1.Effect of human cytomegalovirus on proliferation of hematopoietic progenitor cells of cord blood.

OBJECTIVEThis study was designed to investigate the effect of human cytomegalovirus (HCMV) on the proliferation of colony forming unit granulocyte-macrophage (CFU-GM), CFU-erythroid (CFU-E), burst forming unit-erythroid (BFU-E), CFU-multipotential (CFU-Mix) and CFU-megakaryocytic (CFU-Mk) progenitor cells of cord blood in vitro as well as the possible mechanism.
METHODSTwenty cord blood specimens were collected from the umbilical vein of normal full-term neonates delivered spontaneously. This study consisted of five groups: 3 Infection groups in which 0.1 mL 10(3), 10(4) and 10(5) plague forming unit (PFU) HCMV-AD169 virus solution was added to the culture system, an Inactivated control group in which the equal volume of inactivated virus solution was added, and a Blank control group (normal progenitor cells culture system without HCMV virus infection). Colony forming unit-assay was applied to detect the effects of HCMV-AD169 strain on the colony formation, inhibition rate and colony-maintaining duration of CFU- GM, CFU-E, BFU-E, CFU-Mix and CFU-Mk of cord blood. PCR technique was used to demonstrate the existence of HCMV-DNA in the colony cells of cultured CFU-GM, CFU-E, CFU-Mix and CFU-Mk.
RESULTSHCMV-AD169 (10(3)PFU) in low concentration had inhibition effects on colony formation of the CFU-Mix and CFU-Mk (P < 0.05), whereas 10(5) PFU and 10(4) PFU HCMV-AD169 lead to decreased colonies in CFU-GM, CFU-E, BFU-E, CFU-Mix and CFU-Mk compared with the Blank control and the Inactivated control groups (P < 0.05). The suppression effect of HCMV on the colony formation was dose-dependent. The colony-maintaining duration of the CFU-GM, CFU-E, BFU-E, CFU-Mix and CFU-Mk in the 10(5) PFU and 10(4) PFU HCMV infection groups was significantly shorter than that in the two control groups (P < 0.01). The low concentration of HCMV-AD169 (10(3)PFU) infection resulted in a shortened colony-maintaining duration of the CFU-Mix and CFU-Mk (P < 0.01), but had no effects on the colony-maintaining duration of CFU-GM, CFU-E and BFU-E. PCR amplification demonstrated the existence of HCMV-AD169 DNA in the colony cells of the three Infection groups.
CONCLUSIONSHCMV-AD169 strain can infect hematopoietic progenitors of cord blood and inhibit the proliferation of hematopoietic progenitors, associated with anemia, neutropenia and thrombocytopenia in HCMV patients.

2.Determination of ginsenoside Rd and its metabolites in rat urine by LC-MS.

AIMTo study the metabolic pathways of ginsenoside Rd in rats.
METHODSUrine samples were collected before and after 24 h of single oral administration of 150 mg and intravenous administration of 60 mg of ginsenoside Rd to six rats, separately. The samples were purified by SPE column and then were analyzed by liquid chromatography-ESI-mass spectrometry for putative metabolites.
RESULTSParent drug and its seven metabolites were identified in rat urine based on comparing total ion chromatograms of the blank with the metalolic urine as well as mass spectra. Its main metabolic pathways and possible structures are elucidated.
CONCLUSIONOxidation, combination and deglucosylation were found to be the major metabolic pathway of ginsenoside Rd in rats.

3. Effect of Hei Dihuangwan on Chronic Renal Failure

Chronic renal failure (CRF) is one of the major diseases affecting people's health. The patients' renal function shows chronic progressive irreversible damage, which ultimately leads to complete loss of renal function, and seriously impacts the quality of life of patients and even life-threatening. At early or middle stages, the treatment of CRF focuses on delaying or reversing the progress of the disease, with the aim to prevent the occurrence of end-stage renal disease (ESRD). ESRD has to be treated by renal replacement therapy. Currently, with the continuously developing dialysis and kidney transplantation technology has extended the life of ESRD patients and improved the quality of life. However, there are still many problems, such as more complications, high mortality, high costs and limited transplant conditions. The non-dialysis treatment of traditional Chinese medicine plays an extremely important role in intervening the progress of CRF, and the curative effect of treating this disease is constantly improving, and it has received extensive attention and concerns from kidney scientists at home and abroad. Therefore, it is of great significance to play the role of traditional Chinese medicine and seek effective prescriptions for the treatment of CRF in the early and middle stages. Hei Dihuangwan is an effective prescription for CRF in clinic. It has a certain clinical basis, important values in alleviating renal anemia, bone disease, malnutrition, endocrine and lipid metabolism disorders, microinflammation and residual renal function protection in patients with CRF. There are also extensive experimental studies on the treatment of CRF with Hei Dihuangwan. Through the experimental study on 5/6 nephrectomy in rats with chronic renal failure, it has been found that Hei Dihuangwan can resist inflammation, fibrosis and oxidation, repair immune inflammatory injury, regulate hormone metabolism disorder, impact the regulation mechanisms, such as hemodynamics and hemorheology, delay the process of renal fibrosis in rats with renal failure, and effectively protect rat kidney function. This article reviews the recent advances in the clinical and experimental studies of CRF.
4.Cognitive function in PD patients without dementia

