2.The relationship between serum levels of uric acid and prognosis of infection in critically ill patients

BACKGROUND: Serum uric acid level is associated with some chronic diseases and prognosis of severe infection. This study aimed to investigate the relationship between serum uric acid (SUA) and prognosis of infection in critically ill patients. METHODS: The data from 471 patients with infection admitted from January 2003 to April 2010 were analyzed retrospectively at Huashan Hospital Affiliated to Fudan University, Shanghai, China. The data of SUA, serum creatinine, blood urea nitrogen (BUN) and other relevant examinations within 24 hours after admission were recorded and the levels of SUA in those patients were described, then Student's t test was used to evaluate the relationship between SUA and pre-existing disorders. Different levels of SUA were graded for further analysis. The Chi-square test was used to examine the difference in the prognosis of infection. RESULTS: The mean initial level of SUA within 24 hours after admission was 0.232±0.131 mmol/L and the median was 0.199 mmol/L. Remarkable variations in the initial levels of SUA were observed in patients with pre-existing hypertension (t=–3.084,P=0.002), diabetes mellitus (t=–2.487, P=0.013), cerebral infarction (t=–3.061,P=0.002), renal insufficiency (t=–4.547,P<0.001), central nervous system infection (t=5.096,P<0.001) and trauma (t=2.875,P=0.004). SUA was linearly correlated with serum creatinine and BUN (F=159.470 and 165.059, respectively,P<0.001). No statistical correlation was found between the initial levels of SUA and prognosis of infection (χ2=60.892,P=0.100). CONCLUSION: The current study found no direct correlation between the initial levels of SUA after admission and prognosis of infection in critically ill patients.
3.Therapeutic Effects Observation of Intermittent Eliminating White Blood Cells in Children with Anaphylatoid Purpura Nephritis

Objective To study the different effects between intermittent eliminating white blood cells and immunosuppressive agents pulse therapy in children′s anaphylatoid purpura nephritis on the base of hormone′s treatment.Methods Twelve cases of anaphylatoid purpura nephritis were enrolled in the study and divided into 2 groups.After failure in the hormone′s treatment,the observation group was applied with intermittent eliminating white blood cells(5 mL/kg,1-2 times/week),and the control group was treated with immunosuppressive agents[cyclophosphamide(CTX) 0.8-1.0 g/m~2,or methotrexate(MTX) 8-10 g/m~2].Results There were no significant differences between the 2 groups.The observation group had no obvious side effects,but the control group all had different side effects.Conclusion The treatment of intermittent eliminating white blood cells can have better effects without the side effect resulting from immunosuppressive agents.

In the 40 years of last centry American scientists put forward a concep of high intensity focused ultrasound (HIFU) therapeutic technique and had done some technical and clinical studies. Since 90 years the HIFU surgery technique treating tumors anew rised abruptly in the world. China firstly put out a comprehensive HIFU tumor treating sistem and successfully treated many solid tumors such as breast cancer, bone tumor, liver cancer etc.. Now the treating sistem has already exported to England, Italy,Spain, Japan, Colea etc. and treated tens thousands tumor patients. In the field of noninvasive ultrasound treating tumor technique China has continually kept the leading position in the world.
5.Electrochemiluminescence Biosensor Based on DNAzyme and 3,4,9,10-Perylenetetracarboxylic Dianhydride Derivative Functionalized Hollow Gold Nanoparticles for Detection of Lead Ions

Based on target cycling amplification and 3 ,4 ,9 ,10-perylenetetracarboxylic dianhydride derivative functionalized singal probe, an ultrasensitive electrochemiluminescence ( ECL) sensor was designed for the detection of lead ions. The hairpin substrate DNA was immobilized on the electrode through molecular self-assembly. In the presence of Pb2+and DNAzyme, the substrate was cleaved with single strand DNA fragments left on the electrode surface. Meanwhile, the target and DNAzyme was released for another cleaving circularly. As a result, the single strand DNA fragments hybridized with the assist hairpin probe H1, which leaded to the fabrication of H2 labeled with the 3 , 4 , 9 , 10-perylenetetracarboxylic dianhydride derivative functionalized hollow gold nanoparticles. With the increasing concentration of Pb2+, much more signal probe was been captured and the ECL signal of the biosensor in peroxydisulfate ( S2 O2-8 ) solution would increase. An ECL assay demonstrates that the sensor has a good linear response to Pb2+ concentration in the range of 1í10-12 mol/L-1í10-6 mol/L, with a detection limit of 1í10-12 mol/L. The fabricated sensor shows good selectivity toward Pb2+against other common metal ions.

