1.Concentration of endothelin-1 of plasma in patients with nonarteritic anterior ischemic optic neuropathy

Objective To observe the concentration of plasma endothelin (ET-1) in patients with nonarteritic anterior ischemic optic neuropathy(NAION) and investigate the relationship between ET-1 and NAION. Methods The plasma levels of ET-1 in 41 patients with NAION and 15 age-matched normal control subjects were measured by radioimmuoassay (RIA). The patients with NAION were divided into high papilloedema group, light papilloedema group, and recovery group according to the degree of papilloedema; and were divided into group1 (within 14 days), 2 (within 15-30 days), 3 (within 31-60 days), and 4 (within 61-180 days) according to the disease course. The plasma levels of ET-1 in different groups were detected and compared. Results The plasma level of ET-1 in patients with NAION was significantly higher than that in the normal subjects ( t=5.02,P
2.Radiotherapy of 63 patients with stage I and II non small cell lung cancer

Objective To evaluate the efficacy of Cobalt 60 radiotherapy in patients with stage I and II non small cell lung cancer. Methods From March 1977 to December 1992, 63 patients with stage I and II non small cell lung cancer were treated. According to the 1997 UICC staging system, 7 patients had stage IA disease, 10 stage IB, 5 stage IIA, and 41 stage IIB. Forty nine patients were confirmed by histopathology, and 14 cases by cytopathology. Fifty eight patients had squamous cell carcinoma, 3 adenocarcinoma, and 2 mixed squamous cell and adenocarcinama. All patients were treated by Cobalt 60. Patients would receive 40 Gy plus a boost of 15~30 Gy. The total dose was D T55~70 Gy in 6~12 weeks. All patients were followed for more than five years. Results The overall 5 year survival for all patients was 17.5%. Fifty eight (92%) patients have died and 11 survived. For the 58 succumbed patients, 52 (90%) died of tumor, and 26 (45%) of them from local failure. Four patients (7%) died of other causes. Conclusion Radiotherapy is an effective treatment option for non small cell lung cancer.

Objective Discusses the peeling carotid endarterectomy tcchnique(CEA) operation methods and main points,around the time of surgery to reduce the incidence of stroke and postoperative restenosis rate.Methods Retrospectively analyzed and summarized for the CEA 21 cases in vascular surgery in our hospital from March 2006 to November 2011.Conventional Doppler ultrasonography,preoperative CTA.All the patients were under general anesthesia CEA,conventional set neck artery bypass tube surgery and vascular patch for carotid angioplasty.Results Around the time of surgery and postoperative 30 d were no deaths and stroke occurred and the follow-up both not again hair.In 3 6 months after mild carotid artery stenosis (<40%),patients were no more than the carotid artery stenosis again.Conclusion Carotid endarterectomy exfoliative procedure the routine use carotid artery flow pipe and the blood vessels to patch do artery figuration,which can effectively improve the safety of the procedure and reduce postoperative restenosis happen.

Objective Study of the effects of hepatocyte growth factor on inhibit Intimal hyperplasia of the anastomotic stoma after carotid artery bypass grafting.Methods Thirty-two New Zealand white rabbits were randomly divided into control group and the experimental group.The veins were pretreated with saline solution(control group)only or pretreated with HGF(experimental group ;100ng/ml).The vein grafts were harvested at 14 days,28days after operation,HE Stain and Elastic fibrin Stain,The thickness of Intima and media in the vein grafts,intima-media ratio(I/M) was calculated by computer image analysis system.PCNA Immunohistochemistry was performed.Results The thickness of Intima and media in the vein grafts of control group surpassed experimental group significantly(P <0.01).At 14d I/M in the vein grafts of control group (0.81 ± 0.05) surpassed experimental group (0.47 ± 0.05) (P < 0.01),At 28d I/M in the vein grafts of control group(0.73 ± 0.01)surpassed experimental group (0.65 ± 0.01) (P < 0.01).The vascular smooth muscle cell proliferation in experimental group was significantly lower than that in control group (P < 0.01).Conclusion Treatment of veins grafts with HGF can significantly inhibit intimal hyperplasia in a rabbit carotid artery bypass grafting model.
5.The value of plasma C-peptide levels in early prediction of type 2 diabetes mellitus with peripheral sensory neuropathy

