1.The effect of mental nursing intervention on gastric cancer with liver metastasis patients treated with TACE sequential chemotherapy

Objective To explore the effect of mental nursing intervention on gastric cancer with liver metastasis patients treated with TACE sequential chemotherapy.Methods Sixty four gastric cancer patients with liver metastasis,no hopes of accepting operations,were included.All patients were divided into control group (32 cases) and observation group (32 cases).The former received routine nursing care and the later received extra mental nursing intervention.The anxiety,depression and quality of life in both groups were evaluated and compared.Results After extra mental nursing intervention,the SDS and SAS scores of the observation group [(41.6 ± 1.8) points and (41.6 ± 2.3) points] were significantly lower than those of the control group [(50.9 ± 1.9) points and (53.0 ± 3.5) points,P =0.040,P =0.036,respectively].The quality of life scores,including the physiological function,body pain,general self rating,vitality,social function,emotional function and mental health score were (45.26 ± 38.26)points vs (35.17 ± 33.65) points (P =0.044),(71.98 ± 17.36) points vs (63.01 ± 18.41) points (P =0.039),(55.59 ± 12.78) points vs (49.67 ± 12.43) points (P =0.048),(74.82 ± 14.23) points vs (55.61 ± 10.58) points (P =0.027),(75.47 ± 12.15) points vs (57.22 ± 14.78) points (P =0.031),(70.24 ± 22.00) points vs (45.92 ± 34.76) points (P =0.004),(78.31 ± 19.11) points vs (67.23 ± 11.96) points (P =0.037),respectively,the scores in the observation group were significantly higher than those in the control group (all P < 0.05).Conclusion The extra mental nursing intervention on gastric cancer with liver metastasis patients,treated with TACE sequential chemotherapy,can not reduce the patients'negative mood,improve the patients'compliance with chemotherapy and the confidence to treatment,but improve the quality of patients'life effectively.
2.High intensity focused ultrasound is treatment of 14 hepatic metastases from colorectal cancer

Objective To evaluate the clinical safety and efficacy of HIFU treatment of hepatic metastases from colorectal cancer.Methods 20 tumor lesions in 14 patients with hepatic metastases from colonectal cancer were treated with HIFU therapy.The vital signs,main organs'functions,examination of MRI and complications were observed.Results Vital signs of all the 14 patients were stable.The function of liver and kidney were normal and life quality were improved with elevation of Karnofsky in 5 patients.On MRI examination,the MRI signals of treating area of 20 tumor lesion in 14 patients decreased remarkably with disappearance of blood supply and a thin layer of edematous intensified zone was detected on the margin of lesion.6 patients were found to have cord-like burning scar on skin.Conclusion HIFU treatment is safe and effective in treatment of hepatic metastases from colorectal cancer.
3.Correlations of hyperuricemia and matrix metalloproteinase-9 level with left ventricular remodeling in patients with type 2 diabetes and the clinical effect of intervention for hyperuricemia

Objective To investigate correlations of hyperuricemia and matrix metalloproteinase -9 level with left ventricular remodeling in patients with type 2 diabetes and the clinical effect of intervention for hyperuricemia.Methods 100 type 2 diabetic patients with hyperuricemia in our hospital from June 2011 to June 2012 were selected as hyperuricemia group,50 type 2 diabetic cases with normal uric acid were considered as DM group.Levels of blood glucose,blood lipids,glycosylated hemoglobin and matrix metalloproteinase-9 were detected.All patients underwent echocardiography.Patients in hyperuricemia group were randomly divided into treatment group (receiving allopurinol and alkaline urine-alkalify therapy added to hypoglycemic treatment,n=50) and control group (receiving hypoglycemic treatment,n=50).After 6 months of treatment,levels of blood glucose,blood lipids,glycosylated hemoglobin,matrix metalloproteinase-9 and echocardiography were rechecked.Results The levels of triglyceride (TG),blood uric acid,MMP-9,left ventricular posterior wall thickness (LVPWT),interventricular septal thickness (IVST) and left ventricular mass index (LVMI) were increased in hyperuricemia group versus DM group[(2.3±0.5) mmol/L vs.(1.6±0.30) mmol/L,(498.9±32.5) μmol/L vs.(322.8±35.6) μmol/L,(765±38) g/L vs.(420±26) pg/L,(11.0±1.1) mm vs.(9.2±0.8) mm,(11.5±1.6) mm vs.(10.1±1.1) mm,(138.7±35.6) g/m2 vs.(115.0±10.4)g/m2,t=4.945,5.124,4.895,3.985,3.965,3.989,respectively,all P<0.05].Levels of TG,blood uric acid,MMP,LVPWT,IVST were decreased in treatment group after treatment versus before treatment [(1.6±0.5) mmol/L vs.(2.3±0.6) mmol/L,(323.0±30.5) μmol/L vs.(496.6±32.6) μmol/L,(766±34) pg/L vs.(410±25)pg/L,(9.1±0.8) mm vs.(11.0±1.1) mm,(9.2±1.1) mm vs.(11.6±1.5) mm,(116±10.5)g/m2 vs.(138.8±35.2) g/m2,t=3.386,4.525,4.865,3.256,3.895,3.985,all P<0.05].Correlation analysis showed that LVMI had positive correlations with levels of blood uric acid,TG,MMP-9 (r=0.435,0.348,0.356,respectively).Conclusions Hyperuricemia involves in left ventricular remodeling,and therapeutic intervention on the hyperuricemia can reverse this remodeling.
5. Effects of jiawei danshen yin combined with mir-21 on iri myocardial cells

