1.Analysis of the curative effect and survival of postoperative adjuvant chemotherapy for stage Ⅱ/Ⅲ colon cancer

Objective To study the efficacy of stageⅡ/Ⅲcolon cancer postoperative adjuvant chemotherapy and survival time.Methods 108 stage Ⅱ/Ⅲ colon cancer patients with postoperative adjuvant chemotherapy were selected as the study group,and in the same period 52 stage Ⅱ/Ⅲ colon cancer patients with only simple surgery treatment were selected as the control group.The adjuvant curative effect of the two groups were analyzed and the sur-vival time was followed up.Results The patients of the two groups were followed up for 3 years.The median disease-free survival time of the study group was 18 months,disease-free survival rate was 69.44%,26 cases in recurrence and transfer,accounted for 24.07%,and 7 cases(6.48%) died;The median disease-free survival time of the control group was 12 months and the disease-free survival rate was 46.15%,18 cases in recurrence and transfer, accounted for 34.62%,and 10 cases(19.23%) died.The median disease-free survival,recurrence and transfer rate of the control group were statistically significant better than those of the control group(χ2 =8.07,4.74,all P<0.05).Postoperative adjuvant chemotherapy in patients with adverse reaction was mainly for nausea and vomiting,loss of appetite,pigmentation and hair loss up to see, but were comparatively light, and very little serious most patients could be tolerated.Conclusion The postoperative adjuvant chemotherapy for patients with stageⅡ/Ⅲcolon cancer can reduce postoperative recurrence,metastasis,improve patients'survival time,prolong patient life.

Objective To investigate the electrophysiological characteristics of the atrium in patients with atrial fibrillation (AF) during electrophysiological study (EPS) Methods Seventy two patients with presence of AF during EPS were selected as Group AF , and the other 78 patients without AF as Group control All the patients had no AF history and structural heart diseases P A, P CSd, S1 A1, S2 A2, Max CD, CD zone and ERP HRA were measured as the electrophysiological parameters Results P A, P CSd, S1 A1, S2 A2, Max CD and CD zone were significantly longer in Group AF than in Group control , ERP HRA was shorter in Group AF Conclusion The decreased atrial conduction and the shorter atrial effective refractory period may be the electrophysiological basis for the occurrence of AF
3.Clinical observation of three dimensional conformal radiation therapy in combination with chemotherapy in stage Ⅲ patients with non-small-cell lung cancer

Objective To observe the toxicity and effection of three dimensional conformal radiation therapy(3DCRT)based on conventional fractionation radiotherapy in combination with chemotherapy in non-small-cell lung cancer(NSCLC).Methods 62 NSCLC patients with stage Ⅲ were chosen as research objects.All the patients received chemotherapy two cycles with NP regimen firstly(NVB 40 mg d1,8,DDP 30 mg d2-5),then they accepted radiotherapy in twentith day after chemotherapy when hemocyte got normal.In the first half period of the radiotherapy,conventional radiotherapy were given with 2 Gy/f,after the dose got 40 Gy/20 f,then three dimensional conformal radiation therapy were given with 4~6 Gy/f every 1 to 2 days until the total dose got 24~30 Gy/4~6 f.The patients with supraclavicular lymphnode metastatic carcinoma received conventional fractionation radiotherapy with 60Co or 6 MV-X ray combined electronic ray until the dose(DT)up to 64~66 Gy.After radiotherapy,the patients accepted chemotherapy two cycles with NP regimen again.Results All the 62 patients completed treatment plan,and follow-up survey had lasted for 3 years.The short-term efficacy was evaluated in 3 months after radiotherapy. Among 62 patients, 9 cases gained a complete remission(CR),40 cases partial remission(PR),and the total remission was 79.0%.The acute toxicity showed acute radiation esophagitis,radiation pneumonia and acute gastrointestinal reaction.However,expectant treatment was effective.The 1,2,3 year survival rate was 7 1.0%,48.4%,30.6%,respectively.The 1,2,3 year local control rate was 80.6%,62.9%,40.3%,respectively.Conclusion 3DCRT based on conventional fractionation radiotherapy for NSCLC have good effection and mild toxicity.
4.Prevalence of neurasthenia and its related multivariate analysis among population attending postgraduate exam in medical class