Objective To explore the value of plasma C-peptide levels in early prediction of type 2 diabetes mellitus with peripheral sensory neuropathy.Methods The vibration perception threshold,pain,temperature sensation,touch-pressure sensation,ankle reflex was detected in 500 eases of type 2 diabetes mellitus,and the patients were divided into 4 groups according to peripheral sensory nerve test results:normal group (159 cases),mildly abnormal group (120 cases),moderately abnormal group (121 cases) and severely abnormal group (100 cases).Fasting and 2-hour postprandial C-peptide levels were determined and analysed with peripheral sensory nerve changes.The receiver-operating characteristic (ROC) curve was used to find the best critical point for diagnosis of diabetic peripheral sensory neuropathy.Results The fasting C-peptide among 4 groups had no significant difference (F =1.632,P >0.05).Two-hour postprandial C-peptide from normal group to mildly abnormal group and then moderately abnormal group gradually increased [(1.110 ± 0.526),(1.324 ± 0.490),(1.573 ± 0.716) μ g/L],while 2-hour postprandial C-peptide in severely abnormal group was significantly decreased and lower than that in normal group,and there were significant differences (P< 0.05).The max Youden Index was 0.366 when 2-hour postprandial C-peptide was 1.173 μ g/L.Conclusions The fasting C-peptide might be not related to early diabetic peripheral sensory neuropathy,but 2-hour postprandial C-peptide might be closely related to early diabetic peripheral sensory neuropathy.It is helpful to detect the early diabetic peripheral sensory neuropathy if we can take a dynamical observation of 2-hour postprandial C-peptide.
6.Correlation between left ventricular mass index and NADPH oxidase p22phox in patients with non-valvular chronic heart failure.

OBJECTIVETo study the correlation between the expression levels of phagocytic NADPH oxidase p22phox subunit and left ventricular mass index (LVMI) in patients with non-valvular chronic heart failure and explore the role of oxidative stress caused by NADPH oxidase p22phox subunit in left ventricular remodeling.
METHODSSemi-quantitative RT-PCR was used to examine the expression levels of phagocytic NADPH oxidase p22phox in 59 patients with non-valvular chronic heart failure and 20 control subjects. All the subjects underwent ultrasonic cardiography to record their IVST, LVPWT, LVEDd, LVEDs, and EF. Based on the calculated LVMI, the patients were divided into heart failure without LV hypertrophy (LVH) group and heart failure with LVH group.
RESULTSThe patients with heart failure showed significantly higher expression of phagocytic NADPH oxidase p22phox than the control subjects (0.91∓0.37 vs 0.68∓0.33, P=0.039), and the patients with LVH had significantly higher p22phox expression than those without LVH (1.58∓0.20 vs 0.71∓0.24, P=0.026). LVMI showed a positive correlation with the expression of p22phox in these patients (r=0.508, P<0.05).
CONCLUSIONNADPH oxidase p22phox expression level is positively correlated with LVMI and can be indicative of the level of left ventricular remodeling in patients with non-valvular chronic heart failure.

7.Application of intraoperative neuromonitoring technology in protection of external branch of superior laryngeal nerve

Objective To investigate the application of intraoperative neuromonitoring (IONM) during thyroidectomy for external branch of superior laryngeal nerve(EBSLN).Methods From Jan.2017 to Jun.2017,138 patients undergoing thyroidectomy were randomly divided into monitor group (n=69) and the control group (n=69).The monitor group were used IONM for EBSLN,while the control group were used conventional area protection.Results The overall incidence of EBSLN injury was 1.4%(1/69) in the monitor group,and the overall incidence of EBSLN injury was 11.6%(8/69) in the control group.There was statistical significance between the two groups.Conclusion The application of IONM in thyroidectomy can exactly identify EBSLN,and reduce the possibility of EBSLN injury remarkably.
8.The effect of interventional thrombolytic therapy for central retinal artery occlusion with ipsilateral internal carotid artery occlusion via supratrochlear artery retrogradely or external carotid artery anterogradely