Objective To investigate the protective effect of Jiawei Danshen Yin combined with microRNA-21 on IRI myocardial cells,and to study its protective mechanism. Methods H9C2 cardiomyocytes were cultured and divided into four groups: Blank group, IRI model group of cardiomyocytes,miR-21 group,miR-21 combined with Jiawei Danshen Yin group (JWDSY group). Apoptotic rate, apoptosis-related protein and PTEN signaling pathway expression of cardiomyocytes were detected. Results Electron microscopy showed that mitochondria in model group were damaged significantly , and ultrastructural damage in JWDSY group was alleviated; ELISA demonstrated that cTnl was elevated in model group and decreased in JWDSY group with statistical significance (P < 0. 01); flow cytometry re vealed that the apoptotic rate was significantly increased in model group (36. 79 ±2. 12) ,but markedly decreased in JWDSY group (14. 65 ± 0. 94), and the difference was statistically significant ( P < 0. 05 ). Western blot results showed that PTEN expression was uP-regulated and P-Akt expression down-regulated in model group,while PTEN expression decreased and P-Akt expression increased in JWDSY group, and the difference was statistically significant ( P < 0. 05 ). Conclusions miR-21 combined with Jiawei Danshen Yin can inhibit cardiomyocyte apoptosis and protect IRI cardiomyocytes by reducing PTEN expression and activating PI3K/Akt signaling pathway,.

7.A study of dynamic changes of NO,NOS in serum and CSF in patients with CNS infection

Objective To observe the dynamic changes of NO,NOS in serum and CSF in patients with CNS infection and lay an experimental basis for discrimination of CNS infection due to different agents.Methods The method of nitrate redutase and the method of chromometry were employed to measure NO,NOS in serum and CSF at different time.The dynamic changes of NO,NOS in serum were observed on admission,on 3,5,9,14 day after admission.The dynamic changes of NO,NOS in CSF before treatment and two weeks after treatment were ob- served,too.Results There were no difference between the the concentration of NO and the vigor of total NOS in serum and in CSF of viral meningitis,bacterial meningitis and tubercular meningitis patients due to different agents. Conclusion The changes of the concentration of NO in serum and CSF,the vigor of total NOS in serum and CSF could not he seen as laboratory basis for discrimination of CNS infection due to different agents.
8.Albuterol in the treatment of acute respiratory distress syndrome: A meta-analysis of randomized controlled trials

BACKGROUND: This meta-analysis of randomized controlled trials aimed to systematically evaluate the value of albuterol in the treatment of patients with acute respiratory distress syndrome (ARDS). DATA SOURCES: Randomized controlled trials on albuterol treatment of ARDS from its inception to October 2014 were searched systematically. The databases searched included: PubMed, Ovid EMBASE, Ovid Cochrane, CNKI, WANFANG and VIP. The trials were screened according to the pre-designed inclusion and exclusion criteria. We performed a systematic review and meta-analysis of the randomized controlled trials (RCTs) on albuterol treatment, attempting to improve outcomes, i.e. lowering the 28-day mortality and ventilator-free days. RESULTS: Three RCTs involving 646 patients met the inclusion criteria. There was no significant decrease in the 28-day mortality (risk difference=0.09;P=0.07,P for heterogeneity=0.22, I2=33%). The ventilator-free days and organ failure-free days were significantly lower in the patients who received albuterol (mean difference=-2.20;P<0.001,P for heterogeneity=0.49,I2=0% and mean difference=-1.71,P<0.001,P for heterogeneity=0.60,I2=0%). CONCLUSIONS: Current evidences indicate that treatment with albuterol in the early course of ARDS was not effective in increasing the survival, but significantly decreasing the ventilator-free days and organ failure-free days. Owing to the limited number of included trails, strong recommendations cannot be made.
9.Diverse coagulopathies in a rabbit model with different abdominal injuries

