The incidence of testicular rupture by blunt trauma has been considered rare. But if testicular rupture has developed, the diagnosis of which is difficult. In the 50 cases of scrotal trauma patients, we investigated in the frequency of testicular rupture and early exploration in comparison with delayed exploration. Then the incidence of testicular rupture was in 46.1% of the cases of exploration, and early exploration of for scrotal trauma has resulted in higher testicular salvage rate than delayed exploration.

The intradural lumbar disc herniation which was first described by Dandy in 1942 is unusual. The pathogenesis is obscure and preoperative diagnosis is not easy. The patient usually have more severe neurologic deficits than those found in the much more common extradural disc herniations. We experienced two cases of the intradural lumbar disc herniation who had previous disc operations and the symptoms and neurologic signs were improved after removal of the intradural sequestration of disc fragments.

Rupture of the interventricular septum is a serious complication of acute myocardial infarction, accounting for 5% of death due to acute myocardial infarction. The mortality with medical therapy alone exceeds 90%. Accurate diagnosis, urgent management, and early operative correction are necessary for survival. We report cases of 3 patients with ventricular septal rupture after acute myocardial infarction.

A 14-yr-old male presented with a hemoperitoneum caused by a spontaneous rupture of a primary splenic cyst. The laparotomy showed a ruptured spleen at the lower pole from one of the multiple cyst-like lesions, measuring approximately 2 cm in diameter. After the splenectomy, a microscopic examination confirmed the diagnosis of a primary splenic cyst lined with a mature, well-differentiated squamous epithelium. A hemoperitoneum caused by a spontaneous rupture of a primary splenic cyst is a rare but potentially lethal complication, because most patients with a splenic rupture present with some degree of hypovolemia.

CA-125 is an antigenic determinant expressed by more than 80% epithelial ovarian cancer. Although developed for use in the diagnosis and monitoring of epithelial ovarian cancer, elevations of CA-125 concentrations can also be associated with some normal physiologic conditions, various other malignancies and benign conditions. Adnexal cysts are frequently encountered during the reproductive years. 10 to 35% of benign cystic adnexal masses may be an endometrioma. Because endometriomas are not likely to resolve spontaneously, and may enlarge or rupture during period of observation, a test to detect an endometrioma would assist in the evaluation and management of adnexal cysts. Serum CA-125 concentration is seldom >100 U/ml in endometriosis. We reported a patient with endometrioma presenting with a very high CA-125 concentration, which may confuse diagnosis with ovarian cancer.

8.Rupture of the Pectoralis Major Muscle during Exercise.

Rupture of the pectoralis major muscle may occur in youngers or athletes associated with extreme sports, especially during the weight training. It is uncommon, but the incidence is increased by the recent growth of athletic population. In young active individuals, ruptures of the pectoralis major muscle have the best results after surgical repair. However, if diagnosis of the pectoralis major muscle rupture is missed or delayed, the patient will be limited to return to sport activity. The object of this paper is to report our experience of pectoralis major muscle rupture in 3 cases.

9.Abdominal Aortic Aneurysm.

With the increasing age of the population, abdominal aneurysms are increasing in frequency, especially in the old age. At present, surgical excision is recommended for all abdominal aneurysm as soon as possible after the diagnosis has been made because of its high incidence of rupture, a fatal complication. 10.Antegrade Pyelography.

By antegrade pyelography which was the modified Casey & Goodwin technique, 14 patients were examined in whom relatively accurate visualization of collecting system of the affected kidney could not be made by excretory or retrograde pyelography. Diagnosis established were 5 cases of marked delayed opacification, nine of non-visualization of which one was renal rupture due to trauma and the others were renal tuberculosis. No complications occurred during or after examinations. In selected cases of marked delayed opacification and non-visualization in suspected renal tuberculosis, antegrade pyelography is a safe and accurate diagnostic procedure to other accepted methods of urography.
