1.Clinical analysis of vertebral pedicle bone graft after posterior pedicle screw fixation in the treatment of tho-racolumbar fracture

Objective To analyze the clinical effects of vertebral pedicle bone graft after posterior pedicle screw fixation in the treatment of thoracolumbar fractures .Methods 80 cases with thoracolumbar fractures were ran-domly divided into the study group ( 40 cases ) and control group ( 40 cases ) .The study group used simple pedicle screw fixation , posterior reduction , and the control group adopted conventional inter vertebral internal fixation .The operation time,hemorrhage were compared between the two groups .The clinical effects of posterior transpedicular in-strumentation of bone graft and vertebral pedicle screw in fractured vertebra in the treatment of thoracolumbar fractures were analyzed.Results The operation time of study group was (110.0 ±14.9)min,which was longer than that of the control group[(91.0 ±13.7)min](t=5.93,P<0.05).The intraoperative blood loss of the study group and the control group were (270.0 ±34.4) mL,(258.0 ±42.5) mL,respectively,with no significant difference between the two groups(t=1.38,P>0.05).At the end of the follow-up after the operation,the anterior vertebral height,Cobbangle,VAS scores in the study group were (91.3 ±1.7)%,(11.2 ±1.9)°,(2.5 ±0.2)%,which were better than (86.6 ±2.3)%,(14.5 ±1.1)°,(3.5 ±0.9)% in the control group (t =5.25,3.24,6.17,all P <0.05). Conclusion Vertebral pedicle bone graft posterior pedicle screw fixation in the treatment of thoracolumbar fractures can effectively correct the kyphosis ,reconstruct vertebral height ,increase the stability of the vertebral body ,reduce and prevent postoperative vertebral height lost ,loosening and fracture fixation complications .
2.Comparison of the effect of PFNA and DHS in the treatment of elderly patients with intertrochanteric femur fracture

Objective To compare the effects of proximal femoral nail antirotation ( PFNA) and dynamic hip screw ( DHS) in the treatment of elderly patients with intertrochanteric femur fracture .Methods According to the digital table ,92 elderly patients with intertrochanteric femur fracture were randomly divided into the PFNA group ( n=46) and the DHS group(n=46).The PFNA group was treated with PFNA and the DHS group was treated with DHS . The clinical effect was compared between the two groups .Results The operation time ,postoperative ambulation time , hospitalization time,blood loss and postoperative wound drainage of PFNA group were (58.3 ±19.7) min,(7.4 ± 4.1)d,(16.1 ±6.2)d,(97.6 ±34.7)mL,(64.7 ±12.4)mL,respectively,which were better than those of DHS group[(81.3 ±20.4)min,(11.4 ±3.8)d,(19.7 ±5.8)d,(206.3 ±35.4)mL,(75.2 ±12.8)mL](t=5.500, 4.853,2.875,14.872,3.996,all P<0.01).1 year after operation,the Harris score of hip joint of PFNA group was (88.5 ±6.2)points,which was higher than that of DHS group [(84.3 ±5.8)points](t=3.355,P<0.01).The rate of good hip function was 93.5%,which was significantly higher than that of DHS group (73.9%)(χ2 =6.451,P<0.05).The incidence rate of postoperative complications between the two groups had no statistically significant differ -ence(χ2 =1.011,P>0.05).Conclusion PFNA has shorter operation time,and earlier postoperative ambulation time,good recovery of hip function .It also has smaller surgical trauma ,but not increase the incidence of postoperative complications in elderly patients with intertrochanteric femur fracture ,it's worthy of promotion .
3.Diagnosis and treatment of chondroblastoma

Objective To summarize the keypoints of clinical diagnosis of chondroblastoma and the experience on the treatment Methods The clinical materials of 16 cases(11 males,aged from 12 to 26 years)from June 1992 to February 2003 of pathologically diagnosed chondroblastoma were analyzed retrospectively. The tumor located at distal femur in 6 cases,great trochanter in 1 case,proximal tibia in 6 cases,proximal humerus in 2 cases, talus in 1 case. The main symptoms were swelling and painful joint. All the 16 cases were treated with curettage and autologous bone graft from iliac crest. Results Twelve cases were followed up for 6 months to 5 years .averaging 3. S years. All the bone graft healed well. One case., recurred 2 years after the initial curettage and was treated with curettage again. No recurrence occurred at the last follow-up 6 months after the second curettage. The function of the involved joints was nearly normal in all cases except for one case with slight limitation of knee flexion. Conclusions Mastering the image and clinical characteristics of chondroblastoma can avoid misdiagnosis and mistreatment Curettage can control the tumor satisfactorily. Postoperative rehabilitation can improve the function of the involved limb.
4.High developmental dislocation of the hip in patients with non bone cement (biological) and curative effect of total hip replacement operation method