Objective To analyze the characteristics of cognitive function impairment in PD patients without dementia and their influencing factors, and provide evidence for early recognition and treatment of cognitive deficits in PD patients. Methods Fifty-six PD patients without dementia,admitted to our hospital from January 2010 to October 2011 were assessed with mini-mental state examination (MMSE) and Montreal cognitive assessment (MoCA) for cognitive function. Logistic stepwise regression was employed to analyze the influencing factors of cognitive function impairment.Results Using MMSE scores as the standard for recognition, PD patients with mild cognitive impairment (MCI) accounted for 7.14% (6/56); however,using MoCA scores,they accounted for 71.43%(40/56). Both of the MMSE and MoCA scores were positively correlative with the education degree of the patients (r=0.483,P=-0.007; r=0.503,P=0.000).In the cognitive domain of MMSE,the scores of visuospatial function,and abilities of delayed recall,calculation,attention and repetition had significant decrement; and MMSE indicated that abilities of delayed recall, immediate memory, calculation and attention,and visuospatial function were main cognitive disturbances of PD.In the cognitive domain of MoCA,the scores of visuospatial and executive functions,and abilities of delayed recall,abstraction and repetition had significant decrement; MoCA indicated that visuospatial and executive functions,abilities of delayed recall,denomination,attention,abstraction and repetition were main cognitive disturbances of PD. Logistic regression analysis showed that education degree and clinical types were the main influencing factors of cognitive function impairment in PD patients. Conclusion Cognitive impairment is very common in patients with PD,and the main cognitive deficits involve visuospatial and executive functions,abilities of delayed recall,calculation,attention,abstraction and repetition; education degree and clinical types are the influencing factors of cognitive impairment of PD.
5.Clinical research on fire filiform needle combined with mild moxibustion for postherpetic neuralgia.

OBJECTIVETo compare efficacy differences between fire filiform needle combined with mild moxibustion and gabapentin combined with sham acupuncture for postherpetic neuralgia (PHN).
METHODSOne hundred cases of PHN were randomly divided into a needle group and a medicine group, 50 cases in each one. In the needle group, pricking method of fire filiform needle was given at the Ashi points, and then mild moxibustion was applied for 15 min. In the medicine group, the oral administration of gabapentin capsule and sham acupuncture at non-acupoints in the distal end of lesions were applied. The treatment was required for 21 days in both groups. The visual analogue score (VAS) was recorded before treatment and on the 1st day, 2nd day, 3rd day, 6th day, 9th day and 12th day of treatment. The most severity of pain within last 24 h, preset severity of pain, immediate analgesia effect and starting time of pain relief were observed, also the efficacy was assessed and improvement of symptoms was observed in the follow-up visit.
RESULTSThe total effective rate was 94.0% (47/50) in the fire filiform needle group, which was superior to 86.0% (43/50) in the medicine group (P < 0.05). Compared with medicine group, the VAS of the most severity of pain within last 24 h was obviously reduced after the 2nd treatment in the fire filiform needle group while that of present severity of pain was relieved after the 1st treatment (both P < 0.05). The immediate analgesia effect in the fire filiform needle group was obviously superior to that in the medicine group in the first three times of treatment (all P < 0.05). The average time of pain relief was (3.91 +/- 0.82) days in the fire filiform needle group, which was significantly earlier to (6.53 +/- 1.13) days in the medicine group (P < 0.05). 26 cases were cured in the fire filiform needle group in the follow-up visit, which was superior to 2 cases in the medicine group (P < 0.05). The improvement of VAS, pain range and sleep quality in the needle group were also superior to those in the medicine group (all P < 0.05). The direct medical cost in the fire filiform needle group was (232.32 +/- 48.108) yuan, which was significantly lower than (466.00 +/- 41.09) yuan in the medicine group (P < 0.05). There was only one case of adverse effect in the medicine group during the treatment.
CONCLUSIONThe fire filiform needle combined with mild moxibustion could obviously relieve the pain in PHN patients, which has superior immediate analgesia effect and pain relieving time compared with gabapentin, which also has less adverse effects and cheap cost.

