1.Rlationship between serum levels of uric acid and prognosis of infection in the critically ill

Objective To explore the clinical implications of the initial levels of serum uric acid (SUA) after admission in the critically ill patients with infection,and to investigate the relationship between SUA and prognosis of infection.Methods From January 2003 to April 2010,471 patients with infection were admitted to the ICU of Huashan Hospital,Fudan University,Shanghai.Data,including serum uric acid (SUA),serum creatinine,blood urea nitrogen (BUN) and other relevant laborartory results within 24hours after admission,were retrospectively analyzed.The distribution of uric acid was described.The t test was used to evaluate the relation between SUA and preexisting disorders.Patients with different level of SUA were classfied for further analysis.x2 test was used to examine the difference in the prognosis of infection.Results The mean initial level of SUA within 24 hours after admission was 0.232 ± 0.131 mmol/L and the median 0.199 mmol/L. Remarkable difference in initial levels of SUA were observed in patients with preexisting hypertension (t=-3.084,P=0.002),diabetes mellitus (t=-2.487,P=0.013),cerebral infarction (t=-3.061,P=0.002),renal insufficiency (t=-4.547,P<0.01),central nervous system infection (t =5.096,P < 0.01 ) and trauma (t =2.875,P =0.004).Serum uric acid was linearly correlated with serum creatinine and blood urea nitrogen (F =159.470 and 165.059,respectively; P <0.01).No statistical correlation was found between initial levels of SUA and prognosis of infection (x2=60.892,P=0.100).Conclusions There is no direct correlation between the initial levels of SUA afteradmission and the prognosis of infection in the critically ill patients.

Objective: To investigate the role of cyclin kinase inhibitor P27 on the proliferation of mesangial cell(MC) in Thy1 glomerulonephritis(GN) rats. Methods: The Thy1 model of experimental mesangial proliferative glomerulonephritis was induced by intravenous injection of rabbit anti-thymocyte plasma. The P27 protein of glomerular lysate was detected with Western blotting analysis. The P27 mRNA was determined with RT-PCR. The extracellular matrix (ECM) protein (fibronectin and type Ⅳ collagen) of glomerular lysate was examined with ELISA. Results: Glomerular P27 protein of Thy1 model rat decreased compared with that of normal rat. The level of P27 was associated with the degree of MC proliferation:P27 began to decrease on day 1 in Thy1 model, then reached the lowest on day 3 when MC proliferation was maximal, then returned to 60% of the baseline level on day 5 when MC proliferation began to resolute. AIM To study the nephrotoxicity of various doses of Guangfangji . METHODS Normal Wistar rats were given 1, 5 and 10 g?kg -1 of Guangfangji respectively. Renal pathology and function were observed. RESULTS Rats given 1 g?kg -1 of Guangfangji for 8 weeks showed normal renal function and histology Rats given 5 g?kg -1 of Guangfangji significantly increased 24 hour urinary protein excretion Tubular degeneration and interstitial edema was observed Blood urea nitrogen (BUN) and serum creactinine (Scr) remained in the normal range BUN and Scr increased significantly in the group given 10 g?kg -1 of Guangfangji for 4 weeks The tubulointerstitial abnormalities were more severe in the group given 5 g?kg -1 of Guangfangji CONCLUSION Longterm use of pharmacopoeial dose of Guangfangji shows no harm to the kidney.Renal injury may occur if relatively large dose of Guangfangji is given and the period of treatment using this drug is relatively longer

5.Two transfusion strategies for severe burn patients in non-shock stage: A comparative study

【Objective】 To investigate the effect of two different blood transfusion strategies in non-shock stage of sever burn patients, so as to provide reference for clinical rational use of blood. 【Methods】 74 cases of severe burn patients in our hospital from July 2019 to December 2020 were randomly divided into restrictive transfusion group and liberal transfusion group. The differences of blood transfusion volume, red blood cell (RBC) count before and after blood transfusion, Hb value, incidence of transfusion adverse reactions, postoperative infection rate, length of hospital stay, and 30 day mortality between the two groups were retrospectively analyzed. 【Results】 1) The blood transfusion volume of the restricted blood transfusion group [(9.58±7.43)U] was statistically less than that of the liberal blood transfusion group [(22.24±20.08)U] (P<0.05). 2) The increase of Hb per unit of RBC in the restrictive transfusion group [(4.98±3.37)g/L] was higher than that in the liberal transfusion group [(3.28±3.12)g/L], and the difference was statistically significant. (P< 0.05). 3) There were no significant differences in postoperative infection rate, incidence of transfusion adverse reaction, length of stay and 30 day mortality between the two groups (P> 0.05). 【Conclusion】 Limited blood transfusion treatment for severe burn patients in non-shock stage can reduce the frequency and risk of blood transfusion and save blood resources, which is of great significance for rational and safe blood use in clinic.
6.Application of mixed reality technology in clinical teaching of airway anatomy under bronchoscopy

Objective:To explore the effect of introducing mixed reality technology into traditional atlas teaching to teach airway anatomy under bronchoscopy.Methods:A total of 30 Batch 2017 fifth-year clinical medicine students from Shanghai Jiao Tong University School of Medicine were randomly divided into control group and test group by RAND function in Excel, with 15 students in each group. The control group was taught with the traditional bronchoscopic atlas teaching, and the test group was combined with mixed reality technology. The two groups had the same class time. After teaching, the teaching effect was evaluated by examination and evaluation questionnaire. SPSS 25.0 software was conducted for t test and Mann-Whitney U test. Results:The average score after teaching of test group was (61.67±20.15), and that of control group was (36.67±13.32), with statistically significant differences ( t=4.01, P<0.001). According to the questionnaire results, the scores of the test group on course understanding, course concentration, participation, mastery and satisfaction were better than those of the control group, and the differences were statistically significant ( P<0.05). Conclusion:Using mixed reality technology to assist the clinical teaching of airway anatomy under bronchoscopy can improve the quality of students' study and enhance their understanding of the teaching content and students' participation passion, achieving better teaching effect.

A combined radiation-wound injury refers to a radiation injury combined with a traumatic wound, with the characteristics of repeated ulceration and a long and difficult healing process, which is a focus in the field of research on difficult-to-heal wounds. To research combined radiation-wound injuries, the establishment of animal models is a key part, and appropriate animal models are a guarantee of reliable experimental results. This review summarizes the current research progress on various animal models of combined radiation-wound injuries in terms of radiation types, animal species, and injury types and location, aiming to provide a scientific basis for establishing standardized animal models, studying injury mechanisms, and evaluating prevention and treatment efficacy for combined radiation-wound injuries.

Objective:
To explore the prenatal dietary and environmental risk factors in children aged 6-16 years with asthma (CWA) comorbid neuropsychiatric disorders.