Objective To investigate the clinical effect of high developmental dislocation of the hip in patients with total hip replacement operation to take biology.Methods Thirty-six cases of high developmental dislocation of the hip by patients as the object of study from January 2011 to February 2014 in the First People' s Hospital of Shangqiu, all patients underwent cementless total hip joint replacement operation, 6 months to 2 years after surgery, observed the clinical effect of all patients before and after treatemnt.Results The operation time was 1.20-3.0 h,the average operation time was (2.13±1.01) h,amount of bleeding during the operation of 300.00-700.00 ml,the average amount of bleeding was (342.43±34.23) ml;follow up of 6 months to 2 years, all patients had no prosthesis loosening and dislocation of the hip;all patients after treatment, Harris score (81.56± 14.34) points, better than that before treatment, the difference was statistically significant ((54.34 ± ±9.78) points,P=0.009);The excellent and good rate of all the patients after the treatment (97.22%) .Conclusion The clinical effect of high developmental dislocation of the hip by total hip replacement operation in patients taking biological type significantly, has the advantages of simple operation, effectively relieve pain, improve the life quality of patients and other advantages, the ideal treatment method can be used as high developmental dislocation of hip.
5.Transpedicular osteotomy and internal fixation in the treatment of old thoracolumbar vertebral fracture with kyphosis clinical study with spinal stenosis

Objective To investigate the clinical effect of old thoracolumbar vertebral fracturekyphosis with spinal stenosis by pedicle of vertebral archosteotomy and internal fixation.Methods Twenty patients with old thoracolumbar fracture with kyphosis,and associated with spinal stenosis,were treated by using transpedicular osteotomy and internal fixation treatment in the First People's Hospital of Shangqiu from June 2012 to May 2014.Radiographical assessments including localized kyphosis,thoracic kyphosis,lumber lordosis,sacral tilt angle,sagittal vertical axis,bony fusion and the relative height of the ifiterbody fusion vertebra.Visual analogue scale(VAS),and ASIA were evaluated before and after surgery,and the operative duration,blood loss were recorded.Results The operation carried out in 120-200 min,800-1600 ml of bleeding statistics through operation process.Localized kyphosis was reduced from an average of (42.6±4.2) ° to (8.2± 1.6) °,the difference was statistically significant (P =0.012).All patients were followed up for 10.0 -26.0 months,average (18.0±6.2) months.The average VAS score was 6.88±0.82 before operation and 2.10 ±0.84 at final follow-up,the difference was statistically significant(P=0.023).The bone healed well,no false joints.Neurological function improved from C to D in 2 patients,from C to E in 4 patients,and from D to E in 14 patients.Conclusion Old thoracolumbar vertebral fracture kyphosis with spinal stenosis by transpedicularosteotomy and internal fixation in the treatment of simple operation,definite effect,less complications and can effectively improve the prognosis.
6.Network analysis of the association between adverse childhood experiences and mental health status in college students

Objective:
To investigate the relationship between adverse childhood experiences (ACEs) and mental health status through network analysis in college students, so as to provide empirical evidence for the development of targeted intervention programs.
Methods:
From October to November 2023, a survey was conducted on 1 974 college students from three universities in Hubei Province by using ACEs Questionnaire and Symptom Checklist-90 (SCL-90). The network analysis method was used to construct a network structure to analyze the association between ACEs and SCL-90 symptom factors among college students.
Results:
The network structure of SCL-90 symptom factors and ACEs was stable, each forming a relatively tight cluster; there were 89 non zero edges between ACEs and SCL-90 symptom factors. Emotional neglect-depression played a significant pathway role in connecting the ACEs and SCL-90 symptom factors network. Depression, anxiety, and interpersonal sensitivity of SCL-90 symptom factors had the highest centrality in the ACEs and SCL-90 symptom factors network. Emotional neglect and physical neglect in ACEs and depression and psychoticism in SCL-90 symptom factors had a high node bridge expected influence, acting as Touchpoints to activate the whole network.
Conclusion
Early identification and intervention of emotional and physical neglect, with a focus on depression, anxiety, and interpersonal sensitivity symptoms of childhood neglected individuals, which is beneficial for improving the overall mental health level of college students.
7.Evaluation for optimization effects of sperm floating plate

