OBJECTIVETo compare the clinical outcome between the two different approaches of improved lateral incision and the traditional L-shaped extension incision to treat intra-articular calcaneal fractures.

METHODSFrom January 2005 to January 2008, 36 cases of intra-articular calcaneal fractures were treated by improved lateral incision or the traditional L-shaped extension incision. In improved lateral incision group, there were 19 cases including 12 males and 7 females with an average age of (38.1 +/- 9.1) years; According to Sanders classification: 8 cases were type II, 9 were type III, 2 were type IV; The time from injury to the operation was (10.5 +/- 1.7) days. In the traditional L-shaped extension incision group,there were 17 cases including 11 males and 6 females with an average age of (38.4 +/- 7.3) years; according to Sanders classification of fractures: 7 cases were type II, 9 were type III, 1 was type IV; The time from injury to the operation was (10.6 +/- 1.8) days. Observing and comparing some items of the two groups including (1) operation time, blood loss, incision length, number of graft cases, (2) VAS pain scores after the first day, (3) wound healing, (4) Maryland foot scoring, (5) postoperative imaging results were comparative analyzed.

RESULTSTwo groups of patients were followed up from 10 to 15 months with an average of 11 months. The operative time and number of graft cases were compared between two groups with no statistically significant differences. The blood loss were (94.0 +/- 9.5) ml in improved lateral incision group and (109.9 +/- 13.7) ml in L-shaped extension incision group, incision length were (6.8 +/- 1.1) cm in improved lateral incision group and (15.7 +/- 2.2) cm in L-shaped extension incision group with significant differences. Postoperative VAS pain score at the first day were (1.95 +/- 0.71) points in improved lateral incision group and (3.65 +/- 1.00) points in L-shaped incision group with significant difference. In improved lateral incision group there was 1 case of grade B wound healing, and in the traditional L-shaped extension incision group, there were 6 cases of grade B wound healing, these patients were healing by protensive time for change dress.

CONCLUSIONThese two incision of improved lateral incision and the traditional L-shaped extension incision for treating the intra-articular calcaneal fractures are therapeutic equivalence. However, improved lateral incision has advantage of small incision and operative wound, and fewer wound complications.

OBJECTIVETo investigate the effect of edaravone on renal ischemia-reperfusion injury in rats.

METHODSFifty rats were randomly divided into five groups: sham operation group (Group A), renal ischemia-reperfusion group (Group B) and edaravone treated groups (Group C1, Group C2 and Group C3 with different drug dosages). Serum maleic dialdehyde (MDA) and superoxide dismutase (SOD), renal MDA and SOD, serum creatinine (Cr), blood urea nitrogen (BUN) were measured after the rat kidney was ischemia-reperfused for 24 hours. Renal ultrastructure was observed.

RESULTCompared with Group A, serum and renal MDA, serum Cr, BUN of Group B were significant increased (P <0.01), serum and renal SOD of Group B were significant decreased (P <0.01). After edaravone treatment, serum MDA, Cr and renal MDA of Group C were lower than those in Group B (P<0.01); Serum and renal SOD of group C were higher than those in Group B (P <0.01); Compared with Group B, BUN level of Group C had no significant change (P >0.05). The renal ultrastructure was greatly injured in Group B, meanwhile it was obviously ameliorated in Group C.

CONCLUSIONEdaravone has protective effects on renal ischemia-reperfusion injury in rats.

OBJECTIVETo investigate the therapeutic effects of double filtration plasmapheresis(DFPP) in treatment of hyperlipidemic acute pancreatitis.

METHODSNine patients with acute hyperlipidemic pancreatitis were treated with DFPP in addition to the conventional therapeutic measures. The clinical symptoms,serum levels of triglyceride (TG) and APACHE II scores were observed before and after DFPP.

RESULTAfter DFPP the clinical symptoms of patients were improved greatly; serum levels of TG decreased from (83.48 +/-2.54)mmol/L to (4.09 +/-0.65)mmol/L(P<0.01) and APACHE II scores decreased from 12.2 +/- 2.3 to 6.2 +/- 1.3(P <0.05). There were no significant side effects during and after DFPP.

CONCLUSIONDFPP can be effectively and safely applied in patients with acute hyperlipidemic pancreatitis.

Background: The detection of glutamic acid decarboxylase 65 (GAD65) autoantibodies is essential for the prediction and diagnosis of latent autoimmune diabetes in adults (LADA). The aim of the current study was to compare a newly developed electrochemiluminescence (ECL)-GAD65 antibody assay with the established radiobinding assay, and to explore whether the new assay could be used to define LADA more precisely. Methods: Serum samples were harvested from 141 patients with LADA, 95 with type 1 diabetes mellitus, and 99 with type 2 diabetes mellitus, and tested for GAD65 autoantibodies using both the radiobinding assay and ECL assay. A glutamic acid decarboxylase antibodies (GADA) competition assay was also performed to assess antibody affinity. Furthermore, the clinical features of these patients were compared. Results: Eighty-eight out of 141 serum samples (62.4%) from LADA patients were GAD65 antibody-positive by ECL assay. Compared with ECL-GAD65 antibody-negative patients, ECL-GAD65 antibody-positive patients were leaner (P<0.0001), had poorer β-cell function (P<0.05), and were more likely to have other diabetes-associated autoantibodies. The β-cell function of ECLGAD65 antibody-positive patients was similar to that of type 1 diabetes mellitus patients, whereas ECL-GAD65 antibody-negative patients were more similar to type 2 diabetes mellitus patients. Conclusion Patients with ECL-GAD65 antibody-negative share a similar phenotype with type 2 diabetes mellitus patients, whereas patients with ECL-GAD65 antibody-positive resemble those with type 1 diabetes mellitus. Thus, the detection of GADA using ECL may help to identify the subtype of LADA.

For lack of the biomarker, early diagnosis of prostate cancer is often difficult. Caveolin-1 (Cav-1) is an important oncogene and a major structural coat protein of caveolae, which is involved in multiple cellular functions including molecular transport, cell adhesion, and signal transduction, as well as in the development and progression of prostate cancer. Cav-1 is secreted as a biologically active molecule that promotes cell survival and angiogenesis within the tumor microenvironment, and is overexpressed in the metastatic and primary sites of human prostate cancer. Secreted Cav-1 can be detected in the peripheral blood, and its expression level has an indicative value in the diagnosis and prognosis of prostate cancer. This review focuses on the structure and biological characteristics of Cav-1 and its correlation with prostate cancer.
