Spontaneous hemorrhage from adrenal tumor is most commonly due to rupture of a pheochromocytoma and sometimes due to adrenocortical carcinoma or adrenal cyst. We report 2 cases of spontaneous retroperitoneal hemorrhage that resulted from adrenal tumor, one is an adrenocortical carcinoma and the other an adrenal cyst. They showed atypical imaging features on ultrasonography or computerized tomography.

5.Value of Ultrasound Diagnosis after Testicular Trauma.

The use of ultrasound for evaluation of blunt testicular injury allows contusion to be differentiated from rupture, and some authors advocate reserving surgical management for testicular rupture. We reviewed 22 operated cases of blunt scrotal trauma, 10 cases of them were evaluated with ultrasound preoperatively. In 8 of 10 cases, the tunica albuginea rupture was correctly diagnosed by ultrasonography but there were one false-positive and one false-negative diagnosis of rupture. Without preoperative ultrasound diagnosis, 12 cases were explorated because of suspicious testicular rupture only by physical examination. Three cases of them had intact tunica albuginea. These results suggest that the ultrasound diagnosis of blunt scrotal trauma is not sufficiently accurate to eliminate is highly sccurate, readily available.

Epidermal inclusion cyst of the breast is an uncommon benign lesion and it is usually located in the skin layer. We report here on two cases of ruptured epidermal inclusion cysts in the subareolar area, which is a very unusual location for these cysts and these lesions can be mistaken for breast malignancies.

Simple liver cysts are common and benign lesions, and most of them are detected by ultrasonography (US) or computed tomography (CT) during a medical checkup. Because simple liver cysts are generally asymptomatic, they do not require treatment, although hemorrhage, infection or rupture may occur in rare cases and these cases must receive proper treatment. The diagnosis of simple liver cysts is usually easy because of their typical morphological findings on US or CT. However, when a liver cyst contains hemorrhage or it is inflamed on imaging studies, it is sometimes difficult to differentiate simple hepatic cysts from such conditions as cystadenoma and cystadenocarcinoma. In this report, we describe a case of simple liver cyst that was complicated by intracystic hemorrhage, and this malady was initially diagnosed as biliary cystadenocarcinoma. We successfully treated this lesion by left lateral segmentectomy.

8.Avulsion Rupture of Flexor Digitorum Profundus in Zone 1, Diagnosed in Early Stage.

Avulsion injury of the flexor digitorum profundus (FDP) tendon from the distal phalanx is considered as a rare injury. We report a patient who sustained a FDP tendon rupture insertion on her fifth finger as a result of a closed, hyperextension with no accompanying laceration or predisposing pathologic condition. Preoperative ultrasonography showed complete rupture of FDP and the gap between the tendon ends. According to the classification by Leddy and Pacter, this case is type II. The reconstruction of flexor tendon using pull-out suture and tie-over button. We are reporting a case with brief review of literatures.

9.The Role of Ultrasonography in Blunt Testicular Trauma.

The ultrasonographic features in 26 cases of blunt testicular trauma were reviewed. Ultrasonography diagnosed correctly in 11 surgically proven cases of testicular rupture. There were three false-positive cases, two with a large scrotal hematocele and one with epididymis rupture. there were no false-negative cases. The sensitivity and specificity of ultrasonographic diagnosis were 100% and 80% respectively. The positive and negative predictive values were 78. 6% and 100% respectively. Therefore, the negative diagnosis by ultrasonography can warrant conservative therapy.

Visceral artery pseudoaneurysms occur mostly as a result of inflammation and trauma. Owing to high risk of rupture, they require early treatment to prevent lethal complications. Knowledge of the various approaches of embolization of pseudoaneurysms and different embolic materials used in the management of visceral artery pseudoaneurysms is essential for successful and safe embolization. We review and illustrate the endovascular, percutaneous and endoscopic ultrasound techniques used in the treatment of visceral artery pseudoaneurysm and briefly discuss the embolic materials and their benefits and risks.