Objective To explore the neurasthenia situation of the population attending the postgraduate exam in medical class and its influencing factors .Methods The random cluster sampling method was adopted .The influencing factors were per‐formed the χ2 test and Logistic regression analysis .Results According to the inclusion criteria and exclusion criteria ,36 positive ca‐ses were screened out ,accounting for 8 .89% of participants ,which was higher than the value of the general population ;the statisti‐cally significant differences were noted in the factors of the confidence of attending the postgraduate exam ,stress ,adjusting ability , interpersonal situation ,reading duration ,sleep ,nutrition and dietary (P<0 .05) .Conclusion Attending the postgraduate exam such highly intensitive mental activity is the important reason causing neurasthenia among the population attending the postgraduate ex‐am ,which mainly display in the aspects of stress size ,bearing ability facing stress and regulation .
5.Design and research of medical statistics question database management system

This thesis mainly expounds the design ideas and the realization methods of medical statistics question database management system.Specifically speaking,the function modules,data structure and business processes of the system are designed based on the analysis of questions,test papers and the needs of system functions.We choose Visual Basic 6.0 to develop the system interface,establish the question database with Access 2003,and make use of Word 2003 to output documents.This system can achieve the separation of teaching and testing effectively which can make examinations more normal and scientific.
6.Investigation on health service requirement and utilization in a county medical organization of Chongqing

Objective To understand the health level of people living in county and the situation between the requirement and utilization of health service for rural residents in Chongqing as well as the main influential factors by investigating the basic condition and hospitalization expenditure. Methods Three county hospitals were included in our study: county general hospital, Chinese traditional medicine hospital, and maternal care hospital. The hospitalized patients from the three hospitals were investigated on a certain day by questionaire and the data of discharged patients in a month were collected and analyzed by SAS8.0 software. Results Factors such as the hospital type, sex, age, the cost for sickbed, diagnosis and treatment fee and so on were significantly associated with the total cost for hospitalization. Expensive hospitalization fee harms the quantity and quality of health service required by rural residents. Conclusion The utilization rate of health service by female was lower than by male in rural area. The total utilization rate of health service has been improved since the new policy about rural health service was carried out. People over 45 years old are the main consumers of health service. Peasants are still main consumers to medical organization at county level. Suitable technic of health service for county hospitals should be carried out in time.
7.Experimental research on the characteristics of pancreatic cells regeneration in an animal model for isolated pancreatic trauma

Objective To study the relationships between tissue damage and the ability of the pancreatic cells to regenerate ,and analyze the alteration of the pancreatic cells regeneration .Methods Sixty rats were divided into two groups :impact group(the pan‐creas was injured by a BIM‐Ⅲ biotical impact machine ,40 rats) and control group(sham operated ,20 rats) .All rats were sacrificed at 6 h ,24 h ,72 h ,7 d after operation .The level of AMS ,LPS in the serum were detected by spectrophotometry ,pancreatic cells re‐generation were examined and analyzed by TUNEL staining and flow cytomertry ,and the Bcl‐2 and Bax expression were measured by Western blot .Results In the impact groups ,LPS was activated later than AMS ,and lasted persistently .The results from TUNEL stain ,flow cytometry and Western blot indicated that pancreatic trauma induces cell death and the compensatory prolifera‐tion of pancreatic cells .The characteristics of pancreatic cells regeneration in the animal model of isolated pancreatic trauma indicate that the proper remedial time is in the first 24h after the pancreatic trauma .Conclusion Detecting AMS and LPS at the same time can help us to determine the exocrine function of pancrease .
8.Meta analysis on the necessity for indwelling gastrointestinal decompression after gastrectomy

