1.Diagnosing and treating endometriosis-related fibrosis based on the perspective of the traditional Chinese medicine theory of "coldness produces turbid yin"
Journal of Beijing University of Traditional Chinese Medicine 2024;47(9):1185-1190
Endometriosis is a prevalent disease in gynecology. The symptoms of endometriosis are challenging to treat, affect various organs, and persist in the body, adversely impacting patients' quality of life. Endometriosis-related fibrosis is a primary cause of dysmenorrhea, infertility, and chronic pelvic pain, which represents the main pathological feature of endometriosis. Traditional Chinese medicine identifies blood stasis as the primary pathological basis of endometriosis, emphasizing the use of treatments to promote blood circulation and resolve stasis. The traditional Chinese medicine theory,"coldness produces turbid yin",suggests that coldness plays a crucial role in the formation of blood stasis and damp phlegm. Coldness is harmful to yang qi,leading to the accumulation of blood stasis and damp phlegm in the body, which can cause endometriosis. Based on this theory, the etiology and pathogenesis of endometriosis were discussed in stages. "Strong coldness coagulation is blood stasis, turbidity generating abdominal masses" is the first etiological factor. The progress stage is "yang qi deficiency is difficult to resolve cold blood stasis, and fibrosis is formed over a long period". According to the pathogenesis characteristics at different stages, the treatment concept for endometriosis is warming yang to resolve turbidity and blood stasis. During the initial stage of disease treatment, the treatment principle is primarily based on dispersing cold, activating blood circulation, and removing damp phlegm and blood stasis. The primary treatment is to improve yang qi to dissipate turbidity and circulate blood, and eliminate stasis to reduce abdominal masses if endometriosis is challenging to treat over a long period. In conclusion, endometriosis can be treated when blood stasis and damp phlegm are dissipated, and endometriosis-related fibrosis can be treated to clear abdominal masses when yang qi is filling the body.

Objective To discuss the application of self-developed novel 16-slice mobile CT head scan. Methods A total of 391 patients were performed 16-slice mobile CT scan:145 were scanned in the emergency department,156 in the neurosurgical ICU,55 in the operated room, and 35 in the ambulance vehicle.Sixty-eight patients were with brain injury,122 were with cerebral hemorrhage,120 were with cerebral infarction,59 were with brain tumors, and 22 were with hemifacial spasm.Thirty-five patients were randomly selected from 391 patients and 8-slice mobile CT head scan was performed on them, which included 12 with brain injury,6 with cerebral hemorrhage,12 with cerebral infarction,3 with brain tumors and 2 with hemifacial spasm. The resolution, imaging quality, radiation doses, power consumption and performance stability of novel 16-slice mobile CT and 8-slice mobile CT head scan were compared. Results The resolution line pairs of brain tissues were 91 p/cm by 16-slice mobile CT and 71 p/cm by 8-slice mobile CT,respectively. The imaging quality of the two kinds of mobile CT head scans was high level to the clinic diagnostic criteria. The radiation dose of 16-slice mobile CT were 40.43 mGy,which decreased by 51.01% as compared with that of 8-slice mobile CT (82.52 mGy).The personal power consumption of 16-silce mobile CT (0.29 kW· h) decreased by 38.30% as compared with those of 8-layer mobile CT (0.47 kW· h).The 16-slice mobile CT kept regularly, while 8-slicer mobile CT stopped to work twice during clinical trial. Conclusion The 16-slice mobile CT scan has high resolution, fine imaging quality, low radiation dose, small power consumption and stable working performance.

The combination of sourness, bitterness, sweetness, pungency, and saltness results in different effects.Through the analysis of the theory of combining five flavours in Fu Qingzhu's Obstetrics and Gynecology, this article explores its application in formulation ideas and the characteristics of Chinese medication, summarized as"primarily sweet, with all four flavors harmonized".FU Qingzhu emphasizes the central role of sweet-flavour medicine in facilitating conception, while incorporating the other four flavours in synergy. For instance, sweet and pungent flavours can boost yang energy, eliminating chilliness and warming the uterus;sour and sweet flavours can nourish yin essence and protect the uterus;bitter and sweet flavours can tonify yin, supporting the kidneys and moistening the uterus;and pungent and salty flavours can eliminate stasis, regenerate blood, and enhance uterine functions.By flexibly utilizing the flavours and meridian affinity of traditional Chinese medicine, along with the theory of combining five flavours,it is possible to enhance drug compatibility, deepen the theoretical connotation of Chinese formulas, and regulate the uterus from both yin and yang aspects to facilitate conception.FU Qingzhu's integration of multiple Chinese formulas into a single formula allows for comprehensive treatment.Clear differentiation of dosages within the formula highlights the primary and secondary relationships of traditional Chinese medicines.Additionally, the utilization of pharmaceutical processing techniques optimizes efficacy and regulates meridians and collaterals. This article explores FU Qingzhu's application of the"pure and harmonious traditional Chinese medicines"theory. His prescription thinking enables the attainment of multi-level therapeutic effects, which greatly benefits the optimization of traditional Chinese medicine fertility assistance programs and increases pregnancy rates among infertility patients.

Surgery is the mainstay of lung cancer treatment options.Traditionally, lobectomy has held its place as the gold standard for treating localized lung cancer, while sublobar resection, including wedge resection and segmentectomy, was primarily considered as an alternative, often reserved for patient ineligible to sustain a radical intervention. However, with the widespread application of computed tomography(CT)to clinical practice, the increasing detection rate of pulmonary ground glass nodules(GGNs)has reshaped this landscape.Ground glass opacity(GGO)in persistent lung nodules is an indicative factor of a favorable prognosis, typically corresponding to pathological changes such as atypical adenomatous hyperplasia(AAH),adenocarcinoma in situ(AIS),or adenocarcinomas predominantly featuring a lepidic growth pattern.A large number of retrospective studies have shown that sublobar resection can achieve satisfactory therapeutic outcomes for such lesions.A series of prospective studies from Japan have confirmed that for early-stage lung cancers dominated by GGOs,sublobar resection is also a viable curative surgical option. The follow-up data showed that there was no statistical difference in the survival status of these patients compared with that of pulmonary lobectomy. This article aims to delve into the role of limited lung resection in the context of lung adenocarcinoma presenting with GGO features.
