2.Pulsed Dye Laser Fragmentation of Urinary Calculi : Clinical Experience.

The pulsed dye laser, emitting at wavelengths of 504nm. for 1 micro sec. at a frequency of 10 Hz. transmitted via a 200 micron in-diameter silicon-coated fiber, was send for fragmentation of urinary stones. The mechanism of stone fragmentation by laser is 'shock wave' formation by laser light ( photoacoustic effect). From Nov. 1988 to Apr. 1990, we performed 89 procedures in 86 patients for the removal of stones (80 procedures in 77 ureteral stones, 3 procedures in bladder stones and 6 procedures in urethral stones). Of the calculi 80 were fragmented to spontaneously passable or easily extractable fragments. There was no significant complications attributable to laser lithotripsy. And there was no significant differences in the fragmentation of stones according to the component. So, we think that laser lithotripsy is a safe and effective modality in management of the urinary stones including renal and upper ureter stones with some exceptions if deflecting ureterorenoscope is available.

3.CT findings of early acute cerebral infarction.

The CT findings of the acute cerebral infarction are well known. However the CT findings of early stroke within 24 hours of the onset have not been sufficiently reported. The purpose of this study is to evaluate early acute cerebral infarction on CT within 24 hours after ictus. The early and accurate CT diagnosis could lead to the appropriate therapy and improved outcome of the patients. Authors retrospectively analyzed 16 patients with early acute cerebral infarction. Acute cerebral infarction was confirmed by follow-up CT in 11 patients, SPECT in 4 patients, and MRI in 1 patient. The CT findings of early acute cerebral infarction include effacement of cortical sulci or cistern (n=16, 100%), hyperattenuation of MCA (n=3), obscuration of lentiform nucleus (n=6), loss of insular ribbon (n=6) and subtle low density in hemisphere (n=5). The most frequent findings was effacement of cortical sulci in our study, and it was though to be the most important sign of early acute cerebral infarction.

5.Structual Equation Modeling on Adjustment of Cancer Patients Receiving Chemotherapy.

PURPOSE: This study was to understand and explain the adjustment of cancer patients receiving chemotherapy. Variables in this model were social support, uncertainty, resilience and coping. METHODS: The data were collected for approximately five months from May through to September 2006 with cancer patients receiving chemotherapy or cancer patients finished chemotherapy using self-administered questionnaires. Descriptive statistics program, Amos 5.0, was employed to verify the goodness of the hypothetic model and the hypotheses formulated in the study. RESULTS: The fitness of the finalized model turned out to be improved (chi2=332.644, df=70, GFI=.809, AGFI=.714, RMSEA=.138, AIC=402.644). Those variables made a 60.8% prediction of the patients adjustment. CONCLUSION: The hypothetic model presented in the study could serve to explain the adjustment of cancer patients receiving chemotherapy. Moreover, the use of the conceptual framework of the theory of planned nursing intervention would be very worthwhile as a strategy to boost the adjustment of cancer patients when receiving chemotherapy.

6.Calcific Myonecrosis of the Calf

Calcific myonecrosis is a rare late post-traumatic condition in which an entire single muscle is replaced by a fusiform mass with central liquefaction and peripheral calcification. The compartmental syndrome is suggested to be the underlying cause. We report a case of 70 year-old man whose right tibialis anterior and extensor hallucis longus muscle were replaced by calcific myonecrosis and treated with en-bloc resection & tendon reconstruction.

7.Effect of Emotional Coaching Program for Clinical Nurses on Resilience, Emotional Labor, and Self-efficacy

Purpose:
This study aimed to assess the effect of the emotional coaching program for hospital nurses.
Methods:
The study used anon-equivalent control group pretest-posttest design, and participants included 60 nurses (30 in the experimental group and 30 in thecontrol group) who worked at a general hospital. The experimental group attended four sessions, one per week, with each session lastingtwo and a half hours. Collected data were analyzed using descriptive statistics, Fisher’s exact test, x2 test, t-test, paired t-test, and repeatedmeasures ANOVA using SPSS WIN 23.0 program.
Results:
Significant differences were shown between the experimental and the controlgroups regarding emotional labor (F=68.40, p<.001), resilience (F=48.77, p<.001), and self-efficacy (F=15.31, p<.001).
Conclusion
Theemotional coaching program for nurses is useful for enhancing nurses’ emotional labor management, resilience, and self-efficacy. In addition,this program may serve as a basis for providing emotional coaching to nurses in the future.

Subcutaneous emphysema is a complication of the pneumoperitoneum necessary to perform laparoscopy and will be seen more often as laparoscopic techniques are applied to a growing number of intraabdominal procedures. We report a case of subcutaneous emphysema and hypercarbia without pneumothorax or pneumomediastinum during laparoscopic cholecystectomy, which was treated by multiple puncture with 18G needle on emphysematous site. The suspected cause is inadvertent subcutaneous insufflation of carbon dioxide through the trocar sites by increased intra-abdominal pressure for the establishment of pneumoperitoneum. Immediate recognition, evaluation, and treatment of subcutaneous emphysema is necessary since this can be life-threatening complication.