Objective To compare the difference between sperm floating plate and density gradient centrifugation combined with swim-up in human sperm preparation.Methods The semen samples were obtained from 50 infertile men in the clinic of Reproductive Medi-cine of Northwest Women's and Children's Hospital excluding azoospermia,severe oligoasthenozoospermia and semen volume less than 2 mL.After semen liquefaction,the differences of sperm concentration,total progressively motile sperm count(TMSC),percentages of progressively motile sperm and normal morphology sperm,recovery rate and DNA fragmentation index(DFI)were measured by both the methods of sperm floating plate and density gradient centrifugation combined with swim-up,and the results were compared.Results Compared with the pre-sorting samples,sperm concentrations[(16.08±13.39)x 106/mL,(8.88±8.06)x 106/mL vs(60.05± 27.21)×106/mL],TMSC[(7.41±6.14)×106,(3.98±3.57)×106vs(22.24±13.74)×106]and DFI[(2.20±3.44)%,(5.20± 10.79)%vs(26.38±13.92)%]in the sorting groups by sperm floating plate and density gradient centrifugation combined with swim-up were decreased significantly,and the percentages of progressive motile sperm[(91.67±4.75)%,(87.86±7.90)%vs(40.21± 16.83)%]and normal morphology sperm[(9.58±5.08)%,(7.72±4.01)%vs(3.58±2.06)%]were increased significantly.Com-pared with the density gradient centrifugation combined with swim-up,the results of sperm floating plate were higher in sperm concen-tration,percentages of progressively motile sperm and normal morphology sperm,TMSC and sperm recovery rate[(30.74±13.70)%vs(17.09±9.20)%],but DFI was lower,time-consuming was shorter[(32.38±1.01)min vs(60.08±2.06)min],and the difference between the two groups was statistically significant(P<0.05).Conclusion The sperm floating plate may have certain clinical applica-tion prospects in the future due to better parameters of sperm preparation than those of density gradient centrifugation combined with swim-up in simple operation and shorter time-consuming.
8.Role of microchromosome maintenance protein 5 in promoting malignant progression and its mechanism in glioblastoma

Objective:To investigate the role of microchromosome maintenance protein 5 (MCM5) in promoting malignant progression and its mechanism in glioblastoma.Methods:(1) Three freshly excised brain tissues from non-tumor patients and 3 grading IV glioblastoma tissues were collected in our hospital from September 2020 to September 2021; mass spectrometry and quantitative proteomic analysis were performed to screen differentially expressed proteins for functional enrichment analysis. (2) The target protein MCM5, which was highly expressed in glioblastoma, was screened on the basis of proteomics, and its expression in glioblastoma was verified using The Cancer Genome Atlas (TCGA) database and further validated at the mRNA level in the collected clinical specimens. (3) U251 cells were divided into negative control group, knockdown group-1 (si-1 group) and knockdown group-2 (si-2 group) by siNRA transfection. The regulation role of MCM5 in malignant phenotype of glioblastoma was detected by CCK-8 assay, clone formation assay, 5-ethynyl-2'-deoxyuridine (EdU)staining, Transwell invasion assay and flow cytometry. (4) The transcriptome data of glioma patients from TCGA database were used to explore the possible molecular mechanisms of MCM5 regulation in the malignant process of glioblastoma by gene set enrichment analysis (GSEA) algorithm.Results:(1) In clinical samples of glioblastoma, 322 up-regulated proteins and 94 down-regulated proteins were screened out; MCM5 was highly expressed in these 3 glioblastoma samples. (2) Based on TCGA database, results of 163 patients with glioblastoma and 207 patients with non-tumor brain tissues showed that MCM5 expression was statistically up-regulated in glioblastoma ( t=3.340, P<0.001). Real-time quantitative PCR results of 3 glioblastoma tissues and 3 non-tumor brain tissues clinically collected in our hospital also indicated that significantly increased MCM5 expression in glioblastoma was noted as compared with that in the non-tumor brain tissues ( t=3.876, P<0.001). (3) As compared with the negative control group, the si-1 and si-2 groups had significantly decreased MCM5 mRNA and protein expressions, significantly lower proliferation rate 5 d after inoculation, statistically decreased number of cell clones, significantly decreased proportion of EdU positive cells, and significantly increased proportion of cells at G1 phase, and significantly impaired migration ability ( P<0.05). (4) GSEA analysis showed that mRNA in MCM5 high expression group was enriched in DNA damage repair gene, E2F target gene, MYC target gene, epithelial-mesenchymal transformation (EMT), nterleukin 6-Janus kinase-signal transduction and transcription activation factor 3 (IL6-JAK-STAT3), interferon, KRAS, NOTCH, transforming growth factor-β (TGF-β), Wnt/β-Catenin and other characteristic genes. Conclusion:MCM5 is highly expressed in glioblastoma, and MCM5 regulates the malignant progression of glioblastoma through multiple mechanisms including E2F, MYC, EMT, and Wnt/β-Catenin.
9. Preventive Effect of Panax notoginseng Saponins Combined With Bacillus subtilis on Colorectal Cancer Based on Microbiome