Objective To investigate the influence of coxsackie B virus (CVB) and mycoplasma pneumoniae (MP) superinfection on immunological manifestation of recurrent respiratory tract inflammations(RRI) in children.Methods One hundred and thirty-two hospitalized children with RRI between Jan.to Dec.2005 were divided into negative control group,MP infection group,CVB infection group and superintection group by determining anti-MP IgM and anti-CVB IgM.Blood sedimentation,C-reactive protein,IgG,IgA,IgM and T lymphocyte subpopulation etc.were determined in four groups.The anti-MP IgM was determined by specificity immune agglutination test.Enzyme linked immunosorbent assay was used to detect anti-CVB IgM.The IgG,IgA,IgM were determined by simple agar diffusion method.T lymphocyte subpopulation was tested by flow cytometry.Results The percentage of CVB infection was 32.1%,mainly expressed in the 1-3 years old children;the percentage of MP infection was 22.7%,mainly expressed in children over 3 years old.In both CVB and MP infection group,the ten-dency of IgG increased and that of IgA decreased.IgM in the CVB and MP superinfection group was obviously higher than that in negative control group and MP infection group (Pa
8. Design of novel micellar nano-traditional Chinese medicine formulations based on anti-tumor mechanisms of traditional composition

Clinically traditional Chinese medicine (TCM) has been proven to possess obvious anti-tumor effects. It is critical to further explore the effective components and the corresponding mechanism of the TCM against target cells, which also has great significance for developing novel nano-TCM formulations for clinical treatment of tumor. This paper systematically reviews the anti-tumor effects of Chinese herbal compound and the anti-tumor mechanism of single herb. We also summarized the progress in the current traditional nano-TCM preparations. Taking the shikonin in Herba Arnebiae as an example, using the nano-material self-assembly technology, we discussed the design of novel nano-macromolecule TCM formulations while considering the mechanism of single herb and the clinical obstacles.

Une new flavonoids named as notabilisin K (1), together with four known compounds, morusin (2), mulberrofuran A (3), neocyclomorusin (4) and mornigrol F (5) are separated from 95% ethanol extracts of the twigs of Morus notabilis. Compounds 2-5 are separated from this plant for the first time. Notabilisin I, notabilisin J exhibits certain effect against cells of HCT-116, HepG2 and A2780 with IC50 values ranging from 1.47 μmol x L(-1) to 5.46 μmol x L(-1). Morusin exhibits strong effect against five kinds of human cancer cells (BGC823, A2780, HCT-116, HepG2 and NCI-H1650) with IC50 values ranging from 0.74 μmol x L(-1) to 1.58 μmol x L(-1).

10.Lung transplantation from donor of cardiac death in China (report of 3 cases)

Objective To summarize the clinical experience of harvesting and using the lungs of cardiac death donor.Methods The lungs from donation after cardiac death (DCD) were harvested and used for lung transplantation.The donors suffered from severe craniocerebral trauma or brain neoplasms and were identified after cardiac death post declaration of brain death.Written consent about DCD was obtained from the consanguinities.The donor lungs were harvested after clinical evaluation of donors with considerable function and after the determination of DCD.The preoperative lymphocytotoxic cross match test was negative,ABO blood type was compatible,and the donors were all suitable for the transplant procedure.Results Two bilateral lung transplantations and one single lung transplantation were performed,with the warm ischemic time being 23,27,and 32 min,respectively.The operative course was uneventful The ICU stay was 31,18,and 26 days respectively,with dramatic improvement of pulmonary function postoperatively.Acute rejection occurred in two cases,which was treated with bolus of corticoids.There were no infection in our 3 patients,and the life quality was satisfactory during the follow-up period.Conclusion The lung from DCD may be one of the available resources used for lung transplantation on the basis of efficient management of the potential donors and clear evaluation of the donors.