Objective To observe the effect of interventional thrombolytic therapy for central retinal artery occlusion (CRAO) with ipsilateral internal carotid artery occlusion via supratrochlear artery retrogradely or external carotid artery anterogradely.Methods Nine CRAO patients (9 eyes) were enrolled in this study,including 5 males and 4 females.The mean age was (45.2 ± 18.1) years.The mean onset duration was 24 hours.There were 4 eyes with vision of no light perception,3 eyes with light perception and 2 eyes with hand movement.Fundus fluorescein angiography (FFA) examination showed that the retinal artery was filled with delayed fluorescence.The peak of fluorescence was seen in the anterior part of the artery,and some of the eyes showed retrograde filling.The arm-retinal circulation time (A-Rct) was ≥35 s in 4 eyes,≥35 s-<25 s in 5 eyes.The filling time of retinal artery and its branches (FT) was ≥ 15 s in 2 eyes,≥ 12 s-<15 s in 3 eyes,≥9 s-< 12 s in 4 eyes.All the patients received the treatment of interventional thrombolytic therapy via supratrochlear artery retrogradely (8 eyes) or external carotid artery anterogradely (1 eye) according to the indications and contraindications of thrombolytic therapy in acute cerebral infraction patients.Urokinase (0.4 million U in total) was intermittently injected into the arteries.After artery thrombolysis,the changes of digital subtraction angiography (DSA),filling time of retinal artery and its branches on FFA within 24 hours and the visual acuity were observed.According to the A-Rct and FT on FFA,the therapeutic effects on retinal circulation were defined as effective markedly (A-Rct≤ 15 s,FT ≤2 s),effective (A-Rct was improved but in the range of 16-20 s,FT was in 3-8 s) and no effect (A-Rct was improved but ≥21 s,FT≥9 s).The related local or systemic complications were recorded.Results After the injection ofurokinase into the catheter,the ophthalmic artery and its branches were increased in 6 eyes (66.7%),and the development of the eye ring was significantly more than that of the eyes before thrombolysis.The circulation time in ophthalmic artery was speeded up for 2 s before thrombolysis in 3 eyes,3 s in 3 eyes,and 4 s in 2 eyes.Within 24 hours after thrombolysis treatment,the A-Rct was significantly decreased than that of before interventional therapy.The retinal circulation was effective markedly in 4 eyes (44.4%),effective in 4 eyes (44.4%) and no effect in 1 eyes (11.2%).The vision was improved 3 lines in 4 eyes (44.4%),2 lines in 3 eyes (33.3%),1 line in 1 eye (11.2%) and no change in 1 eye (11.2%).There were no abnormal eye movements,vitreous hemorrhage and incision hematoma,intracranial hemorrhage,cerebral embolism,and other local and systemic adverse effeetives during the follow-up.Conclusions The interventional thrombolytic therapy via supratrochlear artery retrogradely or external carotid artery anterogradely for CRAO with the ipsilateral internal carotid artery occlusion can improve retinal circulation and vision.There are no related local or systemic complications.
9.CloudLCA: finding the lowest common ancestor in metagenome analysis using cloud computing.

Estimating taxonomic content constitutes a key problem in metagenomic sequencing data analysis. However, extracting such content from high-throughput data of next-generation sequencing is very time-consuming with the currently available software. Here, we present CloudLCA, a parallel LCA algorithm that significantly improves the efficiency of determining taxonomic composition in metagenomic data analysis. Results show that CloudLCA (1) has a running time nearly linear with the increase of dataset magnitude, (2) displays linear speedup as the number of processors grows, especially for large datasets, and (3) reaches a speed of nearly 215 million reads each minute on a cluster with ten thin nodes. In comparison with MEGAN, a well-known metagenome analyzer, the speed of CloudLCA is up to 5 more times faster, and its peak memory usage is approximately 18.5% that of MEGAN, running on a fat node. CloudLCA can be run on one multiprocessor node or a cluster. It is expected to be part of MEGAN to accelerate analyzing reads, with the same output generated as MEGAN, which can be import into MEGAN in a direct way to finish the following analysis. Moreover, CloudLCA is a universal solution for finding the lowest common ancestor, and it can be applied in other fields requiring an LCA algorithm.

10.Efficacy analysis of pectoralis major muscle flap transplantation for sternal infection after cardiac surgery

To investigate the therapeutic effect of pectoralis major muscle flap transplantation in thoracotomy patients with sternal infection. Methods From January 2014 to December 2017, the treatment group of 39 patients with thoracic bone infection after cardiac surgery was used pectoralis major muscle flap to close the wound, while 26 patients were treated by debridement and vacuum sealing drainagea at the same time as the control group. The hospitalization time, hospital costs, number of operations, satisfaction survey, and relapse rate were compared between the those. Results Compared with the control group, the treatment group has the benefit of shorter hospitalization time [(18. 1 ± 3. 8)days vs. (36. 7 ± 11. 4) days], less hospital costs [(19429 ±4088)yuan vs. (33495 ±10712)yuan], less number of operations [(1. 1 ±0. 3)times vs. (2. 4 ±0. 8)times], higher level of satisfaction(56. 4% vs. 30. 8%), lower relapse rate(5. 1% vs. 26. 9%), the differences are statistically significant(all P <0. 05). Conclusion Thoracic reconstruction with pectoralis major muscle flap is an effective treatment for sternal infection in postcardiac surgery.
