2. Problems and suggestions for the application of diagnosis-related groups system in burn surgery

The diagnostic codes of diagnosis-related groups system used in our country are different from the clinical diagnostic criteria of burns. The author suggests that the diagnostic codes should be improved according to clinical criteria in terms of the total area and depth of burns and inhalation injury, and the diagnosis of burns with related special causes and sites should be added to the clinical criteria.

Objective:
To observe the neuro-protective effect of Levocarnitine on severe hand, foot and mouth disease (HFMD) after enterovirus 71(EV71) infection, to preliminarily explore the possible mechanism preliminarily.
Methods:
One hundred and thirty-two children with EV71 infection and HFMD combined with serum S100 protein and neuronspecific enolase (NSE) abnormalities who were admitted to Children′s Hospital Affiliated to Zhengzhou University from March 2015 to July 2016 were enrolled in the study.They were divided into the routine group and the Levocarnitine group by the random number grouping method.The routine group (66 cases, including 32 males and 34 females, median age of 2 years and 3 months) was given symptomatic treatment such as antiviral therapy while the Levo-carnitine group (66 cases, including 36 males and 30 females, median age of 2 years and 5 months) was treated with Levocarnitine for neuroprotection on the basis of routine group.Forty healthy children (23 males and 17 females, median age of 2 years and 6 months) who were examined at the Children′s Hospital Affiliated to Zhengzhou University during the same period were selected as the healthy control group.The levels of S100, NSE, soluble apoptosis-related factors (sFas), soluble apoptosis-related factor ligands (sFasL), malondialdehyde (MDA), superoxide dismutase (SOD) in serum were compared between the healthy control group and children with HFMD.The levels of above-mentioned indexes in cerebrospinal fluid and serum, efficacy-related indicators such as duration of fever, white blood cell count on the 3rd day of treatment, time to remission of nervous system symptoms, time of disease progression and critical conversion rate were compared between 2 groups of children with HFMD.The correlation between sFas, sFasL, MDA, SOD and S100, NSE was performed

Objective:
Application of Clinical and Laboratory Standards Institute evaluation protocols-12 approved guideline 2nd edition (CLSI EP12-A2) and EP15-A2 documents in the performance evaluation of Adenovirus IgM CLIA microparticles.
Methods:
Referring to the EP15-A2 method , three samples of high and low concentration were selected. Each sample test was repeated 4 times one day for 5 days, and the total imprecision was calculated. 5.Research progress of microglia in eye disease

Microglia are the resident macrophages of the central nervous system(CNS). Microglia constantly monitor the pathological changes of the surrounding microenvironment. Activated microglia play key roles in tissue protection and injury under the stimulation of various injury factors. Abnormal activation of microglia mediates retinal inflammation and aggravates the original ophthalmic diseases. Our growing understanding of the mechanisms of the interaction between microglia and retinal cells is identifying potential cellular and molecular targets for therapeutic interventions that may be apply to optic nerve injury, diabetic retinopathy, glaucoma and other diseases in the future. In this review, potential links between ophthalmic diseases and microglial activity will be explored.
6.Surgical management of neonate lip and palate cleft.

OBJECTIVETo explore the security and feasibility in ambi-operation of the neonate lip and palate cleft correction.
METHODSUsing Ornizuka's and innovated Langenbeck's method to correct the lip and palate cleft in 30 cases with detailed program, the effect after operation was evaluated.
RESULTSAll physiological index were normal in the group of ambi-operation, which had no difference to other newborn. The effect is satisfactory. 5 of them have the fistula under the gingival.
CONCLUSIONSBased on controlling on surgery indication, screening the well-developed newborn, it is secure and feasible to correct lip and palate in one stage, and it can get excellent effect.

8. Application of blood purification in children with severe adenoviral disease

Severe hypoxia respiratory failure or acute respiratory distress syndrome, acute renal injury, liver function disorder, and multiple organ dysfunction syndrome caused by severe adenoviral disease have high mortality.Blood purification have become more widely available for acute renal injury, fluid overload, hyperlactacidemia, inflammatory reaction, coagulopathy and so on.The methods include renal replacement therapy(RRT) and therapeutic plasma exchange, usually combined with extracorporeal membrane oxygenation (ECMO). The appropriate blood purification strategy has not been available.Based on experience of RRT in acute renal injury, liver dysfunction, currently continuous renal replacement therapy/RRT technologies are reasonable used by personalized decision-making.
9.Efficacy of telbivudine combined with adefovir dipivoxil and pegylated interferon combined with entecavir as antiviral therapy for chronic hepatitis B: a comparative analysis

ObjectiveTo investigate the efficacy of telbivudine combined with adefovir dipivoxil and PEG-IFN-α-2a combined with entecavir as the antiviral therapy for HBeAg-positive patients with chronic hepatitis B (CHB) and a high viral load. MethodsA total of 80 previously untreated HBeAg-positive CHB patients with a high viral load who were treated in The Sixth People′s Hospital of Qingdao from November 2012 to November 2015 were enrolled and randomly divided into two groups. The patients in group A were treated with telbivudine combined with adefovir dipivoxil, and those in group B were treated with PEG-IFN-α-2a combined with entecavir. Biochemical and virologic response rates and seroconversion rate were observed at weeks 12, 24, and 48 of administration. The drug resistance rate and incidence rates of adverse events were observed in both groups. The Chi-square test was applied for comparision of categorical data between the two groups, and the t-test was applied for comparision of continuous data between the two groups. ResultsBiochemical and virologic response rates at weeks 12, 24, and 48 of administration showed no significant differences between the two groups. The seroconversion rates at weeks 12 and 24 of administration showed no significant differences between the two groups, while at week 48 of administration, group B had significantly higher HBeAg clearance rate and seroconversion rate than group A (450% vs 675%, 225% vs 450%,χ2=4.114 3 and 4.528 3, both P<0.05). Group B had significantly higher incidence rates of adverse events than group A. ConclusionTelbivudine combined with adefovir dipivoxil and PEG-IFN-α-2a combined with entecavir can effectively inhibit the replication of high-load HBV DNA, delay disease progression, and realize HBeAg seroconversion. However, the two regimens have their own advantages and disadvantages and should be selected according to clinical situation.
10.Sexual and reproductive health knowledge, attitude, and behavior of young migrant workers in Shanghai

Objective To evaluate the current status of sexual and reproductive health knowledge and behavior of young migrant workers in Shanghai, which would provide information for the development of intervention measures. Methods A cluster random sampling with self-filled questionnaires was used for survey in two factories of Shanghai.The total number of young migrant workers surveyed was 713.The questionnaire contents included socioeconomic characters, knowledge, attitude and behavior of sex and reproductive health. Results There were 428 males and 285 females ranged from 17 to 24 years old.Eighty-eight percent of them were from rural area of China, and 79.7% of them were unmarried.Their awareness of sexual and reproductive health was low, including misunderstandings of sexually transmitted diseases and HIV transmission pathways, and failure to realize the difference between condom and contraceptive pills.Only 46.5% of the males and 31.5% of the females answered correctly about the role of contraceptive pills.Their sexual attitude was open on premarital sex and cohabitation.Males were more tolerant than women; 59.1% of the males thought they could have sex before marriage if they got consent, which was much higher than females (39.6%,