Background: Dysregulation of intestinal flora is a key risk factor for colorectal cancer (CRC). In recent years, traditional Chinese medicine preparations and probiotics have been increasingly applied in the prevention of CRC. Aims: To investigate the preventive effect of Panax notoginseng saponins (PNS) combined with Bacillus subtilis on CRC. Methods: Thirty female BALB/c mice were randomly divided into normal control group (NC group), model group, PNS group, Bacillus subtilis group and PNS combined with Bacillus subtilis group (PaB group). CRC mice model was constructed by azoxymethane (AOM)/dextran sulfate sodium (DSS) method. During the experiment, the mice were weighed, and disease activity index (DAI) score was evaluated. The length of colorectum and tumor number were measured. Serum interleukin (IL) - 6 and IL - 10 contents were determined by ELISA. 16S rRNA sequencing was used to analyze the composition of intestinal flora. Results: Compared with model group, DAI score was significantly decreased (P<0.001), colorectal length was significantly increased (P<0.001), number of tumor was significantly decreased (P<0.001), tumor volume was significantly decreased (P<0.01), serum IL-6 content was significantly decreased (P<0.000 1), and serum IL-10 content was significantly increased in PaB group (P<0.000 1). The results of intestinal flora sequencing showed that Simpson index was significantly decreased in PaB group than in model group (P<0.05), Shannon index and Chao index were significantly increased (P<0.05), abundance of Bacteroidota was significantly increased (P<0.01), abundances of Firmicutes, Helicobacter and Oscillibacter were significantly decreased (P all <0.05), abundance of Lactococcus was significantly increased (P<0.05). Conclusions: The combination of PNS and Bacillus subtilis can effectively alleviate the occurrence of CRC caused by AOM/DSS, and its mechanism may be related to the improvement of composition of intestinal microbial community.
10.Outcomes of total cavopulmonary connection in the treatment of functional single ventricle with heterotaxy syndrome: A propensity score matching study

Objective To comprehensively analyze the clinical outcomes of total cavopulmonary connection (TCPC) in the treatment of functional single ventricle combined with heterotaxy syndrome (HS). Methods A retrospective analysis was conducted on the patients with functional single ventricle and HS who underwent TCPC (a HS group) in Guangdong Provincial People's Hospital between 2004 and 2021. The analysis focused on postoperative complications, long-term survival rates, and identifying factors associated with patient survival. Early and late postoperative outcomes were compared with matched non-HS patients (a non-HS group). Results Before propensity score matching, 55 patients were collected in the HS group, including 42 males and 13 females, with a median age of 6.0 (4.2, 11.8) years and a median weight of 17.0 (14.2, 28.8) kg. Among the patients, there were 53 patients of right atrial isomerism and 2 patients of left atrial isomerism. Eight patients underwent TCPC in one stage. TCPC procedures included extracardiac conduit (n=39), intracardiac-extracardiac conduit (n=14), and direct cavopulmonary connection (n=2). Postoperative complications included infections in 27 patients, liver function damage in 19 patients, and acute kidney injury in 11 patients. There were 5 early deaths. The median follow-up time was 94.7 (64.3, 129.8) months. The 1-year, 5-year, and 10-year survival rates were 87.2%, 85.3%, and 74.3%, respectively. After propensity score matching, there were 45 patients in the HS group and 81 patients in the non-HS group. Compared to the non-HS group, those with HS had longer surgical and mechanical ventilation time, higher infection rates (P<0.05), and a 12.9% lower 10-year survival rate. Multivariate Cox regression analysis identified asplenia was a risk factor for mortality (HR=8.98, 95%CI 1.86-43.34, P=0.006). Conclusion Compared to non-HS patients, patients with HS have lower survival rates after TCPC, and asplenia is an independent risk factor for the survival of these patients.