BACKGROUND:Although coagulopathy can be very common in severe traumatic shock patients, the exact incidence and mechanism remain unclear. In this study, a traumatic shock rabbit model with special abdomen injuries was developed and evaluated by examining indicators of clotting and fibrinolysis. METHODS:Forty New Zealand white rabbits were randomly divided into four groups:group 1 (sham), group 2 (hemorrhage), group 3 (hemorrhage-liver injury), and group 4 (hemorrhage-liver injury/intestinal injury-peritonitis). Coagulation was detected by thromboelastography before trauma (T0), at 1 hour (T1) and 4 hours (T2) after trauma. RESULTS:Rabbits that suffered from hemorrhage alone did not differ in coagulation capacity compared with the sham group. The clot initiations (R times) of group 3 at T1 and T2 were both shorter than those of groups 1, 2, and 4 (P<0.05). In group 4, clot strength was decreased at T1 and T2 compared with those in groups 1, 2, and 3 (P<0.05), whereas the R time and clot polymerization were increased at T2 (P<0.05). The clotting angle significantly decreased in group 4 compared with groups 2 and 3 at T2 (P<0.05). CONCLUSION:This study suggests that different abdominal traumatic shock show diverse coagulopathy in the early phase. Isolated hemorrhagic shock shows no obvious effect on coagulation. In contrast, blunt hepatic injury with hemorrhage shows hypercoagulability, whereas blunt hepatic injury with hemorrhage coupled with peritonitis caused by a ruptured intestine shows a tendency toward hypocoagulability.
10.Comparison of efficacy on functional constipation treated with electroacupuncture of different acupoint prescriptions: a randomized controlled pilot trial.

OBJECTIVETo evaluate preliminarily the efficacy on functional constipation treated with electroacupuncture of different acupoint prescriptions.
METHODSOne hundred and four patients were randomized into a front-mu and back-shu points group (19 cases), a he-sea points group (34 cases), a he-sea, front-mu and back-shu points group (26 cases) and a western medication control group (25 cases). In the front-mu and back-shu points group, electroacupuncture was applied at bilateral Tianshu (ST 25) and Dachangshu (BL 25). In the he-sea points group, electroacupuncture was applied at bilateral Quchi (LI 11) and Shangjuxu (ST 37). In the he-sea, front-mu and back-shu points group, electroacupuncture was applied at unilateral Tianshu (ST 25), Dachangshu (BL 25), Quchi (LI 11) and Shangjuxu (ST 37). In the three groups above, the treatment was given 5 times a week in the first two weeks and 3 times a week in the next two weeks. In the western medication control group, mosapride citrate tablets were prescribed for oral administration, 1 table (5 mg) each time, 3 times a day, continuously for 4 weeks. The period of research was 9 weeks, including 1 week for baseline evaluation, 4 weeks for treatment and 4 weeks for follow-up. The weekly defecation frequency was taken as primary index, while the defecation difficulty and life quality score were taken as the secondary indices for the efficacy evaluation after treatment and in follow-up.
RESULTSAccording to the intention-to-treat (ITT) analytic principle, 104 cases were all enrolled in the final analysis. (1) After treatment, the weekly frequency of defecation was all increased significantly in the four groups (P < 0.05, P < 0.01). The efficacy of the three electroacupuncture groups was similar to that of western medication control group (P > 0.05). In follow-up, the increasing effect on the weekly frequency of defecation was maintained in the he-sea points group (P < 0.01), superior to the front-mu and back-shu points group and the western medication control group (P < 0.05, P < 0.01); the weekly frequency of defecation was not improved in the rest three groups (P > 0.05). (2) After treatment, defecation difficulty was relieved in the he-sea points group, the he-sea, front-mu and back-shu points group and the western medication control group (P < 0.05, P < 0.01). In follow-up, the improvements were still significant in the he-sea points group and the he-sea, front-mu and back-shu points group (both P < 0.01). (3) After treatment, the life quality score was significantly improved in the patients of the he-sea points group (P < 0.05). The difference was not significant in the rest three groups as compared with that before treatment (all P > 0.05).
CONCLUSIONThe weekly frequency of defecation is increased effectively after treatment in the three electroacupuncture groups and the efficacy is similar to mosapride citrate tablets. The bilateral Quchi (LI 11) and Shangjuxu (ST 37) in he-sea acupoints increase significantly the weekly frequency of defecation, relieve defecation difficulty and improve life quality. Acupuncture efficacy is sustained for 4 weeks. This acupoints prescription is the best in the treatment of functional constipation.
