemia => NAFLD" and "NAFLD+hypertension => dyslipidemia".

10.A close contact of coronavirus disease 2019 with severe imported malaria: a case report.

This article presents a severe cerebral malaria patient in shock with a close contact of COVID-19 that was successfully cured in a negative pressure ward during the global pandemic of COVID-19. The patient experienced a sudden onset of high fever and coma in a designated isolation hotel after returning from Africa, and was transferred to a designated hospital. Following antimalarial therapy, blood pressure elevation, increase of blood volume, bedside hemodialysis, mechanical ventilation, plasma and platelet transfusions, the case gradual recovered.
