1.Evaluation of the anal sphincter function by vector manometry in congenital anorectal malformation

Objective: To evaluate the anal sphincter function of congenital anorectal malforotation retrospectively.Methods: With continuous pull through technique, the manometry of 22 congenital anorectal malforotation patients and 24 controls were studied and the results were analysed with their clinical scores. Results: Anal resting pressure in the children with neurogenic anorectum induced by myelodysplasia(21.3?3.4) mmHg was lower than that in normal children(66.7?24.0) mmHg.The maximum contractive pressure of anus in the patients(22.4?3.3) mmHg was lower than that in normal children(129.0?18.8) mmHg. The length of high pressure zone in the patients(12.3?4.6 mm) was lower than that in normal children(23.6?4.6 mm). Rectoanal inhibitory reflex was identified in both patients and normal children. Conclusion:Anorectal manometry might be an effective parameter to evaluate the anal sphincter function of congenital anorectal malformation.
2.The influence of neurogenic anorectum to the function of anorectum

Objective:To study the influence of neurogenic anorectum induced by myelodysplasia on function of anorectum.Methods:Twenty-five patients with myelodysplasia were evaluated by anorectal manometry.The function of anal sphincter was evaluated by resting pressure,contractive pressure and the length of high pressure;The sensation of rectum was evaluated by rectal maximum volume threshold;The function of defecation reflex was evaluated by rectoanal inhibitory reflex.Results:Anal resting pressure in the children with neurogenic anorectum induced by myelodysplasia( 25.8?3.4)mmHg was lower than that in normal children(66.7?24)mmHg.The maximum contractive pressure of anus in patients (86.6?20.1)mmHg was lower than that in normal children(129.0?18.8)mmHg.The length of high pressure in patients (17.5?4.5)mm was lower than that in normal children(23.6?4.6)mm.The rectal volume at sensory threshold in patients(62.1?8.5)ml was higher than that in normal children(36.0?12.6)ml.Rectal maximum volume threshold in patients(141.4?22.6)ml was higher than that in normal children (109.5?12.2)ml.Rectoanal inhibitory reflex was identified in both patients and normal children.Conclusions:Anorectal manometry may provide objective assessment of the neurogenic damage of anorectum in myelodysplasia including the damage of sphincter and the decrease of the rectal sensation. Rectoanal inhibitory reflex was identified in both patients and normal children. The major objective of anorectal treatment for patients with myelodysplasia was to strengthen the function of external sphincter, internal sphincter and pelvis floor muscle and to repair the sensation of rectum.
3.Detection of nucleic acid sequence-based amplification products by gold nanoprobe-based solution hybridization for the diagnosis of invasive aspergillosis

Objective To establish a method of gold nanoprobe-based solution hybridization (GNBSH) to detect nucleic acid sequence-based amplification (NASBA) products for the rapid diagnosis of invasive aspergillosis (IA).Methods The Aspergillus specific 18S rRNA was amplified by NASBA and then the amplified products were hybridized with the gold nanoprobes which were modified with thiol compounds at the 5'end.Serum samples from 106 patients,including 14 with a definite IA,32 with suspected IA and 60 without IA,were detected by the established method,and the obtained results were compared with that of galactomannan (GM) test to evaluate its accuracy.Results The gold nanoprobes only hybridized with Aspergillus NASBA products but not other non-Aspergillus strains.The sensitivity,specificity and the area under the ROC curve (AUCROC) of the established GNBSH method for detecting 106 clinical samples were 82.61% (38/46),81.67% (49/60) and 0.890,respectively.The sensitivity,specificity and AUCROC of GM test were 56.52% (26/46),83.33% (50/60) and 0.723,respectively.Conclusion The established GNBSH method to detect Aspergillus NASBA products has high sensitivity and specificity and simple operation,which may be used to detect the infection of Aspergillus by clinical laboratories.
4.Investigation on the status of traditional Chinese medicine nursing knowledge-attitude-practice of newly recruited nurses in traditional Chinese medicine hospital