Objective To evaluate the necessity of indwelling gastrointestinal decompression after gastrectomy. Methods Eight publications on the necessity of gastrointestinal decompression after gastrecomy were colleted, data on recovery time of gastrointestinal function and hospital stay, complications,and motality were Meta-analyzed using fixed effect model and random effect model. Results Eight randomized trails including 975 patients were qualified and included in this study. The differences in time to oral intake ( WMD =0. 61, 95% CI: 0. 17 - 1.05, P < 0. 05 ) and hospital stay ( WMD = 1.20, 95% CI:0. 05 -2. 36, P < 0. 05 ) between the decompression group and non-decompression group were statistically significant, but the difference in time to flatus (WMD = 0. 31,95% CI: -0. 07- 0. 69, P > 0. 05 ) was not significant. There were no significant differences in complications such as nausea and vomiting ( OR = 1.43,95% CI: 0. 61 - 3.31, P > 0. 05 ), pulmonary infection and atelectasis ( OR = 1.43, 95 % CI: 0. 82 - 2. 49,P>0.05), anastomotic leakage (OR = 1.17, 95%CI: 0.54-2.49, P >0.05), abdominal abscess ( OR = 1.08, 95% CI: 0. 50 - 2. 34, P > 0. 05 ), wound dehiscence ( OR = 1.47, 95% CI: 0. 43 - 4. 95,P > 0. 05 ) between the two groups, except for fever ( OR = 1.76, 95% CI: 1.11 - 2. 78, P < 0. 05 ), which was found more frequent in decompression group than in non-decompression group. Conclusions Routine gastrointestinal decompression after gastrectomy was not conductive to the recovery of gastrointestinal function, and could not reduce the incidence of postoperative complications. Postoperative GI decompression increased fever incidence rate and prolonged hospital stay.
9.Observation on low-dose intravenous cyclophosphamide combined with standard-dose hormone in treatment of old patients with systemic lupus erythematosus

ObjectiveTo assess the efficacy of low-dose intravenous cyclophosphmide (IV CTX) in treatmerit of old patients with systemic lupus erythematosus(SLE).MethodsTwenty-three old patients with newly diagnosed, untreated SLE were included.Patients received 3 times fortnightly IV CTX pulses at a fixed dose of 400mg followed by 3 monthly pulses.All patients were orally administered 0.8mg/kg of prednisone.The changes of the indexes were observed before and after 12 weeks treatment including the scores of SLE disease activity index(SLEDAI) and the levels of C3, C4 ,24-hour urinary protein and anti-dsDNA antibody.ResultsAmong 23 patients, 19 were followed up to 24 weeks, SLEDAI 4 weeks after treatment were all significantly lower than those before the treatment (P <0.01).At week 8, SLEDAI continued to decrease(P <0.05).At week 4,the level of urine protein, the levels of complements C3 and C4 and ds-DNA decreased significantly compared with that before the treatment (P <0.05).ConclusionIntensive low-dose CTX plus prednisone was effective in newly diagnosed, untreated old patients with SLE, and could reduce adverse effect.
10.A 3-year study of patients with Kawasaki's disease complicated with coronary artery lesion

Objective To investigate the prognosis of patients with Kawasaki's disease complicated with coronary artery lesion and to provide evidence for diagnosis and treatment of these patients. Methods This study was conducted during January 2002 to June 2007. All patients diagnosed as Kawasaki's disease complicated with coronary artery lesions were from the Affiliated Hospital of Guilin Medical College, the Second People's Hospital of Guangxi Province and Guilin Women and Childrens' Hospital. All cases were echocardiogram examined in month 3, 6, 12, 24 and 36 in the purpose of observing the morphology of coronary artery. The study subjects were re-categorized to the groups of mild, moderate dilatation of coronary artery and giant coronary aneurysm, based on the severity of coronary artery lesion. The results of these results of the three groups were compared. Results Eighty-four cases in the mild group, 27 cases in the moderate group and 8 cases in the giant coronary aneurysm. The recovery cases were 23 (27%), 3 (11%) and 0 in the 3 groups respectively at month 3. The above numbers were 44 (52%), 8 (30%) and 0 respectively at month 6.The numbers were 69(82%), 13 (48%) and 1 (13%) at month 12. The numbers were 78 (93%), 19 (70%), 3 (38%) at month 24. The numbers were 82(98%), 20(74%) and 4(50%) at month 36. Thirteen patients were treated with adenosine-triphosphate (ATP) stress echocardiography examiantion, 5 patients were evaluated by coronary angiography,and 4 patients were tested by 64-slice CT coronary reconstruction. Part of the patients were found to have coronary stenosis or occlusion. Conclusion Patients of Kawasaki's disease often have concurrent coronary artery lesions. Patients with mild dilatation of the coronary artery are the most commonly seen and have the best prognosis. On the contrast, patients with giant coronary aneurysm are the lest common situation and is the worst in prognosis. Part of them will develop coronary artery stenosis or occlusion in late stage which may lead to ischemic heart disease. We should stress on close follow-up of patients with Kawasaki's disease complicated with coronary artery lesion. Appropriate and timely treatment will increase their clinical outcomes.