6.Clinical application of a quantitative method of atlantoaxial reduction angle in basilar invagination.

Objectives: To investigate the clinical application effect of a quantitative method of atlantoaxial reduction angle in basilar invagination. Methods: A retrospective analysis of clinical and radiographic data was conducted of 38 patients with complicated atlantoaxial dislocation and basilar invagination admitted to the Department of Neurosurgery, First Affiliated Hospital of Chongqing Medical University from May 2020 to May 2022. There were 5 males and 33 females, aged (53.5±9.9) years (range: 38 to 80 years). All patients underwent C1-2 interarticular fusion cage implantation+occipital-cervical fixation by pressing rob with the cantilever technique. The atlantoaxial reduction model of previous studies by our team was used to calculate the reduction angles before surgery. Then titanium rods of prebending angle were prepared according to the calculation before the operation. After that quantitative reduction of angle was performed during the operation. The paired t-test was used to compare the difference between the theoretical and actual reset value. Results: The theoretical reduction angle of all patients was (10.62±1.78)° (range: 6.40° to 13.20°), the actual reduction angle was (10.53±1.63)° (range: 6.70° to 13.30°) and there was no statistical difference between them (t=1.688, P=0.100). The theoretical posterior occipitocervical angle after the operation of all patients was (117.37±5.88)° (range: 107.00° to 133.00°), the actual posterior occipitocervical angle after the operation was (118.25±6.77)° (range: 105.40° to 135.80°) and there was no statistical difference between them (t=-0.737, P=0.466). The postoperative follow-up time of the patients was more than 6 months and the symptoms of all patients were relieved. All patients had satisfactory fusion between small joints without incision infection, internal fixation fracture, displacement, atlantoaxial redislocation, and other long-term complications. Conclusion: The quantitative method of atlantoaxial reduction angle in basilar invagination can calculate the theoretical reduction angle of the clivus axis angle and guide the preparation of the pre-bending titanium rod before surgery, so as to realize the quantification of the atlantoaxial reduction angle.
7.Prevalence and risk factors of Giardia lamblia infections among colorectal cancer patients in Henan Province

Objective To investigate the epidemiological characteristics and identify the risk factors of Giardia lamblia infections among patients with colorectal cancer in Henan Province. Methods A cross-sectional study was performed for questionnaire surveys among colorectal cancer patients in Henan Cancer Hospital during the period from March to July, 2021. Patients’ stool samples were collected, and the triosephosphate isomerase (tpi) gene of G. lamblia was amplified in stool samples using nested PCR assay to characterize the parasite genotype. Univariate analysis and multivariate logistic regression analyses were employed to identify the risk factors of G. lamblia infections among colorectal cancer patients. Results A total of 307 colorectal cancer patients were investigated, including 176 males (57.3%) and 131 females (42.7%). PCR assay detected 8.1% [95% confidential interval (CI): (0.056, 0.117)] prevalence of G. lamblia infections among the study subjects, and there was no significant difference in the prevalence between men [9.1%, 95% CI: (0.057, 0.143)] and women [6.9%, 95% CI: (0.037, 0.125)] (χ2 = 0.495, P = 0.482). In addition, there was no age-specific prevalence of G. lamblia infections among the participants (χ2 = 1.534, P = 0.675). Multivariate logistic regression analysis identified use of septic tanks [odds ratio (OR) = 3.336, 95% CI: (1.201, 9.267)], daily use of well water [OR = 3.042, 95% CI: (1.093, 8.465)] and raising livestock [OR = 3.740, 95% CI: (1.154, 12.121)] as risk factors of G. lamblia infections among colorectal cancer patients, and the prevalence of abdominal pain was significantly greater in colorectal cancer patients with G. lamblia infections than in those without infections (P = 0.017). Among the 25 patients with G. lamblia infections, assemblage A was characterized in 24 (96.0%) cases and assemblage B in one case (4.0%). Conclusions The prevalence of G. lamblia is high among colorectal cancer patients in Henan Province, and assemblage A is the dominant genotype of G. lamblia. Use of septic tanks, daily use of well water and raising livestock are risk factors of G. lamblia infections among patients with colorectal cancer.