Objective:To investigate the current status of the traditional Chinese medicine (TCM) nursing knowledge-attitude-practice of newly recruited nurses in TCM hospitals, so as to provide evidence for improving the standardized training of TCM nursing among newly-recruited nurses.Methods:From March to April in 2020, a total of 800 newly-recruited nurses from 6 TCM hospitals above the municipal level in Hunan Province were selected as research objects by convenient sampling method. The investigation was conducted by TCM Nursing Knowledge-Attitude-Practice Questionnaire. A total of 800 questionnaires were distributed and 768 valid questionnaires were collected, with an effective recovery rate of 96%.Results:The total score of the Knowledge-Attitude-Practice Questionnaire of the newly recruited nurses of the TCM hospital was (191.61±41.80) ; the scores of knowledge and attitude dimension were (73.93±17.21) and (78.17±15.81) , at a middle level; the score of practice dimension was (39.51±8.78) , at a low level. There were significant differences in the total score of the questionnaire among the new nurses with different hospital levels, working years, whether they graduated from a college of TCM and whether they had received the training of TCM nursing knowledge-attitude-practice ( P<0.01) . Conclusions:The newly recruited nurses in TCM hospitals have a high level of TCM nursing knowledge and a positive attitude towards TCM nursing. However, in clinical work, TCM nursing has poor application ability and insufficient use of clinical skills. In-depth study of TCM nursing knowledge should be strengthened, focusing on the cultivation of TCM nursing thinking. It is also important to improve laws, regulations and related rules, and to conduct training according to the different characteristics of new nurses.
5.Evaluation of sphincter damage degree with anal endosonography for children with fecal incontinence.

OBJECTIVETo evaluate the application of anal endosonography in the morphology of internal anal sphincter (IAS), external anal sphincter (EAS) and puborectalis (PR) in order to provide necessary information for analysis of the etiology of fecal incontinence and formulation of the therapeutic schedule.
METHODSFrom December 2010 to November 2012, 18 children of anorectal malformation (n=14) or Hirschsprung's disease(n=4) with fecal incontinence received anal endosonography. The morphology of IAS, EAS and PR was observed. The damage of anal sphincter was classified according to Starck criteria. Anorectal mamometry and anal clinical score were measured simultaneously. Spearman analysis was performed to examine the correlation of anal sphincter damage with anorectal mamometric score and anal clinical score.
RESULTSAccording to Starck criteria, anal sphincter damage was small in 11 children, moderate in 6, and severe in 1. PR damage was found in 4 cases. Starck score was positively correlated with manometric score(P<0.05), while not correlated with anal clinical score(P>0.05).
CONCLUSIONSAnal endosonography can clearly display the morphology of IAS, EAS and PR, and their integrity and damage degree. It is a very valuable technique to evaluate the anal sphincter of the children with fecal incontinence, which however can not reflect the function of anal sphincter and anal continence thoroughly.

6.Correlation analysis of electronic screen use and myopia among primary and secondary school students in six provinces and cities of China

Objective:
To study the correlation between electronic screen use and myopia among primary and secondary school students in six provinces and cities in China, in order to provide a scientific basis for comprehensive prevention and control of myopia.
Methods:
From November 2020 to June 2022, a total of 16 557 primary and secondary school students from six provinces and cities in China (Beijing City, Liaoning Province, Zhejiang Province, Henan Province, Shaanxi Province, Chongqing City) were selected by stratified cluster random sampling and probability smampling methods, and a questionnaire prepared by Beijing Center for Disease Control and Prevention was used to investigate their electronic screen use. According to Screening Criteria for Myopia in Children and Adolescents, 0.5% mass concentration of compound topicamide eye drops was used to paralyze the ciliary muscle and undergo slit lamp optometry. Chisquare test was used to compare the differences between groups, and binary Logistic regression was used to analyze the association between electronic screen use and myopia.
Results:
About 58.3% of primary and secondary school students used electronic screens for more than two hours a day on average, and 63.4% of primary and secondary school students used continuously electronic products for more than 15 minutes at a time for nonstudy purposes. After adjusting for confounding factors, parents unrestricted use of electronic screen time and electronic screen time ≥2 h/d were positively correlated with myopia (OR=1.27, 1.13, P<0.05). Gender stratified analysis showed that boys who used electronic screen time ≥2 h/d had a higher risk of myopia (OR=1.15, P<0.05). The results of grade stratification analysis showed that parents unrestricted electronic screen use time and electronic screen time ≥2 h/d were positively correlated with myopia in the lower grade of primary school students (OR=1.34, 1.18, P<0.05). Among the higher grade of primary school students, continuous use of electronic screens for nonstudy purposes for more than 15 minutes at one time was positively correlated with myopia (OR=1.18, P<0.05). There was a multiplicative interaction between total screen time and one continuous screen time (OR=1.04, P<0.05).
Conclusions
Primary and secondary students in six provinces and cities in China reports excessive electronic screen usage which is associated with myopia. Schools and parents should jointly limit the use of electronic screen among primary and secondary students to reduce the occurrence of myopia.

8.Visualization analysis on research literature about Linggui Zhugan Decoction based on CiteSpace

Objective:To investigate the research status and hotspots of Linggui Zhugan Decoction.Methods:The research literature about Linggui Zhugan Decoction was retrieved from CNKI, VIP, Wanfang Data and CBM databases from January 1, 2000 to July 11, 2023. NoteExpress 3.8 software was used to merge and remove the weight, and CiteSpace 6.2.R4 software was used to analyze the author, research institution, key words and draw the knowledge map.Results:A total of 1 521 articles were included, and the number of published papers showed a fluctuating upward trend. The main source journals were Chinese Journal of Urban and Rural Enterprise Hygiene, Acta Chinese Medicine, Information of Traditional Chinese Medicine, etc. The main research institutions were Tianjin University of Traditional Chinese Medicine, Beijing University of Chinese Medicine, Shandong University of Traditional Chinese Medicine, etc. Among the most published papers were Huang Jinling from Anhui University of Chinese Medicine (22 articles), Wang Liang from the Institute of Integrated Chinese and Western Medicine of Anhui Academy of Chinese Medicine (14 articles) and Shi Hui from Anhui University of Chinese Medicine (10 articles); high-frequency keywords included chronic heart failure, vertigo, clinical efficacy and so on. Conclusions:The research on Linggui Zhugan Decoction focuses on the clinical research of circulatory system, respiratory system, digestive system and urinary system. The research focus has gradually developed from clinical study, prescription composition and efficacy study, network pharmacology study, mechanism and related theory exploration of prescriptions to clinical practical application.
9.Experience on clinical practice training for interventional therapy specialized nurses of Hu'nan Province: a qualitative research

Objective To explore the experience and needs on clinical practice training for interventional therapy specialized nurses of Hu'nan Province, so as to provide a reference for reasonably setting up the training practical courses for interventional therapy specialized nurses and a basis for further perfecting the system of interventional therapy specialized nurses training. Methods From 10th June to August 6th 2017, the semi-structured in-depth interview was used to 14 nurses attending the training for interventional therapy specialized nurses of Hu'nan Province by purposive sampling. The results were analyzed and themes were refined with the Colaizzi seven-step analysis. Results A total of five themes were refined in training experiences including that the practical ability of interventional therapy was improved, the time of clinical practice was too short, clinical rotating department was arranged unreasonably, quality of clinical teachers was uneven, the forms of clinical practice assessment needed to be standardized. Conclusions Interventional therapy specialized nurses training can improve trainees' quality of clinical practice. However, course offered should be adjusted based on trainees' actual situation and the model of interventional therapy specialized nurses training should be perfected so as to improve training effects.
10.Outcomes of newly diagnosed prediabetes and its risk factors in Guiyang: a 3-year follow-up study

Objective:To investigate the outcomes and influencing factors of newly diagnosed prediabetic subjects aged 40 years and above in Guiyang.Methods:A total of 10 015 residents aged 40 years and above were recruited from the Yunyan community, Guiyang, from May to August 2011. Physical examination, laboratory measurements, and questionnaires were conducted. The follow-up survey was conducted in July 2014. A total of 2 530 newly diagnosed prediabetic subjects at baseline were included in the analysis.Results:The 3-year cumulative morbidity of diabetes mellitus was 14.3%, and the risk of diabetes mellitus in combined impaired fasting glucose(IFG)and impaired glucose tolerance(IGT)groups was significantly higher than that in isolated IFG(i-IFG)or isolated IGT(i-IGT)group( P<0.01). High baseline fasting plasma glucose, 2 h plasma glucose, and HbA 1C levels were the independent risk factors for the development of diabetes( OR=1.836, 95% CI 1.374-2.454; OR=1.398, 95% CI 1.261-1.550; OR=2.526, 95% CI 1.804-3.538, all P<0.01)and the inhibitory factors for reversion to normal glucose tolerance( OR=0.511, 95% CI 0.409-0.638; OR=0.715, 95% CI 0.661-0.774; OR=0.638, 95% CI 0.500-0.816, all P<0.01). High level of high density lipoprotein-cholesterol(HDL-C)was an promoting factor for reversion to normal glucose tolerance( OR=1.306, 95% CI 1.017-1.678, P=0.036). Subjects in the highest tertile of baseline HbA 1C level and body mass index(BMI)change before and after follow-up(ΔBMI=follow-up BMI minus baseline BMI)had a higher risk of diabetes mellitus than those in the lowest tertile( OR=2.398, 95% CI 1.733-3.322; OR=2.402, 95% CI 1.859-3.105, both P<0.01). The risk of diabetes mellitus in the significant weight loss group was reduced by 40.4% compared with the non-significant weight loss group when the subjects were divided into two groups according to the cutoff of the lower tertile of ΔBMI( RR=0.596, 95% CI 0.463-0.766, P<0.01). Conclusion:The risk of diabetes mellitus in combined IFG/IGT group was significantly higher than that in i-IFG or i-IGT group. High baseline fasting plasma glucose, 2 h plasma glucose, and HbA 1C levels were the independent risk factors for the development of diabetes. High level of HDL-C was an promoting factor for reversion to normal glucose tolerance. Weight loss can significantly reduce the risk of progression to diabetes in individuals with prediabetes.
